The complete health checkup: fad, fiction, or fact.

The ritual of the annual physical examination is scrutinized with respect to its clinical value. The periodic physical examination is not usually a health examination, because it does not include all components that affect a person's total health. The annual physical examination is usually not of importance to the physician unless disease is discovered. Seldom is the examination used as an opportunity to reinforce behaviors which have kept the patient healthy. It has been suggested that physician extenders be used to deliver primary care services, including comprehensive health examinations, with physicians used as backup. Laymen should not have to seek out specialists to give them examinations for each system of the body. Comprehensive health care should be available and accessible to all segments of the public with an emphasis on greater self-responsibility for maintaining one's health and enhancing one's wellness.

Functional restoration. Returning patients with chronic low back pain to work--revolution or fad?

STUDY DESIGN: A review was performed to assess the basis for functional restoration programs in returning patients with chronic low back pain to work. OBJECTIVE: To review and assess the effectiveness of functional restoration programs in the treatment of chronic low back pain. SUMMARY OF BACKGROUND DATA: Functional restoration programs generally use an aggressive program of physical exercises and psychosocial support with the focus on improving function despite the pain. METHODS: A review was conducted of significant papers presenting results of large clinical trials that used functional restoration programs. RESULTS: Several influential studies have reported statistically significant improvement in return-to-work rates of treated patients. The comparison groups consisted of treatment failures and/or patients refused entry to the programs because of insurers refusal to pay. Selection bias, incomplete follow-up, and inappropriate allocation of compared patients cast doubt on the effectiveness of these functional restoration programs. The only published prospective randomized clinical trial failed to demonstrate the efficacy of a functional restoration program. CONCLUSIONS: Further clinical trials based on sound methodology are needed.

Restless legs syndrome and periodic limb movements of sleep: fact, fad, and fiction.

Restless legs syndrome (RLS) and periodic limb movements of sleep (PLMS) are common disorders seen in sleep disorder centers. Although RLS in universally thought to cause daytime sleepiness, the role of PLMS in causing sleepiness (or any symptom!) has been more controversial. Recent publications have show that PLMS do not cause either subjective or objective sleepiness, but may be a marker for respiratory effort related arousals. In regards to patients with end-stage renal disease, PLMS may mark mortality. In this same group of patients, gabapentin and normalization of hematocrit may be effective therapies for RLS/PLMS. Melatonin may prove to be an alternative therapy for PLMS.

Activity-based cost management in health care--another fad?

By providing improved information for strategic planning purposes activity-based cost management (ABM) systems can help hospitals and other health care providers improve the quality and efficiency of the care they provide control costs and manage their resources better. The NHS is starting to evaluate the ABM approach. Describes a research project in one specialist hospital in Sheffield UK which found that conventional approaches to costing can be inaccurate by as much as 100 per cent; conventional approaches will consistently undercost complex, specialist, infrequent episodes of care and overcost straightforward frequent episodes; and the resulting information from ABM about costs and processes significantly improves the decision-making capabilities of managers particularly in relation to ¿what if¿ situations.
