Test-retest stability of injured workers' MMPI-2 profiles.

Ninety-four workers completed the Minnesota Multiphasic Personality Inventory--2 (MMPI-2) on 2 separate occasions, with an average lag of 21.3 months (SD = 14.1, range = 2-75), within the context of a psychological assessment after suffering an injury due to crime or accident. MMPI-2 profiles were moderately consistent, with correlation coefficients ranging from .61 to .73 for clinical scales, from .52 to .80 for supplementary scales, from .65 to .78 for content scales, and from .32 to .73 for the Personality Psychopathology Five scales (A. R. Harkness, J. L. McNulty, & Y. S. Ben-Porath, 1995). The results suggest that the MMPI-2 provides consistent and stable results across time in injured workers.

Low-dose recombinant IL-2 induces psychological changes: monitoring by Minnesota Multiphasic Personality Inventory (MMPI).

BACKGROUND: Chronic subcutaneous rIL-2 at low doses produces long-lasting immunomodulatory effects and is considered an effective treatment for renal cell carcinoma with marginal activity in malignant melanoma and colorectal cancer. PATIENTS AND METHODS: In this study we evaluated, by Minnesota Multiphasic Personality Inventory (MMPI), the psychological changes induced by rIL-2 in 10 patients with advanced tumors. RESULTS: After 3 months of rIL-2 treatment, 80% of the patients had a significantly increased score on the clinical scale of depression (D) and psychasthenia (Pt) (p<0.01), 70% on the scale of conversion hysteria (Hy) and 60% on the scales of schizophrenia (Sc) and psychopathic deviate (Pd), (p<0.05). These MMPI changes were however not paralleled by disease progression or clinical-overt psychological disease. CONCLUSION: These findings demonstrate that low-dose rIL-2 is a psychoactive treatment, which deserves psychological monitoring The MMPI is feasible for the evaluation of subclinical psychological modifications induced by cytokine immunotherapy.

Exaggerated MMPI-2 symptom report in personal injury litigants with malingered neurocognitive deficit.

Traditional MMPI-2 validity scales, the Lees-Haley Fake Bad Scale (FBS), and the Arbisi and Ben Porath Infrequency Psychopathology Scale (F(p)) were evaluated in 33 personal injury litigants who had failed forced-choice symptom validity testing and other measures of effort in patterns consistent with the Slick, Sherman, and Iverson (1999) criteria for definite and probable malingered neurocognitive deficit (MND). The FBS was more sensitive to symptom exaggeration than F, Fb, and F(p). The definite and probable MND litigants also produced mean elevations on MMPI-2 scales 1, 3 and 7 that were significantly higher than those produced by various clinical groups including non-litigating severe closed head injury, multiple sclerosis, spinal cord injury, chronic pain, and depression. These data suggest that MMPI-2 profiles characteristic of malingered injury differ from those associated with malingered psychopathology.

[MMPI- and BDI-scale-based self-assessment of the psychic condition of patients after a suicidal attempt].

