Erythema, skin cancer risk, and sunscreens.

Ultraviolet-induced erythema is now recognized as an important clinical marker for skin cancer risk. People with a history of repeated erythematous exposures to sunlight are more likely to develop both melanoma and nonmelanoma skin cancers. Is erythema the most important thing we need to be concerned about in terms of protecting the public from the harmful effects of sunlight? What do we know about UV-induced erythema, and why is it a powerful influence on skin cancer risk?

[A patient with acute sarcoidosis associated with fever, polyarthritis, and erythema nodosum: a typical of Löfgren's syndrome].

A 26-year-old female was admitted to Aoyama Hospital in February 1996, for evaluation of abnormal chest shadows and polyarthritis. She visited our clinic in December 1995 with complaints of fever, fatigue, and polyarthralgia which lasted for 3 weeks. Two weeks later she developed erythema nodosum on her lower legs with the exacerbation of polyarthritis. Erythrocyte sedimentation rate was 108 mm/hr and CRP 4.9 mg/dl. A chest radiograph showed hilar lymphadenopathy and multiple nodular shadows in both lung fields. On admission, she had arthritis of the shoulders and knees, erythema nodosum on her lower extremities, and keloid-like skin lesion on her left knee. Thoracoscopic lung biopsy of nodular lesion and skin biopsy of keloid-like lesion revealed noncaseating granuloma of epithelioid cells with lymphocytes, macrophages, and giant cells, confirming the diagnosis of sarcoidosis. She was cured in three months, and was well and free of the symptoms thereafter. Löfgren's syndrome is acute sarcoidosis, characterized by arthritis, erythema nodosum, and bilateral hilar lymphadenopathy. This syndrome is common in Europe and is closely related to HLA-B8 and DR-3. The frequency of HLA-B8 and DR-3 in Japanese is almost 0%, explaining the rare onset of this syndrome in Japan. Our case is the second report of typical Löfgren's syndrome in Japan, although the patient did not have these HLA loci.

Analysis of T cell receptor Vbeta repertoires of annular erythema associated with Sjögren's syndrome.

Sjögren's syndrome (SjS) is an autoimmune disorder characterized by lymphocytic infiltration into the lacrimal and salivary glands. Annular erythema has recently been reported to be a specific, cutaneous manifestation associated with SjS. In this study, the T cell receptor (TCR) Vbeta gene usage and expansion was examined in annular erythema associated with SjS (AESjS) in 7 patients with primary SjS (5 definite and 2 probable), using reverse transcriptase polymerase chain reaction (RT-PCR) amplification of 22 Vbeta gene families. For 4 out of the 7 patients, the TCR V repertoire in lesional skin of AESjS was compared with paired peripheral blood mononuclear cells (PBL). In one case, two lesional tissue specimens biopsied from different sites of AESjS (face and trunk) were examined. As a control, the TCR Vbeta repertoire was examined from the lesional skin of butterfly rash biopsied from 3 cases of systemic lupus erythematosus (SLE). Results showed that TCR Vbeta 2 was detected in 6 out of the 7 cases of AESjS, although diverse usage was observed. TCR Vbeta 2 and 17 (but particularly Vbeta 2) were predominantly expressed in AESjS in comparison with paired PBL. In the case which presented AESjS at two separate sites, Vbeta 2, 6, 18 and 19 were preferentially expressed in both skin sites as compared with PBL. On the other hand, TCR Vbeta 6, 13-2 and 14 were commonly demonstrated in the cutaneous lesions of SLE. These results suggest that (1) the TCR Vbeta usage by infiltrating T cells in AESjS is not strictly limited, however, Vbeta 2 may play an important role in the induction of AESjS, and that (2) different subsets of TCR Vbeta genes are used in the lesional skin of SjS and SLE, which might account for the clinical and histological differences seen in the erythema found in these two autoimmune disorders.

[Erythema multiforme caused by saquinavir].

