[Leishmaniasis in Ecuador. 5. Leishmaniasis and anthropization on the Pacific coast].

We have evaluated the impact of anthropization of the forest on the incidence of leishmaniasis, due to Leishmania panamensis, in three coastal study areas, Corriente Grande (primary forest), Paraiso Escondido and La Tablada (secondary forest). The situation of isolated dwellings, in deforested areas, has also been analysed in the last two stations. In each station, the study of the density of anthropophilic sand flies, specially Lutzomyia trapidoi, has been conducted in the domestic environment, coffee plantations and undergrowth. The incidence of leishmaniasis was nearly non existent in primary forest, though it ranged from 106 to 147% in the more or less cleared forest. At Corriente Grande, none Lu. trapidoi was caught in houses. In the undergrowth, catches were low (8% of the total). At Paraiso Escondido, Lu. trapidoi was the dominant species, with more than 83% of the catches in the undergrowth and in the coffee plantations (41 Man/hour), as well as in dwellings (10.6 M/h). At La Tablada, in the domestic environment, Lu. gomezi, was the dominant species: 2.8 M/h against 0.1 M/h for Lu. trapidoi. In the coffee plantations and in the undergrowth Lu. trapidoi was the main species, 21 M/h and 14 M/h. Thus in the primary rainforest, leishmaniasis transmission can be very low. In disturbed forest, coffee plantations near houses are good biotopes for Lu. trapidoi. The cycle of L. panamensis has been adapted to this new ecological situation, by being closer to the houses. The reservoirs live and circulate throughout coffee plantations. In deforested areas, neither aggressive sand flies have been observed, nor leishmaniasis transmission.

Nutritional status of institutionalised elderly in Ecuador.

The nutritional status of 111 elderly men and women aged 60 to 99 years in two institutions in the Ecuadorian Andes was evaluated using dietary, anthropometric and biochemical measurements. Mean daily energy intakes of men and women were 1720 and 1650 kcal, respectively (7.2 and 6.9 MJ), corresponding to 1.5 times the calculated basal metabolic rate (BMR). 6% of subjects had intakes less than 1.2 x BMR and 40% less than 1.5 x BMR. Body mass index (BMI) was below 20 kg/m2 in 21% of subjects; 45% were between 20 and 24. Only one subject had a BMI over 29. Intakes of protein, calcium, iron, riboflavin and vitamin C were satisfactory, in contrast to those of zinc, vitamin A, niacin and thiamin. Using sex- and altitude-specific cutoffs for hemoglobin, anemia was observed in 11% of subjects. Nutritional status, evaluated using biochemical indicators, was satisfactory with respect to riboflavin, vitamin A, vitamin E and copper. 14% of subjects had low serum zinc levels and 17% had serum albumin values below 35 g/L. Low physical activity was associated with low energy intake and low serum albumin levels.

Three strategies to promote sustainability of CEMOPLAF clinics in Ecuador. Céntros Médicos de Orientación y Planificación Familiar.

The continuing trend of donor attention and resources away from Latin America threatens the sustainability of nongovernmental family planning organizations in that region. Managers can improve sustainability through cost control, cost recovery, and income generation. The Population Council's INOPAL II and INOPAL III projects and Family Health International assisted CEMOPLAF, an Ecuadoran private voluntary organization, in carrying out operations research in each of these areas. Studies included cost-savings analysis from altering IUD revisit norms (cost control), an ability-to-pay study that showed potential gains from increased prices for reproductive health services (cost recovery), and a feasibility study to estimate income from ultrasound services (income generation). Results indicate that any one intervention will probably have a limited impact, and that managers likely will need to undertake several initiatives simultaneously to make significant progress toward sustainability.

The effect of antimalarial chloroquine therapy and prophylaxis on concurrent infection with Onchocerca volvulus in Ecuador.

The effect of chloroquine phosphate on Onchocerca volvulus in vivo was studied in Ecuadorians undergoing treatment for malaria. All persons with a diagnosis of acute malaria and treated with 2500 mg of chloroquine over 3 d showed a 100% reduction of dermal O. volvulus microfilariae 7 d after treatment. However, 28 d after treatment the microfilarial densities returned to their pre-treatment levels and at 35 d they had increased to 121.6% of their pre-treatment values. Treatment did not appear to have any effect on the adult O. volvulus examined histologically in extirpated nodules. Patients treated for acute malaria and subsequently kept on a prophylactic regimen of 500 mg chloroquine weekly showed a reduction of 56.7% from pre-treatment microfilarial density after 27 weeks. Patients who underwent nodulectomy as well as treatment for acute malaria and were given 500 mg of chloroquine prophylactically for 27 weeks showed a reduction in dermal microfilarial density of 93.6%. Symptoms of onchocerciasis were reduced in the latter group of patients, with the elimination of all acute dermatological changes within 6 weeks. Ocular examination of these surgically and chemotherapeutically treated individuals revealed reductions of 94.9% of microfilariae in the anterior chamber, 95.9% of live microfilariae in the cornea, and 95.1% of dead microfilariae in the cornea. There was a reduction of 69.8% in corneal fluffy opacities. No alteration in the visual acuity or in visible lesions in the posterior segment was recorded. The results suggest that a complex interaction between chloroquine and O. volvulus takes place in vivo, which can be beneficial to the patient over a long period.(ABSTRACT TRUNCATED AT 250 WORDS)

Cuticular hydrocarbons of grasshoppers from the Galapagos Islands, Ecuador.

Cuticular lipids of adult Schistocerca literosa, Schistocerca melanocera and Halmenus robustus were analyzed. They have a similar range of hydrocarbons to those previously reported for other grasshoppers, but contain significant amounts of squalene which has not previously been reported from grasshopper cuticle. In Halmenus, squalene comprises 17% of the total hydrocarbons, while internally branched methylalkanes are present in much smaller quantities than in other grasshoppers. The results lend no support for a close taxonomic link between Halmenus and Schistocerca.
