Histomorphometric analysis of bone density in the maxillary tuberosity of cadavers: a pilot study.

BACKGROUND: Maxillary tuberosities have been widely used as a source of autogenous bone for a variety of oral surgical procedures. However, no previous studies have actually demonstrated the histologic and histomorphometric characteristics of this anatomical region in detail. The aim of this study was to evaluate maxillary tuberosities in cadavers histologically and histomorphometrically. METHODS: A block section, including the overlying alveolar mucosa, was obtained from a randomly assigned maxillary tuberosity of 20 cadavers (nine females and 11 males). All cadavers were edentulous distal to the second bicuspid teeth, and 14 out of 20 were edentulous distal to the cuspids. A minimum of 35 serial sections were obtained from each cadaver. Description analysis was performed for the presence of either a fibrotic or fatty infiltration of the marrow spaces and thickness and continuity of alveolar bone. Computer-based histomorphometric image analysis of the percentage surface area of bone was also carried out in three sections of each cadaver. Statistical analysis was performed to establish possible differences between genders. The analysis did not control for nutritional status and/or possible systemic or skeletal diseases. RESULTS: Little variation existed between specimens, regardless of gender, with respect to thickness of oral mucosa overlying the tuberosity. Descriptive bone analysis revealed thin cortical bone and sparse cancellous bone patterns. In addition, the block specimens exhibited no evidence of osteoid or active bone formation, but large marrow spaces infiltrated with loosely organized fibrous connective tissue and/or lipid cells. Histomorphometric analysis demonstrated a mean percentage of vital bone of 24.23% +/- 5.2%. Stratification of the data revealed statistically significant differences in mean percentage of vital bone between genders (27.15% +/- 4.7% for males versus 20.66% +/- 3.4% for females; P = 0.003). CONCLUSIONS: Within the limitations of this study, maxillary tuberosities seem to mainly consist of marrow spaces, adipose tissue, and a low vital bone profile. Females demonstrated a statistically significant lower amount of vital bone than males. The results suggest that this specific area may not be an ideal source of autogenous bone for grafting purposes in older individuals. However, it is imperative to note that this investigation did not control for nutritional imbalances and skeletal disorders. Further studies are necessary to control all confounding factors.

[Prosthodontic treatment for a partial mandibulectomy patient. A mandibular non-reconstructed case].

One of the most consistently frustrating areas of maxillofacial rehabilitation is the treatment of patients who have had radical cancer surgery of the tongue, floor of the mouth, and mandible. The patient described in this report, was a 66-year-old man who had squamous cell carcinoma of the right jaw. The right body of the mandible distal to the right cuspid was resected, and this part of the mandible was not reconstructed. For this mandibulectomy patient, we have treated with the removable partial denture using 30 degree teeth and TS telescope crown as retainer, registrating maxillomandibular relation with Gothic arch tracer and MKG. In addition, masticatory function in this patient was evaluated on tooth contact, tapping pattern, masticatory efficiency and rhythm of mastication. Consequently, 4 to 8 weeks later, tapping pattern and centric occlusion was stabilized, and masticatory function was improved. We will continue to observe this case in long term.

[Case report of prosthetic treatment for defected jaw with enucleation of the right maxillary sinus tumor].

PATIENT: The patient is a 71-year-old woman who had undergone a gingivectomy due to a gingival carcinoma of right maxillary sinus and a denture was made for the defected jaw. DISCUSSION: During three years and six months, |3 serves as the guidance tooth had suffered from various symptoms, from the detachment of the neighboring tooth to the use of an OP anchor followed by a partial fracture of the cuspid root. However, |3 has not been removed in accordance to the patient's request. In addition, a relining provided indirectly to the denture, it is still maintained and used for mastication, pronunciation and swallowing functions without difficulty. CONCLUSION: This is a case that a denture was made for the defected jaw. However, it is important ot take measures in the future, as the remaining tooth is not expected to be preserved for long.

[Effect of various implant materials on cementogenesis].

