Robot-assisted needle placement in open-MRI: system architecture, integration and validation.

This work describes an integrated system for planning and performing percutaneous procedures-such as prostate biopsy-with robotic assistance under MRI-guidance. The physician interacts with a planning interface in order to specify the set of desired needle trajectories, based on anatomical structures and lesions observed in the patient's MR images. All image-space coordinates are automatically computed, and used to position a needle guide by means of an MRI-compatible robotic manipulator, thus avoiding the limitations of the traditional fixed needle template. Direct control of real-time imaging aids visualization of the needle as it is manually inserted through the guide. Results from in-scanner phantom experiments are provided.

Drug formulary commentary: an integrated health systems approach.

Integrated health systems create opportunities and challenges for pharmacists in developing approaches to provision of pharmaceutical care and services. A fundamental area for integration is drug formulary management. This commentary addresses the need for pharmacists in integrated systems to first create a vision of success. From there, pharmacists can capitalize on their expertise in drug formulary management to create and win in terms of quality and cost for the entire system.

Schizophrenia--a disorder of the corollary discharge systems that integrate the motor systems of thought with the sensory systems of consciousness.

BACKGROUND: In spite of intensive research, no causal anatomical lesion has been found in schizophrenia. It may instead be caused by malfunctioning circuits in the corollary discharge, feed forward (CD-FF) systems of thought. AIMS: To integrate with the CD-FF hypothesis recent data showing that subcortical motor systems participate in thinking. METHODS: We review CD-FF concepts in relation to recent evidence that 'motor' brain structures participate in cognitive processing. RESULTS: Malfunctioning of CD-FF systems that integrate thinking and consciousness could produce auditory hallucinations, delusions and disorganised thought. CONCLUSIONS: We hypothesise that the pathophysiology of schizophrenia lies in integrative circuits of basal ganglia, thalamus and frontal cortex. Fruitful research directions would include elucidation of CD-FF circuits at even higher brain levels, the behaviour of these circuits during dreaming, and their responses to late maturational events including synaptic elimination.

Enterprise networks. Strategies for integrated delivery systems.

More integrated delivery systems are making progress toward building computer networks that link all their care delivery sites so they can efficiently and economically coordinate care. A growing number of these systems are turning to intranets--private computer networks that use Internet-derived protocols and technologies--to move information that's essential to managing scare health care resources.

The impact of IAIMS on the work of information experts. Integrated Advanced Information Management Systems.

Integrated Advanced Information Management Systems (IAIMS) programs differ but have certain characteristics in common. Technological and organizational integration are universal goals. As integration takes place, what happens to those implementing the vision? A survey of 125 staff members, or information experts, involved in information or informatics at an IAIMS-funded institution was conducted during the last year of the implementation phase. The purpose was to measure the impact of IAIMS on the jobs of those in the library and related service units, and the computing, telecommunications, and health informatics divisions. The researchers used newly developed scales measuring levels of integration (knowledge of and involvement with other departments), customer orientation (focus on the user), and informatedness (changes in the nature of work beyond automation of former routines). Ninety-four percent of respondents indicated that their jobs had changed a great deal; the changes were similar regardless of division. To further investigate the impact of IAIMS on librarians in particular, a separate skills survey was conducted. The IAIMS librarians indicated that technology and training skills are especially needed in the new, integrated environment.

Leprosy control activities in India: integration into general health system.

Integration of leprosy control into the general health system is an essential element of a leprosy elimination strategy. In India, the process has been undertaken with the assistance of World Bank in a phased manner. In the first phase (2001-2002), 24 low/moderately endemic provinces for leprosy were targeted. Operational research was undertaken in these low/moderate endemic provinces to assess the progress of integration of leprosy control in general health system using defined categories, viz. structural integration, training status, availability of MDT and recording/reporting of cases. Selection of nine provinces, 18 districts, 86 health facilities and 108 sub-centres was performed using multistage stratified random sampling technique. Data were collected by interviewing GHS/vertical staff, scrutiny of records and spot checking of MDT stock by Health officers of three leprosy institutions of the Government of India. The result showed that district leprosy nuclei had formed in 16 of 18 districts. In 56% of health facilities vertical staff were redeployed for delivering general health care. Forty-five percent of medical officers, 71% of health supervisors and 75% of multipurpose workers were trained in leprosy. MDT treatment was available in >80% of health facilities. In only 2% of health facilities 3 months MDT stock of all types was present. Forty-four percent of sub-centres were delivering subsequent doses (second dose onward) of MDT. Reporting through a simplified information system was universal. This study emphasizes the need for reorientation training of Medical Officers, better MDT stock management and decentralized management of cases up to sub-centre level.

System reform in integrated health systems.

As the debate progresses on health care reform and the ultimate form of the U.S. system, important lessons can be drawn from examinations of other health care systems. From the U.S. perspective, European health systems appear to have a certain homogeneity about them. Americans tend to look at all European arrangements as single-source financing systems. Because these systems all provide universal coverage, the assumption is that there must be a strong cohesion and similarity among them. Viewed from the European perspective, the reality appears to be rather different. In this article, the health cae systems of Nordic countries are analyzed in terms of their differences both from other European systems and from the United States approach.

Process improvement program evolves into compliance program at an integrated delivery system.

