Darwinian psychiatry and the concept of mental disorder.

In this paper, we discuss the concept of mental disorder from the perspective of Darwinian psychiatry. Using this perspective does not resolve all of the quandaries which philosophers of medicine face when trying to provide a general definition of disease. However, it does take an important step toward clarifying why current methods of psychiatric diagnosis are criticizable and how clinicians can improve the identification of true mental disorders. According to Darwinian psychiatry, the validity of the conventional criteria of psychiatric morbidity is dependent on their association with functional impairment. Suffering, statistical deviance, and physical lesion are frequent correlates of mental disorders but, in absence of dysfunctional consequences, none of these criteria is sufficient for considering a psychological or behavioral condition as a psychiatric disorder. The Darwinian concept of mental disorder builds from two basic ideas: (1) the capacity to achieve biological goals is the best single attribute that characterizes mental health; and (2), the assessment of functional capacities cannot be properly made without consideration of the environment in which the individual lives. These two ideas reflect a concept of mental disorder that is both functional and ecological. A correct application of evolutionary knowledge should not necessarily lead to the conclusion that therapeutic intervention should be limited to conditions that jeopardize biological adaptation. Because one of the basic aims of medicine is to alleviate human suffering, an understanding of the evolutionary foundations of the concept of mental disorder should translate into more effective ways for promoting individual and social well-being, not into the search for natural laws determining what is therapeutically right or wrong.

Ecological validity of neuropsychological tests: focus on the California Verbal Learning Test and the Wisconsin Card Sorting Test.

To date, few studies have assessed the ecological validity of current measures of memory and executive functioning, particularly the California Verbal Learning Test (CVLT) and the Wisconsin Card Sorting Test (WCST). A battery of tests, including the CVLT and the WCST, was administered to 28 severely head-injured adults who were 1 year or more postinjury. Hierarchical linear regression was performed to assess the relationship between these cognitive tests and work functioning while statistically controlling for physical disability, emotional distress, and memory compensation strategies utilized by the participants. Results suggest that the CVLT was best at predicting performance on the job; however, the CVLT and the WCST were about equally predictive of the type of position held by a participant. Hence, this study demonstrates the ecological validity of measures of memory and executive functioning for predicting work-related skills.

[Optic nerve atrophy after vitrectomy for diabetic retinopathy: its systemic and local risk factors].

PURPOSE: To evaluate the incidence and risk factors of optic nerve atrophy after vitrectomy for diabetic retinopathy. SUBJECTS AND METHODS: Clinical charts of 48 eyes of 40 patients who underwent vitrectomy for diabetic retinopathy were retrospectively reviewed. The relationship between postoperative optic nerve atrophy and patients' physical condition was statistically analyzed. RESULT: Postoperative optic nerve atrophy, distinct from glaucomatous optic nerve atrophy, occurred in 5 eyes (10.4%). Its incidence was correlated with higher plasma creatinine concentration (p=0.001), proliferative diabetic retinopathy (p= 0.046), and retinal white vessel formation (p= 0.007). Maintenance of the best postoperative visual acuity was difficult in the patients with optic nerve atrophy. CONCLUSION: Patients with proliferative diabetic retinopathy accompanied by renal dysfunction were at high risk of optic nerve atrophy after vitrectomy.

Prevalence of tendinitis and related disorders of the upper extremity in a manufacturing workforce.

A cross sectional survey of a randomly selected population of 2,261 textile workers form an overall population of 8,539 eligible workers was performed to evaluate the prevalence of tendinitis in related upper extremity disorders. Of the sample, 2,047 respondents (91.3%) participated in a nurse screening history and examination: 1,091 (53%) had no upper extremity symptoms or abnormalities on examination; 959 (47%) with positive findings were examined by trained physicians. Of these, 347 (36.5%) were found to have no abnormality, whereas, 548 (57.3%) workers were assigned a diagnosis. Of these 227 were considered to fall into the categories of tendinitis (n = 69) or related disorders (n = 158). The projected prevalence of tendinitis and related disorders for the overall group was 11.6% (carpal tunnel syndrome 1.1%, epicondylitis 2.0%, tendinitis 3.5%, shoulder condition 2.3%, ganglion 2.3%, neck conditions 4.0%). Tendinitis was less frequent in the older age group and those employed for a longer time. The prevalence of tendinitis was found to be statistically higher in physically demanding job categories. Ninety-four percent of ailments were of mild or moderate severity. Although our study provides prevalence data for these conditions in a large manufacturing workforce across several job categories.

Impact of age integration on the well-being of elderly tenants in public housing.

The social, psychological, and environmental situations of a national area probability sample of 1875 elderly tenants in 153 public housing sites were studied by means of interviews with tenants and project mangers and direct observation of social and environmental characteristics of the housing. In this report the association between the age mix of the housing and the well-being of the tenants was investigated. With a variety of personal, social, and physical environmental characteristics statistically controlled, age segregation showed small but reliable relationships to on-site activity participation, morale, housing satisfaction, and neighborhood motility. The results are discussed in terms of policy and planning designed to maximize options for the individual.

