Self-employment and economic leadership as two promising perspectives on rural disability and work.

A defining difference between rural and urban circumstances for people with disabilities involves opportunities for work. One of the most significant elements of the rural context is that economic conditions in rural communities consistently trail the national economy. This paper presents an overview of disability in rural America and outlines an ecological model for guiding the development of rural solutions to rehabilitation problems. Two promising examples of such solutions, self-employment as a rural vocational rehabilitation employment option and rural economic leadership by people with disabilities, are outlined.

At the interface of health and community care: developing linkages between aged care services in a rural context.

This paper explores issues concerning the development of linkages across the interface between acute and community aged care services in a small regional Australian city. It addresses a participatory action research project that took place over a 2 year period involving an Aged Care Assessment Team (ACAT). Aged Care Assessment Teams are multidisciplinary teams whose members mediate between hospitals and the aged care system in the community and have a key role in developing networks and linkages between various service providers in the field. In an age of economic rationalist-inspired reform agendas in health and community care, rural infrastructures have been compromised to such a degree that the role of rural ACAT in developing linkages between sectors has never been more important. This paper takes up this issue and addresses the project findings, which highlight a field characterised by ineffective linkages within and between the various sectors, a lack of understanding of the operation of the rural aged care system among nurses working in regional hospitals, and the efficacy of ACAT working collaboratively with nurses to create new and more effective linkages in aged care.

Migration and circulation in Ecuador.

"This study investigates labor mobility in Ecuador as a function of the socioeconomic structure of places and personal attributes of migrants and circulators (short-term movers).... Particular attention is given to the role of agrarian change in altering established migration and circulation patterns." The author finds that "circulation may significantly alter family organization and the role of women in many Ecuadorian communities."

The social/sexual environment of gay men residing in a rural frontier state: implications for the development of HIV prevention programs.

CONTEXT: The incidence and prevalence of HIV/AIDS is increasing among rural men who have sex with men (MSM). Yet little is known about the social/ sexual environment of rural frontier areas. PURPOSE: The purpose of this study was to assess the social/sexual environment of gay men living in rural areas and how this environment contributes to the development of HIV/AIDS prevention programs. METHODS: A qualitative study was conducted in Wyoming. In-depth guided interviews were conducted with 39 self-identified gay men. Data were analyzed for emergent themes using constant comparative analysis. FINDINGS: Four broadly related themes emerged. Participants perceive that they live in a hostile social environment in which the potential for becoming a target of violence is present. In order to cope with this social reality, men adopt strategies to assimilate into the predominant heterosexual culture and to look for sex partners. These, in turn, are related to their attitudes about HIV/AIDS and prevention activities. Notably, the Internet was discussed by participants as a means for men to connect to a larger gay society and look for sex partners and as a potential venue to HIV/AIDS prevention programs. CONCLUSIONS: Data provided a number of implications for developing HIV/AIDS prevention programs targeting rural MSM. Especially apparent was the need for programs to be mindful of the desire to keep one's sexual preferences shielded from public knowledge and the effect this may have on recruiting rural MSM to participate in prevention activities. The Internet, because men can access it privately, might provide a venue for prevention projects.

Parental views of the development of children in a rural South African community.

The purpose of this study is to describe how parents/guardians, reported on the development of their children, their perceptions of social support, satisfaction, and their level of depression. This is an aspect of a larger study conducted to create a database that will help to examine and describe the health and developmental status of children and their families. The results suggested that the parent's satisfaction had a direct correlation with the social support that they received from their families and the community. The study also revealed that the level of depression of the parents was directly associated with their degree of satisfaction.

[Protocol and quality control for Chinese food and nutrition surveillance system in 1998 (1998 CFNSS)].

Establishment of Chinese Food and Nutrition Surveillance System is of great importance in providing information on food consumption behavior and nutrition status of the people which is essential for health, economic planning and agriculture development. Sampling of CFNSS is based on the 145 sites Disease Surveillance Point (DSP) System built by Ministry of Health, and in conjunction with the 600 sites Household Social Economic Survey System founded by State Statistics Bureau. According to indicator system developed by the Ministry of Health for stratification, strata of urban, general rural and less-developed rural were defined and 40 sites were randomly sampled as national sample. The CFNSS sample was the common sub-sample of DSP system and the SSB Household Survey System that makes sharing of information resources possible. The 40 sites are distributed in 26 Provinces/Municipalities/Autonomous Regions. All the sites information was collected on body weight and height of children under 6 years of age, and questionnaire covering the basic family information, feeding practice and disease, health conditions of the households were surveyed by interviews. Whole year food consumption data collected at the sampled households by SSB Urban Household Survey Team and Rural Household Survey Team in the previous year was provided. Comprehensive data analysis was done to formulate the information on the relationship of food, nutrition and health. The protocol, methodologies and principles for the survey are described. 5 sites of CFNSS were selected for quality evaluation on the anthropometric measurement and questionnaire interview. The body weight and length of 20 children of 3-5 years of age in each of 5 sites was measured again. The result showed that the completeness of questionnaires interview was 96%, the rate of logic error was zero. There was no significant difference between the results measured by supervisor and investigator and no significant error on repeated measurements. The reliability of data R = 0.95. The results showed that the data was accurate and reliable.
