Flushing and plasma substance P concentration during infusion of synthetic substance P in normal man.

Six normal subjects received synthetic substance P intravenously in successively increasing doses of 0.5, 1.0, 2.0, 4.0, and 8.0 pmol X kg-1 X min-1. Each dose was given for 20 min. Concentrations of substance P in plasma and infusions were measured radioimmunochemically. The incremental plasma concentration of substance P correlated linearly to the dose infused. Metabolic clearance rate was 0.361 X kg-1 X min-1, and the half-life of exogenous substance P 1.6 min. Lacrimation and sustained flushing were elicited at plasma concentrations above normal range. Transient flushing in spite of continued infusion was seen at 'physiological' levels. Pulse rate and blood pressure did not change significantly.

A double-blind trial with clonidine and a placebo to treat hot flushes.

A double-blind crossover study that compared clonidine and a placebo, which were administered to 19 postmenopausal women who were suffering from hot flushes, failed to demonstrate any statistical reduction in the number of hot flushes. Therefore, clonidine is not recommended as a treatment drug for this symptom.

Clinical Trials as Topic↗

[Clinical thinking and decision-making in practice. A patient with episodes of flushing].

A 74-year-old woman had an 8-year history of spells consisting of facial flushing and swelling, itching and erythema of the hands, rhinorrhoea and gastrointestinal complaints. The ultimate diagnosis was systemic mastocytosis without urticaria pigmentosa. In the clinical decision process the treating physicians used clinical axioms to try to establish a diagnosis, but these axioms were sometimes used inappropriately. In particular, spells accompanied by flushing inappropriately elicited a possible diagnosis of phenochromocytoma. Generalised mastocytosis can occur in the absence of urticaria pigmentosa and is probably an underestimated cause of spell-like complaints.

[Lynestrenol and allylestrenol in the therapy of postmenopausal hot flushes].

In a parallel randomized placebo controlled clinical trial the authors tested two synthetic gestagens--allylestrenol and lynestrenol--in the treatment of postmenopausal flushes. Both preparations were administered during a six-week period in rapidly declining doses. Allylestrenol was administered in initial doses of 30 mg/d, after five days the doses were reduced so that after 15 days a daily dose of 5 mg was reached. The initial dose of lynestrenol was 10 mg/d with a similar gradual decline to 1.25 mg per day. The trial comprised 42 women with menopausal flushes after a natural or artificial menopause (castration). Both preparations improved the subjective condition of the patients and reduced the gonadotropic production significantly better than placebo (p < 0.05). Subjective relief after lynestrenol was recorded after the second week of therapy, in allylestrenol only after the sixth week when the effects of the two preparations were equal and significantly better than after placebo. Suppression of gonadotropin production was similar after both preparation but with a more rapid onset after lynestrenol and a more prolonged effect after allylestrenol even after significant reduction of the doses. No serious undesirable effects were recorded, no changes in indicators of liver functions or serum lipids incl. the HDL/LDL ratio. Endometrial bleeding after administration of hormones was not more frequent than after placebo. In the discussion the authors analyze some aspect of treatment of the climacteric syndrome by means of these hormones. Allylestrenol in particular is an interesting gestagen due to its inherent oestrogenic effect in the absence of an androgenic effect.

Photodynamic therapy for the treatment of erythema, papules, pustules, and severe flushing consistent with rosacea.

We report a case of a 45-year-old woman who presented with facial erythema, papules, pustules, and severe flushing consistent with rosacea. The patient had failed standard pharmacologic treatments. The patient's flushing was so severe that she had undergone an elective sympathectomy. She received 6 sessions of photodynamic therapy (PDT) with 5-aminolevulinic acid (ALA as Levulan Kerastick, Dusa Pharmaceuticals) given at 2-week intervals. Improvement was evident after the second treatment and was considered "excellent" after the sixth treatment. Improvement continued and no flares were observed 1 month after the final treatment.

[Harlequin syndrome (unilateral flushing and sweating attack) due to a spinal invasion of the left apical lung cancer].

