[Medicoepidemilogical assessment of the incidence of noncommunicable diseases in the population living in Yerevan].

To examine the clinical and epidemiological features of non-communicable diseases at the population-based level is a highly promising line of the present-day studies in preventive medicine. The paper presents the data of the author's long-term hygienic studies of the health status of children in the organized collective bodies. Environmental pollution was found to affect the health status of not only children, but also that of adults. There is a geographical irregularity in the prevalence of non-communicable diseases by the microdistricts. At the same time, the pollution is caused by the fact that in Yerevan, there are numerous industrial enterprises and motor vehicles whose emission generates a high pollution, by increasing the incidence of respiratory and gastrointestinal diseases as compared with that in the dwellers living in a control non-industrial microdistrict.

[Topical problems of treatment of acute renal failure in crush syndrome].

The paper analyzes some experience in treating 203 victims with crush syndrome (CS). The victims' status is determined by the force and time of compression, the degree and magnitude of destructive muscular changes, accompanying by lesions and occurring complications. Varying renal dysfunction was revealed in 97 (47.8%) patients, 55 (27%) patients developed mild and severe acute renal failure (ARF) and 38 (18.7%) patients had mild AFR. Renal dysfunction was not detected in 106 (52.2%) patients. The development of severe AFR in CS is a marker of the severity of injury, the body's response to injury and shock; it dictates the tactics of intensive therapy using extracorporeal techniques of detoxification and active or delayed surgical removal of necrotic muscles, which permits prevention of prolonged severe endotoxication and pyoseptic and vital complications. Two-three-week persistent unremoved foci of muscle necrosis and endotoxicosis exhaust the body's adaptive mechanisms, cause severe dysfunction of other organs and systems, promote delayed renal function recovery and ARF resolution.

[Body segments and height in a group of Venezuelan elders].

The height, tibia length, knee height, arm span and arm demi-span were measured in 344 institutionalized elders (152 males and 192 females), with the purpose of identifying which of these variables is the most adequate to estimate the height in the elderly. Descriptive statistic was calculated and a Student's test was used to establish significant differences between sex and age groups. A simple Pearson correlation analysis was used to obtain the association between height and body segments. Sexual dimorphism was evidenced, with male predominance in all the variables (p < or = 0.001). 60-79 year-old women were taller than women of 80 of older (p < or = 0.05). The knee height showed the highest correlation with men's height, but this variable showed a lower correlation in women, especially in the 80 year-old or older group. The arm-span variable had the highest correlation with women's height. The tibia lenght was the body segment with the lowest correlation with height, both in males and females. These results suggest the need to develop predictive equations using the knee height, since it is the body segment that showed the greatest associaction with the height in elders.

[The lactic acid enterococci Enterococcus faecium and Enterococcus durans: nucleotide sequence diversity in 16S rRNA genes].

Among the strains used as starters for making sour milk products on the territory of the CIS, the bacteria Enterococcus faecium and Enterococcus durans are frequently found. In this work, we studied a new collection of lactic acid enterococci and also obtained more complete data on the nucleotide sequences of 16S rRNA genes in some strains studied earlier and found that most strains had certain distinctions in their 16S rRNA genes as compared with the E. durans and E. faecium genes available in the NCBI database. Based on these data, it is suggested that the strains of lactic acid enterococci represent new, earlier unknown taxa of enterococci that use milk as an ecological niche.

[Study of volatile fatty acids in the blood of patients with periodic disease].

Microecological "failures" are an important pathogenetic factor of different diseases and, in the authors' opinion, periodic disease (PD) is one of them. PD is a recessive disease characterized by fever attacks and neutrophil-mediated serous inflammation. A genetic factor has been established to be responsible for half the cases of PD, the influence of non-hereditary factors, particularly a role of the host automicroflora in the genesis of an inflammatory process, has been little studied. The authors' early studies indicate that there are changes in the qualitative and quantitative composition of microbial molecules in the blood of patients with PD. The anaerobic bacterial metabolites that are volatile fatty acids (VFAs) represent biologically active substances that affect the growth of the microflora, on the one hand, and the host's immunological responsiveness, on the other. Out of VFAs, only is acetic acid detectable in small quantities in the blood of healthy individuals. The other VFAs, namely propionic, valeric, butyric, and caproic acids and their isomers, are absent. Gas chromatography was used for qualiitative and quantitative determination of the metabolites of anaerobic microorganisms in the blood of patients with PD (n = 13) during an attack and remission and in that of healthy volunteers (Armenians) (n = 5) of a control group from one Yerevan region. The blood samples from all the patients with PD displayed a significantly higher concentration of caproic acid while the latter was absent in the blood of the controls. This finding suggests that there is a specific shift in the structure of the microbiocenosis in patients with PD. It is conceivable that caproic acid plays a certain role in the pathogenesis of the disease under study. Further studies will deal with the association of some microbial molecules with the manifestation of an attack of PD, which may provide the key to the goal-oriented regulation of detected homeostatic disorders and to the management of the frequency of its attacks.
