Sensory impairment in late-life schizophrenia.

We reviewed 27 published studies examining a possible association between sensory (visual or hearing) impairment and late-life psychosis with paranoid features. A majority of these investigations supported the postulated association between hearing impairment and late-onset schizophrenia or paranoid disorder. Many of the published studies, however, had important methodological limitations. In a case-control study, we assessed visual and hearing impairments in 87 middle-aged and elderly subjects (16 with late-onset schizophrenia, 25 with early-onset schizophrenia, 20 with mood disorder, and 26 normal comparison subjects). Visual and hearing impairments were assessed in a blind manner by means of standardized quantitative assessments. Compared with normal subjects, both of the schizophrenia groups and the mood disorder group had greater impairment in most variables of corrected visual acuity and in self-reported hearing deficit, but not in uncorrected (constitutional) visual acuity or on pure-tone audiometry. Our results suggest that the observed relationship between sensory impairment and late-life psychosis may be due, at least in part, to a suboptimal correction of sensory deficits in older psychiatric patients.

Bifocal contact lenses today.

Bifocal contact lenses produced today are far from perfection; however, there are several different types produced, one of which should work out for most presbyopic patients who can be successfully fit with single vision contact lenses. This paper looks at the products we have available now. It is not designed to be a cookbook that the optometric practitioner can follow from A to Z and come up with the ideal contact lens bifocal fit. It is pointed at making the optometrist aware of the growing population of contact lens wearers that the health professions are now creating, and a resume of the products and fitting techniques now available to the eye care field.

The design and prescription of multifocal lenses for civil pilots.

The aging (presbyopic) pilot has difficulty with near vision which can be a serious problem in the cockpit because of the complexity and uniqueness of his visual tasks. Numerous individual studies have been reported, but the aviation industry and vision care professionals do not have available to them a set of guidelines to permit the optimum fitting of spectacles for civil aviation pilots. In this study the cockpit vision environment was studied in ten contemporary aircraft. Twenty-five pilots of seven of these aircraft types participated in a spectacle design study. Experimental spectacles were designed for each pilot and evaluated for performance. Problems of pilot acceptance of reading aids, custom designing to the geometry of a given aircraft, balancing of the visual gains from complex designs versus difficulties in use and manufacture have been analyzed. Results of this study should be of value for human factors engineering, vision testing and accurate prescription for the presbyopic pilot.

The state of accommodation during the measurement of axial chromatic aberration of the eye.

The axial chromatic aberration of the eye was determined as in previous studies by finding the lens required to eliminate or minimize blur of the fixation target when it is illuminated by a series of monochromatic sources. Objective measurement of the refraction of the eyes during this procedure showed that accommodation remained constant throughout. Hence, the conclusion that the accommodative state does not in this case respond automatically to changes in the vergence of light falling in the retinal plane. It is suggested that some other stimulus, perhaps perceived distance, predominates. Further, the least amount of plus lens required to blur a near target was about the same for white and red but much less for blue. This substantiates an earlier finding that the eye accommodates for near only sufficiently to place the blue end of the chromatic interval on the retina.

Vision training for presbyopic nonstrabismic patients.

Home vision training was prescribed for 161 presbyopic patients (ages 45 to 89) who had vision-related symptoms and convergence insufficiency or visual skills deficiencies. Most patients required 10 weeks or less of treatment, the longest treatment period being 15 weeks. Elimination of the symptoms and improved responses on certain optometric tests was achieved by 92% of the patients. Evaluation 3 months after therapy indicated that 77 patients required additional training, the older patients requiring it more often, to retain the initial improvement.

Depth of field for the presbyope.

As a general rule the near addition (add) for a presbyope is selected so that the range of clear vision is dioptrically and not linearly balanced. Implicit in such thinking is the assumption that the dioptric and linear range of clear vision cannot be simultaneously balanced. Assuming that the range of clear vision for the absolute presbyope is due to depth of field and assuming also that the blur circle theory adequately describes the limits of depth of field, I will show that the dioptric and linear range of clear vision can indeed be simultaneously balanced. The results are independent of the actual position of the reference plane from which the measurements are made.

The range of zero-associated phoria in an asymptomatic clinical population.

The clinical concept of the range of zero-associated phoria is developed, and normative data for this range are presented based on a clinical study of 201 asymptomatic patients. The relation of patient age, sex, and amount of near lens addition to this range is examined. The range at far is not related to patient age or sex. The range at near appears to bae independent of patient age, sex, and the amount of near addition required by presbyopes.

Space shuttle suit alert system visual acuity problem.

The National Aeronautics and Space Administration (NASA) requested solution of a visual acuity problem that presbyopic astronauts had when viewing an alert system visual display attached to their Space Shuttle suits. Solutions wee analyzed in the context of constraints which prohibited major modifications of the suits. The solution to the problem was to mount Fresnel lens strips on the helmet so that presbyopic astronauts could view the visual display clearly.
