Reduced P300 amplitude in a visual recognition task in patients with schizophrenia.

We studied top-down visual processes in schizophrenia by analyzing visual event-related potentials (ERPs) during a gestalt recognition task, after subjects (patients with schizophrenia, n = 10; controls, n = 14) were trained to perceive three different geometrical shapes. Recognition performance of patients was poorer under both the figure and the nonfigure conditions then that of normal controls. ERPs analysis indicated that P300 amplitudes of the patients were significantly smaller than those of controls during correct figure detection trials. Moreover, topographical analysis of the differences in ERPs during the figure vs the nonfigure condition showed an earlier, more positive and more widely distributed P300 in controls than in patients with schizophrenia. Our study supports the misconnection hypothesis of schizophrenia and highlights the difficulty of the patients to refer to previous experience in order to filter incoming information. In a visual recognition task, this misconnection syndrome might induce a failure to integrate information stored in the frontal and prefrontal sites with incoming stimuli.

Integrating external clinical information with the ten-step monitoring and evaluation process: from theory to practice.

Traditionally, Sisters of Mercy Health System (SMHS)-St Louis hospitals have had difficulty integrating quality information from external sources into their own monitoring and evaluation activities. This difficulty became apparent at the 14 hospitals of the SMHS when the system headquarters began providing its hospitals with comparative clinical outcome information. Although the hospitals found value in the comparative studies, the information was not fully used in individual hospitals' quality improvement efforts. As a result, SMHS developed a model for integrating external quality information into the local hospitals' ten-step monitoring and evaluation process. This article explains the integration model and shows how it has worked in several clinical scenarios.

Evaluation of the vector space representation in text-based gene clustering.

Thanks to its increasing availability, electronic literature can now be a major source of information when developing complex statistical models where data is scarce or contains much noise. This raises the question of how to deeply integrate information from domain literature with experimental data. Evaluating what kind of statistical text representations can integrate literature knowledge in clustering still remains an unsufficiently explored topic. In this work we discuss how the bag-of-words representation can be used successfully to represent genetic annotation and free-text information coming from different databases. We demonstrate the effect of various weighting schemes and information sources in a functional clustering setup. As a quantitative evaluation, we contrast for different parameter settings the functional groupings obtained from text with those obtained from expert assessments and link each of the results to a biological discussion.

Combining HIV and AIDS surveillance: an experience from an Italian region.

BACKGROUND: Surveillance systems based on the reporting of AIDS cases do not provide a completely up to date picture of the trend of the HIV epidemic, stressing the need for systems based on the diagnosis of HIV infection. However, implementation of these systems has been hindered by low feasibility, poor access to HIV testing, and problems related to confidentiality. The advantages and disadvantages of the two systems and of combined use were explored by comparing and integrating information from AIDS and HIV testing registries in a region of Northern Italy. METHODS: Linkage of AIDS and HIV testing registries allowed the annual number of incident and prevalent infections to be calculated. For linked cases, concordance of exposure category was determined. RESULTS: Up to the end of 1995, 2186 AIDS cases and 5306 HIV-positive individuals were diagnosed. Linkage identified 1212 individuals reported to both registries. From 1990 to 1995, annual AIDS incidence steadily increased, while incidence of new HIV diagnoses decreased. The AIDS-to-AIDS-free ratio among those infected decreased from 1:5.6 in 1989 to 1:4.2 in 1995. The proportion of women and noninjecting drug users was higher among AIDS-free cases than among AIDS cases. The concordance of the exposure category was high (K = 0.70; 95% CI: 0.67-0.74), but it varied by exposure category; the highest concordance was for injecting drug users. CONCLUSIONS: Integrated use of the information provided by these surveillance systems allowed us to better understand and foresee the AIDS epidemic dynamics. The data also suggested that the reliability of information on exposure category may vary among categories.

Processing of color, form, and motion in macaque area V2.