[Acute pharmacological test of clonidine in essential tremor].

Thirty-seven patients with essential tremor were given intramuscular clonidine injections of 0.3 mg. Physical examination and statistical analysis showed that response was highly positive (t = 13.47; p < 0.0001) 60 min after administration. The absence of peripheral alpha-adrenergic receptors for tremor suggest that the drug acts centrally, consistent with the theory that essential tremor originates in a central oscillating pacemaker.

Transmyocardial laser revascularization in the treatment of end-stage coronary artery disease.

Transmyocardial laser revascularization has been used in 171 patients suffering from severe angina pectoris unresponsive to medical therapy at Deutsches Herzzentrum Berlin within the period from 7/1994 to 12/1997. In all patients conventional bypass surgery or PTCA, did not seem to offer the possibility of success. Postoperatively, the majority of the patients experienced a significant decrease in angina and better physical endurance. A statistically significant increase in myocardial perfusion or left ventricular contractility has not been detected. Perioperative mortality after TMR decreased from 8% among the first 40 patients to 2.5% among the 40 patients operated on most recently, with an overall mortality of 7%. The relief of angina symptoms and the improved quality of life after TMR seems to justify the use of the method in patients who presented with urgent indications for CABG surgery, although their coronary vessel status did not seem to promise success for this procedure. However, considering its experimental foundation and long-term effects, it seems that a conclusive assessment of TMR is not yet possible.

Reliability and validity testing of the revised 12-item Short-Form Health Survey in older adults.

The SF-12 Short Form Health Survey was developed to describe mental and physical health status of adults, and to measure the outcomes of health care services. The purpose of this study was to test the reliability and validity of this measure in a sample of 185 older adults living in a continuing care retirement community. The mean age of the participants was 86 + 6.1 and the majority were female (82%), unmarried (82%), Caucasian (99%), and all had at least a high school education. Initial testing of the SF-12 measurement model as proposed by Ware, Kosinski, and Keller (1995) demonstrated that the model did not fit the data (chi-squared/df = 7.8). The model was revised based on modification indices and the revised model provided evidence of reliability (alpha coefficients of .70, and .89; the majority of the R2 values were greater than .50), and validity as there was improved fit of the model to the data (chi 2 difference of 123, df difference of 1.0, p < .05). There was also evidence of construct validity, based on hypothesis testing, with a statistically significant correlation between physical health and number of chronic illnesses (r = .33, p < .05) and mental health and chronic illnesses (r = .27, p < .05), and a statistically significant difference in physical health (F = 30.5, p < .05) and mental health (F = 18.5, p < .05) between those who exercise regularly and those who do not. The revised SF-12 is a reliable and valid measure of health status in independent living older adults, and has the potential for use as either a predictor or an outcome measure.

Brief report: Acceptance of physical violence (APV) among adolescents in a Norwegian normal sample; statistical description of the assessment.

The article presents a psychometric description of 11 statements related to use of physical violence. The items were tested in a normal sample (N=1700, age: 15-16) from urban and rural areas in Western Norway. The internal reliability was alpha=0.86, and the factor analysis resulted in two factors. Boys had higher mean scores than girls. Self-reported violence was predicted by acceptance of physical violence.

The characterization of human brain tumor using magnetization transfer technique in magnetic resonance imaging.

The clinical applicability of magnetization transfer (MT) technique in magnetic resonance imaging (MRI) for the estimation of the histological and constitutional feature of brain tumors was investigated. MT effect was evaluated by measuring the MT ratio (MTR). The parameters in 1.5-tesla MRI system were as follows: TR, 50 msec; TE, 5 msec; flip angle, 30 degree; offset frequency of off-resonance MT pulse, 1000 Hz. The sequence was performed in 20 normal volunteers and 45 patients with brain tumors which were characterized histologically and surgically. The MTR for brain tumors was significantly lower than that for normal brain tissue (p < 0.05). The MTR for meningioma was higher than that for the other brain tumors (p < 0.05). In the meningiomas, MTR for fibrous type was higher than that for meningothelial type, but there was no statistical significance. Regarding the physical consistency for the brain tumors, as classified by surgery, there was a statistically significant difference in MTR between the soft tumor group (0.22 +/- 0.03, n = 6) and the hard tumor group (0.36 +/- 0.04, n = 10) (p < 0.01). This study suggested that the MT technique for patients with brain tumor may be useful to understand the characteristics of the tumors presurgically, based on the degree of intermolecular interaction of macromolecule such as protein.

Probabilistic structure calculations: a three-dimensional tRNA structure from sequence correlation data.

