Forty-eight adult patients with juvenile rheumatoid arthritis (JRA) (onset before age 16 years) were evaluated at the age of 17 years or more for the presence of hidden 19S IgM rheumatoid factors (RF), i.e., 19S IgM RF that can be detected by the complement-dependent haemolytic assay in the IgM-containing fraction after separation of the serum by acid gel filtration. The average age of the patients was 25.3 years. The mean duration of disease was 16.5 years. Thirty-two of 48 patients (67%) showed the presence of hidden 19S IgM RF in their serum. Disease activity correlated with hidden RF titres in 62% (55/88) of the evaluations. The results indicate that patients with seronegative JRA onset continue to have significant titres of hidden 19S IgM RF in their sera into early adulthood.
In the diagnosis of Pneumocystis carinii infection in man, rather exacting methods of staining must be used for the organism to be visualized in lung tissue. Techniques for sample collection, which range from open lung biopsy to endobronchial brush methods or collection of sputum, are discussed. Recent advances in the serologic diagnosis and in the culture of the organism are also covered.

Most samples of cerebrospinal fluid from 49 patients with or without coccidioidomycosis yielded positive agglutination with the Hyland ("CM") latex-particle agglutination test. Furthermore, simple dilution of sera from patients with or without coccidioidomycosis yielded a strong agglutination reaction with the latex particle agglutination test reagent. These reactions were found with three lots of the latex-particle agglutination test reagent. One lot latex particles was found to give agglutination reactions merely by addition of saline solution or water, though a second lot did not. It is advisable, therefore, that the latex-particle agglutination test not be applied to cerebrospinal fluid, and that it be applied cautiously, particularly with diluted sera.
Eight Aspergillus antibody detection assays--three indirect haemagglutination assays (IHA-LD, IHA-Roche, IHA-Fumouze), three enzyme immunoassays (EIA-IgG, EIA-IgM, EIA-IgA, DDV) and two complement fixation tests (CF-metabolic and CF-somatic, Virion--and one latex agglutination test (LAT) for Aspergillus galactomannan antigen detection (Sanofi Pasteur) were evaluated in 14 patients with proven invasive aspergillosis (a total of 47 serum samples and one cerebrospinal fluid sample) and in 68 selected control individuals (one selected serum sample each). For the antibody tests, sensitivity ranged from 14% to 36% and specificity from 72% to 99%. The antigen detection test had a sensitivity of 36% and a specificity of 100%. Currently commercially available antibody detection assays for the serodiagnosis of invasive aspergillosis are inadequate. The antigen detection test appears to be highly specific, but lacks sufficient sensitivity.
