Spinally projecting neurons in the dorsal column nuclei: distribution, dendritic trees and axonal projections. A retrograde HRP study in the cat.

The distribution, dendritic trees and axonal courses of spinally projecting cells in the dorsal column nuclei were studied after labelling by retrograde HRP transport. The region of densest distribution was at the base of the two nuclei and in the area between them, extending for about 2 mm caudally from the obex. Only very few cells were found inside the cell cluster regions of the nuclei, where their dendrites had a free stellate form. The great majority, lying between, deep, or rostral to the cluster regions, also had a stellate form, except where they impinged on the boundaries of the cluster regions or on other nuclear borders; the spread of dendrites was dramatically restricted at such boundaries, often leading to a fusiform appearance in transverse sections which however was not evident in the parasagittal plane. No justification was therefore found for subdividing the population on morphological grounds. Axons of these cells descended ipsilaterally in either the medial part of the dorsolateral fascicle or in the adjacent lateral part of the cuneate fascicle, at cervical levels, and probably in about equal numbers. Most axons destined for the DLF followed a deep caudolateral trajectory, while many destined for the DC had a more dorsal or lateral course. Collateral branches were seen within the nuclei but could not be followed far. The fact that few if any cells lying in the region of maximum distribution of the spinally projecting cells were labelled following injections of HRP into the thalamic ventroposterior nucleus emphasizes that they form a distinctive entity within this medullary nuclear complex, and that any axon branches they give into the contralateral brainstem must have some other destination than the VPL. Two other groups of neurons were labelled by HRP implants into the dorsal columns - one in the ventrolateral medullary reticular formation, and the other in the nucleus of the solitary tract.

Immigrant suicide in Australia, Canada, England and Wales, and the United States.

"Factors influencing the suicide rates of numerous immigrants in groups in Australia, Canada, England and Wales, and the United States during the period 1959-73 were examined.... For males, the foreign-born in England and Wales had the lowest suicide rates and the foreign-born in the United States the highest. For females the variation was smaller, with immigrants in the United States having the lowest rates, and those in Australia the highest.... In each destination significant correlations existed between the suicide rates of the immigrants and those of the origin populations, indicating that the suicide rates for individual immigrant groups were to some extent predisposed by their experiences in the origin countries. Factors in the destination country also influenced immigrant suicide rates, as the rates of the majority of the immigrant groups converged towards the rates of the destination native-born.... The analyses also suggested that migration is more deleterious for females than males."

[Preclinical care of trauma patients in air rescue. Results of the medical quality management for patients with severe head injury and polytrauma in the years 2000 and 2001].

In a retrospective study of mission data of ADAC Air Rescue of the years 2000 and 2001 the quality of preclinical care of 1,946 patients with severe head injuries and 1,878 polytraumatized patients was examined. The actual preclinical care of these patients was compared with a catalogue of eleven thesis-like recommendations. These recommendations were previously derived from corresponding publications of national and international specialist companies and were introduced in a binding manner by the senior doctors of the participating air rescue centres. The results of the study show that 73.3% of the severe head injuries were preclinically intubated and 94.4% were supplied with oxygen, 82.2% were analgosedated. 94.8% could be delivered to the hospital of destination. 65.9% had a systolic blood pressure of >120 mmHg upon admission to the hospital. 71.4% of severe head injury victims were equipped with a cervical support, 23.3% had the blood pressure documented. 47.3% reached the hospital of destination in less than 60 minutes. Among polytraumatic patients the intubation was performed in 75.7%, the supply with oxygen in 90.7%, 88.6% of the patients were analgosedated. 78.7% of patients suffering from concomitant head injuries were provided with a cervical support and only 22.8% had a blood sugar measurement documented. A concomitant severe thorax trauma was treated by a thorax drainage in 59.2%. 35.6% of the polytraumata reached the hospital of destination in less than 60 minutes. The work describes the preclinical patient care of severe head injuries and polytraumata, pointing out deficits and presenting optimization possibilities, particularly in the area of training. Furthermore, the work shows the concept of the medical quality management in an air rescue enterprise. The total evaluation of all air rescue centres participating in data collection forms the basis of an external quality comparison. The data evaluation of a single station makes regional strengths and weaknesses visible, deficits can be proven and proposals for optimization be developed. The presentation of the time history of data yields continuous standard information on the state of the patient care at the relevant air rescue location and enables the analysis of improvement concepts based on the updated data.