Various implant materials have been used to stimulate the regeneration of supporting bone lost from periodontal disease. In addition, the histologic features of bone regeneration associated with their implantation have been evaluated. Very little, however, seems to be known about the effect of implant materials on cementum formation. The aim of this study was to determine whether implant materials stimulate the cementogenesis on adjacent planed root surfaces. Twelve monkeys with healthy gingivae were used in this experiment. Following mucoperiosteal flap elevation, "windows" were chiseled in the bone to the proximal root dentin surfaces and adjacent root surfaces were planed. Each of the three implant materials [tricalcium phosphate (TCP), decalcified bone matrix (DBM) and hydroxyapatite (HA)] were then placed in the cuspid and incisor root "windows" before the flap was sutured back into the previous position. Windows with no implantation served as a control. Animals were sacrificed 2, 4 and 8 weeks postoperatively. Biopsy specimens including the tooth and surrounding bone were examined by light and electron microscopy. At 2 weeks, all implant particles were surrounded by fibrous tissue. On the other hand, fibrous tissues filled the control defect. On the planed root surfaces after the implantation of TCP and DBM, furthermore, cementoid tissue appeared. At 4 weeks, a considerable amount of new cementum was deposited on the root surfaces except in the implantation of HA. It was especially pronounced after implantation of TCP and DBM which promoted bone regeneration after resorption. These results suggest that resorbable implant materials such as TCP and DBM not only facilitate the formation of new bone, but also of new cementum.

Outcome of 6-week treatment with transcutaneous electric nerve stimulation compared with splint on symptomatic temporomandibular joint disk displacement without reduction.

The aim of the present study was to compare the effect of transcutaneous electric nerve stimulation (TENS) with the flat occlusal splint in the treatment of temporomandibular joint (TMJ) disk displacement without reduction. Thirty-one patients were included and randomly selected to be treated 6 weeks with either TENS (90 Hz, 30 min, three times/day) or with a flat occlusal splint (24 h/day. Those selected for the TENS group had one electrode placed over the painful TMJ and another electrode over the anterior temporal muscle. The splint group used a conventional flat occlusal splint with cuspid guidance. Both treatment groups visited the clinic once a week. Symptoms and signs were registered before and after treatment. The intensity of pain was recorded with a visual analogue scale (VAS) and with an electronic pocket-sized recorder (Pain-Track) carried 1 week before and also the last week of treatment for continuous registration of pain. Measured with the VAS, half of the patients treated with splints became pain-free or their TMJ pain improved at least 50% both at rest and with jaw function compared with only 6% in the TENS-treated group. With regard to strictly chewing pain, the VAS-registered pain improved in two-thirds of the splint group, compared with 50% of the TENS group. With the Pain-Track device it was found that in most individuals pain was aggrevated at mealtimes. The conclusion was that flat occlusal splints in several respects are better than TENS in the treatment of symptoms associated with TMJ disk displacement without reduction.

Light- and electron-microscopic localization of primary dental afferents to medullary dorsal horn (pars caudalis).

Light-microscopic (LM) and ultrastructural (electron-microscopic, or EM) identification of primary dental afferents to medullary dorsal horn (MDH) was demonstrated in the cat following injections of horseradish peroxidase (HRP) into pulpal chambers of unilateral maxillary and mandibular posterior teeth, including the cuspids. Use of a new osmication protocol improved and simplified the EM localization of reaction product within the brain stem terminals. LM examination showed that the projection pattern varied between the different levels of MDH. At caudal levels, the labeling was primarily confined to a narrow band consisting of a dense projection to the dorsomedial portion of laminae I and superficial II and a less intense projection to lamina V. The pattern to rostral levels became increasingly more dense and extensive within these same laminae. LM examination of the tooth apex region showed that a limited spread to the periodontal ligament occurred in some cases. EM investigation of the ipsilateral MDH demonstrated reaction product in terminals with synaptic vesicles that are presynaptic to small and medium-sized dendrites. Labeled axonal endings in close association with cell bodies were also observed. No labeled structures were identified in the contralateral MDH. Some of the reaction product found with EM was below the LM limit of resolution, and thus ultrastructural investigation is necessary for a complete analysis of any pathway when using HRP.

Cystic management in a 10-year old child.

Cysts are among the most common lesions found in the oral cavity. These may be developmental in origin or secondary to chronic infections. Untreated, cysts may expand causing local tissue destruction and deformities. Basically, two treatment methods are employed. Enucleation and/or Marsupialization. The following case involves a child treated at the College of Dentistry, University of the Philippines, for a large cystic lesion involving the permanent cuspid and the deciduous dentition. The lesion was accidentally discovered on a panoramic radiograph. The child underwent extraction of deciduous teeth and marsupialization, which later helped in the eruption of the malposed permanent teeth.

[Quadricuspid aorta diagnosed by transesophageal echocardiography. A case report].

We report a case of a 64 year-old woman who had a quadricuspid aortic valve associated with aortic regurgitation of mild degree diagnosed by transesophageal echocardiography (TEE). The four cuspids were of the same size. This valve anomaly could not be diagnosed by transthoracic echocardiogram (TTE). The TEE is a diagnostic noninvasive procedure that gives the best information in quadricuspid aortic valve malformation.

