Age-dependent impairment of IgG responses to glycosylphosphatidylinositol with equal exposure to Plasmodium falciparum among Javanese migrants to Papua, Indonesia.

Immune responses directed at glycosylphosphatidylinositol (GPI) anchors of Plasmodium falciparum may offer protection against symptomatic malaria. To independently explore the effect of age on generation of the anti-GPI IgG response, we measured serum anti-GPI IgGs in a longitudinal cohort of migrant Javanese children (6-12 years old) and adults (> or = 20 years old) with equivalent numbers of exposures to P. falciparum in Papua, Indonesia. While the peak response in adults was achieved after a single infection, comparable responses in children required > or = 3-4 infections. Significantly fewer children (16%) than adults (41%) showed a high (optical density > 0.44) anti-GPI IgG response (odds ratio [OR] = 3.8, 95% confidence interval [CI] = 2.3-6.3, P < 0.0001), and adults were more likely to show a persistently high response (OR = 5.5, 95% CI = 1.0-56.8, P = 0.03). However, the minority of children showing a strong response were significantly less likely to experience symptoms with subsequent parasitemia compared with those with a weak response (OR = 4.0, 95% CI = 1.1-13.8, P = 0.02). This effect was not seen among high- and low-responding adults (OR = 1.2, 95% CI = 0.5-2.8, P = 0.60). Host age, independent of cumulative exposure, apparently represents a key determinant of the quantitative and qualitative nature of the IgG response to P. falciparum GPI.

Efficacy of permethrin-impregnated bed nets on malaria control in a hyperendemic area in Irian Jaya, Indonesia III. Antibodies to circumsporozoite protein and ring-infected erythrocyte surface antigen.

A two years intervention study was carried out using permethrin impregnated bed nets in a hyperendemic area, in Irian Jaya, Indonesia. To assess the influence of this intervention on natural immunity, concurrent immunological studies to determine levels of antibodies to the circumsporozoite (CS) and ring-infected erythrocyte surface antigen (RESA) proteins were conducted. Prevalence and titers of immunoglobulins (Ig)G and IgG subclasses were periodically measured in 138 individuals (30 children under the age of ten and 108 villagers ten years old and older). In the younger group, seropositivity of total IgG against CS fluctuated according to the parasite infection rates; however, IgG seropositive reaction against RESA gradually increased. In the older age group, seropositivity of both kinds of antibodies was stable during the whole study period. Nevertheless, the geometric mean titers of total IgG against CS and RESA were significantly reduced in this latter group in individuals who contained these antibodies before and after intervention. The geometric mean titer of IgG3 subclass against RESA was decreased at a highly significant level (p = 0.0005), and that of IgG4 against the same antigen was also decreased although to a lesser extent (p = 0.02).

Seroepidemiological studies of malaria in different endemic areas of Indonesia.

A total of 618 sera from inhabitants living in various endemic areas in Indonesia were examined for IgG against Plasmodium falciparum utilizing young trophozoites and mature schizonts as antigens by the method of ELISA and IFAT. In general, antibodies against trophozites (RESA) based on ELISA and antibodies against schizonts based on IFAT showed a correlation of malarial antibodies with the level of endemicity of the area examined. Anti-RESA antibody, detected either by ELISA or IFAT was more pronounced in the aparasitemic group compared to the parasitemic group. On the contrary, anti-schizont antibody measured by IFAT was more pronounced in the parasitemic group. Malarial antibody levels against the schizont-merozoite fraction of P. falciparum as assayed by ELISA appeared to develop more slowly compared to levels based on IFAT.

The role of community participation in the malaria control program in Indonesia.

An operational field trial on community-based malaria control efforts in West Timor, one district in Indonesia, had been implemented since 1987. The trial investigated the utilization of Posyandu cadres in case finding and treatment of malaria. From this trial it appears feasible to utilize these cadres for malaria control. Efficacy of cadres in case finding and treatment of chills and fevers surpassed all expectations.