The riddle of the plant vacuolar sorting receptors.

Proteins synthesized on membrane-bound ribosomes are sorted at the Golgi apparatus level for delivery to various cellular destinations: the plasma membrane or the extracellular space, and the lytic vacuole or lysosome. Sorting involves the assembly of vesicles, which preferentially package soluble proteins with a common destination. The selection of proteins for a particular vesicle type involves the recognition of proteins by specific receptors, such as the vacuolar sorting receptors for vacuolar targeting. Most eukaryotic organisms have one or two receptors to target proteins to the lytic vacuole. Surprisingly, plants have several members of the same family, seven in Arabidopsis thaliana. Why do plants have so many proteins to sort soluble proteins to their respective destinations? The presence of at least two types of vacuoles, lytic and storage, seems to be a partial answer. In this review we analyze the last experimental evidence supporting the presence of different subfamilies of plant vacuolar sorting receptors.

Gender, bladder distension and hypogastric nerve activity in the cat.

In the pentobarbitone anaesthetised cat, postganglionic activity was recorded from the whole hypogastric nerve and from hypogastric filaments destined for the bladder wall during bladder distension. A gender difference was observed in whole hypogastric nerve recordings, cardiac-related activity obscuring other components in females with intact internal reproductive organs. In contrast, nerve activity in the vesical hypogastric filaments was dominated by bladder distension-related activity in both sexes. These observations suggest that bladder distension-related activity, which has a continence promoting action on the bladder detrusor muscle, is destined for the bladder wall, while the cardiac-related activity is largely destined for the uterine and vaginal vasculature.

Somatotopic analysis of fibre and terminal distribution in the primate corticospinal pathway.

This is an anatomical study of the precision of fibre and terminal orderliness in the direct corticospinal projection. It was conducted to assess the degree of somatotopy in this projection in primates and to deduce the mechanism(s) possibly responsible for guiding fibres to their segmental destinations in development. As fibres leave the cortex they are grouped in an orderly way so as to be placed (within the pathway cross-section) according to their points of origin. Systematic neighbourhood relations are rather abruptly broken down as the descending pathway traverses the pons. Fibres are randomly distributed within each medullary pyramid and this disorderliness is maintained throughout the spinal pathway in the dorsolateral columns. Nonetheless, fibres exciting thumb movements end selectively at cervical levels. Fibres exciting foot movements end preferentially at lumbar levels but also show a secondary maximum of terminations in cervical segments, with relatively few terminations at intervening thoracic levels. These two sets of fibres ('thumb', 'foot') are derived from distinct, separate subareas of motor cortex and pass through exclusive, separate zones of the internal capsule. Thus, developmental contact guidance between somatotopically originating neighbours is inadequate to account for the observed specificity of corticospinal fibre destinations. Fibres must be observing distinctive local segmental cues (perhaps of cytochemical nature) and/or utilising somatotopically distinctive arrival sequencing (i.e. securing specific destinations without neuronal recognition).

Timing of major ontogenetic events in the visual cortex of the rhesus monkey.

