[Indications and contraindications in the surgical treatment of patients with destructive pulmonary tuberculosis and diabetes mellitus].

The additional indications for operative intervention performance are established in patients with destructive pulmonary tuberculosis and diabetes mellitus due to mutually burdened course of both diseases, poor reparative reaction of affected parenchyma, low efficacy of the tuberculosis chemotherapy. High clinical efficacy was achieved in 96.86% of operated patients, including in 92.7% in 1-15 years, the ability to work restoration--84.41% of patients.

Perforated appendicitis is not a contraindication to laparoscopy.

Recent studies have reported an increased risk of intra-abdominal abscess formation following laparoscopic operation for perforated appendicitis. We undertook this study to compare laparoscopic versus open appendectomy in the treatment of perforated appendicitis. Records of all patients undergoing an appendectomy between January 1994 and June 1997 were reviewed, classifying appendicitis as acute, gangrenous, or perforated based on the intraoperative findings. Operative procedures were categorized as open, laparoscopic converted to open, or laparoscopic. The study group included 690 patients; four hundred fourteen (60%) were acute, 77 (11%) were gangrenous, and 199 (29%) were perforated. Although mean length of stay was shorter for all patients undergoing laparoscopic appendectomy, patients with perforated appendicitis had similar length of stay between treatment groups. Mean operative time for open appendectomy was significantly shorter than for converted or laparoscopic appendectomy regardless of diagnosis (P<0.01). Ten patients (1.4%) developed an intra-abdominal abscess: six after open appendectomy (1.7%), one after converted appendectomy (3.7%), and three after laparoscopic appendectomy (1%). There was no significant difference in rate of abscess formation in patients with perforated appendicitis undergoing open, converted, or laparoscopic appendectomy. We conclude that laparoscopic appendectomy for perforated appendicitis is not associated with an increased rate of intra-abdominal abscess formation.

[In the year 2000, are there still anesthesiologic contraindications for laparoscopic abdominal surgery?].

Indications for laparoscopic surgery have been extended in the last ten years. Patients with several organ failures may also benefit of such technique. Therefore, it seems interesting to study and analyse the interactions between clinical status and the consequences of intra-abdominal gas insufflation. Because the postoperative benefits of laparoscopic surgery are notable, this technique is more and more proposed to old patients. The only imperative is to anticipate medical problems.

[How safe are the new obesity drugs? Indications and contraindications of orlistat and sibutramine].

Sibutramine and Orlistat are suitable "supporting drugs" for use in patients trying to lose weight. Orlistat reduces the absorption of fat from the intestine by about one-third. Over the long term too, the weight loss achieved under Orlistat (9%) has been greater than that seen under placebo (6.5%). Increased fat losses via the stools are associated with side effects and abandonment of treatment. Sibutramine inhibits the uptake of serotonin and noradrenaline in the synaptic gap, thus enhancing the CNS effects of these two transmitters, and prolonging the sensation of satiety. The most common side effects of sibutramine are dry mouth, headache and fatigue. The effects of sibutramine on weight reduction are similar to those of orlistat. For both drugs, the indications have been defined, and in the case of sibutramine, interactions with other medications have to be taken into account.

[Diskectomy of the lumbar spine: indication and contraindications].

The authors present an overview of the principles of surgical treatment of lumbar disc herniation which are currently applied at spinal surgery ward by the Orthopedic Department of the Poznan School of Medical Sciences. Most of these principles are valid in spinal surgery centres in USA and most European countries. The authors reviewed surgical technique, indications, contrindications and complications related to discectomy in the lumbar spine. Although the articles deals mainly with "classical" discectomy, other surgical techniques (microdiscectomy, percutaneous discectomy, chemonucleolysis, and endoscopy-assisted discectomy) are also briefly reviewed. Careful selection of patients, meticulous analysis of radiographic findings and proper surgical technique yield good results which allow most patients to improve their life conditions and lead a pain-free existence.
