Different models of analytic group psychotherapy. A comparative survey.

The purpose of this paper is to reflect on and to compare various psychoanalytic models which are used in the conceptualisation of the processes occurring in small psychotherapeutic groups. Three tendencies are roughly differentiated: (1) the tendency to analyse the individual patient in the group (Slavson, Wolfson and Schwartz, Kuiper); (2) the tendency to stress the importance of the group as a whole (Bion, Eziel, Whitaker and Lieberman), and (3) the tendency to consider the group more as a network of interpersonal relations (Foulkes). For each model the following aspects are analysed: the importance of the group or the individual; the role and the attitude of the therapist, and the factors which are considered therapeutic.

Dreams as an aid in determining diagnosis, prognosis, and attitude towards treatment.

The are certain types of dreams, in both psychosomatic and general medical patients, that address themselves directly - in undisguised form - to illness. From a collection of 60 such dreams, examples are presented which illustrate the objective value dreams can have in determining diagnosis, prognosis, and patient's attitude towards treatment. These dreams bring to light crucial factors that could affect treatment, such as the unconscious need for symptoms, the purposive function of symptoms, and unconscious resistances to treatment. Dreams may also reveal an unconscious knowledge of impending death in terminal patients. Although it is usually the patient who has dreams relevant to an existing illness, family members or the physician or therapist may also have them. This study makes a link with ancient Greek medical tradition in which dreams had a central role in both diagnosis and treatment. The possibility of revitalizing this tradition is noted, and the need for systematic research into dreams associated with illness is emphasized.

Assessment for three different forms of short-term dynamic psychotherapy. Findings from the Bergen Project.

Forty-four patients were assessed for three different short-term dynamic therapies, with an evaluation form based on Sifneos' criteria for Short-Term Anxiety-Provoking Psychotherapy (STAPP). Ten patients were ascribed to STAPP, 22 patients to Malan's Brief Psychotherapy (BP), and 12 patients to a more eclectic/integrative form of brief psychotherapy in this project called the FIAT model. 78% of the patients completed their treatment in agreement with the original ascription to therapy, with good results for all three therapies. The evaluation form seems to be a reliable and valid instrument offering a good and systematic basis for designing a tailor-made treatment format for different types of patients.

Individual and group training program.

We describe the experience of a didactic analysis group during a 3-year period in the Academy of Analytical Psychotherapy placing special emphasis on the training of the psychoanalyst, who, apart from carrying out his individual analysis, must complement it with group training. The advantages of the group analysis are given particular attention when evaluating the training of the candidate from the point of view of the psychoanalyst and the 6 doctors who have received this education. In this type of training for individuals and groups the program aims at a deeper study of the aspects of narcissism, the formation of the self and the social ego which we consider to be the fundamental elements in the development of this didactic experience.

Training in short-term psychotherapy.

A new approach to short-term psychotherapy, based on psychoanalytic theory is presented as 'Therapy of limited time and objectives'. Personal experience, psychoanalytic knowledge and flexibility for accepting technical modifications allows for familiarity with concepts such as 'available setting', 'operational time' and 'adequate therapeutic time', which together with supervision and deep psychoanalytic learning, emphasizing counter-transferential knowledge, shall help to achieve a true epistemophilic sentiment, essential for empathic observation and listening, basic for training in short-term psychotherapy.

Somatic reenactment in the treatment of posttraumatic stress disorder.

Somatic reenactments, like other intrusive symptoms in post-traumatic stress disorder, such as flashbacks and nightmares, reproduce the mental content of traumatic events. Four cases are presented from survivors of military trauma and civilian catastrophes. The patients were part of larger research projects carried out by the University of Cincinnati Traumatic Stress Study Center. Understanding such symptoms as repetitions of the trauma itself proved useful therapeutically, especially in consolidating the working alliance.