In vitro sensitivity of Plasmodium falciparum to chloroquine and other antimalarials in east Timor and east Kalimantan, Indonesia.

In Indonesia resistance of Plasmodium falciparum to chloroquine has spread to all provinces except Yogyakarta since the first report in 1974. The proportion of resistant cases is relatively low except in Irian Jaya and East Timor. The results of in vitro tests performed in East Timor showed 81.0% resistance to chloroquine, 87.5% to amodiaquine, 20.0% to sulfadoxine-pyrimethamine (S-P), 4.8% to mefloquine, and 100% sensitivity to quinine. The percentage of failures was between 11.1% and 71.4% and highest with the S-P combination. Multidrug resistance was observed in 9 cases (or 11.1%). The results of in vitro tests in East Kalimantan were: 62.8% resistance to chloroquine, 100% to amodiaquine, 85.7% to S-P combination, 3.2% to quinine, and 2.5% to mefloquine. Failures rates ranged between 21.2% and 37.1% the highest being with mefloquine. Multidrug resistance was more prominent and observed in 59 cases (84.3%) of which one case was resistant to 5 antimalarials, while 4 cases (5.7%) were resistant to chloroquine only, compared to 72 cases (88.9%) in East Timor.

Malaria control program in Indonesia.

The malaria situation in Indonesia is reviewed in the major island group of Java-Bali and the remainder of the archipelago called the Outer Islands. Based on their varying epidemiological patterns the areas for control have been stratified and efforts are directed towards the rational use of antimalarial drugs and the institution of integrated vector control ie chemical control in conjunction with biological control and environmental management. The targets of malaria control vary as well between island groups. Administrative, technical and operational constraints are identified. Drug-resistant malaria, forest-related malaria, lack of personnel, supervision and coordination, inadequate resources, and community participation are among the main issues confronting the national malaria control program. Research and training needs are emphasized in the recommendations.

Intestinal parasitic infections in Likupang, North Sulawesi, Indonesia.

A parasitological survey was conducted on the inhabitants of 6 villages of Likupang, Minahasa Peninsula, North Sulawesi, Indonesia, in August 1991. A total of 419 fecal samples were examined by using direct smear, flotation, formalin ether concentration, Harada-Mori culture and agar-plate culture techniques. Five nematode and 7 protozoan parasites were detected, while trematode and cestode infection was not observed. Soil-transmitted nematode infections were predominant. Among the younger inhabitants aged less than 15, positive rates of Ascaris, Trichuris and hookworm infections were almost same, namely 45.7, 45.3 and 47.7%, respectively. Among the elder people aged 15 or more, positive rate of hookworm infection (89.4%) was much higher than Ascaris and Trichuris infections (19.3 and 26.1%, respectively). Village to village differences in parasite prevalence, probably due to socio-economic and sanitary-environmental differences were observed. Both Necator americanus and Ancylostoma duodenale were detected. The agar-plate culture was proved to be an efficient method for detection of hookworm as well as Strongyloides stercoralis.

Integrated ergonomics approach toward designing night and shift work in developing countries based on experiences in Bali, Indonesia.

Recently, the effort in carrying out an integrated ergonomics approach known as "SHIP" (systemic, holistic, interdisciplinary and participatory) approach has been intensively undertaken in Bali with the aim of sustaining improvements being done. The People's Consultative Assembly of the Republic of Indonesia issued for the 1999-2004 period a "SHIP" Act on the Macro Guidelines of Tourism Development in which ergonomics and other factors must be considered comprehensively to attain sustainable development in tourism. Therefore the night and shift work that is recently increasingly applied in the tourism industry must also be designed and organized through this approach. In fact, however, economic factors have still been the predominant reason for workers to accept any type of night and shift work decided by the management, without taking into account possible impacts and consequences. For example, rapid forward rotation schemes seem more adapted to the hotel industry instead of traditional 6-6-6 rotation. Further, inter-city bus drivers are approved to work a 24-hour shift followed by one day off. These drivers often work an additional risky night shift after two consecutive night shifts so as to meet needed expenses for the family. Cultural or religious activities still presented constraints for workers as they carried out subsequently the night work. Therefore, proactive steps should be taken in a timely manner through the integrated SHIP approach in designing night and shift work so as to achieve work schedules compatible with both social life of shiftworkers and business concerns.

