[Effect of chloridine on some biochemical indices in pregnant rats].

Experiments conducted on rats demonstrated that introduction of chloridine (35 mg/kg) on the 13th day of gestation causes maldevelopment of the fetuses in 81.5 per cent of the cases. The blood serum of female-rats receiving the teratogenic compound showed a change in the content of the pre-albuminic and albuminic fractions, the appearance of an unusual proteinic band in the beta-globulins zone and also perversion of the isoenzymatic spectrum of the serumal lactate-dehydrogenase, as well as a 2.9-fold increase of its specific activity.

Aubuku v. The State, 29 July 1987.

The Supreme Court of Justice of Papua New Guinea in this case set out guidelines for sentencing in rape convictions. These guidelines follow principles expressed in the English Court of Appeal decision R. vs. Billam (1986). They are as follows: "1) the offence is a serious crime which is to be punished by an immediate punitive custodial sentence other than in wholly exceptional circumstances; 2) for rape committed by an adult without any aggravating or mitigating features, a figure of five years should be taken as the starting point; 3) for rape committed by two or more persons acting together, or by a person who has broken into or otherwise gained access to a place where the victim is living, or by a person who is in a position of responsibility towards the victim, or by a person who abducts the victim and holds her captive, the starting point should be eight years; 4) for rape committed as part of a concerted campaign, where the accused represents more than an ordinary danger, a sentence of 15 years or more may be appropriate; 5) for rape committed in circumstances which manifest perverted or psychopathic tendencies or gross personality disorder, and where if the accused is likely, if at large, to remain a danger, a life sentence will not be inappropriate; 6) where any one or more of the following aggravating factors are present, the sentence should be substantially higher than the suggested starting point; a) violence over and above the force necessary to commit rape, b) use of a weapon to frighten or wound the victim, c) the rape is repeated, d) the rape has been carefully planned, e) the accused has previous convictions for rape or other serious offenses of a sexual or violent kind, f) the victim is subjected to further sexual indignities or perversions, g) the victim is either very old or very young, h) the effect upon the victim, whether physical or mental; and 7) matters which may be relevant to mitigation include: a) a plea of guilty, b) the age of the defendant, and c) the degree of involvement of the defendant in the planning and carrying into effect of the crime, but do not include d) the fact that the victim may be considered to have exposed herself to the danger by acting imprudently, and e) the previous sexual experience of the victim."

[Psychopathological study of parents who abuse their children].

After recalling the studies of French authors, especially those of P. Parisot and L. Caussade at the Congress of Legal Medicine in Nancy (1929), and more recent, those of the Nancy school, and, in Paris, those of P. Straus and A. Wolf, of L. Kreisler and P. Straus, the author studies Anglo-Saxon research on subject, particularly that undertaken in the U.S.A. The study by Steele and Pollock on the psychology of parents of battered children has shown anomalies with an infantile characteristic in the psychology of some parents (apparently having no problems and living in easy circumstances, but demanding of their children a constant reassurance which the latter are unable to give, hence a permanent frustration leading to agressivity and actual tormenting. Eight observations are presented, covering the whole range of clinical forms observed with parents of battered children: psychopathic personality with perverse sado-masochism, affective immaturation with decompensation during pregnancy, lack of psycho-affective balance with aggression upon some of the children (target-children), a need for domination in the form of tyrannical matriarchate, nevropathy with aggressive perfectionism, and finally subnormal personality with nevropathy, the latter categories leaving a hop for a possibility of attaining transformation through analytical psychotherapy, such as practised by the author in one of his cases.

[A case of autoerotic fetishism: psychodynamic evaluation (author's transl)].

This paper presents a psychodynamic evaluation of a case of fetishism, whose main features are the homosexual or autoerotic choice of the fetish (hair), social and occupational maladjustment and occurrence of epysodes of psychotic depression. After a survey of the literature, with particular emphasis on psychoanalitic and psychodynamic approach, the authors discuss the diagnostic problems presented by this case and decide for a diagnosis works as a largely unsuccessful attempt to escape psychotic regression. In contrast with the situation which prevails in most cases of fetishism, there is no satisfactory adjustment to social and occupational life; the mecanism of ego-splitting is found to be impaired. The authors stress the importance of Kohut's recent contributions to the study of the early phases of narcissism for a better understanding of the differential diagnosis between psychosis, character disorders and perversions, Kohut's narcissistic personality disorders and "borderline syndrome".

