[New approaches to the treatment of postoperative peritonitis].

A retrospective comparative analysis of results of surgical treatment of 245 patients with postoperative peritonitis was made. In 114 patients (control group) furacin was used for sanitation of the abdominal cavity, and in 131 patients (main group) sanitation of the abdominal cavity was performed with water-soluble antibacterial ointments. It was established that the proposed ointment sanitation of the abdominal cavity in combination with active surgical methods based on using programmed relaparotomy resulted in 24.1% lower lethality as compared with the traditional method of sanitation. The authors assert that under conditions of marked polyorganic insufficiency the method of programmed relaparotomy with the ointment sanitation of the abdominal cavity is not very effective while using this method before the development of systemic complications is the decisive factor of the favorable prognosis.

[Effect of light and heat on the stability of furacilin aqueous solution].

AIM: To study the effect of both light and heat on the stability of furacilin aqueous solution and the probability of substituting for isothermal accelerated tests by nonisothermal accelerated tests upon exposure to light at high temperatures. METHODS: The isothermal and nonisothermal accelerated tests were employed. The accelerated tests were proceeded in the dark and exposed to light at high temperature. Tungsten, ultraviolet and fluorescent lamps were employed in exposure tests. RESULTS: The degradation of furacilin aqueous solution in isothermal heating experiments or the exposure experiments to light at high temperatures obeys zero-order kinetics. The total degradation rate constant k caused by both light and heat can be divided into two parts: k = kdark + klight, where kdark and klight are the degradation rate constant caused by heat and light, respectively. The klight can be expressed as klight = Alight.exp(-Ea,light/RT).E, where E is the illuminance of light; Alight and Ea,light are both experimental constants. The parameters obtained in nonisothermal accelerated tests were comparable to those obtained in classic isothermal accelerated tests. CONCLUSION: Nonisothermal accelerated tests may substitute for isothermal accelerated tests during the study of the effects of both light and heat on the stability of drugs, in order to save time, labor and drugs.

[Topical pectins for the treatment of burn wounds].

The results of the burned wounds treatment with 1-2% solution of apple or beet pectins were analysed. The control results were examined at the same patients with symmetrical wounds treated by chlorhexidine, levomecol, furacillin liniment. Clinical evaluation demonstrated good tolerability of the treatment regime, absence of side effects and complications, inhibition of inflammation, acceleration of epithelisation of the burns II-IIIA class, allowed to diminish the preliminary period before autodermoplastic operation (burns of IIIB class), diminished microbial dissemination and dissemination with Pseudomonas aeruginosa. The best results were demonstrated when the pectins application was performed since the first day after the burned injury.

The ability of bacteria to synthesize a new cyclopyrophosphate correlates with their tolerance to redox-cycling drugs: on a crossroad of chemotherapy, environmental toxicology and immunobiochemical problems.

Many redox-cyclers were recently shown to induce, in some bacterial species, large-scale biosynthesis of a new 2-methylbutan-1,2,3,4-tetraol-2,4-cyclopyrophosphate believed to be involved in anti-stress reactions. In the present study Mycobacterium smegmatis, Micrococcus luteus and Brevibacterium ammoniagenes were shown to begin synthesis of the new cyclopyrophosphate when cultivated in a medium containing furacilin or furadonin (widely used nitrofuran antibacterial drugs) and to maintain close to normal growth rates, whereas Staphylococcus aureus, Bacillus subtilis and Escherichia coli were inhibited by the drugs and were unable to synthesize the cyclopyrophosphate compound. Preferential binding of Mg2+ and Cd2+ with one or other phosphoryl groups of the cyclopyrophosphate, which was indicated by selective changes of 31P-NMR chemical shifts and intramolecular hydrogen bonding, is suggested as a reason for this selectivity.

[Methods of local antimicrobial therapy and prevention of postpartum endometritis].

To increase the effect of the treatment of puerperal endometritis, long-term washing of the uterus cavity with cool solutions of furacillin was used in 57 puerpera. The most favourable results were observed in 27 patients who at the final stage of the washings were treated with antibiotic solutions administered to the uterus cavity. The recovery terms, as well as the terms of the hospitalization lowered. For confirming the diagnosis of endometritis and control of the therapy efficacy cytograms of the metroaspirates were used. High efficacy of the prophylactic administration of antibiotics to the myometrium tissues during the cesarean section with high risk of infectious complications was also shown. Both the methods were recommended for their wide use in obstetric hospitals.