The place and time of origin, the migration and eventual disposition of neurons of the monkey visual cortex were studied by autoradiography in animals killed at various intervals after 3H-thymidine pulse labeling at embryonic (E) and early postnatal (P) stages. All neurons destined for the visual cortex are generated during about a 2-month period between E45 and E102. Neuron position in the cortical laminae correlates systematically with time of cell origin; neurons destined for deeper cortical positions are generated earlier, and more superficial ones progressively later. Thus, most neurons in layer VI are born between E45 and E60, in layer V between E60 and E70, in layer IV between E70 and E80, and in layers III and II between E80 and E102. No neurons, but numerous glia, are generated within the cortical plate itself. Initially young neurons are produced almost exclusively in the ventricular zone. Later they are probably generated in both ventricular and subventricular zones, and by the end of the proliferative period the subventricular zone becomes the predominant source of new cells. At the time when all neurons destined for the monkey visual cortex have already been produced, primary fissures are barely indicated on the cerebral surface and no secondary fissure appears as yet. Autoradiographic analyses indicate that at early stages young neurons move to the cortical plate relatively synchronously and at a fast rate, whereas at later stages there are considerable differences in the rates of cell migration. At early stages when the migration pathway is relatively short, the external process of the ventricular cell may stretch across almost the entire migratory distance. It is possible that nuclei move without interruption within their own cylinders of cytoplasm, a mechanism which might account for the rapid, synchronous movement of cell bodies as seen in the autoradiographic material. During later stages when young neurons, mostly of subventricular origin, move across a distance which is more than 10 times the length of their leading process, migrating cells follow radial glial guides across the widened intermediate zone and through densely packed cortical plate. This type of cell displacement seems to proceed less synchronously and require more time. It is proposed that two different mechanisms of cell displacement might exist: one in operation at early stages of cortical formation when mostly ventricular cells migrate, and another utilized by subventricular cells at later stages.

Features of the neighborhood environment and walking by U.S. adults.

BACKGROUND: Manipulating the physical environment may be an efficacious way to promote physical activity. This study examined the relationships between features of the neighborhood environment and walking in the neighborhood by U.S. adults. METHODS: A random sample of women (n =266) and men (n =208) aged >18 years participated. Door-to-door interviews were conducted in 2003 to collect information about demographics, walking behavior, and features of the neighborhood environment. RESULTS: The analyses conducted in 2004 revealed that women were more likely to walk for exercise (odds ratio [OR]=4.6, 95% confidence interval [CI]=1.01-20.72) or walk a dog (OR=3.3, 95% CI=1.01-11.08) in the neighborhood if neighborhood safety was average as opposed to below average (p <0.05). Women with an average number of neighborhood destinations were more likely to walk for transportation in the neighborhood (OR=5.7, 95% CI=1.63-19.73) than women with a below average number of neighborhood destinations (p <0.01). In men, none of the neighborhood features were significantly associated with walking for exercise or walking a dog. Men were less likely to walk for transportation in the neighborhood if the functional (OR=0.22, 95% CI=0.06-0.89) or aesthetic (OR=0.17, 95% CI=0.03-0.89) features of the neighborhood were average versus below average. CONCLUSIONS: Prospective studies are needed to determine if changes in neighborhood safety and awareness of neighborhood destinations promote increases in walking by women. Evaluations of the relationships between other environmental features and walking behavior in men are warranted.

mRNA localisation gets more complex.

Recent advances in techniques for visualising mRNA movement in living cells have led to rapid progress in understanding the mechanism of mRNA localisation in the cytoplasm. There is an emerging consensus that in many cases the mRNA signals that determine intracellular destination are more complex and difficult to define than was first anticipated. Furthermore, the transacting factors that interpret the mRNA signals are numerous and their combinations change during the life of an mRNA, perhaps allowing the selection of many sub-destinations in the cell. Lastly, an emerging theme over the past few years is that many proteins that determine the destinations of mRNAs are recruited on nascent transcripts in the nucleus. They often function in many different processes in the biogenesis of mRNA and probably act in concert to provide specificity.

HeartMate VE LVAS design enhancements and its impact on device reliability.