[Naked on the lawn. The sexual life of 60-year-old Danish women].

INTRODUCTION: The sexuality of older women has attracted little scientific interest. In a literature search only 29 fairly representative cross-sectional studies were found. There was a single population study of elderly Danish women. The aim of our study was to describe the sexual life of a group of 60-year-old women from the Copenhagen area. MATERIAL AND METHODS: A random sample of 122 women was taken from a cohort found to be fairly representative of 60-year-old women in the County of Copenhagen. The women were asked about their attitudes and sexual experiences in a semi-structured interview. Socio-demographic data were collected. RESULTS: Half the women felt sexual desire once a month or more and had intercourse once a month or more. Two thirds had had no more than three sexual partners, and 85 per cent had a regular partner at the time of the study. The most common dysfunction was vaginal dryness, which one third had. 25 per cent said they seldom or never achieved orgasm. Most of the women had experienced different sexual practices and were tolerant toward prostitution and pornography, but took exception to a number of perversions. DISCUSSION: These elderly women have developed and maintained their sexual life in step with the changes in the sexual norms occurring during their adult life, despite prejudices against elderly people's sexuality.

Social basis of deviant sexual behaviour: a historical perspective.

Sexual perversions are often a product of civilized life. Most societies permit some degree of deviant sexual behaviour as a relief from stress of routine life. Criteria for labelling such behaviour have changed over a period of time. There have been instances of normalizing deviant sexual behaviour by formation of pressure groups by the deviants e.g. homosexual clubs, particularly in Western countries. Attitude of church on masturbation & homosexuality has fluctuated from forbidding these activities to accepting these as harmless acts. Extra marital sex, premarital sex, homosexuality, fellatio, masturbation have been reported from almost all societies. Swinging and mate-swapping is more prevalent in the West. Social factors associated with deviant sexual behaviour are discussed. Incest lobbies have come up in U. S. Prostitution has also been there in all societies since antiquity. Earlier, prostitutes enjoyed a relatively higher social status. Their degradation started with the dawn of Christianity. In 1960s there was sexual revolution in U. S. with emphasis on free sex. There is evidence of slowing down of sexual revolution with the advent of AIDS. Safe-sex and fidelity are now being emphasized.

Cyclodextrin nephrosis in the rat.

The renal toxicity of the Schardinger dextrins, alpha and beta-cyclodextrin, is manifested as a series of alterations in the vacuolar organelles of the proximal convoluted tubule. These changes begin as an increase of apical vacuoles and the appearance of giant lysosomes. The giant lysosomes characteristic of cyclodextrin nephrosis are notable because of the prominent acicular microcrystals embedded in the lysosomal matrix. Giant vacuoles devoid of acid phosphatase reaction product are found in advanced lesions. The vacuolar apparatus shows advanced changes prior to manifestation of lesions in mitochondria and other organelles. These observations indicate a role of the vacuologenic apparatus in the nephrotic process. Intracellular concentration of toxin via the lysosomal pathway represents a perversion of the physiologic function of the proximal tubule which ultimately leads to cell death.

A psychiatric evaluation of the Kinsey Report.

In Kinsey's investigations there seems to be a tendency to study human sexuality from a biological point of view and to neglect the psychological with which sexuality is intimately involved. He does not sufficiently differentiate between sexual behavior in animals and humans which appear to be similar but which are really very different. His sociological conclusions suffer from this limited approach. Experience with patients strongly suggests that his statistics on women may include gross distortion. From a clinical psychiatric standpoint his concept of perversions is oversimplified. To Kinsey, all sexual activity represents the expression of sexual desire whereas it not uncommonly may be used to relieve tensions which are not of sexual origin.

[A comparative evaluation of the biochemical and psychopathological characteristics in subjects with signs of pedophilia].

RIA was employed to examine 54 test subjects who committed sexual crimes with regard to children of tender age. A syndrome of sexual perversions in the form of pedophilia was discovered in 42 persons. Different patients' groups manifested significant differences in the content of hypophyseal and sexual hormones in the blood serum. A significant lowering of testosterone was revealed in persons with heterosexual pedophilia and with pedophilia without any aggressive and sadistic disorders. They also demonstrated a high prolactin content.

