Acrylic splints for dental alignment in complex facial injuries.

Twenty-four patients with complex facial injuries were managed by wide subperiosteal exposure, precise anatomical reduction, rigid internal fixation, and immediate bone grafting when indicated, in conjunction with dental impressions, model surgery, and fabrication of dental splints to establish proper preinjury occlusion. The study population consisted of 18 men and 6 women, whose ages ranged from 18 to 49 years (mean, 30.7 yr) at the time of injury. High velocity motor vehicle accidents were responsible for facial injuries in 18 patients, gunshot wounds in 2, low velocity blunt trauma in 3, and falls in 1. All facial fractures involved the occlusion, and unstable and/or comminuted palatal/maxillary and mandibular fractures, often with edentulous segments, were the major indications for fabrication of acrylic splints. Depending on the nature of the fracture pattern, model surgery was performed on the maxillary and/or mandibular models and segmented along fracture lines. These fragments were then repositioned according to dental wear facets and preinjury occlusion. When possible, preinjury occlusal records were obtained before splint fabrication. Models were mounted on a Galetti articulator and palatal, lingual, and/or occlusal splints were fabricated. Edentulous segments were compensated for by local buildup of the splints to produce an occlusal stop. Arch bars were fixed directly to the splint with acrylic. Twenty-six splints were used in the 24 patients to establish proper occlusal relationships before internal fixation of fractures. Patient age, service mix and dental practice productivity.

To further explore economic relationships between patient age and dental practice productivity detected in previous studies, this study investigated the effects of variations in the mix of services used by different age groups on time and dollar-based dental practice outputs. Production function models for dental services were estimated using data from 31 Minnesota general dental practices reporting increased visits by older adults between June, 1980 and December, 1984. Analysis suggested that standardised time-based measures of productivity were not greatly affected by differences in the types of services used by different age groups. In contrast, differences in service mix between age groups exerted a substantial influence on dollar-based productivity that was independent of patient age. Overall, dental practices were less productive with older patients, but the decline in monetary productivity was not evident until patient age was disaggregated from differences in service use by different age groups.

[The importance of the method investing for accuracy of Sipal inlays].

The fit of three-area inlays (phantom) made from Sipal 306 was determined in an experimental study by means of microscopic measurements of the gap width. Six different investment techniques were compared. The necessity of processing Expansit adequately is emphasized. Core embedding using the LAW precision-casting investment compound is recommended.

In vitro study of transmission of bacteria from contaminated metal models to stone models via impressions.

The purpose of this study was to determine the risk of microbiological transmission into the dental laboratory via impressions and dental stone models. Metal master models were contaminated with Bacillus subtilis and Streptococcus sanguis. Impressions in alginate, polyvinylsiloxane, and polyether were used, and models were cast in two brands of dental stone. Samples were taken from the impression surfaces before and after casting, and sections from the stone models were taken by a technique developed for this study. After incubation, the numbers of colonies were counted and the numbers of colony-forming units per milliliter were calculated. The reductions were statistically significant but considered to be clinically insignificant. The conclusion is that even after severe contamination, the risks to dental laboratory personnel are minimal. "Normal" hygienic procedures are recommended instead of disinfection.

The morphogenetic triangle: a new conceptual tool for application to problems in dental morphogenesis.

A hypothetical mathematical model of human dental crown height attainment during morphogenesis was developed. The model is based on well-known histologic stages of tooth development from initiation to crown completion, with special emphasis on the dynamics and life-cycle of the cells of the inner enamel epithelium. From three temporal events (P, initiation of proliferation; D, initiation of differentiation; C, crown completion), and crown height (H) measured at the dentino-enamel junction, a morphogenetic triangle PDZ can be constructed where Z = coordinates of H and C at the time of crown completion. The slope of lines PZ and DZ represent average proliferation rate (Rp) and average differentiation rate (Rd) of the inner enamel epithelium, respectively, during crown formation. From data available in the literature, and based on morphogenetic triangle principles, Rp and Rd were calculated for the deciduous and succedaneous dentitions. A strong positive correlation between Rp and Rd was observed throughout the 20-tooth series (r = 0.996); however, Rp and Rd showed moderately strong negative associations with H, P, D, C, and the intervals between PD, PC, and DC. The smaller deciduous tooth in each deciduous:permanent pair was associated with faster Rp and Rd, and shorter intervals between initiation of proliferation, initiation of differentiation, and crown completion. The data generated in this analysis of a hypothetical dentition, while not definitive, provide new insights and a theoretical basis for a broader understanding of the basic mechanisms involved in the attainment of certain tooth dimensions during morphogenesis.

Preradiation dental extraction decisions in patients with head and neck cancer.

OBJECTIVE: The first objectives of this international survey were to study how dental-related and radiotherapy-related risk factors influence clinicians' preradiation dental decision making in patients with head and neck cancer and to evaluate clinicians' degree of certainty in making such decisions. A further objective was to examine the correlation of clinicians' policies with a policy based on a model for dental decision support that was presented in an earlier article. STUDY DESIGN: A consensus questionnaire was mailed to 54 oral-maxillofacial surgeons and hospital-based dentists at a number of international locations. The responses were aggregated and anonymously analyzed through use of a multiple regression procedure. RESULTS: Forty-four clinicians returned the questionnaire (response rate, 81%). Nine clinicians (20%) were using printed clinical guidelines for preradiation dental screening. Eighty-eight percent of clinicians' preradiation decisions and 49% of their certainty could be explained by the studied risk factors. Not all risk factors were significant at P <.001. Clinicians' policies showed high correlation (.85) with the policy based on the model for dental decision support. CONCLUSIONS: The findings support our previous assumption that policies in this field seem to be based primarily on clinical experience and opinions rather than on evidence-based clinical guidelines. We conclude that the clinical usefulness and validity of the model for dental decision support should now be tested and that it could also serve as a training tool.