Frequency distribution of antimalarial drug-resistant alleles among isolates of Plasmodium falciparum in Purworejo district, Central Java Province, Indonesia.

Treatment failures to the first- and second-lines antimalarial drugs chloroquine and sulfadoxine-pyrimethamine have increased in the Purworejo district on the island of Java, Indonesia. A molecular epidemiologic study was conducted to determine the frequency distribution of mutant alleles of the genes associated with the resistance among the isolates of Plasmodium falciparum from the area. Analyses using a polymerase chain reaction and restriction fragment length polymorphism showed that nearly all of the 111 samples carried mutant alleles in genes associated with chloroquine resistance: P. falciparum multi-drug resistance 1 (pfmdrl) 86Y (92%), 1042D (4.5%), and P. falciparum chloroquine resistance transporter (pfcrt) 76T (99.1%). Mutant alleles of the in the dihydrofolate reductase (dhfr) gene were also high (84.7%), either as 108N and 108T or paired with 59R, and 16V, respectively. Mutant alleles in the dihydropteroate synthase gene were the least common, either as a single 437G mutation (35.3%) or paired with 540E (26.5%). These results are consistent with the antimalarial drug resistance situation in the area and emphasize the need for a proper treatment strategy.

A small yaws survey on the island of Sumatra, Indonesia.

Yaws (framboesia tropica), caused by Treponema pallidum subspecies pertenue, is a chronic infectious disease, found in tropical rural regions. As a result of mass treatment campaigns conducted in the 1950s and 1960s the prevalence of yaws and the other endemic treponematoses (pinta and endemic syphilis) has decreased greatly. However, in several tropical regions in the world resurgence of yaws was reported in the 1980s. In this article we present the results of a small survey on yaws in six health centres in the Pariaman region in West Sumatra, Indonesia. A general conclusion from this small survey is that yaws is far from being eradicated and that in this region the disease is on the increase again. Some major findings of T. pertenue research are presented here.

Epidemiological and clinical study of autistic children in Yogyakarta, Indonesia.

This epidemiological study of autistic children was carried out in Yogyakarta, Indonesia. The approaches considered included the analysis of cross-sectional data on non-verbal intelligence, social maturity behavior or adaptive skills test by using statistical or sampling controls. The 5,120 children studied were all Javanese. Doctors, nurses of Public Health Centers and school teachers assisted in assessing the above subjects. The Bryson's screening scale for evaluating autistic children was used. A total of 66 children who scored above 16 were identified. Final diagnosis was done by means of CARS. Six children, who lived within Yogyakarta Municipality, were identified. The prevalence rate of autism within the birth cohort who were born between June 1984 and May 1991 was 12 per 10,000. Observations of the social interactions of 6 autistic and 6 normal children of equivalent mental age were carried out during a free play situation and reunion situation with mother and a stranger. A comparison of the two groups during social interaction made it clear that the autistic children showed more attachment, directed more social-interaction and more physical contact toward their mothers than to strangers during the reunion situation.

Distribution of two species of malaria, Plasmodium falciparum and Plasmodium vivax, on Lombok Island, Indonesia.

Medical and entomological surveys were conducted to determine the risk factors of Plasmodium falciparum and P. vivax infections on Lombok Island, Indonesia, to find the risk factors and the main mosquito vectors for each malaria. Multivariate longitudinal analysis demonstrated two significant risk factors for infection with P. falciparum: disappearance of P. vivax parasitemia (p<0.001) and a specific study site (p<0.001). In contrast, younger age (p=0.024) and the interpolated virtual density of An. subpictus (p=0.041) were significantly associated with increased risk of infection with P. vivax. Thus, it seems that the distribution of P. vivax was determined largely by the presence of An. subpictus, whilst that of P. falciparum was influenced by antagonism with P. vivax. This result shows the importance of following-up treated P. vivax patients to identify recrudescence of P. falciparum in this area.