The child's sexual life; a consideration of clinical implications.

A healthy sexual life begins in childhood, and the groundwork for later difficulties also begins in childhood. For the child, sexuality is a rather general, pleasurable excitation, not a specific genital stimulation. Excessive attention to the perineal area, operation upon or injury to the genital area and injections are vicissitudes which may have an unhealthy influence on later sexual development. Healthy and informed parental attitudes are the key here, just as they are in the normal exhibitionism, curiosity and intense emotional attachments to parents. Parents infect children, healthily or unhealthily, with their attitudes. In healthy growth exhibitionism and peeping become transformed in socially acceptable ways. In unhealthy growth, either perversions or strong reactions like over-modesty or shame result. Masturbation is common and transitory in most children. Parents, and especially pediatricians to whom parents turn, have a golden opportunity to direct healthy growth by being well informed about the infant and child's sexual growth and thus be enabled to advise upon or manage the common developmental phenomena with good commonsense and patience. Infants and children do not enter the world possessing the morals, standards or inhibitions of adults.

Etio-pathogenic views on transsexualisms (TS).

Starting from the analysis of six cases of transsexualism (TS) the authors performed complex investigations of all sex levels and identified primary T.S. (disturbances of sexual differentiation), secondary TS (sexual perversions, sexopathy). This distinction appears to the authors as unavoidable for ethico-pathological and legal drive in view of the implications of T.S.--implications able to be analysed from standpoint.

THE FREQUENCY OF LYMPHOGRANULOMA VENEREUM IN PERSONS WITH PERIRECTAL ABSCESSES, FISTULAE IN ANO, OR BOTH. WITH PARTICULAR REFERENCE TO THE RELATIONSHIP BETWEEN PERIRECTAL ABSCESSES OF LYMPHOGRANULOMA ORIGIN IN THE MALE AND INVERSION.

Altogether 94 patients with perirectal abscesses and/or fistulae in ano were tested for lymphogranuloma venereum in a Washington, D.C., clinic. They included men and women of low socio-economic status, many of the men being overt psycho-sexual hermaphrodites. The findings were compared with those in a control group of similar sex distribution (females, male homosexuals, male heterosexuals). All persons in the study were tested for lymphogranuloma venereum by the complement-fixation and Frei tests. Homosexuals and persons with rectal lesions were also examined by proctoscopy.Among other findings, a significantly higher frequency of Frei reactors or positive complement-fixation reactions was observed among patients (both male and female) with abscesses than among the controls and among male homosexuals than heterosexuals.The author concludes that lymphogranuloma venereum should be excluded in the differential diagnosis of perirectal abscess and fistual in ano, that homosexuals should be routinely tested for lymphogranuloma venereum, and that sexual perversion should be considered in male patients with ano-rectal disease of lymphogranuloma venereum origin.

Legal anxieties and end-of-life care in nursing homes.

Many persons spend their final days as nursing home residents. It has been suggested that one set of factors powerfully and unfavorably influencing the quality of end-of-life (EOL) care provided in American nursing homes involves the anxieties that nursing home providers experience regarding potential negative legal entanglements and repercussions associated with the provision of EOL care to their residents. This article critically examines the hypothesis that the quality of EOL medical care provided in nursing homes often is skewed in a perverse way because providers are driven unduly by legal apprehensions. The author offers practice and policy recommendations for trying to resolve or mitigate the tension present between legally defensive practice (real or perceived) by nursing homes, on one hand, and ethically optimal EOL care, on the other.

Evaluating the role of confounding in pharmacoeconomic studies.

Confounding may be present in nonrandomized etiological research involving human populations. It can result in erroneous conclusions about the effect of exposure on a disease outcome or about any form of causality between predictors and outcomes. Confounding can wholly or partially account for the apparent effect of the risk factor under consideration or mask the underlying, true association. Not controlling for the effects of confounding can lead to biased results, thus compromising the validity of study conclusions. The three goals of this article are: (1) to define a confounder or a confounding variable, (2) to discuss strategies for controlling the effects of confounding, and (3) to illustrate the perverse effects of confounding with the help of an example.

About interpretation of self-envy.

