[Relationship between lipometabolic disorder in stroke with TCM syndrome differentiation and typing].

OBJECTIVE: To analyse the relationship between lipometabolic disorder in stroke with TCM Syndrome Differentiation and Typing. METHODS: The serum lipid, lipoprotein and apo-lipoprotein level of TCM treated group (142 cases) and control group (100 cases) were determined. The patients were subdivided into three Type-groups according to TCM Syndrome Differentiation: Phlegm-Dampness Stagnation (PDS), Yin Deficiency with Yang Hyperactivity (YDYH) and Qi Deficiency with Blood Stasis (QDBS) group. RESULTS: The results showed that the level of TC, TG, HDL, LDL, apoA1, apoB and apoA1/apoB were significantly different in PDS and QDBS as compared with control group (P < 0.05, P < 0.01, respectively), while lipometabolic disorder of PDS was most serious in TCM Syndrome Differentiation of stroke. The level of TG, apoA1/apoB were significantly different in YDYH in comparing with control group (P < 0.05). While the level of TC, HDL, LDL and apoB were insignificantly different than those of control group (P > 0.05). CONCLUSIONS: This study showed that lipometabolic disorder of stroke were correlated with Syndrome Differentiation of TCM. Stroke with PDS and QSBS should give removing Phlegm-Dampness, dredging Meridian and removing blood stasis treatment besides routine treatment.

Survival time of Class II molar restorations in relation to patient and dental health insurance costs for treatment.

The aim was to evaluate the median survival time (MST) of direct molar class II restorations (glass ionomer, composite, amalgam) in the Nordic countries and the initial cost, as well as, the theoretical cost per year of function of treatment for patients, Social Insurance Office (SI), and total cost, at Public Dental Services (PDS) in Sweden. Restoration longevity studies conducted in general practice settings in the Nordic countries were used to calculate the MSTs of class II restorations. The initial costs were based on fee schedules from all PDS in Sweden. The MSTs of class II molar restorations in Nordic general practices were shortest for glass ionomer and longest for amalgam. Glass ionomer molar class II restorations had the lowest and composite restorations had the highest initial total cost at PDS in Sweden. The highest theoretical cost per year of function was seen for composite restorations. Amalgam restorations seem to have the longest functional lifetime in Nordic general practices and the lowest theoretical cost per year of function for the patients at PDS in Sweden.

[Protective effect of ginsenosides on human peritoneal mesothelial cell against lactate-based peritoneal dialysis solutions--induced injury in vitro].

OBJECTIVE: The aim of this research was to observe the influence of ginsenosides (GS) on viability and proliferation of human peritoneal mesothelial cell (HPMC) which were inhibited by L-PDS in vitro. METHOD: Mesothelial cells were isolated from human omental specimens by trypsin disaggregation and established a stable HPMC culture model. The viability and capacity of proliferation of HPMC were assessed by lactate dehydrognase (LDH) release and tetrazolium salt colorimetry assay (MTT assay). RESULTS: Compared with the control group, the release of LDH in L-PDS groups (2.5%, 4.25% glucose) significantly increased. But there was no difference between the two glucose concentrations. As HPMC pre-exposed to L-PDS prolonged, the viability of cells decreased gradually. GS decreased the release of LDH and increased the cells viability (P < 0.01), as well as enhanced the proliferation of HPMC. CONCLUSION: The results suggest that GS has some protective effects on cell viability and proliferation of HPMC inhibited by L-PDS in vitro.

[Metabolism of ginsenoside Rb1 and panaxadiol saponins by fungi].

AIM: To study the metabolic process of ginsenoside Rb1 (G-Rb1) and panaxadiol saponins (PDS) by fungi. METHODS: Ten strains of fungi were incubated with G-Rb1 and PDS at a certain temperature with shaking. A portion was taken out at different time and mixed up with butanol. The butanol extract was analysed by thin layer chromatography (TLC), high performance liquid chromatography (HPLC) and electrospray ionazition mass spectrometry (ESI-MS). RESULTS: It was found that there were ginsenoside-Rd (G-Rd), ginsenoside-F2 (G-F2), compound K (CK) and 20(S) protopanaxadiol (Ppd) metabolites beside the prodrug G-Rb1 induced by fungi (No. 1, 2, 3, 6, 8, 9). CONCLUSION: The six strains of fungi have different degrees of ability to metabolize G-Rb1 and PDS. The possible metabolic process could be as follows: G-Rb1 (or PDS)-->G-Rd-->G-F2-->CK-->Ppd.

