[Correlations of ventilation measurements with alpha 1-antitrypsin deficiency chronic suppurative bronchopneumopathies].

The corrleations of ventilometry and alpha-1-antitrypsin deficiency were studied in a lot of 105 patients with chronic obstructive bronchopneumopathy associated with various forms of chronic pulmonary suppuration. Alpha-1-antitripsin deficiency was determined by Erikson's gelatin film test, simplified by Keith et al. Ventilation dysfunctions were found in 79% of the cases, predominantly obstructive, mixed, advanced or severe. Alpha-1-antitrypsin deficiency was detected in 11.4% of the patients as against 4.1% in the controls, the deficiency being more accentuated in the mixed (75%) and obstructive (25%) ventilation dysfunctions. Determination of alpha-1-antitrypsin deficiency, corroborated with other investigations may supply useful information for detecting a predisposition and for the evolutive prognosis of these respiratory diseases.

[Surgical treatment of chronic suppurative otitis media].

The results of surgical treatment of chronic suppurative otitis media with or without cholesteatoma were presented in 708 patients over the period of ten years. Microtoilet of the ear and treatment of pathologic processes in the upper respiratory tract in the preoperative period the authors consider important for success in surgery. The best functional results were achieved with tympanoplasty bearing in mind adjustment of operative technic for each case depending on the nature and spreading of the process. The most frequent recurrences occurred in patients with cholesteatoma surgery (32%).

Suppurative cervical lymphadenitis after Yersinia enterocolitica bacteremia.

Yersinia enterocolitica is increasingly recognized as a pathogen causing diverse complications. We have reported the case of a man with fever, abdominal tenderness, and Y enterocolitica bacteremia. After antibiotic therapy, his condition improved initially, but later, suppurative cervical lymphadenitis developed. This suggests that the hematogenous spread of Y enterocolitica to a distal lymphatic focus of infection is a possible complication of Y enterocolitica bacteremia.

Recurrent acute suppurative thyroiditis.

Recurrent acute left-sided suppurative thyroiditis was observed and treated in an eight-year-old girl. Barium swallow performed after the third episode revealed the presence of a pyriform sinus fistula extending to the upper pole of the left lobe. Despite resection of the fistula, she relapsed for a fourth time. The fistula was again seen on esophagogram. A probe was left inside the tract during subsequent surgery, thus facilitating its complete removal. AST occurs most commonly as a result of the persistence of a connection between the throat and the thyroid gland which is otherwise extremely resistant to infection. AST is usually left-sided and not associated with thyroid dysfunction. Recurrences are common unless the congenital fistula is identified and completely excised.

[Antibacterial activity of ciprofloxacin against fresh clinical isolates from superficial suppurative foci].

The minimum inhibitory concentrations (MICs) of 5 drugs (ciprofloxacin (CPFX), and 4 drugs used as standard) were determined to investigate antibacterial potencies of CPFX against bacterial strains isolated in 1989 from superficial suppurative foci. The clinical isolates tested included 375 strains from 11 aerobic bacterial species, and 50 strains from 2 anerobic bacterial genera (group) for a total of 425 isolates. Interpreting MIC level distributions of these drugs as the expression of antibacterial potencies, the results are as follows. 1. When activities of new-quinolone antibiotics were tested, we found that, CPFX expressed far superior antibacterial potency to ofloxacin (OFLX) and norfloxacin (NFLX) against coagulase-negative staphylococci, Enterococcus faecalis, Enterococcus faecium, Escherichia coli, Klebsiella pneumoniae, Enterobacter cloacae, Proteus mirabilis, Proteus vulgaris, Morganella morganii, Pseudomonas aeruginosa and Peptostreptococcus spp., although the activity of CPFX against Bacteroides fragilis group was weaker than that of OFLX, and CPFX had similar activity against Staphylococcus aureus to OFLX. 2. In comparison to beta-lactam antibiotics, CPFX was inferior to amoxicillin (AMPC) against E. faecalis and inferior to AMPC and cefaclor (CCL) against Peptostreptococcus spp. Against all other bacterial species, however, CPFX expressed superior antibacterial potency to AMPC and CCL. 3. Scattered findings of low sensitivity or resistance to CPFX were observed among the S. aureus, E. faecalis, E. faecium, P. vulgaris, M. morganii, P. aeruginosa and B. fragilis (group) species, but with an exception of E. faecium, the incidence of resistance strains was low.

Antagonism among Bacteroides fragilis group strains isolated from middle ear exudates from patients with chronic suppurative otitis media.

The production and sensitivity of a bacteriocin-like substance in Bacteroides fragilis group strains isolated from middle ear exudates in children with suppurative otitis media were studied through antagonism assayed by the well method. The results of the crossed reactions showed that 10 strains (66.6%) were bacteriocinogenic, 9 were sensitive to at least 1 bacteriocin (60%), and none showed inhibitory activity against homologous strains. Different patterns of susceptibility to bacteriocin-like substances were observed among strains isolated from the same patient as well as different strains isolated from another patient. These findings indicate that bacteriocin typing of anaerobic bacteria isolated from middle ear exudates in children with otitis media might have use in epidemiologic studies.

