[Cytopharmacologic effect of using polyvinylpyridine-N-oxide in experimental silicosis].

Methods of scanning, transmission electron microscopy and cytochemistry were applied to the study of the development of experimental silicosis against the background of polyvinylpyridine-N-oxide administration. The majority of the macrophages retained their functional activity and realized quartz phagocytosis, eliminating it through the airway. Bringing the proteins on the cell surface in the fibroblasts was delayed, and formation of collagen fibers decreased. Application of the polymere prevented development of fibrosis of the lungs.

[Silicosis associated with autoimmune hemolytic anemia].

A 60-year-old man with pneumoconiosis complained of general fatigue, exertional dyspnea, and anorexia. The patient had severe anemia, and laboratory examination revealed autoimmune hemolytic anemia with positive direct and indirect Coombs tests. After corticosteroid therapy, the anemia resolved markedly, and the antinuclear antibody test became negative. However, the silicotic shadow on chest X-ray film showed no remarkable change. The autoimmune hemolytic anemia in this case was probably due to an immunological disturbance caused by silicosis.

[Spirometric investigation and spiro-ergometric test in evaluation of exercise tolerance in patients suffering from aluminosis and silicosis].

The main purpose of our research was to estimate the ventilation and physical efficiency in 6 patients suffering from aluminosis and silicosis. Chest X-ray examinations, statical spirometric investigations and spiroergometric tests (TSE) were performed. Oxygen levels achieved in this group were equal or even higher than medium values achieved by healthy people. Ability to physical effort can be estimated only using TSE.

[Effects of antioxidant state on individual susceptibility to silicosis (an experimental study)].

The authors proved that animals having higher natural antioxidant activity and lower intensity of lipid peroxidation are more resistant to fibrogenic effects of quartz. The peritoneal macrophages obtained from those animals show higher resistance to quartz exposure "in vitro". Parameters characterizing antioxidant system should be included into a group of indexes describing propensity to silicosis.

[Effect of different edible fats on the lipid metabolism of rats with experimental silicosis].

With a long-term (1 and 4 months) introduction of an additional amount of edible fats (beef, hog fats, butter, sunflower seed oil) to intact and intratracheally quartz-dust laden sexually mature male rats an organ-specific reaction to the supply of fat, and in intact rats, also some peculiarities of the reaction depending upon the kind of the introduced fats, were discovered. An additional administration of fats to rats with developing silicosis tended to increase accumulation of lipids in the lungs and led to a higher content of oxyproline in the lungs and liver, which bears proof to an adverse effect produced by the introduced fats on the lipids metabolism and the processes of silicotic fibrogenesis.

[Percutaneous effects of iodine inhibit the development of experimental silicosis].

Experiments on white rats proved that the silicotic pulmonary changes slowed with the tails periodically immersed into iodine solution. Those pulmonary changes were due to inhalation of quartzite dust and especially due to intratracheal administration of standardized quartz dust DQ12. Influenced by inhalation of dust containing quartz, the rats demonstrated characteristic alterations in cytologic features of bronchial and alveolar lavage water. The "iodine baths" also declined those alterations, that may prove higher resistance of pulmonary macrophages to cytotoxic effects of quartz. This hypothesis finds a support in experiments with culture of rat peritoneal macrophages. If incubated in a medium with low iodine concentrations and subsequently washed off, the macrophages demonstrate lower incidence of cytotoxic effects due to the next incubation with particles of DQ12. The macrophage's higher resistance against cytotoxic agents is believed to result directly in antisilicotic effect of iodine, so "iodine baths" could be recommended for pathogenetic therapy and medical prophylaxis of silicosis.

The association: scleroderma-silicosis or Erasmus' syndrome (author's transl).

Classically more common in women, scleroderma nevertheless affects with predilection men exposed to the inhalation of silica particles. The association scleroderma-silicosis is essentially a statistical finding. It has no particular clinical, radiological nor histological distinguishing characteristics, other than the syndromic juxtaposition of the stigmata of each of the two disorders. This serious pathological combination is not due merely to chance. It is the result of depressed cellular immunity, related to the cytotoxicity of silica and responsible for auto-immune reactions and the formation of circulating immune-complexes. In the presence of an underlying predisposition, the polyvisceral diffusion of inhaled toxic mineral particles could also facilitate the sclerodermic process by stimulating fibroblastic proliferation.

[Silicosis and primary Gougerot-Sjögren syndrome].

We report three cases of primary Sjögren's syndrome in silicotic coal miners. All patients fulfilled the diagnostic criteria for Sjögren's syndrome recently established by the European Community study group. One patient had cryoglobulinemia and polynevritis. Another had Raynaud's phenomenon, arthralgia, purpura and polynevritis. Capillary microscopy was normal in all the three patients. Antinuclear antibodies were detected only in one patient, who had also anti-SSa and anti-SSb antibodies. The prevalence of systemic sclerosis, rheumatoid arthritis and probably systemic lupus erythematosus is significantly higher after longstanding occupational exposure to silica. On the contrary, any case of Sjögren's syndrome was till now described in the course of pulmonary silicosis. The physiopathological mechanisms of these associations are misunderstood.

["Phtisis atra"--a now seldom disease picture as a special disease course of silicosis].

Progressive massive fibrosis (PMF) of the lung is caused by coalescence of fibrotic nodules. The center of the PMF often displays necrotic areas. If the necrosis gets in contact to the bronchial system cavitation may occur. We report on a 68 year old patient suffering from severe silicosis of the lung and metastatic spread of a histologically proven lung cancer into the brain. The patient who was administered to the hospital under the intention of cerebral radiotherapy showed a colliquative PMF in the right upper lobe with cavitation and expectoration of large amounts of black-stained sputum (melanoptysis).