Use of power Doppler sonography for differential diagnosis of small hepatocellular carcinoma and adenomatous hyperplastic nodule.

OBJECTIVE: To evaluate applicability of power Doppler sonography (PDS) in differential diagnosis of small hepatocellular carcinoma (SHCC) and adenomatous hyperplastic nodule (AHN). METHODS: Twenty-two cases of SHCC and 15 cases of AHN were investigated by PDS and the findings were compared with those of digital subtraction angiography (DSA). RESULTS: The rates of arterial and portal flow in an afferent tumor vessel were 86.4% and 40.9% in SHCCs, respectively. The rate of portal flow in an afferent tumor vessel was 60.0% in AHNs, while no arterial flow was detected (P < 0.01). In addition, PDS revealed a constant flow in an efferent tumor vessel (50.0%) continuing to a portal branch in 10 (45.5%) of the 22 SHCCs cases to a hepatic vein in 1 (4.5%) of the 22 SHCCs, but to nothing else in the AHNs (P < 0.01). CONCLUSIONS: Power Doppler sonography is of value in distinguishing SHCC from AHN, and arterial afferent tumor vessels from constant flow efferent tumor vessels at PDS.

[Evaluation of sternal deformity after pediatric minimally invasive cardiac surgery].

Sternal shape is one of the most important esthetic factors of the chest appearance after pediatric minimally invasive cardiac surgery (pMICS) as well as length of skin wound. We evaluated the grade of postoperative sternal deformity in 20 patients who underwent total repair of pediatric congenital heart disease [atrial septal defect (ASD): 17, ventricular septal defect (VSD): 2, partial anomalous pulmonary venous connection (PAPVC): 1] with minimal skin incision and lower partial median sternotomy. The sternum was closed with stainless wire in 3 patients, with absorbable polydioxanone (PDS) cord in 5 patients, with combined use of reabsorbable radiolucent poly (L-lactate) acid sternal pin and absorbable PDS cord in 12 patients. The evaluation of postoperative sternal deformity was made according to the vertebral index (VI) and frontosagittal index (FSI) in 3 groups with each sternal closure method. VI and FSI of the 3 groups showed no significant difference. Sternal deformity in the group with sternal closure with PDS cord group was more severe than that in other 2 groups. The combined use of sternal pin with PDS cord offered the most sufficient fixative strength for sternal closure.

Psychomotor disadaptation syndrome. A new clinical entity in geriatric patients.

Psychomotor Disadaptation Syndrome (PDS) is characterized by backward disequilibrium in a sitting and standing position, reactional hypertonia, an alteration of postural reactions, gait modifications, and fear of falling. PDS may be the result of a loss of postural reserves, reaching a frailty threshold that does not allow subjects to maintain an adequate functional level. This clinical picture may be associated with frontal-subcortical dysfunction in which microangiopathy could play a key role. PDS may be triggered by numerous factors, such as cardiovascular and metabolic diseases, falls, and bed rest. In addition to medical and neurological evaluations, a specific rehabilitation program is one of the most important aspects of the management of patients showing PDS.

Correlative microscopy of Purkinje dendritic spines: a field emission scanning and transmission electron microscopic study.

Purkinje dendritic spines (Pds) of mouse cerebellar cortex were examined by field emission scanning electron microscopy (FESEM) and by transmission electron microscopy (TEM) using ultrathin sections and freeze-etching replicas, to study their three-dimensional features and intramembrane morphology. FESEM showed unattached mushroom-type, elongated and lanceolate Pds separated by 100-500 nm on the dendritic shaft surface. High resolution FESEM showed 25-50 nm globular subunits at the spine postsynaptic density corresponding to the localization of postsynaptic proteins and/or postsynaptic receptors. TEM images of ultrathin sections showed gem-like, mushroom-shaped, lanceolate and neckless or stubby spines. Freeze etching replicas exposed postsynaptic intramembrane particles that can be correlated with the globular subunits observed at high resolution FESEM. Parallel and climbing fiber endings were observed making asymmetric synaptic contacts with the Pds heads. Simultaneous contacts with the necks and heads were also found. The variety of Pds shapes were interpreted as spine conformational changes related with spine dynamic, and spine plasticity.

[Observation on therapeutic effect of jianpi huoxue herbs combined with chemotherapy in treating post-operational colonic cancer patients].