BACKGROUND: Saquinavir is a protease inhibitor used for the treatment of HIV infection. Adverse skin reactions have been rare. We report here the first case of erythema multiforme in a patient given saquinavir. CASE REPORT: A 32-year-old man was seropositive for HIV and consulted due to the development of round maculo-papular lesions centered on a bulla and two erosive lesions of the palate five days after the introduction of saquinavir. Histology was compatible with erythema multiforme. After withdrawal of saquinavir, the skin and mucosal lesions regressed in 15 days, with no recurrence at 3 months. DISCUSSION: Adverse skin reactions to saquinavir are exceptional (eruptions, pruritus). We describe here the first case of erythema multiforme caused by saquinavir (imputability criteria 12 BO). Due to the structural analogy of saquinavir with other protease inhibitors (indiravir, ritonavir, nelfinavir) it would be difficult to prescribe a compound of the same class.

[Bullous erythema nodosum leprosum. A case report in French Guiana].

BACKGROUND: Polar and borderline lepromatosis leprosy can be complicated by type 2 reactional states, including erythema nodosum leprosum. CASE REPORT: A 36-year-old man was treated for lepromatous leprosum. He consulted for recurrent erythema nodosum leprosum. Certain nodular lesions were bullous. Histopathology demonstrated major dermal edema causing bullous blisters. Treatment with thalidomide led to rapid regression. DISCUSSION: The bullous form of erythema nodosum leprosum is rarely described in the literature. It raises the diffential diagnosis with other bullous dermatoses, particularly Sweet's syndrome.

Apoptosis with positive direct immunofluorescence findings in a patient with necrolytic migratory erythema.

A 45-year-old man with a glucagonoma and necrolytic migratory erythema is described. Clinical, radiographic, and histologic features were typical for this syndrome. However, a skin biopsy specimen for direct immunofluorescence examination revealed apoptotic keratinocytes that stained positive with immunoglobulins, fibrinogen, and C3. These immunofluorescence findings were initially interpreted erroneously as showing erythema multiforme or a related disorder. We present a unique case of necrolytic migratory erythema with positive direct immunofluorescence findings reflecting immunoglobulin and complement deposition within dyskeratotic epithelial cells.

[Erythema nodosum in sarcoidosis. 37 cases].

Erythema nodosum was seen in 37 out of 564 cases of sarcoidosis, i.e. 6.4. per cent. The female preponderance of the association was 56.8 per cent. It was accompanied by fever in 84 per cent of cases and with an increased sedimentation rate, greater than 50 mm in the first hour, in 44 per cent of cases. Articular manifestations (arthralgia or arthritis) were seen in 73 per cent of cases, with a predilection for the ankles. Progression by acute excerbations of the erythema nodosum and/or articular manifestations was rare. Mediastino-pulmonary involvement accompanying the erythema nodosum included mediastinal lymphadenopathy in all cases. Infiltration of the pulmonary parenchyma was seen in 27 per cent. Spontaneous resolution of the mediastino-pulmonary lesions occurred in 82 per cent of cases, regardless of the initial radiological appearance.

Observations on the cytological diagnosis of an infectious erythema.

The clinical features of an infectious erythema which affected children and young people are described. Other writers have described the same disease and have suggested a viral origin. A cytological method is described which may establish the diagnosis, and the cytological appearances might also be useful in differentiating between erythema and measles.

The effect of topically applied agents on ultraviolet erythema in guinea pigs.

It has been shown previously that ultraviolet erythema (UV) is partially the result of a local release of prostaglandins. We have studied the effects of different classes of pharmacological agents applied topically to the skin of guinea pigs and have found indomethacin and pirprofen, both prostaglandin synthetase inhibitors, to be highly effective suppressors of UV erythema. Our studies appear to further substantiate that prostaglandin synthesis and release may be the primary mechanistic process in the production of erythema and that the model itself can be predictive of both therapeutic and prophylactic effects of agents against sunburn.

[Pacemaker erythema with telangiectasis ].

