A hip joint simulator study using simplified loading and motion cycles generating physiological wear paths and rates.

In some designs of hip joint simulator the cost of building a highly complex machine has been offset with the requirement for a large number of test stations. The application of the wear results generated by these machines depends on their ability to reproduce physiological wear rates and processes. In this study a hip joint simulator has been shown to reproduce physiological wear using only one load vector and two degrees of motion with simplified input cycles. The actual path of points on the femoral head relative to the acetabular cup were calculated and compared for physiological and simplified input cycles. The in vitro wear rates were found to be highly dependent on the shape of these paths and similarities could be drawn between the shape of the physiological paths and the simplified elliptical paths.

Combined anterior and posterior approach to the hip joint in reconstructive and complex arthroplasty.

The combined anterior and posterior approach permits access to the front and back of the hip joint for reconstruction and complex arthroplasty, usually without osteotomy of the greater trochanter, with minimal muscle release. There is ease of access to the anterior and posterior capsule, which can be resected under direct vision, thus permitting accurate hemostasis. The entire acetabular rim is exposed for bone grafting as required. The authors review the useful standard approaches and illustrate the combination of the anterior and posterior approaches through a straight lateral incision. Concomitant exploration of the sciatic nerve is demonstrated, as is the osteotomy of the trochanter performed when the ilium superior to the acetabulum must be reconstructed. The utility and limitations of the operation is illustrated by representative cases. This extensive procedure usually can prevent neurologic and vascular compromise and allows excellent observation of the skeletal structures for accurate component alignment.

[Characteristics of the structure and dynamics of disability in degenerative and dystrophic diseases of the hip joint].

The author has analysed the structure of disability of 142 patients with degenerative and dystrophic diseases of the hip joint (91 with coxarthrosis and 51 with osteonecrosis of the femoral head) after studying its dynamics for 3 to 10 years. Among coxarthroses the share of posttraumatic ones was especially significant, which was often due to inadequate treatment of the displaced trans-articular++ fractures of the pelvis. Attention is paid to a considerable occurrence of idiopathic osteonecrosis (up to 1/3 of the whole number of cases with degenerative and dystrophic pathology of the hip joint). A necessity of differential approach to these patients during the examination is stressed with more decisive acknowledgement of group 2 of disability at stage I and II of the disease with compulsory early surgical treatment which may prevent the development of persistent disability. At the same time the surgical treatment of coxarthrosis (intertrochanteric osteotomy), while contributing to a stabilization of the process, hardly affects the dynamics of disability.

[Treatment of hip joint dislocation with the Hanausek apparatus].

The Hanausek apparatus has been used for reposition and retention of luxation of the hip joint in the Orthopädische Klinik Hessisch Lichtenau in 37 cases aged up to 2 years, if there was no hindrance of reposition in arthrography. It is a functional method which avoids the extreme Lorenz position that is claimed to endanger the caput femoris respecting necrosis. In a retrospective roentgenological observation it became evident, that the method of Hanausek leads generally to a good development of the acetabulum. The rate of necrosis in our patients was high (17%). Our patients started treatment later (38% aged more than one year). 2/3 of the patients had a high luxation of the hip joint.

Normal pregnancy and delivery following total hip joint replacement.

Total joint replacement procedures should be restricted to older individuals or persons with limited physical activities or life expectancy. However, on occasion a total joint replacement may be the only means to avoid serious disability in a young, otherwise healthy individual. This report of a 19-year-old woman who underwent a total hip joint replacement following unsuccessful treatment of a severely comminuted intracapsular hip fracture. Subsequent to the total hip joint replacement the patient experienced a normal pregnancy and delivery.

Arthroscopic management of chondromatosis of the hip joint.

In selected cases, we present the arthroscopic surgery of the hip joint as an alternative to the conventional arthrotomy. It is distinguished by less surgery, minimal invasive exploration, and slight traumatization. Also, it comprises a short morbidity, and thereby, the method reduces the costs. If the operation succeeds, the patient and the surgeon are both enthusiastic.

The diagnosis of congenital hip-joint dislocation by the ultrasonic Combound treatment.

Due to poor penetration of bone, ultrasonic techniques have not been popular in orthropedic procedures. Since they are non-invasive and are not associated with a radiation hazard, their application in the diagnosis of congenital hip-joint dislocations merits investigation. Methods are presented in Part I which should enable the experienced investigator to make the diagnosis and avoid flash interpretations. In Part II, the practical results of these techniques over an 18 month period are presented and discussed. The small size of the object under examination and the, in general, poor resolving power of ultrasonoscopes constitute the major difficulties encountered when using ultrasonic diagnostic devices. The fact that these methods are harmless to the patient, economical, simple to perform, and noninvasive, make them attractive diagnostic tools for screening infant hip-joints congenital luxations.

Functions of hip joint muscles.

The relative torque components (percentages of total hip torque) of 26 hip muscles about three joint axes were calculated using a human multibody model to determine muscle actions as a function of hip joint angle. It was demonstrated that the muscle actions obtained from this straight line model of hip musculature were in good agreement with published functions; the data on muscle function changes during hip joint motions exceed the data available in the literature and could extend the anatomical knowledge which is available today.

A cement spacer for two-stage revision of infected implants of the hip joint.