Short report: polymorphisms in the chloroquine resistance transporter gene in Plasmodium falciparum isolates from Lombok, Indonesia.

The polymorphisms in the Plasmodium falciparum multidrug resistance 1 (pfmdr1) and P. falciparum chloroquine resistance transporter (pfcrt) genes, which are associated with chloroquine resistance, were examined in 48 P. falciparum isolates from uncomplicated malaria patients from the West Lombok District in Indonesia. The point mutation N86Y in pfmdr1 was present in 35.4% of the isolates and mutation K76T in pfcrt was found in all but one of the samples studied. Identified pfcrt haplotypes were mainly identical to the Papua New Guinea type S(agt)VMNT (42 of 48, 87.5%), and a few isolates had the Southeast Asia type CVIET (5 of 48, 10.4%). Moreover, one P. falciparum isolate harbored the K76N mutation, giving rise to the haplotype CVMNN, which was not previously reported in field isolates. Our findings suggest that chloroquine resistance in this area might have the same origin as in Papua New Guinea.

Laboratory and field evaluation of spatial repellency with metofluthrin-impregnated paper strip against mosquitoes in Lombok Island, Indonesia.

Spatial repellency of a new multilayer paper strip impregnated with metofluthrin, a newly synthesized pyrethroid, was evaluated in the laboratory and in the field at Kerandangan, Lombok Island, Indonesia, with the use of cow- and human-baited double nets. Spatial repellency was observed in both cow- and human-baited collections. Metofluthrin treatment reduced mosquito collection by >80% during the 1st 4 weeks. However, repellency seemed to reduce with the loss of metofluthrin by evaporation within 6 wk after treatment.

Dietary intake and the risk of coronary heart disease among the coconut-consuming Minangkabau in West Sumatra, Indonesia.

Several nutrition and non-nutritional pathways are recognised in the development and occurrence of cardiovascular disease. In many populations, high intakes of saturated fat are associated with elevated serum cholesterol concentrations and increased coronary heart disease (CHD) mortality. However, several studies report that hyperlipidaemia and heart diseases are not common among populations who consume coconut, a source of saturated fat. A case-control study was conducted among the Minangkabau known to be high coconut consumers to examine the difference in food patterns and risk of coronary heart disease (CHD) between the coronary cases and their gender- and age-matched apparently healthy counterparts serving as controls. Eligible subjects with CHD were identified through the co-operation of five participating hospitals located in Padang and Bukittinggi in West Sumatra, Indonesia. A total of 93 eligible cases (62 men and 31 women) in the Case group and 189 subjects (113 men and 76 women) in the Control group were recruited. Information on the intakes of individual foods and dishes over the preceding 12 months was obtained using a semi-quantitative food frequency questionnaire. The Case groups had significantly higher intakes of meats, eggs, sugar, tea, coffee and fruits, but lower intakes of soy products, rice and cereals compared to the controls. Coconut consumption as flesh or milk was not different between cases and controls. The cases had significantly higher intakes of protein and cholesterol, but lower intake of carbohydrate. Similar intakes of saturated and unsaturated fatty acids between the cases and controls indicated that the consumption of total fat or saturated fat, including that from coconut, was not a predictor for CHD in this food culture. However, the intakes of animal foods, total protein, dietary cholesterol and less plant derived carbohydrates were predictors of CHD.

Trial of spatial repellency of metofluthrin-impregnated paper strip against Anopheles and Culex in shelters without walls in Lombok, Indonesia.

Trials of metofluthrin-impregnated multilayer paper strips against mosquitoes in shelters with no walls were carried out at 3 sites in Lombok, Indonesia. Major reductions in human biting by Culex quinquefasciatus, Anopheles balabacensis, and An. sundaicus were achieved. The device is a very practical measure of preventing outdoor mosquito biting, with no need for electricity or heating to evaporate its active ingredient.