OBJECTIVE: To observe and compare the therapeutic effect of Jianpi Huoxue herbs (JPHXH) combined with chemotherapy (CT) in treating post-operational colonic cancer patients with Pi deficiency Syndrome (PDS). METHODS: Adopting randomized control trial method, 64 patients were divided into two groups. The treated group (n = 43) received JPHXH plus CT and the control group ( n = 21 ) received CT alone. A treatment course of 3 months was applied to both groups. Therapeutic effect, changes of PDS and incidence of adverse reaction in the two groups were observed. RESULTS: The tumor remission rate in the treated group and the control group was 39.5 % and 33.3 %, respectively. The effective rate on PDS in the treated group and the control group was 72.1% and 19.0%, respectively, showing significant difference (P < 0.01). The incidence of adverse reaction was lower in the treated group than that in the control group (P < 0.05). CONCLUSION: JPHXH is effective in treating post-operational colonic cancer patients with PDS and relieving the adverse reaction of chemotherapy to certain extent.

[Tendon holding capacities of the suture materials used in repairing Achilles tendon rupture].

OBJECTIVES: We evaluated tendon holding capacities of suture materials that are commonly used in repair of Achilles tendon ruptures. METHODS: Achilles tendons of 60 sheep were removed by incisions 2-cm proximal to the calcaneal insertion and 1-cm distal to the musculotendinous junction. The tendons were randomly divided into six groups and sutures were placed with the Kessler technique at the distal end of the tendons using one of the following suture materials: 2 polydioxanone (PDS), 1 PDS, 2 Vicryl, 1 Vicryl, 2 Ethibond, and 1 Prolene. The distal end of the suture material was left free. Each specimen was mounted in an Instron machine, with the tendon being placed proximally and the suture material distally. The system was loaded with a displacement rate of 20 mm/min. RESULTS: Failure of all the specimens was due to pull-out of the suture material through the tendon. There were no failures due to suture breakage. The highest and the lowest tendon holding capacities were found with 2 PDS and 2 Ethibond sutures, respectively. CONCLUSION: Following Achilles tendon repair, the healing period, in particular the first three weeks, is precarious for pull-out of the suture material through the tendon. Thus, tendon holding capacity of the suture material is an important factor for the strength of the repair. Among the tested suture materials, 2 PDS was found to have the highest tendon holding capacity.

Cost of composite and glass ionomer class II molar restorations and theoretical analyses of cost per year of function at public dental services in Sweden.

The aim was to evaluate the cost of direct composite and glass ionomer class II molar restorations, and the theoretical cost per year of function, at Public Dental Services (PDS) in Sweden, years 2000 and 2005. Costs for patients, Social Insurance Offices (SI; Försäkringskassan), and total cost, were calculated based on fee schedules from all PDS in Sweden. Theoretical cost per year calculations were based on the median survival times (MST) of failed direct composite and glass ionomer class II molar restorations, derived from a set of clinical studies conducted in Nordic general practices. Due to lack of national statistics from SI, the number of direct restorations including more than one surface, made in adults, in general dentistry at PDS in the county of Halland were studied. From the year 2000 to year 2005, the total cost of composite class II molar restorations increased by 25%, whereas the total cost of glass ionomer restorations more than doubled. Theoretical calculations implied a higher cost per year of function for composite restorations in year 2000, whereas in year 2005, glass ionomer restorations had a higher cost per year of function. The cost of direct composite and glass ionomer class II molar restorations increased from year 2000 to 2005, at PDS in Sweden. In the context of planning public health care funding, theoretical models for cost prediction may prove useful.

Cytokine release syndrome induced by the 145-2C11 anti-CD3 monoclonal antibody in mice: prevention by high doses of methylprednisolone.

The hamster mAb 145-2C11 specific for the CD3 complex of murine T lymphocytes shares many properties with OKT3, including the induction of T cell activation. In vivo, the injection of 145-2C11 entails a variety of pathologic changes in relation to the systemic release of cytokines. We tested the effects on this cytokine release syndrome of different doses of methylprednisolone (m-PDS) given at various intervals of time before the 145-2C11 mAb. The administration of high doses of m-PDS (50 mg/kg) 2 to 3 h before the mAb resulted in an almost complete inhibition of the systemic release of TNF-alpha, IL-2, and IL-6. As far as the pathologic changes are concerned, the hypothermia, the acute renal tubular necrosis, and the fatty infiltration of the liver were completely prevented whereas the hypoglycemia was only partially attenuated. The protective effect of m-PDS on the toxicity of 145-2C11 was confirmed by the reduction of the mortality rate among galactosamine-sensitized mice. The inhibition of the release of cytokines by m-PDS did not affect the immunosuppression triggered by 145-2C11 as assessed by the CTL activity against alloantigens measured 48 h after the injection of the mAb. We conclude that the administration of very high doses of glucocorticoids 2 to 3 h before 145-2C11 prevents the release of cytokines and attenuates the acute toxicity of the mAb. Similar protocols could allow mitigation of the cytokine-release syndrome induced by the OKT3 mAb in man.

