In contrast to an increased oral health level in the Western countries, this improvement is not the case in age matched handicapped. The dental mutilation is generally related to periodontal problems (plaque, gingivitis, periodontitis, calculus), dental caries, traumatic injuries and self-injuring behaviour. Via a questionnaire, data from 88 institutions in Flanders were obtained. It became clear that there is a striking lack of oral health care and follow-up within institutions. There is a strong need for mouth hygiene instructions programmes for educators, parents, as well as for dentists involved in the special care of these patients.

As a support for the guidelines published in 2001, a review of the literature was carried out using the evidence-based approach in order to update the state of the art regarding epidemiology and treatment of traumatic dental injuries in the primary dentition. An online search in Medline, and a review of expert literature, lead to the conclusion that most luxation injuries heal spontaneously, and unless more conclusive evidence is available, conservative treatment of acute dental trauma for preschool children is suggested. Pain control, the dentist's ability to cope with the child's anxiety, and follow-up instructions in oral hygiene techniques will allow many primary teeth that are currently lost at the time of injury to be saved.

Traumatic injuries require a vast and complex treatment methodology. This manuscript although in a "cookbook" format will still require from the practitioner sound clinical judgment prior to treating a traumatized tooth.
Traumatic injuries to primary and permanent dentition may lead to severe therapeutic problems. The purpose of the investigation was to evaluate epidemiologic data of dento-alveolar injuries collected in a university dental clinic. The sample consisted of 300 patients (198 boys, 102 girls) representing 480 injured teeth, mostly maxillary incisors (94.6%). The results pointed-out a high predominance of traumatized primary teeth. Types of injuries differed between deciduous and permanent dentition: luxation injuries were most often recorded in primary dentition (81%), while a high percentage of crown and crown-root fractures were recorded in permanent dentition (38%). Epidemiological observations concerning location, etiology and distribution of dento-alveolar injuries are briefly discussed.
Traumatic injures distribution of children's teeth is determined of different factors at different ages: beginning to walk, games, sport, orthodontic anomalies. The aim of this investigation is to compare tooth traumatic injures distribution of temporary and permanent incisors at children aged 3 to 14. Object of investigation are 1825 children aged 3 to 14 from the town of Plovdiv. A clinical dental examination has been carried out and had been registrated: expulsio dentis, fractura coronae dentis involving only the enamel, involving and the dentin, exposing the pulp. There also has been pointed out if the fracture is treated or not. The data received has been statistically processed. The results show that tooth traumatic injures distribution at primary teeth is 31-40 per cent of boys and 16-30 per cent of girls and at permanent teeth is 12-33 per cent of boys and 4-19 per cent of girls. There is a statistical significance of differences between tooth traumatic injures of primary and permanent incisors.