We investigated the representation of color in cortical area V2 of macaque monkeys, and the association of color with other stimulus attributes. We measured the selectivity of individual V2 neurons for color, motion, and form. Most neurons in V2 were orientation selective, about half of them were selective for color, and a minority of cells (about 20%) were selective for size or direction. We correlated these physiological measurements with the anatomical location of the cells with respect to the cytochrome oxidase (CO) compartments of area V2. There was a tendency for color-selective cells to be found more frequently in the thin stripes, but color-selective cells also occurred frequently in thick stripes and inter-stripes. We found no difference in the degree of color selectivity between the different CO compartments. Furthermore, there was no negative correlation between color selectivity and selectivity for other stimulus attributes. We found many cells capable of encoding information along more than one stimulus dimension, regardless of their location with respect to the CO compartments. We suggest that area V2 plays an important role in integrating information about color, motion, and form. By this integration of stimulus attributes a cue invariant representation of the visual world might be achieved.

Maps versus clusters: different representations of auditory space in the midbrain and forebrain.

The auditory system determines the location of stimuli based on the evaluation of specific cues. The analysis begins in the tonotopic pathway, where these cues are processed in parallel, frequency-specific channels. This frequency-specific information is processed further in the midbrain and in the forebrain by specialized, space-processing pathways that integrate information across frequency channels, creating high-order neurons tuned to specific locations in space. Remarkably, the results of this integrative step are represented very differently in the midbrain and forebrain: in the midbrain, space is represented in maps, whereas, in the forebrain, space is represented in clusters of similarly tuned neurons. We propose that these different representations reflect the different roles that these two brain areas have in guiding behavior.

A framework for integrating the songbird brain.

Biological systems by default involve complex components with complex relationships. To decipher how biological systems work, we assume that one needs to integrate information over multiple levels of complexity. The songbird vocal communication system is ideal for such integration due to many years of ethological investigation and a discreet dedicated brain network. Here we announce the beginnings of a songbird brain integrative project that involves high-throughput, molecular, anatomical, electrophysiological and behavioral levels of analysis. We first formed a rationale for inclusion of specific biological levels of analysis, then developed high-throughput molecular technologies on songbird brains, developed technologies for combined analysis of electrophysiological activity and gene regulation in awake behaving animals, and developed bioinformatic tools that predict causal interactions within and between biological levels of organization. This integrative brain project is fitting for the interdisciplinary approaches taken in the current songbird issue of the Journal of Comparative Physiology A and is expected to be conducive to deciphering how brains generate and perceive complex behaviors.

The HELP system and its application to infection control.

The HELP system is a comprehensive hospital information system that is linked to an allied financial data base. The clinical data base integrates information from areas such as admitting, pharmacy, radiology, surgery, pathology, nursing, respiratory therapy, and the clinical laboratories, including microbiology. This allows for the creation of an electronic medical record that contains all the clinical and financial data for each patient. The HELP system combines both communication and advice features through the use of data- and time-driven algorithms. We have used the HELP system to automate the surveillance and analysis of hospital-acquired infections and to identify patients at high risk for nosocomial infection. The expert system features have also been used to suggest alternatives for patients receiving inappropriate antimicrobial therapy, to improve the timing of antibiotic prophylaxis in surgery, and to curtail unnecessarily prolonged prophylaxis. Automated hospital information systems such as HELP can facilitate the investigation of a broad range of infection control, quality improvement, and cost-containment issues.

14C-deoxyglucose mapping of the monkey brain during reaching to visual targets.