Oral health in pre-school children living in Sweden. Part III--A longitudinal study. Risk analyses based on caries prevalence at 3 years of age and immigrant status.

A decreasing proportion of children require operative treatment each year and an increasing proportion of children will at recall require no operative care. This would provide resources to give individualised prevention to children at risk for developing new carious lesions. From 3 years of age, however, the prevalence of caries increases up to the age when the primary dentition exfoliates, even though the children attend regular dental service programmes yearly, and for some children dental caries remains a significant problem. The purpose of the present study was to describe the dental health of a group of 6-year-old children living in Sweden, with special reference to caries prevalence at 3 years of age and to immigrant status. At 6 years of age, 45% of the children were free of initial and manifest carious lesions in the primary cuspids and molars. The mean caries increment between 3 and 6 years was 0.9 tooth surfaces for children who were caries free at 3 years of age compared to 4.5 tooth surfaces for children with manifest carious lesions at the same age. The mean caries increment from 3 to 6 years was 1.3 in the non-immigrant group and 3.6 in the immigrant group. For the majority of the children in this study, current preventive dental care seems sufficient. However, for about one-third of the children (children with carious lesions at 3 years of age and/or immigrant status), current caries preventive programme used in the Public Dental Service are inadequate.

[Transcutaneous balloon aortic valvuloplasty as the 1st stage of surgical treatment of aortic valve stenosis].

The results of conduction of transcutaneous balloon aortic valvuloplasty in 99 patients were analysed. After the operation conduction was noted the systolic pressure gradient lowering on aortal valve by 67.8% from the initial one. In 28.3% of patients in such conditions the retrograde blood flow to aorta occurs or enhances, but in 7% of patients only the substantial aortic regurgitation increase by 2 points occurs. The occurrence or enhancement of retrograde blood flow in the aorta is more probable if there is a two cuspid valve and while the two balloon method application. Hospital mortality in the intervention performance had constituted 4%.

Evaluation of the carbon dioxide laser on vital human primary pulp tissue.

PURPOSE: The purpose of this study was to evaluate the response of the human primary pulp to the carbon dioxide laser and formocresol for vital pulp therapy. METHODS: Fifteen healthy children with intact, caries-and-restoration-free, contralateral primary cuspids with at least two-thirds of the roots remaining who were scheduled for orthodontic extraction were randomly assigned to pulpotomy treatment with a carbon dioxide laser or formocresol. The treated teeth were clinically and radiographically evaluated at 28 and 90 days post-treatment prior to extraction. The extracted teeth were evaluated histologically for pulpal response. RESULTS: All teeth were asymptomatic and clinically normal at both observation periods. Internal root resorption was observed in one formocresol and two laser treated teeth. There was a significant inverse correlation between the laser energy applied to the pulp and the degree of inflammation at 28 days (P = .01) but not at 90 days (P = .27). CONCLUSION: Carbon dioxide laser treatment compared favorably to formocresol for pulpotomy in primary teeth.

[The caries prevention campaign in the Canton of Zürich during 1962-1974 along with a statistical result report from the first 8 years].

In November 1962, a caries prophylactic campaign was inaugurated in the canton of Zurich. At the end of 1965, two thirds of the communities, 115 of 171, had introduced already toothbrushing with fluorine compounds at school, under supervision. The following years though did not bring notable increase in participation of the remaining communities. Here are the main results of the campaign: -reduction of dental caries in the permanent dentition by 45 to 50% (fig. 2) -reduction of dental caries on cuspid and incisor teeth and on the buccal and lingual surfaces of lateral teeth by about 80% -reduction of first molar extractions (fig. 5) -massive reduction of the number of untreated carious teeth (table VI) -better condition of the deciduous dentition (fig. 6, 7) -reduction in the incidence of gingival inflammations (table VII, fig. 8) -reduction of the supra- and subgingival calculus deposits. It is deduced from the discussion that supervised toothbrushing with fluorides, the spread of fluoridated toothpastes, and in a lesser degree--the fluoridation of table salt are a certain cause for the massive improvement of juvenile dentitions. It is still uncertain whether the avoidance of sugar containing snacks have also contributed to the favourable result. The reduction of gingival inflammations can only be explained by the improved home care. It cannot be proven if this has also an influence upon caries incidence.

T426I a new mutation in the thyroid hormone receptor beta gene in a sporadic patient with resistance to thyroid hormone and dysmorphism. Mutations in brief no. 192. Online.

