In this report we review our preclinical studies on the antitumor efficacy of L-DXR, using negatively charged sonicated vesicles. Animal studies indicate that various L-DXR formulations are more active than F-DXR on tumors infiltrating the liver and spleen, organs where liposomes are accumulated, and are equally effective on bone marrow-residing leukemic cells. In contrast, F-DXR was more effective than L-DXR when i.v.-administered, mg-equivalent doses were tested against ascitic and subcutaneously implanted tumors. Intraperitoneal administration of L-DXR was significantly more effective and approximately twofold less toxic than F-DXR in the treatment of an ascitic tumor. The antitumor effect correlated well with differences in drug levels in the relevant anatomic areas. These observations stress the site-specific activity of L-DXR and its dependence on biodistribution factor.

The levels of the serum carbohydrate antigen (CA) 19-9 were assayed in 284 cases including 77 patients with gastric cancer. The CA 19-9 levels were positive in 38% of gastric cancer cases, and they were significantly higher than those of benign gastric diseases. Extremely increased levels were found in advanced gastric cancer, but not in early gastric cancer. There was no significant correlation between CA 19-9 and CEA, and CA 19-9 seemed to be superior to CEA in specificity for gastric cancer, but inferior in sensitivity. Thus, the assay of CA 19-9 in combination with CEA could be helpful in the differentiation between benign and malignant gastric diseases.

In infancy and childhood, as in adults, organ defect is the most direct adverse effect of cancer treatment. Anatomical defects due to surgical treatment and functional defects due to radiotherapy are the two main types. In this paper, the relationship between the radicality of surgical treatment and preservation of organ form and function are discussed with regard to the thyroid, chest wall, lung, stomach, intestine, liver, pancreas, kidney and intra-pelvic organs. The more radical the treatment, the less preservation of organ form and function. The role of surgery has been changing in accordance with the development of chemotherapeutic drugs and drug combination, and must always be reevaluated in accordance with any progress made in other treatment modalities. Some typical cases from a series of patients we have treated were presented. A neonatal case of broad right chest wall defect due to surgical removal of chondroblastoma was presented, showing our new method of chest wall reconstruction. A case of bilateral Wilms' tumor was presented, which was treated using bench surgery to prevent total removal of kidney tissue. A case of huge and infiltrative intra-pelvic rhabdomyosarcoma was also described, which was treated by tumor extirpation and 2/3 partial resection of the urinary bladder, in combination with chemotherapy and radiotherapy.
A highly metastatic variant (LLT-HH) of liver metastasizing Lewis lung tumor (LLT) has been selected. Comparative studies were made on proliferation rate, morphological characteristics and host cell interaction of these two lines. Increased metastasis formation seemed to result from the selection of cells with increased resistance to nonspecific host effector cells.
