[Blunt abdominal injuries. Retrospective study of 697 cases].

In our country the number of abdominal traumata is steadily increasing. Mostly they are caused by traffic accidents in the overcrowded cities. In this retrospective study we analyse all our cases of the last 4 years, which were admitted to our clinic. Only in rare cases diagnosis could be made immediately on the basis of clinical symptoms. The decision to operate or not could only be taken after diagnostic irrigation of the abdominal cavity. In this way early diagnosis could also be made in doubtful cases and the patients operated in time.

Biomechanics of abdominal injuries by armrest loading.

Anesthetized pigs were exposed to impact against a padded side interior that included an armrest with either a SOFT or STIFF crush characteristic. The purpose was to assess liver and spleen injury under specific impact conditions. The STIFF armrest resulted in severe abdominal and thoracic injury in five side-impact animal tests. Injuries of the liver and spleen included deep lacerations, tears of major hepatic arteries and veins, and serious hemoperitoneum. The injuries averaged AIS = 4. In contrast, five animals exposed to the SOFT armrest design experienced injuries of lower severity than any animal in the STIFF armrest exposures (p < 0.005). The average injury was AIS = 2. The STIFF armrest protruded into the abdomen and showed little sign of deformation with abdominal loading. This situation is consistent with the occurrence of lacerations at hepatic junctions and between lobes, and displaced rib fractures in several cases. The SOFT armrests crushed fully in each test, indicating that abdominal compression was lower and was limited by armrest deformation.

[Thoracic and abdominal injuries caused by safety belts (author's transl)].

Based on experiences over half a year authors found that the introduction of safety belts, beside obvious advantages, resulted in new types of injuries. They describe own experiences and draw attention to heart contusions joining thoracic fractures, and to possible thoraco-abdominal organ injuries with characteristic symptom free or delayed appearance, caused by blount trauma.

[Postoperative treatment of abdominal injury victims].

Among other procedures of a complex intense therapy applied upon 265 abdominal trauma cases the authors have included an early peritoneal perfusion and longlasting peridural blockade. The results evidence a high prophylactic and therapeutic effectiveness of the suggested methods

[Duodenopancreatic lesions in abdominal injuries. Clinical and therapeutic considerations].

The frequency of duodeno-pancreatic lesions in abdominal or thoraco-abdominal trauma hospitalized in the surgery clinic of the Emergency Hospital from Bucharest between 1960 and 1975 was of 0.57%. In the present study are stressed the important factors which influence the outcome of the interventions (age, the interval from the accident to hospitalization and that from hospitalization to surgery). The causes are described, that have generated abdominal trauma with duodeno-pancreatic lesions the intensity and the extension, the localization of the lesions, as well as the importance of the association with lesions of other organs. An analysis is made of the interventions that have been made, in which the type of duodeno-pancreatic lesion was taken into account, as well as the variety of the procedure applied, from the simple draining of the pancreatic lodge, and of the peritoneal cavity, to complex interventions including exeresis of the duodenum or of the pancreas. To conclude the authors show that duodeno-pancreatic lesions are of considerable severity and are one of the causes of high mortality. These lesions also represent a serious test for the surgeon when one has to make a decision on the technique to be performed during the intervention. These lesions are also very demanding from the viewpoint of the surgeon's technical capability.
