The steady state is a physiological condition whereby the metabolic requirements are met and the criteria parameters are stable. Four groups of male subjects were evaluated on their VO2 for a one hour duration ergocycle effort at 75%, 50%, 25% and 10% of their maximum individual VO2. Three cardio-respiratory parameters (heart frequency, oxygen consumption, pulmonary ventilation) and one biochemical parameter (lactic acid) were measured The evolution of the control parameters during the task were completed according to the requirements of the literature. For the long-duration efforts (one hour) we notice the absence of a steady-state for the cardio-respiratory parameter. We cannot however rule-out the existence of a steady state for the biochemical parameter for the lighter efforts of 10% and 25% of the VO2 maximum.

An analysis has been made of the data of direct and indirect methods for evaluation of myocardial reserve and occult cardiac insufficiency in varying groups of patients with postinfarction cardiosclerosis and hypertensive disease. Absolute and relative values of the systolic AD, cardiac contraction rate and ECG data have been studied. A combined characteristics of the hyposystolic type reaction and the scheme for the examination of patients are presented.

The authors appraise a number of indices of left-ventricular apex cardiogram recorded at rest and after submaximum or threshold physical load in 52 healthy males and in 122 patients with ischemic heart disease. It was found that after physical load the electromechanic interval of the apex cardiogram of patients with ischemic heart disease is prolonged, the period of rapid filling is shortened and the period of slow filling of the left ventricle is prolonged with a general decrease in the whole diastole as compared to that which should be. An increase in the ratio of the atrial wave amplitude to the total systolic wave, which is clearly demonstrated after physical load, is one of the earliest signs of cardiodynamic disorders recorded by apex-cardiography. A parallelism was established between the degree of the pathological changes on the apex cardiogram, physical load tolerance and the pronounced character of the clinical manifestation of ischemic heart disease.
The sensitivity of various criteria of the physical exercise test in revealing myocardial ischemia was studied in 2 groups of patients with ischemic heart disease: 1st group of 64 patients with normal ECG at rest, 2nd group of 96 patients with cicatricial changes in the myocardium. Selective coronography demonstrated atherosclerotic narrowing (stenosis of more than 70%) of one or more coronary arteries of the heart in all patients. During physical exercise an attack of angina pectoris without "ischemic" changes on the ECG occurred in 36.1% of patients of the 1st group and in 22.4% of patients of the 2nd group. The frequency of other clinical signs of cessation of the exercise (dyspnoea, change of arterial pressure, extrasystole and others) was three and a half times higher in patients with cicatricial changes in the myocardium. It is concluded that the frequency with which signs of myocardial ischemia are revealed during physical exercise depends not only on the pronounced character of the pathological coronary condition and the development of collateral circulation but also by the sensitivity of the separate ECG leads and the presence and localization of cicatricial changes in the myocardium.