The strategies used by the macaca monkey brain in controlling the performance of a reaching movement to a visual target have been studied by the quantitative autoradiographic 14C-DG method. Experiments on visually intact monkeys reaching to a visual target indicate that V1 and V2 convey visuomotor information to the cortex of the superior temporal and parietoccipital sulci which may encode the position of the moving forelimb, and to the cortex in the ventral part and lateral bank of the intraparietal sulcus which may encode the location of the visual target. The involvement of the medial bank of the intraparietal sulcus in proprioceptive guidance of movement is also suggested on the basis of the parallel metabolic effects estimated in this region and in the forelimb representations of the primary somatosensory and motor cortices. The network including the inferior postarcuate skeletomotor and prearcuate oculomotor cortical fields and the caudal periprincipal area 46 may participate in sensory-to-motor and oculomotor-to-skeletomotor transformations, in parallel with the medial and lateral intraparietal cortices. Experiments on split brain monkeys reaching to visual targets revealed that reaching is always controlled by the hemisphere contralateral to the moving forelimb whether it is visually intact or 'blind'. Two supplementary mechanisms compensate for the 'blindness' of the hemisphere controlling the moving forelimb. First, the information about the location of the target is derived from head and eye movements and is sent to the 'blind' hemisphere via inferior parietal cortical areas, while the information about the forelimb position is derived from proprioceptive mechanisms and is sent via the somatosensory and superior parietal cortices. Second, the cerebellar hemispheric extensions of vermian lobules V, VI and VIII, ipsilateral to the moving forelimb, combine visual and oculomotor information about the target position, relayed by the 'seeing' cerebral hemisphere, with sensorimotor information concerning cortical intended and peripheral actual movements of the forelimb, and then send this integrated information back to the motor cortex of the 'blind' hemisphere, thus enabling it to guide the contralateral forelimb to the target.

Time-accuracy relationships in young and old adults.

The relationships between reaction time and classification accuracy were compared in young (18 to 21 years) and old (60 to 84 years) adults in a choice reaction time task. Both young and older adults showed equivalent rates of increasing accuracy with greater time, but the temporal duration at which the accuracy first exceeded the chance level was shorter for young than for older adults. It was suggested that aging is associated with a slowing of the information integration and/or response preparation processes but not with a slowing of the actual rate of information extraction.

Modeling shared health and social care information.

The article describes an analysis of the detailed data used by different groups of health care professionals and their requirements to share data as part of an integrated information systems design project. This systematic recording of their data usage and sharing requirements led to the development of a novel analysis technique: the complex object grid. The use of the complex object grid facilitates the description of information usage and needs at several levels of detail.

The telematic network of referee hospital "V. Monaldi" in Naples: state of the art and perspectives.

The new advances in I.T. both in Hardware (wideband network) and in Software are rapidly changing the Health Information Systems scenario. In many hospitals of Campania Region this leads in many case to rebuild, starting from zero, both infrastructure and applications. Ericsson Enterprise has recently developed for the A.O. Monaldi and Integrated information System which consists of an advanced LAN (Local Area Network), a number of software infrastructures and some application systems as WEB site, Dicom PACS, E-mail server, Streaming Video from Operating Theatres, Internal TV Network. This integrated system represents the starting point for modern health information systems, which is compliant with new standards. The start-up of such systems represents always a problem for the organization and management point of view, therefore a number of problems concerning: training, education, security, privacy, operative procedures, co-ordination with existing applications, system management at the start-up and after. This paper deals with the technical aspects of this information system and discusses the problem met in introducing these IT products in a big and important hospital of Campania Region in Italy, in order to suggest a model, useful for other similar experiences.

Multilevel selection in models of prebiotic evolution: compartments and spatial self-organization.

In this paper we explore the impact of new levels of selection in models of early evolution. We contrast two types of higher levels of selection. On the one hand we look at spatially explicit models of replicators in which, by a process of self-organization, new levels of selection arise as large scale spatial patterns with a dynamics of their own. Alternatively externally imposed levels of selection above the basic replicators are created by enclosing the replicators in vesicles. In this paper we first review some results on the impact of emerging higher levels of selection on the evolutionary persistence of interacting co-evolving replicator systems. Moreover, we present a vesicle model, which can potentially integrate emerging and imposed levels of selection. We use the models to examine the classical problem information integration in early evolution.

Spatial integration in polarization-sensitive interneurones of crickets: a survey of evidence, mechanisms and benefits.

Many insects exploit the polarization pattern of the sky for compass orientation in navigation or cruising-course control. Polarization-sensitive neurones (POL1-neurones) in the polarization vision pathway of the cricket visual system have wide visual fields of approximately 60 degrees diameter, i.e. these neurones integrate information over a large area of the sky. This results from two different mechanisms. (i) Optical integration; polarization vision is mediated by a group of specialized ommatidia at the dorsal rim of the eye. These ommatidia lack screening pigment, contain a wide rhabdom and have poor lens optics. As a result, the angular sensitivity of the polarization-sensitive photoreceptors is very wide (median approximately 20 degrees ). (ii) Neural integration; each POL1-neurone receives input from a large number of dorsal rim photoreceptors with diverging optical axes. Spatial integration in POL1-neurones acts as a spatial low-pass filter. It improves the quality of the celestial polarization signal by filtering out cloud-induced local disturbances in the polarization pattern and increases sensitivity.