A 54-year-old man was admitted to our hospital because of a persistent pain of the left cervix and scapular region of three-month duration and an abnormal shadow in the chest roentgenograms. Neurological examinations, chest roentgenograms, chest CT scanning, vertebral tomograms and myelogram revealed Pancoast's syndrome concomitant with Horner's syndrome. Four months later, the patient complained of a sudden onset of unilateral flushing and sweating appearing on the right face, cervix and upper chest. Eye drop tests with cocaine, epinephrine and tyramine indicated the lesion of ciliospinal centers between the 8th cervical and 2nd thoracic spines. The unilateral flushing and sweating attack appearing on the intact side without Horner's syndrome seemed to be an excessive response by an intact sympathetic pathway, the other side failing to respond because of a sympathetic deficit.

Effect of nitroglycerin ointment placement on the severity of headache and flushing in patients with cardiac disease.

Fifty-six patients with cardiac disease participated in a study to determine the effect of site placement on patient reports of headache and flushing after topical nitroglycerin ointment placement. Nitroglycerin was placed on the upper arm, chest, and pelvis in random order. Approximately 30 minutes after application, patients were asked to evaluate their headache and flushing using a visual analogue scale. No significant difference in the severity of side effects was found when the three sites were compared by multivariate analysis of variance with repeated measures. The clinical practice of instructing patients to vary the site placement of nitroglycerin ointment to avoid side effects was not supported.

Elimination kinetics of ethanol and acetaldehyde in healthy men during the calcium carbimide-alcohol flush reaction.

In a crossover design experiment, we investigated the elimination kinetics of ethanol and acetaldehyde during the calcium carbimide (CC)-alcohol flush reaction. Ten healthy men swallowed a tablet of calcium carbimide (50 mg) or placebo and about 2 hours later drank 0.25 g/kg ethanol within 5 min. The pulmonary blood concentrations of ethanol and acetaldehyde were estimated indirectly by analysis of end-expired alveolar air. The onset of facial flushing and associated cardiovascular response coincided with the peak concentrations of ethanol and acetaldehyde in blood. The speed of absorption of alcohol was faster in subjects treated with CC. A smaller volume of distribution of ethanol was evident after pretreatment with CC; 0.636 L/kg compared with 0.675 L/kg after placebo. The rate of elimination of ethanol from blood was about 5% slower in subjects given the CC tablet. The disposition kinetics of acetaldehyde were markedly different when aldehyde dehydrogenase (ALDH) was inhibited. The maximum blood-levels of acetaldehyde ranged from 40-242 microM compared with 1.7-6.5 microM in the placebo control experiments. The elimination half-life of acetaldehyde after CC treatment ranged from 18-31 min. Our results do not support a significant role of acetaldehyde in regulating in-vivo metabolism of ethanol in humans.

[Consequences of sudden flushes in 50-year-old women].

In the year 2000 there will be 4 millions of women undergoing menopause in France. Sudden flushes represent the most characteristic symptom of that period. It is a destabilizing disorder compelling the women to modify their behavior, for instance their clothing. The sudden flushes are often experienced as ridiculing and women often hesitate to talk about it. At 50, this disorder is added to the socio-psychological problems that the physician must be able to tackle and treat with confidence.

[Pronounced flush symptoms in carcinoid syndrome without liver metastases].

The case of a 78 years old patient with impressive flush symptoms is reported. 5-Hydroxyindoleacetic acid and serotonin were elevated. The responsible pathomorphological structure is a 8 x 4.5 x 6 cm ileocaecal neoplasm. Pre-, intra- and postoperatively liver metastases could not be detected during a 18 month period. After surgery of the carcinoid flush disappeared, so that the tumour seize resulting in enormous secretion of metabolites with endocrine activity should be claimed responsible for causing this symptom.

[Contribution of the ALDH2 and CYP2E1 genes to flushing response and drinking patterns in Japanese healthy volunteers].

