Microscopic and histochemical evaluation of demineralized freeze-dried bone allograft in association with implant placement: a case report.

A case report is presented in which a titanium implant was placed into a defect resulting from the extraction of an impacted tooth; the defect was filled with demineralized freeze-dried bone allograft particles without a membrane barrier. After a 6-month healing period the bone defect had completely healed and the tissue present had macroscopic features similar to mature bone. Histologic examination of this tissue showed that, in all demineralized freeze-dried bone allograft particles, mineralization nodules were scattered inside the demineralized bone; in the areas where the mineralization nodules were present, osteocyte lacunae could be observed. In the case presented, significant new vital bone formation was observed 6 months after placement of a demineralized freeze-dried bone allograft.

Treatment of the infected implant site using platelet-rich plasma.

Implant therapy has become a highly effective treatment modality for patients seeking the replacement of lost dentition. Potential complications in the delivery of implant therapy may cause either failure of implant integration or the development of peri-implantitis. In an effort to enhance the healing phase in the treatment of the infected implant site, the incorporation of platelet-rich plasma and its growth factors has been introduced into the surgical protocol. Documentation of cases requiring the treatment of infected implant sites with combined bone grafting, tissue regeneration, and enhanced healing phase management with platelet-rich plasma is presented.

Use of a resorbable hydroxyapatite-collagen chondroitin sulfate material on immediate postextraction sites: a clinical and histologic study.

This study looked at a bone-substitute biomaterial (Biostite) used to fill bone defects around immediate implants in humans. Ninety-six peri-implant bone defects were treated without the use of membranes, comparing two groups: group A = filling the defect with Biostite, and group B = no defect fill. After 4 to 6 months of healing, group A revealed higher percentages of implants covered by new, bone-like tissue (67% in group A, 34% in group B). Endochondral osteogenesis was demonstrated histologically in the regenerated tissues in alveolar sites treated, allowing the hypothesis of bone induction from the filling material used.

Esthetic re-creation of soft tissue architecture in replacing missing maxillary anterior teeth.

Replacing missing maxillary anterior teeth while providing an adequate esthetic can present a considerable challenge. A substantial effort is directed toward achieving a better tooth-to-soft tissue relationship, regardless of whether the restorative modalities are implants, naturally supported fixed bridges, or removable partial dentures. The interdental papilla is critical to the esthetically pleasant architecture and contributes to successful harmonious restoration, especially in patients with a high smile. There are several techniques for restoring the indental papilla. The best result comes from careful planning and good communication with the patient.

The utilization of an interdisciplinary team approach in esthetic, implant, and restorative dentistry.

To execute interdisciplinary care for complex dentofacial abnormalities, a team of providers must develop a consistent philosophy and mission regarding examination, diagnostic, and treatment planning procedures. To obtain optimum therapeutic results, patients must rely on the collaborative expertise of a team. This article and case study illustrates a multidisciplinary team's role in the prosthetic and esthetic rehabilitation of the maxillary anterior region.

Complicated comprehensive reconstruction: report of a case.

This patient presented an interesting problem surgically from the standpoint of placement of implants immediately following removal of periodontally involved teeth at which time it was necessary to completely debride the alveoli. A 20-year history of smoking mitigated the prognosis in this particular case. Prophylactic antibiotics were used perioperatively, and his healing was uneventful. The prosthetic rehabilitation also details a specific technique to reproduce superior anterior esthetics to retain virtually the same appearance in the final implant retained fixed bridge. The case also demonstrates the efficacy of immediate implant placement and augmentation of the maxilla for adequate prosthetic management.

Double-fold connective tissue pedicle graft: a novel approach for ridge augmentation.

Localized defects of the alveolar crest are deformities that impair prosthetic restoration of the damaged ridge area, causing esthetic, phonetic, and oral hygiene complications. Among the alternatives that have been proposed to restore the damaged ridge area and dentition to normal form, function, and esthetics, the procedures that involve ridge augmentation with soft tissues have been widely accepted. The present clinical report describes a novel surgical procedure to treat these deformities.

Secondary bone grafting in cleft lip and palate with eruption of tooth into the graft: a case report.

Secondary bone grafting in cleft lip and palate patients is performed preferably before the eruption of permanent canine in order to provide adequate periodontal support for eruption and preservation of the teeth adjacent to the cleft. Presented here with is a case of unilateral cleft lip and palate, which was followed up from birth to 15 years of age. The role of an orthodontist in the team approach for management of such anomalies is described. Also discussed in detail is the entire range of treatment procedures the child underwent, especially the role of secondary bone grafting.

[Mid-premaxillary sutural distraction osteogenesis for repair of alveolus cleft: an experimental study in dogs].

OBJECTIVE: To explore a new technique for repair of alveolar cleft by sutural distraction osteogenesis. METHODS: Nine 8-weeks mongrel dogs were used in this study, three being in the control group, six in the experimental group. Alveolar cleft model was created surgically in all animals. Two weeks later, a U-shaped distractor made of Ni-Ti memory alloy wire was insterted into the premaxilla to distract the mid-premaxillary suture. When the premaxilla of the cleft side approached the ipsilateral maxilla, periosteoplasty of the alveolar cleft was performed. The distractor was removed at two weeks after periosteoplasty. The results were evaluated clinically, radiographically, morphologically and histologically. RESULTS: The cleft model in dogs was stable and similar to the human alveolar cleft. In experimental dogs, the premaxilla was moved gradually toward the maxilla so that the cleft was closed. The distracted mid-premaxillary suture showed a gradually widened traingle, with its tip being posterior. The density of the distracted traingle suture was increased gradually. Bony repair was achieved completely at the cleft three months post-periosteoplasy. The morphology of the mid-premaxillary suture was restored. CONCLUSION: The alveolar cleft could be repaired by the technique of mid-premaxilla suture distraction.

[Experience with utilization of the Michigan splint for temporary splinting during osteogingivoplasty].

The experience of use Michigan splint for temporary splinting of the maxillary tooth at realization of flap surgery at 23 patients. Stages of clinical and laboratory procedures making the splint are described and analysed. Is appeared, that the reduction of mobility of teeth in the period following surgical intervention occurs on the average within 0.5-1 month faster, than with the patients after similar interventions, but without splinting. At the same time it is proved obvious, that the usage Michigan splint promotes normalization of functioning of muscles activity masticatory system and TMJ and helps in revealing of occlusal interferences.

Closed-flap laser-assisted esthetic dentistry using Er:YSGG technology.

Use of a hard-/soft-tissue laser is a wonderful adjunctive tool for esthetic and restorative dentistry. The cases described here demonstrate some of the many ways in which this laser technology allows clinicians to make significant soft- and hard-tissue changes. These changes not only improve the final esthetic outcome of the case but also provide the biologic functional parameters required for successful dentistry.

Maintaining vector control during alveolar distraction osteogenesis: a technical note.

Distraction osteogenesis is a predictable method for restoring missing tissue prior to implant placement. However, pull from the soft tissue can alter the desired direction of the distraction. This article describes and illustrates techniques that are useful for maintaining the desired vector during alveolar distraction osteogenesis. These methods can prevent the need for further intervention and allow for ideal placement of endosseous dental implants without compromising results.
