[Tension relief in nerve sutures].

In short defects up to 25 mm following injuries of peripheral nerves, a method of suturing is outlined using two PDS-ribbons which are sutured to the epineurium, reducing tension at the site of coaptation. The ribbons are 20 mm long and 1 mm wide. They distribute and reduce the tension at the suture site and permit early mobilisation. They induce a restricted scar on the epineurium, maintaining tension relief. Of 105 nerves sutured with this method results have been obtained in 53 cases with an average follow-up time of at least 12 months. The overall results: Median nerve (N = 28) S3-4 64.5%, M3-5 82%. Ulnar nerve (N = 25) S3-4 77%, M3-5 84.5%. Clean cuts had the best results, secondary sutures (with an average interval of 30 days) were somewhat better than primary sutures.

Anterior cruciate ligament reconstruction and augmentation with PDS graft.

The article briefly explains the biomechanical properties of the PDS and the most important ways of its use in anterior cruciate ligament (ACL) reconstruction. The different techniques of ACL reconstruction with gracilis and semitendinosus tendons are outlined. The authors explain their technique of ACL reconstruction with gracilis and semitendinosus plus PDS-band. The possible variations and pitfalls are pointed out. The results of this surgery are expounded.

[Prevention of adhesions in the middle ear].

In certain cases of tympanoplasty implantation of silicone sheets is recommended to prevent postoperative adhesion. However, many surgeons feel that silicone would not be the best material in these cases. Here we recommend using PDS sheets instead of silicone.

Investigation of microcirculatory changes in the duodenum of dogs caused by surgical suture materials.

Surgical suture materials play an important role in the safe performance of surgical interventions. In our experiments we made an attempt to investigate what microcirculatory changes result from pulling the thread through the wall of the duodenum in Lembert stitches by including 3 kinds of absorbable and non-absorbable suture materials each (Catgut, Dexon, PDS, Silk, Ethibond, Ethilon). Research is still in the pre-experimental stage. In the long run, we hope to enrich the description of these suture materials by some new details. It could help prevent suture insufficiency, facilitate wound healing and thus, improved surgical safety.

Breaking strength and diameter of absorbable sutures after in vivo exposure in the rat.

Although absorbable sutures are commonly used in clinical practice, the rate of decay of strength in various tissues has not been studied. The purpose of this study was to assess breaking strength (BS) and diameter of monofilament (chromic gut, polydiaxanone, Maxon, Monocryl) and multifilament (Vicryl, Dexon, Polysorb) absorbable sutures implanted in various sites and measured at specific time intervals. A 15 cm length of 4-0 suture from a single lot of each material was implanted in the pleural space, rectus abdominus muscle, subcutaneous tissue, intravascular space, peritoneal cavity, and stomach lumen in the rat. A precipitous decrease in BS was noted in all multifilament sutures after 7 days, and in chromic gut and Monocryl sutures after 1 day. Polydiaxanone and Maxon sutures maintained the highest BS over the 28-day period, 71 per cent and 59 per cent of their initial BS, respectively. Suture diameter remained essentially unchanged except for chromic gut and the multifilament sutures which exhibited increased diameter. This increase was attributed to inflammatory tissue infiltration.

[Dislocation of clips of the cystic duct stump].

The security of six different laparoscopic clips was investigated using 60 kryopreserved specimens of human cystic ducts. After fixation of the cystic duct the cross placed clips were distracted in axial direction and the maximum forces to remove the clip were registered. The mean dislocation-force was 4.6 Newton for titanium clips and 9.3 Newton for absorbable clips with a highly significant difference (p < 0.001, Wilcoxon-test). Additional to the advantage of complete biodegradibility these data favour the use of absorbable clips, because of a better hold force compared to titannium clips.