Pacemaker erythema is a rare, irreversible side effect of cardiac pacemakers, clinically characterized by epifocal teleangiectatic erythema. Histology reveals teleangiectatic vessels in the superficial dermis and a slight perivascular infiltrate. We report on three patients presenting with pacemaker erythema following either pacemaker or defibrillator implantation. The pathogenesis of this histopathologically uncharacteristic lesion is unknown. In contrast to peri- or postoperative pacemaker infections, physical irritation can be considered a possible triggering mechanism. Allergic reactions to the implanted material have been ruled out by negative patch test results.

Ixodes ricinus spirochete and European erythema chronicum migrans disease.

From three endemic locations of erythema chronicum migrans disease in North Rhine-Westphalia, Germany, we recovered 19 isolates of a spirochete from Ixodes ricinus ticks. The infection rate in adult ticks was 16 percent. The isolated spirochete is immunologically related to the Ixodes dammini spirochete, Borrelia duttoni, and Treponema pallidum. Using indirect immunofluorescence, the sera of 90 patients with erythema chronicum migrans disease showed antibody titers against the isolated spirochete, which correlated with the clinical course. Similarly, antibodies were demonstrated in the sera of 21 patients with acrodermatitis chronica atrophicans. These results suggest an etiologic role for the Ixodes ricinus spriochete in European erythema chronicum migrans disease.

Early Lyme disease: a flu-like illness without erythema migrans.

The existence of a form of early Lyme disease characterized by a flu-like illness without erythema migrans is controversial. To confirm the existence and define the clinical characteristics of the flu-like illness without erythema migrans of localized Lyme disease, the authors studied patients from a Lyme disease endemic area of Connecticut who visited their primary care physicians with an undefined flu-like illness. Patients kept a diary of their symptoms. Acute and convalescent sera were obtained. The diagnosis of Lyme disease was based on the appearance of IgM or IgG antibodies to Borrelia burgdorferi as demonstrated by both enzyme-linked immunosorbent assay and immunoblot assay. Twenty-four untreated patients were studied. In five patients acute serologic evidence of Lyme disease developed. The flu-like illness in these five patients was characterized by fever and fatigue and resolved spontaneously in 5 to 21 days. Symptoms recurred in three of these five patients. The existence of a flu-like illness without erythema migrans of early Lyme disease has been clearly established. Prospective, controlled studies are needed to better define its incidence, characteristics, and prognosis so that appropriate diagnostic and therapeutic strategies can be developed.

HISTOPLASMOSIS IN MONTREAL DURING THE FALL OF 1963, WITH OBSERVATIONS ON ERYTHEMA MULTIFORME.

Although Montreal is in an endemic area, significant clinical histoplasmosis with systemic manifestations has been, until recently, infrequently diagnosed. However, since the autumn of 1963, 31 cases of clinically significant histoplasmosis have been seen by the authors. These were divided into two groups: (1) patients in whom the diagnosis was established on the basis of histological and/or cultural demonstration of the fungus; (2) patients in whom the diagnosis was based on a positive histoplasmin skin test, a complement fixation antibody titre of 1:32 or greater and compatible clinical and radiological findings. An additional group of 11 patients who presented with erythema multiforme was investigated and a heretofore unrecognized relationship between histoplasmosis and erythema multiforme was established.

Erythema multiforme and urticaria. Eruptions induced by chemically related ophthalmic anticholinergic agents.

Erythema multiforme developed in an 80-year-old man following the use of scopolamine hydrobromide ophthalmic drops. The erythema multiforme cleared when the medication was discontinued and recurred on challenge. Later, he was given tropicamide, an anticholinergic ophthalmic preparation that, like scopolamine, has a tropic acid residue. Within 15 minutes an immediate hypersensitivity reaction with generalized urticaria developed in the patient.

Erythema induratum of Bazin. Evidence of T-lymphocyte hyperresponsiveness to purified protein derivative of tuberculin: report of two cases and treatment.