OBJECTIVE: The HeartMate VE left ventricular assist system (LVAS) has supported more than 2300 patients and has been shown to be effective for bridge to cardiac transplantation and has demonstrated improved outcomes in survival as a destination therapy. Improvements in device durability are needed as bridge to transplant times increase and as we move into the era of LVAD as destination therapy. The purpose of this study is to determine if design enhancements to the HeartMate LVAS have improved device reliability and durability. METHODS: A retrospective analysis of serious mechanical failures was performed in 1865 devices (1458 VE, 407 XVE). The analysis of data included devices used to support patients from September 1998 for bridge to transplantation and destination therapy. Serious mechanical failures were defined as inflow valve dysfunction, percutaneous lead breaks, diaphragm fractures or punctures, bearing failures, outflow graft erosion and pump disconnects. RESULTS: Median device duration for the VE was 97 days (max 1206 days), and 85 days (max 517 days) for the XVE. A total of 134 serious mechanical failures occurred and included inflow valve dysfunction (5.3% VE, 2.4% XVE) (P = 0.853) percutaneous lead breaks (1.9% VE, 0% XVE) (P < 0.001) diaphragm fractures (0.1% VE, 0% XVE) (P = 0.134) outflow graft erosion (0.2% VE, 0% XVE) (P = 0.1096), pump disconnects (0.1% VE, 0% XVE) (P = 0.1336) and bearing failures (0.6% VE, 0.2% XVE) (P = 0.5538). Of the XVEs 97% were free of serious mechanical failures at 6 months and 82% at 1 year compared to 92 and 73% for the VE, respectively. The 6-month difference between the devices was statistically significant (P = 0.0063) and there was no statistically significant difference at 1 year (P = 0.1492). CONCLUSIONS: Preliminary experience with the HeartMate XVE LVAS demonstrated a significant reduction in percutaneous lead breaks. Early trends indicate positive impact of recent design modifications on XVE performance. These design modifications may improve device durability and reliability, which is crucial as we enter the era of LVADs as an alternative to medical therapy.

Pulse-step models of control strategies for dynamic ocular accommodation and disaccommodation.

Dynamic properties and control strategies of step responses by accommodation and disaccommodation differ from one another. Peak velocity of accommodation increases with response magnitude, while peak velocity and peak acceleration of disaccommodation increase with starting position. These dynamic properties can be modeled as control strategies that use independent acceleration-pulse and velocity-step components that are integrated respectively into phasic-velocity signals that control movement and tonic-position signals that control magnitude. Accommodation is initiated toward its final destination by an acceleration-pulse whose width increases with response magnitude to increase peak velocity. Disaccommodation is initiated toward a default destination (the far point) by an acceleration-pulse whose height increases with dioptric distance of the starting position to increase peak velocity and peak acceleration. Both responses are completed and maintained by tonic-position signals whose amplitudes are proportional to the final destination. Mismatched amplitudes of phasic-velocity and tonic-position signals in disaccommodation produce unstable step responses.

Continuous flow pumps and total artificial hearts: management issues.

Feasibility studies are underway for new axial flow ventricular assist systems and with a total artificial heart (TAH). The axial flow pumps provide continuous flow from the left ventricle (LV) to the aorta; the TAH provides pulsatile flow to the pulmonary and systemic circulation. Understanding the differences between these systems is necessary for appropriate patient selection and management. We compared the Jarvik 2000 axial-flow pump and the AbioCor TAH. The Jarvik 2000 pump is placed in the LV with its outflow graft anastomosed to the aorta. This system is used for bridge-to-transplantation and destination therapy. The AbioCor TAH provides complete circulatory support. The AbioCor is used for destination therapy in patients expected to die in less than 30 days. Worldwide, 45 patients have received the Jarvik 2000 as a bridge to transplantation (n = 34) or destination therapy (n = 11) for an average duration of support of 132.8 days (5 to 853 days). In 30 bridge-to-transplantation cases, 14 patients (47%) have undergone heart transplantation, 5 (17%) continue to be supported with the Jarvik 2000 device, and 11 (37%) have died. Five of 7 patients supported by the AbioCor TAH survived beyond the perioperative period; 4 were ambulatory, 2 were discharged from the hospital, and 1 is at home 13 months after implantation. Anticoagulation therapy and infection management are necessary for both systems. Therapy with inotropic agents, vasoactive drugs, a pacemaker, and electrolyte normalization is necessary for Jarvik patients. AbioCor-supported patients do not require medications to support heart function. Vasoactive agents may be useful for controlling blood pressure.

Urinary incontinence and stroke outcomes.