UNLABELLED: Parasuicide (suicidal attempt) is an intentional, self-aggressive behaviour intended to damage the body rather than cause death. About 75% of suicidal attempts are those committed for the first time. The majority of suicidal attempts are impulsive. About 66% of the parasuicide patients said that they had thought about the act for less than an hour beforehand. Only 30% of the patients had been psychiatrically diagnosed before they were admitted to the hospital. After prompt re-diagnosing, the percentage was considerably lower. The aim of the work is to analyse, using Minnesota Multiphasic Personality Inventory (MMPI-DMS subscale) and Beck Depression Inventory (BDI), the self-assessment of the psychical condition of patients hospitalised at the Acute Poisonings Clinic, Nofer Institute of Occupational Medicine, Lodz, Poland, for attempted suicide. The study subjects included 222 patients (148 women and 74 men) treated in 2003. The control comprised 137 volunteers random-selected from among patients visiting Institute's Outpatient Clinic of Occupational Diseases. The psychiatric testing revealed mood and personality disorders, and acute stress disorders. Depression or other psychotic reactions were not diagnosed in any of the cases. Mean MMPI subscale scoring for the women was 10.7 (subjects) and 3.5 (control). The MMPI rating after 6 months since the attempted suicide was 6.03, i.e. by 4.3 scores lower. The corresponding values for the men were 10.1 (subjects) and 3.06 (control), and after 6 months the value was 4.64, by 5.46 scores lower. Mean BDI scale scoring for the women was 10.56 (subjects) and 7.9 (control). The BDI score after 6 months since the poisoning was 7.74, i.e. by 2.83 scores lower. The corresponding values for the men were 10.77 (subjects) and 5.27 (control), and after 6 months the value was by 2.86 scores lower. The results are statistically significant (p<0.001). CONCLUSION: The results show that 6 months after the suicidal attempt, the self-assessment of patients' psychical condition improves, the mood normalises. The suicidal attempt is often an impulsive act intended to relieve the rapidly growing stress.

Assessment of MMPI profiles of male batterers.

Fifty-six adult male batterers completed the Minnesota Multiphasic Personality Inventory (MMPI) as part of the standard screening and assessment process for a court-mandated aggression management treatment program. Data were analyzed via composite-mean and cluster analyses. MMPI results revealed a 4-2 profile for both the composite-mean analysis and the elevated profile revealed by the cluster analysis. Fifty-two percent of the MMPI profiles exhibited Scale 4 as one of the two scales in the high-point pair. Future research must incorporate multidimensional behavioral and personality assessment in order to enhance specific treatment approaches via matching procedures.

Dimensional analysis of the MMPI Alexithymia scale.

Recently, the alexithymia construct has emerged as a heuristically useful personality construct in the explanation of the pathogenesis of a variety of physical illnesses. Research that has evaluated the alexithymia construct, however, has been conducted with little attention to the psychometric properties of the various scales constructed to measure it. This study investigated the underlying dimensions and factor structure stability of the MMPI Alexithymia scale (MMPI-A). The MMPI-A was administered to 552 undergraduates, and the data then were split randomly into two samples. Factor analysis of both samples produced a three-factor solution with little theoretical relevancy to the alexithymia construct. Comparisons of the factor structures from the two samples revealed only moderate congruence, which indicates an unstable factor structure.

Kinetic Family Drawings and MMPI diagnostic indicators in adolescent psychiatric inpatients.

Correlations among Kinetic Family Drawings and MMPI indicators of depressive, anxiety, behavioral, and thought disorders and diagnostic category were estimated for a group of 52 adolescent psychiatric inpatients. No statistically significant values were found between test indicators and corresponding MMPI scales or diagnoses, although MMPI D and Sc scales were significantly related to diagnosis. Results do not support the concurrent or construct validity of the drawings.

Life stress and clinically elevated MMPI scales: gender differences in the moderating influence of coping.

Coping styles were evaluated as moderators of life stress-psychopathology relationships. Five hundred twenty-one undergraduates (271 females and 250 males) completed the Life Experiences Survey, the Coping Strategies Inventory, and the MMPI. To assess maladaptive coping styles, groups were constructed for each of 8 clinical scales (Hs, D, Hy, Pd, Pa, Pt, Sc, and Ma) composed of all subjects with a clinically significant elevation on that scale. The coping styles of these "clinical" groups were compared to the coping styles of "normal" groups, made up of subjects whose life stress scores were approximately the same but whose corresponding MMPI scale scores were within normal limits. To assess adaptive coping styles, a group of effective copers (with high life stress but normal MMPI profiles) was compared to a group of less effective copers (with high life stress but with at least one scale outside of normal limits). All groups were constructed separately for gender. The predicted gender differences and specific coping style-psychopathology relationships were, by and large, found. Coping style differences between the effective and less effective copers were also found. These findings suggest that several coping styles are important moderators of life event stress-psychopathology relationships.