BACKGROUND AND DESIGN: Erythema induratum of Bazin, a chronic form of nodular vasculitis, may be associated with chronic infections by Mycobacterium tuberculosis. However, the true origin of the disease is a subject of speculation and remains elusive. Two female patients (58 years old and 33 years old) with a minimum 10-year history of chronic tender ulcerating nodules on the lower aspects of the legs were studied both clinically and in the response of their peripheral T cells to purified protein derivative of tuberculin. RESULTS: Both patients with no previous history of tuberculosis had strongly positive skin test results at a Mantoux 10(-4) dilution (1 unit of purified protein derivative). In response to full-course triple-agent (isoniazid, rifampicin, and ethambutol) chemotherapy, a complete remission of clinical symptoms was seen in both cases and no relapse occurred after discontinuation of therapy. A marked increase in peripheral T-lymphocyte response to purified protein derivative was found before onset of and during successful therapy. CONCLUSIONS: The present clinical observations together with the pronounced cellular response to purified protein derivative suggest a tuberculous origin of erythema induratum of Bazin.

CC and CXC chemokines are differentially expressed in erythema multiforme in vivo.

BACKGROUND: A characteristic feature of erythema multiforme is an acute inflammatory reaction of the skin with an infiltrate largely composed of mononuclear cells around the upper dermal vessels and in the dermal-epidermal interface. OBJECTIVE: To determine the composition and localization of leukocyte subsets and corresponding expression of chemokines with chemoattractant properties for lymphocytes and macrophages. MATERIALS AND METHODS: Immunohistochemical analysis was performed to localize leukocyte subsets (CD1(+), CD3(+), CD4(+), CD8(+), and CD68(+)). Expression of transcripts and proteins of chemokines (macrophage chemoattractant protein [MCP] 1); macrophage inflammatory protein [MIP] 1 alpha and MIP-1 beta; regulated on activation, normal T-cell expressed and secreted [RANTES]; growth-related oncogene alpha; epithelial-derived neutrophil attractant 78; interleukin 8; macrophage interferon-gamma inducible gene [Mig]; and interferon-gamma inducible protein 10) was determined by in situ hybridization and immunohistochemical analysis. SETTING: Department of Dermatology, University of Würzburg Medical School. RESULTS: High levels of messenger RNA expression of MCP-1, RANTES, Mig, and interferon-gamma inducible protein 10 were detected and localized in the interface zone and subepidermal infiltrate. In contrast, other investigated chemokines (growth-related oncogene alpha, interleukin 8, epithelial-derived neutrophil attractant 78, I-309, MIP-1 alpha, and MIP-1 beta) were minimally expressed or absent. Protein expression of MCP-1, RANTES, Mig, and interferon-gamma inducible protein 10 was high in the interface zone and low in the subepidermal infiltrate. The messenger RNA expression and protein immunoreactivity patterns overlapped. According to the expression profiles, Mig, interferon-gamma inducible protein 10, MCP-1, and RANTES were expressed by basal keratinocytes above and mononuclear cells within the inflammatory foci. CONCLUSION: These cytokines are important agents in the cytokine network and contribute to the cell-specific and spatially restricted recruitment of mononuclear cells in the acute inflammation of erythema multiforme lesions.

Erythema annulare centrifugum associated with ascariasis.

Erythema annulare centrifugum (EAC) is a figurate erythema that has been associated with man diverse entities. A case of EAC was related to infestation by the nematode, Ascaris lumbricoides. A careful examination of the patient did not define an immunologic basis for EAC. Results of lymphocyte stimulation tests of the patient showed an immune response to A lumbricoides extracts but not to A suum extracts. This finding is of practical importance, since most laboratoires use A suum extracts in routine skin and serologic tests for human ascariasis.

Rapid resolution of necrolytic migratory erythema after glucagonoma resection.

A 55-year-old man presented with an 11-year history of necrolytic migratory erythema and glossitis. After the patient's serum glucagon was demonstrated to be elevated, computed tomography scan revealed a mass involving the head of the pancreas. The patient underwent a Whipple-type pancreatico-duodenectomy and his rash resolved completely 6 days after tumor resection. He received no adjuvant treatment. A discussion of the varying theories regarding the pathogenesis and treatment of glucagon-associated necrolytic migratory erythema is presented.