Survival and in vitro fertilization potential of sperm following washing and incubation with different protein supplements.

Three protein supplements, preovulatory donor serum (PDS), Cohn Fr V human serum albumin (HSA), and highly purified HSA were used for washing sperm. The ability of the different protein supplements to support sperm survival and in vitro fertilization of zona-free hamster oocytes was compared following 18 hours of incubation. Sperm motility, velocity, linearity, and motility index were the same for the various proteins. The percentage of sperm with a functional plasma membrane was significantly greater (P less than .02) for highly purified HSA than for PDS or Cohn Fr V HSA. The percentage of zona-free hamster oocytes penetrated and penetrations per oocyte were two times greater (P less than .0001) for PDS and Cohn Fr V HSA compared with highly purified HSA. These data indicate that (1) proteins can possess an equal ability to support sperm survival, yet differ in their ability to support capacitation and the acrosome reaction; (2) HSA preparations derived from dissimilar purification methods may differ in their ability to support in vitro fertilization; (3) PDS and certain types of HSA possess an equal ability to support in vitro fertilization.

[Reducing tension in sutures of peripheral nerves].

A method is presented to reinforce normal suturing techniques in median and ulnar nerve repairs at the level of the forearm or wrist. If the distance between the stumps does not exceed 20 to 30 mm the coaptation can be protected from tension by two ribbons of polydioxanone (PDS) which are sutured to the epineurium bridging the site of the suture of the nerve. PDS ribbons proved to give remarkable support to experimental nerve repairs using epineural, perineural or Tsuge suture techniques. In further experiments with tibial nerves of rats the PDS ribbon did not interfere with the nerve function or with axon sprouting. Follow-up investigations of thirty-two median and ulnar nerve injuries treated with primary or secondary interfascicular sutures using PDS ribbons of 20 mm length and 1 mm width to protect the suture line revealed satisfactory reinnervation. The late results with this method were superior to a comparable group treated with autografts.

[Ultrastructure and elemental composition of pancreatic stones].

Four patients with pancreatic duct stones (PDS) and one with calcium deposits (CD) were diagnosed and surgically cured. The ultrastructure and elemental composition of PDS were studied by scanning electron microscopy and energy dispersive X-ray fluorescence. The results indicated that the outer shell of PDS, with dense texture, contained 96.29% of calcium (Ca), while the amorphous reticular inner nidus mainly contained sulphur (S) and chlorine (Cl). CD and PDS were manifestations in the process of pancreatic stones (PS) formation at different stages. As regards the formation of PS, the depressed gene expression played a key role in the reduction of PSP (Pancreatic Stone Protein), which made pancreatic acini reduce PSP biosynthesis rates. The consequence arising therefrom was that the supersaturated calcium carbonate (CaCO3) in pancreatic juice became inadequately inhibited. Thus the calcium carbonate became crystallized and deposited onto the reticular inner protein nidus (RIPN), which developed from the compilation of micro-protein-plugs that had grown from pathologically hyperconcentrated total proteins (mainly containing lactoferrin) in pancreatic juice. The RIPN gave a basis on which the calcium carbonate precipitated and formed PS.

The effect of a penetrating biodegradable implant on the growth plate. An experimental study on growing rabbits with special reference to polydioxanone.

This study examined the fate of a biodegradable polydioxanone (PDS) implant in growing bone in rabbits. In 20 immature rabbits, a PDS rod 2.0 mm in diameter and 25 mm in length was driven into a drill hole of equal bore in the intercondylar portion of the right femur across the central portion of the growth plate. In another 25 immature rabbits, a PDS rod 3.2 mm in diameter and 25 mm in length was driven into a corresponding drill hole using the same technique. The follow-up intervals were three, six, 12, 24, and 36 weeks. The distal femoral growth plates of both femora were analyzed by roentgenographic, microroentgenographic, oxytetracycline-fluorescence, histologic, and histomorphometric studies. Histomorphometric studies showed clear changes at the site of injury and appeared to be an excellent means of providing an exact numerical description of the different cellular areas of the growth plate in experimental studies. A transphyseal PDS implant 2.0 mm in diameter did not cause any permanent growth disturbance of the femur in a growing rabbit. An implant 3.2 mm in diameter caused a growth disturbance similar to a drill hole of equal bore.
