The effect of using T-activin in the therapy of the newborn with suppurative surgical infection.

A total of 156 newborn infants with suppurative surgical infection (SSI) were observed; 73 of them had sepsis and 83 a severe localized process. In 47 patients with sepsis and 34 with localized infection, T-activin was included in complex therapy while the other infants formed the control group. It has been established that T-activin leads to an increase in the quantity of the active population of T-lymphocytes in the peripheral blood and to enhanced functional activity of T-lymphocytes in the newborn with SSI independent of generalization of the process. Bactericidal activity of circulating phagocytes is improved. The clinical course of SSI is less severe with more pronounced positive changes in the symptoms, hospital stay of the children is shortened, lethality is reduced. The effect of T-activin on the dynamic of the indices of the immune state is more marked in a septic process.

Bacteriology and antibiotic treatment of perineal suppurative hidradenitis.

A plausible bacterial pathogen (Streptococcus milleri, Staphylococcus aureus, anaerobic streptococcus, or Bacteroides species) was isolated at least once in 26 of 32 patients with active perineal suppurative hidradenitis. The main pathogen was S milleri, whose presence was significantly associated with disease activity and whose disappearance significantly correlated with clinical improvement; S aureus nd anaerobic streptococci were also implicated.

Results of bacteriophage treatment of suppurative bacterial infections in the years 1981-1986.

In the years 1981-1986 bacteriophage therapy was applied in 550 cases (100 treated in 1986) of suppurative bacterial infections. Positive results were obtained in 508 cases (92.4%). In 38 cases (6.9%) a transient improvement was observed and in 4 cases (0.7%) phage treatment proved ineffective. Considering that majority of patients (518 cases, 94.2%) were resistant to antibiotic treatment, the results of phage therapy may be regarded as favorable.

Chronic suppurative otitis media in the Solomon Islands: a prospective, microbiological, audiometric and therapeutic survey.

Chronic suppurative otitis media affected 3.8% of 3500 Solomon Island children under 15 years (and 6.1% under 5 years) and was the sole cause of conductive hearing loss recorded in 265 children tested audiometrically. It was characterised by early onset (65% under 18 months) male preponderance and large central tubotympanic perforations. Measles, respiratory infections, swimming and malnutrition were identified as aetiological factors amenable to intervention. Proteus and pseudomonas were the principle aerobes isolated from ear pus and gentamicin the only antibiotic tested to be effective against them. However although a prospective therapeutic trial demonstrated a significantly improved outcome after aural toilet, no additional benefit was imparted by concurrent ototopical boric acid or aminoglycoside solution or oral antianaerobic clindamycin. Parental tuition in aural cleaning, avoidance of ear water entry, nose blowing and breathing will yield a good result in up to 60% of children in half of whom tympanic healing occurred.

Isolation of group eugonic fermenter-4 bacteria from a cat with multifocal suppurative pneumonia.

A pure growth of Group Eugonic Fermenter-4 bacteria (gram-negative) was isolated from the lungs of an 11-year-old male domestic cat that had been ill for 2 days before death. Clinical signs included anorexia, severe dyspnea, and salivation. The lungs contained several firm, slightly raised, yellowish-gray, spherical nodules of various sizes. Purulent exudate was found in the pericardial sac and left pleural cavity. Histologic evaluation revealed multifocal suppurative pneumonia.

Pulmonary resection in the prone position for suppurative lung disease in children.

Lung resection for suppurative inflammatory disease is hazardous in children whose small airway diameter precludes the use of standard methods of bronchial separation. A prospective evaluation of the prone position for thoracotomy in 17 children referred for operation with severe inflammatory disease was done. Bronchography showed whole lung bronchiectasis eight, destroyed lung in three, and lobar bronchiectasis five. Pulmonary resections performed with the child prone included left pneumonectomy (nine), right pneumonectomy (four), lingulectomy with lower lobectomy (two), and other lobectomy (two). No endobronchial or intrapleural spillage occurred. One child required reexploration for bleeding and one child developed a postoperative empyema that ultimately caused death. The remaining 16 children were discharged within 8 days of operation, and follow-up of 1 to 18 months records favorable progress.

Reversible and permanent bone conduction threshold shift in cases of chronic suppurative otitis media.

Bone conduction thresholds were measured in sixty-six cases of chronic suppurative otitis media before and after successful oral antimicrobial chemotherapeutic treatment. Comparison of pre- and post-treatment cumulative clinical audiograms of the diseased and the control ears of the same patients disclosed the presence of sensorineural hearing loss with a reversible and a permanent component in the diseased ears. The reversible threshold shift was observed over the higher frequencies, while the permanent threshold shift was equally imposed upon the whole range of the five tested frequencies.

[Clinical study of ceftizoxime suppositories in acute suppurative otitis media in children and tissue concentration of ceftizoxime in the palatine tonsil after administration of ceftizoxime suppositories].