Assessment of PIC and MMPI scales in adolescent psychosis: a caution.

This study investigated the sensitivity of two frequently used multidimensional instruments--the Personality Inventory for Children (PIC) and the Minnesota Multiphasic Personality Inventory (MMPI)--in assessing psychotic states in adolescents. This issue arose as a result of the fact that the PSY scale on the PIC was constructed and cross-validated only on samples of children between the ages of 5 and 12 years. T-score comparisons between psychotic and nonpsychotic samples were performed, as well as Pearson correlations between PIC Psychosis (PSY) and MMPI Schizophrenia (Sc) scale T-scores. T-score comparisons indicated no significant differences between the groups on either PSY or Sc. A nonsignificant relationship between PSY and Sc scale elevations was also found. These results suggest the need for a profile-analytic approach to PIC and MMPI interpretation in identifying psychotic symptomatology in adolescents rather than PSY or Sc T-score elevation in isolation.

Color preference and MMPI scores of alcohol and drug addicts.

Sixty-seven inpatients (12 women, 55 men) treated for alcohol and drug abuse were administered the MMPI and Luescher Color Test (LCT); as required by the test manual, the interpretations are based on a second LCT administration. Scores on Luescher's scales were unrelated to MMPI scores. The rank order of relative preference for each of the 8 LCT cards also was unrelated to MMPI scores. Older patients more often preferred brown and less often black. Female patients showed more preference for purple. The group of patients showed more preference for brown than a control group of 20 normal adults (3 women, 17 men) and also obtained higher (i.e., presumably more pathological) scores on Luescher's total score scale and compensation scale.

Predictive value of MMPI scales on smoking cessation programs outcomes.

Five hundred and fifty-eight cigarette smokers were randomized in 4 groups: acupuncture, behavior therapy, placebo and waiting list. The MMPI scales showed stability across pretest, posttest and a one-year follow-up. Principal components analysis isolated a "depression-psychasthenia" factor accounting for 61% of the variance. Moreover 43% of the subjects had an abnormal MMPI profile. Segmentation isolated predictive factors: a high number of pathological MMPI scales predicted failures in any kind of treatment. Acupuncture yielded better outcomes when the profiles were normal. Behavior therapy and placebo had better outcomes when the anxiety index was abnormal. The study underscores the role of personality factors in tobacco addiction and their influence on cessation programs outcomes.

The delineation of an MMPI symptom pattern unique to lithium responders.

The author attempted to determine whether the Minnesota Multiphasic Personality Inventory (MMPI) would be sensitive to a symptom pattern unique to lithium-responsive patients regardless of diagnosis. Statistical analyses of the MMPI scores of lithium responders, lithium nonresponders, and a group of patients receiving other psychotropic drugs revealed that the depression, psychasthenia, mania, and social introversion scale scores significantly differentiated the lithium response and lithium nonresponse groups. No strong indications of significant difference were found between the MMPI scale scores of the lithium nonresponse and psychotropic groups. The results suggest that there is a lithium-responsive syndrome that exhibits a distinctive symptom pattern regardless of its varying phenomenological features in different individuals.

MMPI and drug treatment in alcohol withdrawal.

Thirty-four inpatients in a mild phase of alcohol withdrawal, exhibiting anxious and depressive symptoms, were administered an MMPI before and immediately after a 4-week drug treatment period. A double-blind design was used; 17 patients were treated with chlordiazepoxide and 17, with thioridazine. The pretreatment MMPI profile was typical of alcoholic patients; namely, peaks on scales 4 and 2. After the fourth week, the MMPI indicated that thioridazine was significantly more effective than chlordizaepoxide. Decreases in 7 of the 10 clinical scales averaged 7.29 T-score points, whereas only scale 6 was significantly decreased for the patients treated with chlordiazepoxide.