OBJECTIVE: To examine the relation of urinary incontinence to functional status and discharge destination in stroke rehabilitation patients. DESIGN: Historical cohort. SETTING: Freestanding, not-for-profit rehabilitation hospital. PARTICIPANTS: Nonrandom sampling. The charts of all admissions to the stroke rehabilitation unit were screened to identify patients with a medical diagnosis of completed stroke occurring 2 to 4 weeks before admission to rehabilitation, documented evidence of urinary incontinence at least daily the first 2 days after admission, and Functional Independence Measure (FIM) bladder sphincter score of 5 or less. Ninety patients met the inclusion criteria. MAIN OUTCOME MEASURES: FIM, change in FIM scores from admission to discharge, weekly FIM gains, and discharge destination. RESULTS: Continence status at discharge was not associated with discharge destination, but did predict total FIM score at discharge and the change in FIM scores from admission to discharge. Subjects who regained continence had higher weekly FIM gains on all subscales except Communication. A critical area of difference between the groups was in Social Cognition scores. The time interval from stroke greatly influenced the findings; in patients evaluated 14 to 18 days after stroke differences between continent and incontinent subjects were found only in Social Cognition scores, whereas in patients evaluated 27 to 30 days after stroke differences were found in Self-Care and Sphincter Control scores. CONCLUSION: Further investigation into cognitive characteristics should be undertaken with a more comprehensive tool appropriate for an impaired population.

Development of a new mobility scale for people living in the community after stroke: content validity.

Mobility inside and outside the home, and in the community, is important to independence of lifestyle after stroke. Existing measures lack content validity relating to these three environments. The aim of this study was to contribute to the content validity phase of developing a new scale for assessing mobility of people with stroke in these settings. An open-ended questionnaire was used to obtain responses from 15 physiotherapists with substantial experience in neurological physiotherapy. Responses were coded to provide a comprehensive list of tasks and destinations. A wide range of destinations was identified for i) inside the home: bedroom, bathroom/toilet, living room, kitchen; ii) outside the home: access to and from property, outside buildings, clothes line, garden and letterbox; iii) in the community: access to health and shopping facilities, leisure sites and public transport. Although a diverse range of tasks was identified, a common theme emerged to emphasise the impact of the environment on mobility. Responses were recorded in all dimensions of a recently published conceptual model of mobility that focused on environment. These dimensions included distance, time constraints, ambient conditions, terrain characteristics, external physical load, attentional demands, postural transitions and traffic level. Further work is required to quantify environmental dimensions which are relevant to the destinations identified in the three environmental settings of this study.

Use of diffusion weighted MRI to predict the occurrence and severity of hemorrhagic transformation in a rabbit model of embolic stroke.

Severe hemorrhagic transformation (HT) is an important complication of thrombolytic therapy. A method to identify stroke victims destined to severe HT could improve the patient selection and thus the safety of such treatment. In this study, we investigated whether very early serial diffusion weighted magnetic resonance imaging (DWI) could predict the occurrence of HT in an embolic model of experimental stroke. We tested the hypothesis that the ischemic brains with very low initial apparent diffusion coefficients (ADC) are destined to severe early (<or=5.5 h) HT. We retrospectively analyzed DWI scans of 45 New Zealand white rabbits subjected to thromboembolic stroke and treated with thrombolysis. DWI was obtained 0.5, 2, 3 and 5 h after embolization. Various thrombolytics were administered 1 h post embolization. The percentage of pixels within the ischemic hemisphere with ADC values below 550 x 10(-6) mm(2)/s was calculated and then compared to the severity of HT observed on gross brain sections at 5.5 h. As early as 30 min after embolization, ischemic brains destined to severe HT exhibited a significantly greater percentage of pixels below the cut-off value compared to those without HT: severe HT: 25%, 18.75-37.25% vs. no HT: 12%, 5.00-16.00% (median, 25th-75th %, P<0.001). Petechial HT when percentages were in the intermediate range. Quantitative analysis of initial ADC value might identify individual stroke patients at risk of severe HT.

[Management of vaccinations and prophylaxis of international travellers from community pharmacy (VINTAF study)].