Relationship between genotypes of the loci for ALDH2 and CYP2E1 and the flushing response as well as drinking pattern were investigated among 31 Japanese healthy persons. In 14 persons who showed flushing symptom and whose ALDH2 genotype was heterozygote (ALDH2*1/ALDH2*2), 6 persons whose CYP2E1 genotype was mutant homozygote or heterozygote (C2/C2 or C1/C2), reported to drink more frequently and their alcohol consumption was higher than that of C1/C1 carriers. However, 2 subjects possessing C2 allele in 3 persons carrying ALDH2 homozygous genotype showed very low alcohol consumption. It is concluded that a person heterozygous for ALDH2 and either mutant homozygous or heterozygous for CYP2E1 may become able to drink more alcohol, if his drinking pattern grows more frequent.

Human kidney preservation by flushing with intracellular solution and cold storage.

Simple flushing and cold storage of human kidneys for transplantation has not been accepted for preservation times exceeding 24 hours. A prospective study of 25 primary cadaver kidney transplants was performed to evaluate Collins C2 solution for human kidney preservation. Cold storage time was 10 to 23 hours in 14 cases and 26 to 44.5 hours in 11 cases. Seventeen (68%) of the kidneys had immediate, sustained function. The one-month function rate was 88%. There were no significant differences between the 10- to 23-hour and the 26- to 44.5-hour groups with respect to the incidence of first-week dialysis, one-month graft function, or mean lowest serum creatinine value. Human kidney preservation time can be safely extended beyond 24 hours with Collins C2 flushing followed by simple cold storage.

Prominent ECG repolarization changes associated with intracoronary infusion of normal saline: comparisons with alternate coronary catheter flush solutions.

Normal saline (NS) is commonly used as a coronary catheter flush solution. We tested the hypothesis that intracoronary (i.c.) infusions of lactated Ringer's solution (LR) and LR with 5% dextrose (D5LR), both of which contain potassium and calcium, would be associated with less prominent surface ECG changes compared with i.c. infusions of NS. In 34 patients, 10 mL each of NS, LR, and D5LR at 37 degrees C were infused over 5 sec into the left main coronary artery. A 12-lead ECG was recorded before, continuously during, and after each infusion. Blinded ECG analysis revealed T-wave amplitude changes > 0.2 mV in 94%, 12%, and 3% of patients with the use of i.c. NS, D5LR, and LR, respectively (P < 0.0001, NS vs. D5LR or LR). QT prolongation > 40 msec occurred in 88%, 15%, and 18% of patients with i.c. NS, D5LR, and LR, respectively (P < 0.0001, NS vs. D5LR or LR). QT dispersion was increased by > 40 msec in 26% of patients during i.c. NS infusion compared to only 3% of patients with i.c. LR and D5LR infusions (P < 0.01). In conclusion, i.c. NS infusion is associated with more marked repolarization changes as compared with i.c. LR and D5LR infusions. Since such changes may lower arrhythmogenesis thresholds, the routine use of LR as a coronary catheter flush solution should be considered. Cathet. Cardiovasc. Intervent. 48:359-364, 1999.

Immune heparin-induced thrombocytopenia resulting from preceding exposure to heparin catheter flushes.

Immune heparin-induced thrombocytopenia (HIT) is a life-threatening adverse effect of heparin. It can result from any type of heparin exposure and by any route of administration; however only a few cases are reported after exposures to small quantities of heparin from catheter flushes. The major clinical problem associated with HIT is thrombosis. Early detection and institution of alternative, non-heparin anticoagulation are important. We report a patient with HIT associated with use of therapeutic-dose unfractionated heparin in whom immune sensitization to heparin was triggered by two 500-unit exposure to UFH associated with intravascular catheter flushing for antineoplastic chemotherapy in a patient with colon adenocarcinoma.

Flushing technique in the management of retained common bile duct stones with a T tube in situ.

