Growth factor expression in breast tissue.

We have studied mRNA levels for a variety of growth factors in biopsy specimens from malignant, benign and normal breast tissue. We found TGFb mRNA in all breast cancers and neoplastic breast tissues but the level of the TGFb mRNA were found to be higher in breast cancer (P = 0.01). TGFa mRNA was detected in a similar proportion of cancers as in neoplastic breast tissues but the TGFa receptor EGFR mRNA was detected in only 55% of breast cancers but in all non-neoplastic breast tissue tested. The presence of EGFR mRNA was inverted related to oestrogen receptor status and coexpression of TGFa and EGFR was observed in 28% of carcinomas, and significantly more commonly in ER negative tumours (P = 0.01). PDGF a and b chain transcripts coexisted in all normal and malignant breast tissue. Insulin-like growth factor II mRNA was present in all 15 samples of non-malignant breast tissue but in only 11 of 21 (52%) of carcinomas.

Metanephric neoplasms: the hyperdifferentiated, benign end of the Wilms tumor spectrum?

Metanephric neoplasms represent a spectrum of differentiated lesions that seem most likely to be related to Wilms tumor. These neoplasms include a pure stromal lesion, a pure epithelial lesion (MA), and a mixed epithelial-stromal lesion (MAF). The continuity of these lesions with Wilms tumor has been demonstrated best in the epithelial lesions. The relationship of Wilms tumor, MAF with mitoses or combined MA/Wilms tumor lesions, and usual MAF or usual MA may be viewed as analogous to that of neuroblastoma, differentiating neuroblastoma, and ganglioneuroma, in which progressively more mature or differentiated counterparts of malignant embryonal lesions are associated with a greater probability of benign clinical behavior. Such a spectrum already is recognized for cystic ILNR-derived nephroblastic lesions, ranging from cystic Wilms tumor, cystic partially differentiated nephroblastoma, and cystic nephroma. Although this concept implies that the more active lesions (Wilms tumor) mature with time into inactive ones (usual MAFs or MA), the converse (that an active Wilms tumor can arise within an inactive usual MAF or MA) remains possible.

Breast biopsy in women 30 years old or less.

Forty women 30 years of age or less underwent breast biopsy at Roswell Park Cancer Institute between January 1980 and January 1989. Thirty-eight of the 40 women had a palpable breast mass. Thirty-one of these young women had self-detected breast masses, and the median duration before presentation was 6 months. Physical characteristics were described in 30 of the masses. Twenty-three were described as "fibroadenomas" or smooth, firm, and mobile. Seven masses were described as irregular. The median size of the breast mass was 1.5 cm (range: 0.5 to 9.0 cm). Mammography was performed in 20 patients, but results were reported as abnormal in only 6. Twenty of the masses were described histologically as fibroadenoma. Twelve were described histologically as "fibrocystic disease" or "stromal fibrosis." One case (2.5%) was invasive adenocarcinoma. Probability of serious underlying breast pathology in young women is low but not nil. Noninvasive and minimally invasive techniques are proposed by some authors as cost-efficient methods that may substitute for open biopsy in these patients. Unfortunately, false-negative results persist and are particularly unacceptable in these young women. During the same time period as our study, 54 women aged 30 years or less were referred to Roswell Park Cancer Institute with a previously diagnosed invasive breast cancer. The incidence of breast cancer in this biopsy series was 2.5%. The potential costs of misdiagnosed early breast cancer in young women negate any rationalization for less invasive biopsy techniques. Following aspiration to rule out a benign cyst, and a possible period of brief observation for spontaneous resolution (2 or 3 months), excisional biopsy is recommended for young women with a breast mass.

Breast masses in adolescent patients in Trinidad.

In our study, fibroadenoma was the most common breast mass found in adolescent girls. The frequency of mammary dysplasia seems to increase with age. There was one case of granular cell tumor of the breast and an unusual case of metastatic ovarian carcinoma presenting as a breast mass.

Modified Dumon stent for the treatment of a bronchopleural fistula after pneumonectomy.

We describe the case of a 72-year-old man in whom a bronchopleural fistula developed after induction chemotherapy followed by right pneumonectomy for lung cancer. Resuturing of the right main bronchial stump, endoscopic treatment, and repair using a latissimus dorsi muscle flap were not effective. Consequently we placed a modified Dumon stent in the carina, which effectively closed the stump.

Ultrasonic attenuation in fibroadenoma of the breast.

Fifteen patients are described who attended out Breast Screening Programme, and were found to have sclerosed fibroadenomas, the imaging of which raised the possibility of carcinoma. In six of these a reflective zone between a mass lesion and distal acoustic shadowing might have been used to infer the benign diagnosis.

Direct contact B-scan ultrasound in the diagnosis of solid breast masses.

Our experience of breast ultrasound in the differentiation of benign and solid malignant breast masses indicates a specificity of 93% for carcinoma; 7% of cancers were regarded as benign. Eight of 89 proven fibroadenomas were considered possibly malignant. Typical, and overlapping, features are described, and the world literature is discussed. Whereas most previous workers have used a water-path technique, we employ direct contact B-scanning, which is quick, simple, and apparently at least as successful. The small group of solid masses where there is overlap between fibroadenoma and carcinoma needs further study; this might be helped by histological correlation with the ultrasound appearances of malignancy, the causes of which are still debated. The exclusion of a focal solid mass in a patient with a breast lump can reduce the biopsy rate.

Management of breast lesions in children and adolescents.

The records of 207 patients evaluated and treated for breast abnormalities during a 10-yr period were reviewed. Patients ranged in age from 1 wk through 16 yr. Seventy-eight per cent were female. Operative procedures were performed in 134 patients (64%). Most common diagnoses were fibroadenoma, premature thelarche, and precocious puberty in the females, and pubertal gynecomastia in the males. Age and sex separate patients into natural subgroups. In females beyond the neonatal period, premature thelarche and precocious puberty are the most likely cause of breast enlargement. Breast biopsy is only rarely indicated for a distinct mass lesion in the prepubertal breast. Mass lesions in the breast of adolescent females require excisional biopsy. In the male, pubertal gynecomastia is the most common form of breast enlargement and only occasionally requires subcutaneous mastectomy for cosmetic and psychologic reasons. Careful evaluation of all children with breast enlargement is indicated to uncover underlying causes and to avoid unnecessary operative procedures.

Breast disorders in children and adolescents.

A review of pediatric breast disorders is provided, with particular attention given to the topics of fibroadenoma, cystosarcoma phylloides, breast infections, breast cancer, and benign breast disease. The pediatric clinician is strongly encouraged to include the breast system as part of a complete examination and educational process.