OBJECTIVES: To improve and evaluate the travel advice by community pharmacists. DESIGN: Cross-sectional and prospective study. SETTING: Community pharmacies from Asturias, Barcelona, Madrid and Valencia. PARTICIPANTS: 483 travellers to high health risk countries who visit the community pharmacies and were agreed with the aims of the study. METHOD: Community pharmacists gave information regarding immunization and prophylaxis about travel-related disease according to the destination, type and duration of travel and other features of the tourist (age, taking medicines, diseases, etc.). MAIN MEASUREMENTS: Destination, adherence to the prophylaxis and vaccinations recommended, adverse effects and diseases the tourists have in the travel and one month after they are back. RESULTS: Only 6.3% of the travellers were effective vaccinated and took prophylaxis (all vaccines and prophylaxis according to the destination). 36.2% of the travellers experienced an illness while abroad. The commonest disease were travellers' diarrhoea (15.7%). The pharmacists were the only source of information about sunscreens, repellents, and other sanitary goods. CONCLUSIONS: The community pharmacists can give accurate information regarding immunization and prophylaxis about international travels. It is necessary to improve the communication between family physician, the tropical travel clinic and community pharmacists in order to improving compliance. It is necessary keep the bureaucratic barriers away to get this kind of drugs. The international tourists still travel without the necessary vaccines and prophylaxis to the high health risk areas.

Travel epidemiology--a global perspective.

In order to have a rational approach to necessary preventive measures it is essential to know the health risks. The 80 million travellers each year with destinations in Africa, Asia, Latin America, Pacific Islands and remote areas in Eastern Europe are exposed to a broad range of pathogens that are rarely encountered at home. The risk depends on the degree of endemicity in the area visited, the duration of stay, the individual behaviour and the preventive measures taken. Travellers' diarrhoea (TD) is the most frequent ailment of visitors to countries with poor hygiene. The incidence rate is 25-90% in the first 2 weeks abroad. The risk of TD is far less in travellers originating in a high risk country, as some immunity develops. Malaria is an important risk for travellers going to endemic areas. Without chemoprophylaxis, the monthly incidence is high in some destinations, among them frequently visited tropical Africa where 80-95% of the infections are due to Plasmodium falciparum. The incidence rates are lower in most endemic areas of Asia and Latin America where Plasmodium vivax predominates. The risk is nil in all capital cities of South America and SE Asia, as well as in many frequently visited tourist destinations. The diseases preventable by immunization will be discussed in a separate paper (Vaccination priorities; page 175). Sexually transmitted diseases occur frequently, as some travellers (5% of Europeans) engage in casual sex, approximately half of them without being protected by a condom. The prevalence for HIV-infection, syphilis, gonorrhoea, etc. often exceeds 50% in prostitutes. In some European countries, a major proportion of heterosexuals with newly acquired HIV-infection have acquired it while abroad.

Distribution of cartilage molecules in the developing mouse joint.

This study describes the precise spatial and temporal patterns of protein distribution for aggrecan, fibromodulin, cartilage oligomeric matrix protein (COMP) and cartilage matrix protein (CMP) in the developing mouse limb with particular attention to those cells destined to form articular chondrocytes in comparison to those cells destined to form a mineralized tissue and become replaced by bone. Mouse glenohumeral joints from fetal mice (12-18 days post coitus (dpc) to the young adult (37 days after birth) were immunostained with antibodies specific for these molecules. Aggrecan staining defined the general chondrocytic phenotype, whether articular or transient. Fibromodulin was associated with prechondrocytic mesenchymal cells in the interzone prior to joint cavitation and with the mesenchymal cells of the perichondrium or the periosteum encapsulating the joint elements of the maturing and young adult limb. Staining was most intense around developing articular chondrocytes and much less abundant or absent in those differentiating cells along the anlage. CMP showed an almost reciprocal staining pattern to fibromodulin and was not detected in the matrix surrounding articular chondrocytes. COMP was not detected in the cells at the articular surface prior to cavitation but by 18 dpc, as coordinated movement of the mouse forelimb intensifies, staining for COMP was most intense around the maturing articular chondrocytes. These results show that the cells that differentiate into articular chondrocytes elaborate an extracellular matrix distinct from those cells that are destined to form bone. Fibromodulin may function in the early genesis of articular cartilage and COMP may be associated with elaboration of a weight-bearing chondrocyte matrix.