Borderline personality disorder: symptomatology and MMPI characteristics.

Using the Schedule for Interviewing Borderlines and the MMPI, 20 subjects diagnosed as borderline and/or schizotypal personality disorder were examined to determine 1) whether having one diagnosis affected the likelihood of having the other, 2) the amount of diagnostic overlap between the two diagnoses, and 3) whether different MMPI profiles could be found. The results indicated some overlap but also some differentiation between the borderline and schizotypal disorders. The modest overlap in diagnostic criteria was consistent with other studies and justifies the continued separation of the two diagnoses. The MMPI may be used as an initial screen to identify persons who warrant further evaluation for a borderline personality disorder but would not be appropriate as a screen for a schizotypal personality disorder.

MMPI assessment of psychopathology in the adopted-away offspring of schizophrenics.

Previous studies completed in Denmark show a higher frequency of schizophrenia-related psychopathology in the adopted offspring of schizophrenics than in matched controls. This study adds a psychometric dimension to the earlier reports with an analysis of psychopathology in the adopted offspring of schizophrenics based on the Minnesota Multiphasic Personality Inventory (MMPI). A combination of MMPI criteria and interview-based diagnosis identifies 22% of the index cases and 6% of the control cases (P less than .01) as having a schizophrenia spectrum disorder. The findings support a genetic hypothesis, but the use of the MMPI does not fully clarify some issues raised in the earlier reports.

Are the MMPI subtle subscales subtle measures of their scales?

This study investigated the ability of the MMPI Wiener-Harmon subtle subscales (on scales D. Hy, Pd, Pa, and Ma) to serve as subtle or unobtrusive measures of their scales. Forty outpatients completed the MMPI under standard instructions, followed by a fake-good or fake-bad instructional set. First, we investigated the paradoxical effect found in the MMPI faking literature (in which, overall, the subtle subscale T-scores change in a direction opposite of the faking instructions) and found that not every subtle subscale shows this effect. Secondly, the subtle subscale T-scores achieved under faking conditions showed no significant relationship to their respective full-scale T-scores achieved under standard conditions. Therefore, our results do not support the Wiener-Harmon subtle subscales as subtle measures of their scales.

MMPI profile characteristics of women with varying levels of normal dissociation.

MMPI-2 correlates of normal dissociation as measured by the absorption subscale items of the Dissociative Experiences Scale (DES) were examined in patients with diagnoses in the dissociative spectrum (n = 119) and not in the dissociative spectrum (n = 97). All patients completed the MMPI-2 and DES and were assigned to subgroups based on a cut-off score of 30 on normal dissociation. Results indicated that normal dissociation was unusually prominent among patients with mental disorders. The MMPI profiles of those with high imbalances of normal dissociation were skewed in the direction of more pathology across a broad range of mental symptoms. This was true for both dissociative and nondissociative patients. These findings provide preliminary evidence that high imbalances in normal dissociation may be related to psychopathology of a general nature.

The Hispanic MMPI-A across five countries.

The Hispanic MMPI-A was developed for use with Spanish-speaking adolescents living in the United States. In order to sample its potential usefulness in other countries, the test was administered to 385 Spanish-speaking adolescents in Colombia, Mexico, Peru, Spain, and the United States. The results showed a high degree of similarity across the five countries on the basic content and supplementary scales. Most scales were within 1/2 standard deviation of the U.S. Hispanic mean, and no scale elevations were greater than T = 65. Slight differences (T = 55-60) were obtained on scale F for males and scales F, Hs, Mf (Peru and Colombia), Pa, and Sc for females. This study would suggest that the Hispanic MMPI-A, with its established norms, is appropriate for adaptation in Spanish-speaking countries other than the United States. Further studies with the Hispanic MMPI-A are recommended to determine its effectiveness in clinical assessment in Spanish-speaking countries.
