Underutilization of breast-conserving therapy in a predominantly rural population: need for improved surgeon and public education.

Though breast-conserving therapy (BCT) was first recommended as the preferred treatment for women with early-stage breast cancer in 1990, little is known about the factors influencing or limiting the use of BCT in rural women. We retrospectively surveyed all surviving patients (227) referred to the Roger Maris Cancer Center over a 2-year period. Disease characteristics were verified by the tumor registry and random chart review. Responses were obtained from 171 patients (75%), a median of 26 months from diagnosis. The majority of patients were from rural areas; only 32% resided in towns with a population greater than or equal to 15,000. Thirty-five percent of those patients meeting published criteria had BCT. Patients who underwent BCT were younger (mean age 56.8 vs. 62.5, P = 0.01), more likely to have benign axillary lymph nodes (82% vs. 64%, P = 0.008), and more likely to be employed away from the home (66% vs. 44%, P = 0.01) than patients who underwent mastectomy (MRM). Distance from the nearest radiation facility did not affect treatment decisions (mean: 59.5 miles BCT vs. 52.6 miles MRM). Most patients (83%) ranked their surgeon as the most important source of information about treatment options. Perceived surgical recommendations were generally followed. Only three patients who felt their surgeon recommended MRM underwent BCT; eleven patients chose MRM though they believed their surgeon recommended BCT. The choice of local therapy is predominantly a surgeon-driven process; logistical barriers unique to a rural population had little impact. Unfortunately, many surgeons continue to apply much more stringent criteria when recommending BCT than those in published guidelines.

[Kleine-Levin syndrome. Female cases and catamneses].

The Kleine-Levin syndrome (periodic hypersomnia and megaphagia in adolescence) was considered till 1962 to be a disorder limited to the male adolescents. Since 1924, however, a few female cases have been reported about. These, being approximately 20 patients, are registered by their essential symptoms together with 2 additional cases observed by the author. Moreover, prospective catamneses, covering periods of more than 12 years, were described for the first time. Probably, the Kleine-Levin-syndrome is the product of disturbed functions involving structures of hypothalamus and the reticular system. The syndrome seems to belong to the same groups of periodical diseases as migraine: in particular it exhibits similar relations to the genital cycle. Therapeutical success was attained by administration of amphetamines. In course of time, the episodes become spontaneously rarer and mostly cease.

Predictors of biological aggressiveness of prostate specific antigen screening detected prostate cancer.

PURPOSE: It is not known whether screening of asymptomatic men with prostate specific antigen (PSA) decreases the mortality of prostate cancer. We evaluated the extent to which PSA screening identifies clinically significant prostate cancer by analyzing markers of biological aggressiveness. MATERIALS AND METHODS: We compared prostate cancer in 56 patients in the screening and 21 in the randomized control arm of a population based cohort of 8,975 men 55 to 67 years old participating in the Finnish arm of the European Randomized Study of Screening for Prostate Cancer to 47 clinically detected organ confined, 30 clinically detected metastatic and 16 latent prostate tumors identified at autopsy in 46 consecutive subjects. Biological aggressiveness was determined by histological grading using the Gleason and Mostofi scales, tumor proliferation rate by Ki-67 immunostaining, p53 over expression by immunostaining and aneuploidy by fluorescence in situ hybridization using formalin fixed, paraffin embedded tumor specimens. RESULTS: A total of 56 neoplasms were detected in 2,781 men (2%) who participated in PSA screening and 21 were detected in 5,975 nonscreened controls (0.35%) during the study period. Disease in nonscreened controls more often involved a high tumor proliferation rate (p = 0.004) and p53 over expression (p = 0.015) than screening detected disease. At least 1 feature of biological aggressiveness was present in 19% of latent, 34% of screening detected, 51% of clinically detected and organ confined, 62% of randomized control and 87% of metastasis cases. Of the screening detected tumors defined as biologically aggressive 74% were identified at organ confined stages pT1-2N0M0. CONCLUSIONS: PSA screening detects a significant number of biologically aggressive cancers at an early clinical stage, implying that screening may decrease mortality.

Growth retardation in children with chronic renal insufficiency.

Progressive growth retardation is a complication of chronic renal insufficiency (CRI) in children and often results in patients attaining a final adult height SD score of -1.88 or worse. Although a host of factors may contribute to the subnormal height velocity associated with CRI, disorders of the growth hormone/insulin-like growth factor (IGF) axis predominate. An excess of one or more of the six IGF binding proteins in the 35-kD serum fractions results in depressed bioactivity of the IGF and impaired stimulation of linear growth. Treatment with recombinant human growth hormone likely increases the IGF levels in the 35-kD serum fractions and restores IGF bioactivity, which is associated with an increase in height velocity. Nevertheless, the use of recombinant human growth hormone remains limited in patients with CRI, and more than one-third of this population are severely growth-retarded. Strategies designed to prevent/correct the growth retardation that accompanies CRI should ideally occur during the pre-end-stage renal disease period.

Telomerase activity in human bladder tumors and bladder washing specimens.