Algorithms based on probability theory can address issues of uncertainty directly through their representational framework and their theory for data combination. In this paper, we discuss the advantages of probabilistic formulations for molecular-structure calculations, describe one implementation of such a formulation, and show its performance on a data set derived from analysis of the statistical correlations within a set of aligned transfer RNA sequences. By assigning reasonable physical interpretations to certain statistical correlations, we are able to calculate three-dimensional structures for tRNA from a random starting structure. The constraints that we use are associated with different variances, and so their effects are not uniform, and must be reconciled by a probabilistic algorithm to yield the most likely structure. As might be predicted, the uncertainty in the position for each base is a function of both the number and strength of the constraints, and is reflected in the variances in atomic position calculated by the algorithm. For example, the hinge region in the tRNA is shown to be the most uncertain. In addition, the algorithm retains information about positional covariation that is useful for understanding the relationships between different parts of the structure. These experiments also demonstrate that we can define a single-sphere representation for each base that is useful for nucleic acid structural calculations in the same way that alpha-carbon representations are useful for protein structural calculations.

The importance of considering biological maturity when assessing physical fitness measures in girls and boys aged 10 to 16 years.

It is widely considered that biological maturity influences physical fitness test performance, children can be advantaged/disadvantaged in physical fitness tests by being more or less mature than counterparts of the same chronological age. The current study sought to investigate the effect sexual maturity had upon performance in physical fitness tests. A cross-sectional study involving 161 girls and 152 boys was carried out. Each subject was assessed for stature, mass, self-assessment of sexual maturity, vertical jump, hand grip strength and the 20 m shuttle run test, all procedures were standardized. Spearman's rank correlation coefficients were developed to assess the relationship between maturity and physical fitness measures. ANCOVA inferential statistics were performed to investigate if performance in physical fitness tests differed between children of different sexual maturity stages irrespective of mass and stature. Significance was set at p < 0.05. Stage of sexual maturity was significantly correlated with all physical fitness measures (boys: r=0.56 to 0.73; girls: r=0.24 to 0.46). ANCOVA revealed that when stature and mass were taken into account significant differences were evident between sexual maturity stages in boys but not girls. This suggests that increases in mass and stature are primarily responsible for variation in girls' physical performance throughout maturation, whereas in boys there are some qualitative differences in performance due to other factors. It was concluded that sexual maturity has a large influence on physical fitness measures in boys but less effect in girls. Rating of physical fitness, particularly for boys should take into account biological maturity.

Twelve-month self-reported quality of life after retropubic radical prostatectomy: a prospective study with Rand 36-Item Health Survey (Short Form-36).

OBJECTIVES: To evaluate the health-related quality of life (HRQoL) in patients undergoing retropubic radical prostatectomy (RRP) for clinically localized prostate cancer. PATIENTS AND METHODS: From February 2002 to September 2003 all patients undergoing RRP in our department were invited to participate in the study; the data from 75 of them comprised the present analysis. For evaluating HRQoL the RAND 36-Item Health Survey (SF-36) was used. RESULTS: Comparing the baseline scores of the SF-36 domains to those at 3, 6 and 12 months, there was a statistically significant difference in 'physical function', 'role limitations due to physical health problems', 'role limitations due to emotional problems', and 'energy/fatigue'. There were no statistically significant changes in the follow-up values for the other scales of the questionnaire. However, the baseline scores overlapped the 12-month follow-up values for all the SF-36 scales. The mean SF-36 scores reported by incontinent patients were lower than those of the continent patients, although this trend was not statistically significant. At the 12-month follow-up some variables were independent predictors of lower mean scores of some SF-36 scales, i.e. age >65 years, education level less than secondary school, pathological extracapsular extension of cancer and erectile dysfunction. CONCLUSION: At 1 year after RRP, HRQoL levels in each of the SF-36 domains overlapped those of the baseline in >80% of patients. The age, educational level of patients, local extension of the tumour, and erectile dysfunction could significantly affect the HRQoL scores.

Physiological reaction to workload in women performing manual or mental work.

The aim of our study was to evaluate the reaction of the circulatory system on work with various levels of physical and mental load. The examinations were carried out at a department store in two groups of women: 20 cashiers (mental work) and 53 saleswomen (physical work). There were no statistically significant differences in age, duration of employment or physical fitness between those groups. Work at this department store was performed on a two-shift system with irregular rotations. The psychic load was evaluated as a relation between subjective estimation of work demands and the ability to cope with them. Physical effort was measured as an energy expenditure during work. Cardiovascular reaction was expressed by heart rate during work, leisure time and sleep estimated according to the 24-hour Holter monitoring method. No statistically significant differences in heart rate were found to exist between cashiers and saleswomen. This, taking into account the much higher energy expenditure in saleswomen, suggests that mental effort is likely to affect heart rate. In fact, mean values of subjective assessment of work demands in cashiers were much higher (5.1) than in saleswomen (4.4), unlike the evaluation of the ability to cope with them, which points to the occurrence of a significant psychic load in the work of cashiers. A lower difference between maximum and minimum frequencies of heart rate in cashiers in comparison with saleswomen confirmed this observation, although it might have resulted from different levels of physical activity during work.(ABSTRACT TRUNCATED AT 250 WORDS)