I present clinical material from two patients in order to describe self-envy interpretation in borderline structures, where negative therapeutic reaction type of mechanisms are usually present, and where massive and intrusive projective identifications are continuously used with the main unconscious purpose of establishing a perverse collusion with the analyst. In these cases there is also a need for splitting and projection of sadistic superego components, in order to provide a corporeity to the persecutor by placing it outside. Self-envy interpretation may help the analyst to deal with the transferential pressure exercised by these patients, and as a consequence improving the 'working space' and providing a better analytical objectivity.

Quality measurement of care for people with type 2 diabetes in Tayside, Scotland: implications for the new UK general practice contract.

BACKGROUND: The new United Kingdom general practice contract proposes that up to a third of general practitioners' income will come from achieving quality targets. AIM: To examine selected quality markers in terms of their robustness to case-mix variation and chance effects, and in the attribution of quality to practices. STUDY DESIGN AND METHODS: Data were extracted from a population-based diabetes clinical information system in Tayside, Scotland, for patients with type 2 diabetes registered in 67 practices with complete ascertainment. RESULTS: Most practices would have received relatively high levels of payment for the process measures examined. Outcome measures appeared more challenging. Case-mix adjustment for age, sex, and postcode-assigned deprivation altered measured performance by up to 7%, but payment by up to 14%. Despite no strong evidence of any real difference in quality, chance effects meant that there was greater apparent variability for smaller practices from year to year. Hospital attendance was common, but highly variable between practices. CONCLUSION: Case-mix adjustment to allow fairer comparison is routine in national performance indicators, and ignoring it risks making the new contract quality framework inequitable. Because of chance effects, smaller practices may have greater year-to-year variability in income. Reflecting National Health Service structure, the new contract provides no incentives for integrated care and offers a perverse incentive to refer more patients to hospital. There are trade-offs between the validity of measures, and the cost and bureaucracy of collecting data. The planned evaluation of the new contrast should examine the effectiveness and equity of the quality framework, and rapidly act on deficiencies found.

[Electrically stimulated olfactory evoked potential in olfactory-lesioned rabbit].

OBJECTIVE: To develop olfactory dysfunctioned animal model by injuring olfactory blub(s) and to investigate the characteristics of olfactory evoked potential. METHODS: The changes of OEP in rabbits were observed at 24 h, 48 h and 1 w after olfactory bulb(s) were electrolytically injured. The pathological and ultrastructural changes of olfactory mucosa and olfactory bulb were also observed. RESULTS: When unilateral olfactory bulb was damaged, N2 vanished with elongated latencies and lowered amplitudes of N1 and P1. When both olfactory bulbs were damaged, N1 and N2 were absent, only P1 could be recorded. The latency was extended obviously and the amplitude had a big change. Histopathology and ultrastructure showed that there were a lot of inflammatory cells and degenerated neurons scattered around OB 24 hours to 48 hours after injure. It was observed that the olfactory cilia had distraction perversion and colliquefaction changes. CONCLUSION: Different degree of olfactory bulb injury caused different effect on OEPs. These changes of potential were based on the histopathology and ultrastructure.

Implications of the Precautionary Principle: is it a threat to science?

Scientific research is of proven value to protecting public health and the environment from current and future problems. We explore the extent to which the Precautionary Principle is a threat to this role for science and technology. Not surprisingly for a relatively simple yet still incompletely defined concept, supporters of the Precautionary Principle come from different viewpoints, including a viewpoint that is at least uneasy with the role of science, and particularly its use in risk assessment. There are also aspects of the Precautionary Principle that inherently restrict obtaining and using science. The Hazardous Air Pollutant (HAP) provisions in the US Clean Air Act Amendments are an example of the Precautionary Principle, which both shifted the burden of proof so that the onus is now on showing a listed compound is harmless, and required maximum available control technology (MACT) instead of a primarily risk-based approach to pollution control. Since its passage in 1990 there has been a decrease in research funding for studies of HAPs. Other potential problems include that once MACT regulations are established, it may be difficult to develop new technological approaches that will further improve air pollution control; that by treating all regulated HAPs similarly, no distinction is made between those that provide a higher or lower risk; and that there is a perverse incentive to use less well studied agents that are not on the existing list. As acting on the Precautionary Principle inherently imposes significant costs for what is a potentially erroneous action, additional scientific study should be required to determine if the precautionary action was successful. If we are to maximize the value of the Precautionary Principle to public health and the environment, it is crucial that its impact not adversely affect the potent preventive role of science and technology.