An integrated delivery system discovered questionable practices when it undertook a process-improvement initiative for its revenue-to-cash cycle. These discoveries served as a wake-up call to the organization that it needed to develop a comprehensive corporate compliance program. The organization engaged legal counsel to help it establish such a program. A corporate compliance officer was hired, and a compliance committee was set up. They worked with counsel to develop the structure and substance of the program and establish a corporate code of conduct that became a part of the organization's policies and procedures. Teams were formed in various areas of the organization to review compliance-related activities and suggest improvements. Clinical and nonclinical staff attended mandatory educational sessions about the program. By approaching compliance systematically, the organization has put itself in an excellent position to avoid fraudulent and abusive activities- and the government scrutiny they invite.

Stable incorporation of genetic material into the chromosome of Rhizobium meliloti 41: construction of an integrative vector system.

An integrative vector system has been developed from the site-specific recombination elements of temperate phage 16-3. The system can be used for highly efficient stable introduction of genetic material into the chromosome of the symbiotic nitrogen-fixing organism, Rhizobium meliloti 41 (Rm41) at the attB site. Vectors carrying the phage-borne attachment site were constructed, and helper phages providing the site-specific recombination functions in trans were isolated. Other possible applications of the system are discussed.

Are provider integrated delivery systems the final step to integration?

Provider integrated delivery systems are not the final step to full-fledged integration, states author R. Daniel King, president of Medi-Call of St. Louis Inc. While a PIDS is on the way, true interdependent integration takes place when the community joins with the integrated system to represent all phases of the health care continuum and assumes some of the responsibility for planning. These community integrated delivery systems (CIDS) should be the aim of integration.

Developing a patient care model for an integrated delivery system.

An integrated healthcare delivery system requires a consistent patient care delivery model. The authors describe the process used to define common elements of the patient care model. These elements include the roles of chief nurse executives, first-line managers, staff registered nurses, and unlicensed assistive personnel. In addition, the philosophy of nursing and support functions (staff education and nursing dashboard for quality measurement) in place across the system are discussed.

Partners HealthCare. Creating and managing an integrated delivery system.

Supporting the "integration" of an integrated delivery system is an exceptionally complex and difficult challenge. Lack of organizational clarity about integration strategies and value, political challenges in effecting integration, the idiosyncratic and evolutionary nature of integration, and the technical challenges of integrating heterogeneous technologies all contribute to the scale of the challenge. Information systems integration strategies and tactics should be guided by overall concepts that frame the organization's understanding of the nature of integration. Effort must be directed to working with IDS leadership to define the approach to specific classes of integration, e.g., clinical integration. And, as always, one should learn from the experiences of others. While it may be difficult to fully define the value of integration in an IDS, the IDS seems to be a permanent feature of the healthcare landscape. And the integration of the IDS has value, value that is often nuanced, intangible, and complicated. Hence, the efficient and effective application of information systems to further integration is a critical organizational undertaking.

Computer-assisted diagnosis of postmenopausal osteoporosis using a fuzzy expert system shell.

This paper shows how the AMDIS (Automated Medical Diagnosis with Intelligent Systems) integrated system can be employed to build a fuzzy medical expert system in the domain of postmenopausal osteoporosis. The fundamental aims of the expert system are to standardize knowledge and support physicians in the early detection of postmenopausal osteoporosis. A wide range of diagnostic situations has been considered for both categories of the disease, with judgments that range from disease is excluded to disease is definite. The salient aspects of the approach are the use of fuzzy logic as an analytic language for the representation and manipulation of knowledge and strategies and the integration of structured interview techniques and learning-by-example to address the knowledge acquisition task.

Computerized manufacturing of transparent face masks for the treatment of facial scarring.

In this work an integrated system of computer-aided design and computer-aided manufacturing specifically designed to fabricate affordable transparent face masks is discussed. FaceScan, a custom-designed software system, integrates shape capture, mask design, and pattern fabrication. The software controls a linear scan noncontact laser imager for facial topography acquisition and a milling machine for pattern fabrication. Compared with conventional methods of mask fabrication, this system is faster, more accurate, and less stressful for the patient and allows for greater control of the finished product. Masks for two subjects have been successfully fabricated using this system.

Analysis of a Bacillus subtilis genome fragment using a co-operative computer system prototype.

Analysis of the huge volume of data generated by large scale sequencing projects requires the construction of new, sophisticated computer systems. These systems should be able to manage the biological data as well as the results of their analysis. They should also help the user to choose the most appropriate methods, and to string them together in order to solve a global analysis task. In this paper we present the prototype of a software system providing an environment for the analysis of large-scale sequence data. As a first step toward this end, this environment has been put to the test within the Bacillus subtilis genome sequencing project. This system integrates both the descriptive knowledge of the entities involved (genes, regulatory signals and the like) and the methodological knowledge comprising an extensible set of analytical methods. A knowledge representation based on two existing object-oriented models is used to implement this integrated system. In addition, the present prototype provides a suitable user interface both for displaying simultaneously the results generated by several methods and for interacting with the objects. We present in this paper the analysis of a B. subtilis genome fragment, present in data libraries but not annotated. Annotation of the genes present in the fragment allowed us to combine the results of several methods used for predicting coding sequences, and to characterize it as comprising a cryptic phage, the skin element. Comparison between the annotation of the skin element and a standard region of the chromosome indicated that local features of the nucleotide sequence could discriminate between phage and non-phage DNA sequence.