Over a 12-year-period, 6046 cholecystectomies and 918 common bile duct explorations were undertaken with 697 stone-positive choledochotomies (76 per cent). Forty patients (5.7 per cent) had retained common bile duct stones with a T tube in situ. Twenty-eight patients underwent flushing of the common bile duct with a continuous infusion of heparinized saline solution with parenteral aminophylline to induce relaxation of the sphincter of Oddi. Complete duct clearance was achieved in 17 patients; infusion was stopped in four patients and no deaths occurred. The use of percutaneous stone extraction (Burhenne's technique), gave an overall successful common bile duct clearance rate of 86 per cent. The flushing technique represents the first step in the non-operative management of retained common bile duct stones in patients with a T tube in place. It is a safe and effective method for use in the early postoperative period with a prolongation of hospital stay of not more than 1 week.

Coronary arteriography: prevention of thromboembolic complications using a pressure-drip flushing technique.

All selective coronary arteriographic examinations (1,833) performed in the authors' laboratory during a five-year period (1/1/70 to 12/31/74) were analyzed for mortality and total morbidity according to method used. During the first two years, the control period, the classic brachial artery cutdown (Sones) and percutaneous femoral artery puncture (Judkins) techniques were utilized. Mortality rate for the total 589 patients was 1.01%. This included a mortality of 0.26% (1/386) for the brachial arteriotomy method, and 2.5% (5/203) for the percutaneous femoral puncture approach. After introduction of the pressure-drip flushing technique, the subsequent three-year mortality rate for a total of 1,244 patients was 0.16%. This included an incidence of 0.17% (1/585) for brachial arteriotomy and 0.15% (1/659) for modified percutaneous puncture techniques. The morbidity incidence during the initial two-year period was 3.0% (18/589). This included an incidence of 2.0% for brachial arteriotomy and 5.0% for percutaneous puncture techniques. After institution of the new pressure-drip flushing technique the total incidence fell to 1.2% equally divided between arteriotomy and percutaneous techniques. Modification of the classic percutaneous femoral artery puncture techniques has resulted in major reduction of mortality and morbidity complications which are chiefly thromboembolic in nature. It has not significantly influenced local thrombotic complications of arteriotomy.

Interstitial accumulation of Na+ and K+ during flush-out and cold storage of rat livers: implications for graft survival.

Intracellular-type electrolyte solutions were introduced into organ preservation to prevent K+ efflux and Na+ and Cl- influx into cells and cell swelling during cold ischemia. We studied cation accumulation in the interstitial space by microdialysis, during rat liver cold storage and after flush-out with high-K+ and low-K+ solutions. The effect of Na+ and K+ on graft function and survival was studied in an isolated perfused liver model and an orthotopic transplantation model after rat liver storage in iso-osmolar high-K+ and low-K+ solutions. After 24 hours of cold ischemia [Na+]o dropped from 136 +/- 2 mmol/L to 91.8 +/- 1.1 mmol/L, and [K+]o increased from 5.9 +/- 0.1 mmol/L to 12.2 +/- 1.6 mmol/L (P < .001 vs. control). [Na+]o and [K+]o after flush-out did not equilibrate with [Na+]sol and [K+]sol after 24 hours of cold storage. Rat livers preserved in low-K+ solutions produced significantly more bile during isolated reperfusion and released less alanine transaminase, aspartate transaminase, and lactate dehydrogenase into the reperfusion medium than high-K+ solutions. Rat liver survival after 14 hours of preservation was higher in low-K+ solutions (13 of 13) than in high-K+ solutions (7 of 13). Those studies indicate that during cold storage of rat livers, transmembraneous Na+-K+ sodium-potassium exchange might not follow the 3:2 stochiometry of a sole sodium-potassium exchange via Na+-K+ sodium-potassium adenosine triphosphatase (ATPase), and that low-K+ solutions might improve graft function and survival after rat liver preservation.

Sodium ion concentrations in uterine flushings from "implanting" and "delayed implanting" mice.

The uteri of mice were flushed with isotonic mannitol (1) on the day of implantation during normal pregnancy or (2) during delay of implantation. The Na+ concentration in flushings was reduced during diapause but had increased by 65% 24 h after progesterone-treated ovariectomized mice received estrogen injections. The results suggest that the apparent metabolic quiescence of diapausing blastocysts may, in part, be a consequence of lower (Na+) in the uterus during delay of implantation.