Risk assessment and health surveillance of pesticide workers.

Occupational health has two main objectives: the protection of workers from health hazards associated with the working environment, and the promotion of workers' health. Fundamental aspects of health protection are health risk management, information and training, first aid and medical treatment. Risk management comprises risk characterization, and exposure assessment. Integrating information on the identified hazards and exposure levels, and assessing the likelihood and severity of health effects performs risk characterization. Health surveillance includes medical surveillance and biological monitoring. The physician in charge of the occupational health programme, taking into account the results of the risk assessment process should determine frequency and contents of periodical medical examinations. Pre-employment medical examinations should be carried out in order to determine the physical ability to do the job the candidate is recruited for. Periodical medical examinations are aimed at detecting, in an early and reversible phase, any adverse effect attributable to exposure. A worker should undergo a broad medical examination when returning to work after a significant illness. Records of occupational health programmes should be filed and kept for at least 30 years.

What have HMOs learned about clinical prevention services? An examination of the experience at Group Health Cooperative of Puget Sound.

Over the last 20 years, HMOs have begun to develop population- and evidence-based systems for their clinical prevention services. Because of their integrated information systems and staff communications, HMOs are uniquely positioned to help physicians to identify systematically the primary risk factors or secondary prevention needs of individuals and to provide specific referral or screening services and off-site telephone counseling services to effect seamless, organized intervention. A review of the relevant published literature was combined with the empirical experience of the Group Health Cooperative of Puget Sound in Seattle to illustrate the approach of one large staff-model HMO. Challenges for the future include creating clinical information systems, establishing adequate funding, instituting practitioner incentives, training practitioners for behavioral change, and developing and integrating patient self-care.

Using the DNA language model, GROVER, to parse effects of sequence, chromatin and regulatory features on genome stability.

MOTIVATION: Genome stability is shaped by DNA sequence and chromatin context, but their relative contributions to double-strand break (DSB) sensitivity remain unclear. RESULTS: We show that the DNA language model, GROVER, can infer DSB location based on sequence. DSB hotspots tend to contain GC-rich sequences that belong to promoters, genes and short interspersed nuclear elements (SINEs). Additionally, we identified several specific short sequences (tokens) that are associated with modulating DSB sensitivity. Another model using chromatin and genome regulatory features outperforms the sequence-only model, highlighting complementary and cell-type specific information. Integrating sequence and genome biological features yields the best performance, demonstrating their synergy. Analyzing this model revealed that, dependent on the sample, genome stability information encoded in H3K36me3 and DNase-seq can be learned from the sequence, but not H3K27ac or H3K9me3. Embedding chromatin data directly into the GROVER architecture enabled cell-type specific modeling with performance matching the full chromatin feature model. Our results suggest that while chromatin and regulatory context provides important information, such as cell-type specificity, much of the information shaping DSB patterns is already encoded in the DNA sequence itself. Our integrative modeling approach not only reveals DSB patterns but also provides a generalizable strategy for tracing predictions in genomic data. Optimizing point-of-care testing in clinical systems management.

The goal of improving medical and economic outcomes calls for leadership based on fundamental principles. The manager of clinical systems works collaboratively within the acute care center to optimize point-of-care testing through systematic approaches such as integrative strategies, algorithms, and performance maps. These approaches are effective and efficacious for critically ill patients. Optimizing point-of-care testing throughout the entire health-care system is inherently more difficult. There is potential to achieve high-quality testing, integrated disease management, and equitable health-care delivery. Despite rapid change and economic uncertainty, a macro-strategic, information-integrated, feedback-systems, outcomes-oriented approach is timely, challenging, effective, and uplifting to the creative human spirit.