We report the technical details and clinical results of twelve patients who had deep infections of implants in the hip joint and were treated by two-stage revision, using a gentamicin-loaded, hand-moulded cement spacer inserted for the period between resection and reimplantation arthroplasty. During management with the spacer, usually for 4 months, patients were almost free of pain and mobile with good leg control, spending 2/3 of the treatment period at home. Six of twelve spacers failed locally due to dislocation [5] or cement fracture [1], and more than two further episodes of surgery were required in 3 patients. Problems with dislocation of the spacer were significantly higher when the head to neck offset was lacking (P < 0.05) or when anchorage in the femoral shaft was poor. Nevertheless, infection after reimplantation arthroplasty did not occur by the time of follow-up (2.2 years). Based on these data, we consider that the use of the cement spacer is a promising approach to the treatment of complicated infections of the hip joint.

[Prosthetic hip joint infection caused by Listeria monocytogenes].

The authors report an unusual case of prosthetic hip joint infection caused by Listeria monocytogenes. The patient, an 87-year-old lady who had undergone a right total hip replacement 10 years previously, presented with pain and restriction of hip motion three weeks after an episode of abdominal pain. Aspiration of the joint yielded a dark fluid, from which Listeria Monocytogenes type 4-b was isolated. Blood cultures remained negative. After prolonged antibiotic therapy, symptoms gradually resolved. A few months later, pain recurred with radiological signs of loosening of the femoral component. One-stage revision arthroplasty was performed combined with antibiotic treatment. The patient remains asymptomatic at one year follow-up. Laboratory data and x-ray control are normal. Prosthetic hip joint infection with Listeria monocytogenes is uncommon; few cases have been reported. The literature review shows that prolonged antibiotic therapy alone may be used in patients for whom removal of the prosthesis is not desirable, although revision arthroplasty or prosthesis removal remains necessary in the other cases.

Ultrasonography of hip joint effusions.

In order to evaluate ultrasonography in patients with suspected hip joint effusions, 123 consecutive patients were examined prospectively. Twenty healthy subjects were used as a control group. The normal sonoanatomy, the recommended scanning approach, and the diagnostic features of intra-articular joint effusions are presented. Even minor fluid collections of 1 or 2 ml could be accurately detected. Transient synovitis and fresh hemorrhagic effusions proved to be echofree, whereas clotted hemorrhagic collections or septic arthritis showed non-echofree effusions. Based on intraoperative and computed tomography (CT) data, ultrasonography is clearly superior to X-ray films in detecting joint effusions. A negative sonogram will exclude a fluid collection; the depiction of an echofree effusion will virtually rule out septic arthritis. The use of additional imaging techniques can be reduced greatly, as ultrasonography seems to be an improved method for the early diagnosis of septic arthritis.

[Rotation-abduction contracture of the hip joint].

The paper presents the rotation-abduction contracture of the hip, a problem of growing magnitude in recent years. The anatomy of the hip joint, the perils related to muscle physiology, the symptoms of the disease, the etiology and means of surgical treatment according to Fernandez de Valderrama and Esteve de Miguel are discussed. The results of surgical treatment of 12 patients at the Orthopedic Department of Wroclaw. The presented procedure has been used in our center since 1993, and the results encourage further use of this technique.

Combined administration of low dose heparin and aspirin as prophylaxis of deep vein thrombosis after hip joint surgery.

The study was carried out on 75 patients undergoing hip joint surgery. The efficacy of a prophylaxis of deep vein thrombosis (DVT) by acetylsalicyclic lysine (ASL) and by a low dose heparin (LDH) combined with the ASL treatment was investigated using the 125I-fibrinogen test. DVT was not significantly decreased by the ASL application (53%) compared to the controls (60%). DVT occurred significantly less under the combined administration (27%) than in the other two groups.

Transient osteopenia of the hip joint in children.

Two pediatric cases of transient osteopenia of the hip joint are reported. Roentgenograms showed bone atrophy in the femoral head, the femoral neck, and the pelvis. Bone scintigrams showed a higher accumulation. At the follow-up study, osteopenia disappeared in both cases, however coxa magna was recognized in one case. The emphasis is on differential diagnosis from more severe disease, because the syndrome is characterized by spontaneous resolution.

[Temporary arthrodesis of the hip joint with fixateur externe in osteolytic pelvic metastasis].

The report concerns a patient with extended osteolytic metastasis of the pelvic region including the left hip joint with a primary kidney carcinoma. A simple fixateur-exterior apparatus for arthrodesis of the hip joint including immediate painlessness is presented. The procedure is relatively simple and with little strain for the patient. Due to the overall poor prognosis of the underlying disease, in our opinion the most simple procedure should be chosen over any complex external constructions or invasive procedures.

[Anesthetic support of operations on the hip joint in senile patients].

The impact of spinal anesthesia on a surgical stress response during prosthetic repair of the hip joint was studied in 56 senile patients (mean age 76 +/- 2.1 years). Hemodynamic changes, the time course of fluid distribution in different sectors, the activities of sympathoadrenal, cholinergic, serotoninergic, and histaminergic systems were analyzed. Studies were conducted in 4 steps: before a planned operation, before anesthesia, after the end of surgery, and 24 hours after intervention. The characteristic features of hemodynamic changes were elevated left ventricular filling pressure and high specific peripheral resistance, which was regarded as signs of latent heart failure and age-related vascular tone regulatory changes. Involutional tissue processes in senile patients appeared as decreased water levels in the cellular sector. The patients' sympathoadrenal and enterochromaffin systems were in a state of hypofunction. The adequacy of anesthesia supports no trend in the study neurohumoral regulatory parameters. The low level of hormones and mediators confirms the true depletion of the neurohumoral regulatory system.