Telomerase appears to be an important factor for the control of cellular proliferation and tumorigenesis. Enzyme activity dramatically increases in almost all human tumors. The purpose of our study was to evaluate the role of telomerase activity as a marker for bladder cancer diagnosis and follow-up. By using the PCR-ELISA based on the TRAP (telomerase repeat amplification protocol) method, telomerase activity of bladder tumors (n = 77), normal-appearing adjacent tissues (n = 21) and bladder washings (n = 37) were analyzed. Telomerase activity was detected in 87% (67/77) of cancer tissues and in 38% (8/21) of normal-appearing adjacent tissues. However, the levels of enzyme activity were significantly higher in cancer tissues than in normal-appearing adjacent tissues (p < 0.05). Telomerase activity in bladder cancer tissues was not correlated to the tumor stage or grade. During a 26 months follow-up period, disease progression occurred in 66.7% of patients with invasive tumors where telomerase activity of the normal-appearing adjacent tissue was detectable, as compared to only 14.3% for patients who showed undetectable telomerase activity in adjacent, normal-appearing tissues (p = 0.094). When telomerase activity of bladder washing fluid was compared with its corresponding tumors, sensitivity of detection was 81% and specificity was 75%. In contrast, urine cytology only yielded a sensitivity of 31% in the detection of cancer. The detection ability between telomerase activity measurement in washing fluid and cytological examination had a trend toward the telomerase measurement identifying more cancer cases than the cytologic examination (p = 0.07). In conclusion, telomerase activity is present in early-stage bladder cancer and is a potential molecular marker for bladder tumors diagnosis. The expression of telomerase activity in normal-appearing mucosa adjacent to bladder tumor is probably an indicator of disease progression. Using the telomerase activity to detect exfoliated cells in bladder washing fluids could be a useful method in adjunct to urine cytology and cystoscopy in establishing the diagnosis and follow-up of bladder cancer.

Cyclical shock with hyperglobulinemia.

Two personal cases are compared with CLARKSON's case, enabling one to describe a syndrome arising in adults without a family history, and characterised by episodes of repeated shock (cyclical shock) with hemoconcentration and hypoproteinemia. The physiopathological mechanism is an acute hyperpermeability. Two cases out of three took a fatal course, with a negative autopsy. In the three cases, the presence of light kappa chain IgG type monoclonal dysglobulinemia was proven. The diagnosis of multiple myeloma can be excluded. Comparison of this syndrome with periodic disease and primary amyloidosis can be envisaged.

[Evaluation of cerebrospinal fluid proteins in patients with subacute sclerosing panencephalitis (SSPE)].

CSF proteins of 62 patients with SSPE aged 6 to 26 years were analysed by polyacrylamide gel electrophoresis. The total IgG concentration in the CSF was increased to 16.8 +/- 10 mg/dl and the IgG index was raised to from 2.7 +/- 0.5 to 3.6 +/- 3.0. All SSPE patients had pathological subfractions gamma 4 and gamma 5. The per cent proportion of the gamma 1 subfraction was raised only in the youngest patients. A tendency was observed for higher values of the prealbumin:albumin index and the prealbumin: transferrin index. The youngest SSPE patients had a tendency for greater total protein concentration, lower prealbumin concentration, and highest IgG index values (3.6 +/- 3.0). In early disease period a tendency was noted for higher concentration of total protein, higher total IgG level and higher IgG index. These results suggest that SSPE patients have a constant stimulation of B-cells and variable increase of blood-brain barrier permeability.

[Anesthetic managements of the patients receiving catheterizable cecostomy].

To improve the quality of life of patients suffering from spinal cord injury, "Catheterizable cecostomy" operation have recently been carried out. In the procedure, cathetelizable stoma is opened and patients are able to control the excretion by injecting enema solution through the stoma. Although operations are usually performed under general anesthesia with tracheal intubation, special considerations sometimes need to be taken for airway management. In such cases, we select spinal anesthesia. Because the surgery is achieved on a fixed charge, efficiency of pre-operative assessment should be done for the cost-cutting with shortening of perioperative period. Diseases including hypertension, diabetes mellitus, etc., must be controlled before admission to the hospital. With these considerations, proper pre-operative assessment can be made.

[Serum level of various biologically active substances and their connection with vessel permeability in hemorrhagic fever with kidney syndrome].

Content of total free serotonin, histamine, kallikrein and prekallikrein was studied in blood of 45 patients with hemorrhagic fever accompanied by kidney syndrome; vessel permeability was also evaluated by means of radiological procedure. Analysis of correlation between vessel permeability and content of these biologically active substances in blood was carried out. Content of kallikrein was distinctly increased in blood plasma of the patients; content of serotonin and histamine was less markedly altered. Vessel permeability was elevated during all the disease periods; it was especially high in the period of oligoanuria. Analysis of the data obtained showed that an increase in vessel permeability correlated most distinctly with augmented content of kallikrein in the blood, while correlation between vessel permeability and content of histamine and free serotonin was less distinct.

[Indicators of immunologic reactivity and the alpha fetoprotein level in newborn infants with thymomegaly and bronchopneumonia].

Forty-two newborns with bronchopneumonia were under observation. Of these, 22 also presented with thymomegaly . The acute disease period was characterized by higher concentrations of alpha-fetoprotein and the decrease of the transforming activity of lymphocytes in response to phytohemagglutinin in newborn patients with thymomegaly as compared to the same parameters in newborn patients without thymomegaly . Study into correlative interrelations has revealed a significant reverse relation between the content of alpha-fetoprotein and IgG.