The newly developed ceftizoxime rectal suppository (CZX-S) contains 125 mg or 250 mg ceftizoxime (CZX) in potency. From the laboratory and clinical studies on CZX-S, the following results were obtained. Concentration of CZX in serum and palatine tonsil when 250 mg of CZX-S was rectally administered reached the peak level rapidly. The serum levels were 9.39 micrograms/ml in 30 minutes, 6.00 micrograms/ml in 45 minutes, 4.55 micrograms/ml in 60 minutes, 3.87 micrograms/ml in 90 minutes and 2.65 micrograms/ml in 120 minutes. The palatine tonsil levels were 2.73 micrograms/g in 30 minutes, 1.83 micrograms/g in 45 minutes, 1.54 micrograms/g in 60 minutes, 0.99 micrograms/g in 90 minutes and 0.74 micrograms/g in 120 minutes. About 30% of serum concentrations were distributed into palatine tonsil. CZX-S was administered at a daily dose of 375 mg or 750 mg divided 3 times for 4 approximately 9 days in 19 cases of acute suppurative otitis media of children. The overall clinical effect was excellent in 7 cases, good in 7 cases, fair in 2 cases and poor in 3 cases. The effectiveness rate was 73.7%. No side effects were observed in any cases.

Prevalence of beta-lactamase-producing bacteria in chronic suppurative otitis media.

Aspiration of the exudate through open perforation was performed in 48 children with chronic suppurative otitis media. Eighty-two aerobic and 93 anaerobic isolates were recovered. Aerobic bacteria only were involved in 22 patients (46%), and anaerobic organisms only were involved in five patients (10%). Mixed aerobic and anaerobic isolates were recovered from 21 patients (44%). The most common bacteria isolated were Bacteroides melaninogenicus group (40% of patients), Pseudomonas aeruginosa (29%), Klebsiella pneumoniae (10%), and Staphylococcus aureus (10%). There were 50 beta-lactamase-producing organisms (26 anaerobes and 24 aerobes) recovered from 33 patients (69%). Forty-four of these bacteria were isolated from the 37 patients recently treated with a penicillin. These included all ten isolates of S aureus and Bacteroides fragilis group, 11 of 19 of B melaninogenicus group, five of the 14 P aeruginosa, three of ten K pneumoniae, three of six of Bacteroides oralis, four of six of Haemophilus influenzae, two of three of Staphylococcus epidermidis, and two of four of Branhamella catarrhalis. The beta-lactamase-producing organisms have a possible role in the failure of penicillin therapy.

Acute suppurative thyroiditis.

A 25 year old woman suddenly developed pain in a neck mass which had been present for 10 years. Aspiration of a large cyst revealed by ultrasonography produced blood stained fluid from which E. coli was subsequently grown. Acute suppurative thyroiditis is the least common inflammation of the gland, and E. coli infection has been reported only once before in the past decade. A technetium scan is helpful in distinguishing acute from subacute thyroiditis.

[Clinical evaluation of lenampicillin in the treatment of superficial suppurative skin and soft tissue infection. A double-blind study comparing amoxicillin].

A double-blind controlled clinical study between lenampicillin (LAPC), a newly developed oral ampicillin (ABPC) prodrug, and amoxicillin (AMPC) was conducted for the treatment of suppurative skin and soft tissue infection as grouped in 6 disease types. LAPC or AMPC were orally administered at a daily dose of 1,000 mg, in 4 equally divided doses. Each group was treated for 14 days. The results indicated that LAPC was equal to AMPC in evaluations of effectiveness and usefulness, although incidence of severe side effects was slightly lower in LAPC. The number of cases studied was 235 (115 in the LAPC group, 120 in the AMPC group). Among these, 10 patients (4 in LAPC, 6 in AMPC) were excluded and 12 patients (5 in LAPC, 7 in AMPC) dropped out. Final global improvement rating was evaluated in 213 patients (106 in LAPC, 107 in AMPC). General usefulness rating was evaluated in 215 patients (106 in LAPC, 109 in AMPC), and overall safety rating was evaluated in 231 patients (115 in LAPC, 116 in AMPC). Final global improvement rating of LAPC was, "cured", 55.7% and "cured" and "remarkably improved", 79.2%. The rate increased to 88.7% when "improved" was included. On the other hand, in the AMPC group, "cured" was 50.5%, and "cured" and "remarkably improved" was 76.6%. The rate increased to 91.6% when "improved" was included. No significant difference was found between the 2 drug groups. In overall safety rating of LAPC, "safe" was 93.9%, while in the AMPC group, "safe" was 94.0%. No significant difference was found between the 2 drug groups. Side effects were noted in 2 of 115 patients (1.7%) among the LAPC group and in 5 of 116 patients (4.3%) among the AMPC group. Incidence of severe side effects was slightly lower in LAPC (P less than 0.1). General usefulness rating of LAPC was, "remarkably useful", 56.6% and the rate increased to 86.8% when "useful" was included. In the AMPC group, "remarkably useful" was 51.4%, and increased to 84.4% when "useful" was included. No significant difference was found between the 2 drug groups.

Studies on stimulation and properties of antiglobulin antibodies. III. Antiglobulin antibodies with different specificity in patients with bronchial asthma complicated by suppurative catarrh of the respiratory tract.

Sera from 14 of 50 patients suffering from bronchial asthma complicated by suppurative catarrh of the respiratory tract contained antibodies to homologous and heterologous gamma-globulins. The findings were attributed to hyperimmunization causing changed in antigenic properties of antibodies. Antigenically altered antibodies can act as autoimmunogenic stimuli inducing an immune response manifested by the appearance of antibodies to autologous gamma-globulins and cross reacting with homologous and heterologous gamma-globulins.