Resistance to thyroid hormone (RTH) is a rare inherited autosomal syndrome caused by mutations in the thyroid hormone receptor beta (TRb) gene. Although RTH is generally a familiar disease, 15% of sporadic cases have been also reported. So far, about 80 different mutations of TRb gene have been identified in patients affected by RTH. All these mutations localize to the binding domain and most of them cluster within two "hot spots" (codons 310-349 and codons 429-460). Here we describe in a patient with RTH, a new mutation in codon 426 (T426I) of the TRb gene leading to a threonine to isoleucine substitution. This is a "de nova" mutation which localizes in the so-called "cold" region, outside the two known "hot spots". The patient had the hallmark of RTH: elevated FT3 and FT4, normal TSH, and clinical features of both hypo and hyperthyroidism. Moreover, several dysmorphisms were present including triangular face appearance, synophris, low set ears, micrognathia with malocclusion, large upper incisors and apparent lack of lower cuspids which have not previously described in RTH patients.

[Separation and purification of taxol using normal- and reversed-phase chromatography in tandem].

A method is described for separation and purification of taxol from the extract of Taxus cuspidate callus culture with Al2O3 normal-phase chromatography and C18 reversed-phase chromatography, The operating conditions of the two chromatographic steps were investigated and optimized. The content of taxol was greatly increased after Al2O3 chromatography. The purity of taxol reached 95% from less than 1.0% after the two chromatographic steps. The final purity of taxol was more than 98% after the process of crystallization. The 13CNMR spectrum of the product was identical to that of taxol reported in the literature.

[The effects of the complex of rhBMP2 and fibrin sealant on dental pulp].

OBJECTIVE: To investigate the reparative dentine formation induced by the complex of recombinant human bone morphogenetic protein 2 (rhBMP2) and fibrin sealant (FS) in dog tooth. METHODS: Freshly exposed molars, premolars and cuspids were treated with the complex of rhBMP2/FS, rhBMP2 and Ca(OH)(2) paste which served as control. RESULTS: The wound healing of exposed pulp treated with FS seemed to be better than that without FS. Dentine bridge formation was observed in pulp treated with the complex at 7 days, with rhBMP2, Ca(OH)(2) at 28 days. At 63 days after operation, the cases of tubular dentine bridge formation of the pulp treated with the complex were more than that of pulp treated with rhBMP2. CONCLUSION: The synergistic effects of FS and the complex rhBMP2 exist and can be used as bioactive pulp capping agent.

[Intracardiac hemodynamics after closure of atrial septal defects with the endovascular device, Amplatzer septal occluder].

Changed size and forms of the right and left ventricles of the heart and intraventricular hemodynamics were studied in 5 patients after closing the defects of the interventricular septum with an intravascular device. It was found that after occlusion of the defects by the safe and atraumatic endovascular device the right ventricular myocardium of the heart relaxed, the stroke index became less, the ejection fraction became greater, the diastolic diameter of the right ventricle decreased, and of the left ventricle increased. The indices of the regional contraction function of the tri-cuspid and output sectors of the input part and external and pulmonary output sectors of the right ventricle were found to normalize.

The use of biomechanical orthodontic treatment concepts for removable appliances in maxillofacial prosthodontics. Part I: review of prosthesis design principles.

Maxillary cuspids are commonly included in ablative block resections in maxillectomy patients. Although the remaining incisors are poor abutments for interim or definitive obturators, they must be used if adequate retention and support is to be achieved. Orthodontic biomechanical concepts for removable appliances offer solutions for the treatment of these patients. The proper application of these concepts can improve retention, support, and force distribution to the anterior teeth and is particularly useful in younger patients when long-term maintenance of teeth is critical. The purpose of this article is to describe the rationale for this approach and its application to interim and definitive Aramany Class I and II obturators.

[Surgically assisted rapid maxillary expansion: a retrospective study].

Transverse maxillary hypoplasia, in adolescents and adults, may be corrected by means of a 'surgically assisted rapid maxillary expansion' (SARME). The results of 7 patients who were treated by means of a SARME, are presented. Attention is paid to the technique, the type of expansion, and the stability of the expansion of the maxilla 1-1.5 years after SARME. The mean expansion between the cuspids was 4.7 mm (sd 1.7) and between the first molars 5.6 mm (sd 3.2). The SARME technique appeared to give rise to a predictable result and a sufficient amount of expansion, although the sample is small and the follow-up period relatively short. All patients had a satisfactory result without complications.