[Suppurative-inflammatory diseases in newborn infants].

Changes in the etiological patterns of suppurative-inflammatory diseases of neonates during 1978-1988 were analysed. Basic trends in the etiological structure changes, growing numbers of individual microorganism families or species, and changes in the clinical manifestations corresponding to the etiology variation were demonstrated. Preventive antibiotic therapy administered to neonates in the maternity hospital is proved ineffective, and its deleterious effect on the postnatal microbial colonization is shown.

Randomised comparative efficacy of clindamycin, metronidazole, and lincomycin, plus gentamicin in chronic suppurative otitis media.

Chronic suppurative otitis media is major health problem seen frequently in the ENT clinics in Nigeria and the outcome of most medical management in the past have been disappointing. A comparative randomised clinical trial involving combination therapies with systemic clindamycin, metronidazole, lincomycin, each with gentamicin, was conducted on a total of 14 patients. At the end of one week, and three-week, follow-up end points the clinical response with bacteriological cure were, for clindamycin and gentamicin 21%, metronidazole and gentamicin 33%, lincomycin and gentamicin 22%, and the control (aural toilet alone) 14%. However, when the clinical response was measured only by the ceasation of discharge, the outcome was more impressive. By this assessment the clinical response with clindamycin and gentamicin was 52% of the 140 patients, metronidazole and gentamicin 69%, lincomycin and gentamicin 47%, and the control 24%. The metronidazole and gentamicin regime was significantly more effective than the other regimes and it is suggested for use in prophylactic treatment of CSOM patients undergoing surgical procedures.

[Leukocyte transfusion in complex treatment of acute pulmonary suppurations].

The authors studied the effect of transfusions of a leukocyte suspension on the main clinico-immunological values of homeostasis in 56 patients with acute pyodestructive diseases of the lungs. They found that inclusion of leukocyte transfusions in the complex of therapeutic measures produced a marked clinical effect, reduced the term of treatment, and allowed successful control of endotoxicosis and immunodeficiency in patients with pulmonary suppuration. Immune leukocytes obtained according to a specially developed method had a more marked immunocorrective effect.

[Specificities of bacterial death in culture and suppurative wounds].

Electron microscopic autoradiographic study has been performed. In tissue sections, obtained from suppurative wounds, diploforms of cocci and bacilli have been observed. In the former cells DNA and RNA synthesis was normal, however, in the latter cells no synthesis was detected. Such diploforms were not observed in fresh and exhausted cultures. Possible existence in the body of a factor directly injuring bacterial genome is discussed.

[Anaerobic bacteria in intracranial suppuration].

In four cases of intracranial suppurative disease, studied in the laboratory of Anaerobic bacteria of Institute Pasteur of Algiers, three cases are caused by anaerobic bacteria and one remain sterile. Two of them are empyema with unknown origin and the others are brain abscess with bilateral chronic ear infection to one. In the three cases, gram negative anaerobic bacilli were grown in culture: Bacteroides asaccharolyticus, Bacteroides vulgatus and Bacteroides ruminicola brevis. In two cases, the bacilli are associated with gram positive cocci: Streptococcus intermedius (anaerobic bacteria) and Streptococcus NG (facultative anaerobic bacteria).

[A method for assessing the patient's clinical status and for predicting the clinical course of suppurative-inflammatory processes using chemiluminescence].

A new method for the assessment of the severity of the patients' status and prediction of the clinical course of suppurative-inflammatory processes is described, making use of the peripheral blood luminol-dependent chemiluminescence. Early assessment of the patient's condition and prediction of the disease course helps correctly choose the treatment strategy. The technique is simple and rapid, recommending its wide clinical application.

[Suppuration of old open cavities. Radical treatment].

The use of small, Davis-type, cutaneous grafts, enabled drying up of draining cavities where suppuration persisted despite all treatment. At the same time, hearing was improved by a cartilage arch from the septum which had vibratory and anti-retractile effects.

[Status of humoral defense mechanisms of the middle ear in young children with acute suppurative otitis media].

The concentration of lysozyme and immunoglobulins was measured in serum and middle ear secretion of young children suffering from acute suppurative otitis. It was found that the local synthesis of lysozyme increased. The high concentration of lysozyme and low concentration of immunoglobulins of all classes in the middle ear secretion of children aged up to three years can be associated with an immature local humoral immune response.

Neutrophil function and humoral immunity in children with recurrent infections of the lower respiratory tract and chronic bronchial suppuration.

Neutrophil motility and superoxide anion production and serum immunoglobulin levels were assessed in 51 children with recurrent infections of the lower respiratory tract and chronic bronchial suppuration, not due to anatomic or functional cause. In 26 children (50.9%), a significant defect of the immunologic defenses, likely to be responsible for the disease, was observed. A precocious diagnosis and an adequate therapy could result, at least in some patients, in the prevention of progressive lung damage.
