Information model and terminology model issues related to goals.

Goal statements are a significant component of structures that support the process of health care delivery such as practice guidelines, standards of care, critical pathways, disease management plans, patient education plans, and nursing care plans. Although these structures are increasingly computer-based, there has been little attention to the formal representation of goal statements. This is a necessary prerequisite for enabling semantic interoperability. Existing and evolving information model and terminology model standards offer some approaches that may be applicable to goal statements, however, a number of issues require resolution

Integration of a standard gastrointestinal endoscopy terminology in the UMLS Metathesaurus.

MST(c), a standard terminology for gastrointestinal endoscopy reporting, was integrated in the January 2002 UMLS Metathesaurus in order to ease the practical interoperability of clinical data repositories in gastroenterology. The integration required full specification of names, resolution of discrepancies between English, French and Italian versions of MST, appropriate categorization with UMLS Semantic Types and MST-level Class attributes, assignment of explicit intra-table (and some useful inter-table) relationships mainly at concept level but also at the source level in order to retain and fully represent the original explicit and implicit MST organization. Main results, problems encountered and future plans are discussed.

[Depression in menopausal period--prevalence, classification, terminology].

Depression, understood as an illness requiring medical treatment, is a frequent phenomenon in woman in menopausal period. For years the literature has been presenting discussions concerning etiology and pathogenesis of depressive syndromes of this period, their specificity in comparison with other affective illnesses as well the problems connected with adequate classification and terminology. The following article is an attempt at ordering the knowledge concerning the above mentioned subjects while taking into account recent research results and diagnostic criteria in accordance with ICD-10.

[A simplified terminology for abnormalities of the lumbar disks].

The terminology for abnormalities of the lumbar disk has always been a source of confusion. Recent advances in pathological studies have inspired the authors to propose a simple classification of common disk anomalies suitable not only for diagnostic radiologists but also for referring clinicians. Although the diagnosis of a few pathological entities will only be possible with specific imaging techniques, the proposed classification is appropriate for reporting observations from plain films, conventional tomograms, myelograms, discograms, computed tomography scans and magnetic resonance images. All lumbar disks can thus be classified into one or more of the following categories: normal, aging, scarred, ruptured and herniated. A disk herniation is defined as a localized exit of disk material beyond the limits of the original intervertebral space.

BLOOD PRESSURE. A CONSIDERATION OF TERMINOLOGY.

Concepts of hypertension have changed and changes in terminology to reflect this state of affairs are suggested. Statistically, the best mortality experience is associated with blood pressure commonly regarded as subnormal, and increments of blood pressure above this level are associated with progressive increases in mortality. The terms "normal", "benign" and "essential" in relation to blood pressure should be abandoned. "Optimal", "acceptable" and "hypertensive" ranges of blood pressure are suggested. Hypertension is regarded as a symptom of disease, rather than as a disease in itself, and "hypertension", when used as a diagnostic label, should be qualified always by the primary disease, if known, or by the modifying phrase, "of unknown cause", if not known.

Incorrect terminology in nasal anatomy and surgery, suggestions for improvement.

This article discusses why nomenclature such as 'upper lateral cartilage', 'lower lateral cartilage', 'weak triangle', 'empty triangle', 'soft triangle', 'hemitransfixion', and 'marginal incision' should be abandoned and replaced by anatomically, linguistically, and surgically correct terms. Suggestions for better terminology are made. The division of the nasal organ into five anatomical areas and three physiological parts is questioned. The terms 'interdomal ligament', 'intercrural ligament', and 'septocrural ligament' should not be used, as histological studies have demonstrated that no such ligaments exist.

An evaluation of hybrid methods for matching biomedical terminologies: mapping the gene ontology to the UMLS.

Integration of disparate biomedical terminologies is becoming increasingly important as links between biological science and clinical medicine grow. Mapping concepts in the Gene Ontology (GO) to the UMLS may help further this integration and allow for more efficient information exchange among researchers. Using a gold standard of GO term--UMLS concept mappings provided by the NCI, we examined the performance of various published and combined mapping techniques, in order to maximize precision and recall. We found that for the previously published techniques precision varied between (0.61-0.95), and recall varied from (0.65-0.90), whereas for the hybrid techniques, precision varied between (0.66-0.97), and recall from (0.59-0.93). Our study reveals the benefits of using mapping techniques that incorporate domain knowledge, and provides a basis for future approaches to mapping between distinct biomedical vocabularies.

OpenGALEN: open source medical terminology and tools.

The GALEN programme of research into medical terminology began in 1991. In 1999 OpenGALEN was formed to provide an open source route both for disseminating the results of that programme and as a framework for its future development. Currently available open source resources include a sophisticated ontology development environment and a large open source description logic-based ontology for the medical domain.

[The Bethesda system: new revision of the terminology from 2001 year].

The aim of the present review is to briefly look through the new components of the 2001 revision of the cytological terminological classification Bethesda, created in 1989 and modified in 1991. In general, the changes are connected with the more clear definition of circumstances and different categories, in order to allow the comparison of data provided in international publication as well as to facilitate diagnostic and therapeutic strategies. It is pointed out although single attempts to use the Bethesda system have been made in Bulgaria since 1994, there is no serious scientific discussion between the experts in this field--gynecologists, cytologists, pathologists and cytotechnicians, about the opportunities for its implementation in a large scale.

[Terminology and classification of macular degeneration of old people (MDO)].

The paper presents the main clinical and histopathological research, relevant for the evolution of concepts regarding the terminology and classification of MDO. Because of the insufficient knowledge of the etiopathogenic under-stratum of the affection, a thorough classification could not have been elaborated until now. We propose a classification based on the clinical aspect and the histopathological under-stratum of the affection. Two main degenerative categories could be revealed: the senile macular degeneration of the old man, located at the level of the "choroid papilla, EPR, Bruch membrane" complex; other forms of macular degeneration of the old man, located at the level of retinal structures. For the Romanian literature, we propose the term "macular degeneration of the old man". The term is considered stylistically adequate and more accurate, compared with the term "macular degeneration related to age".

Evaluation of reliability in the use of endoscopic terminology.

Variability in the reporting of gastrointestinal endoscopic findings may affect the validity of analyses of data collected from clinical reports of those findings. In this project, images of 10 endoscopic findings were collected from the data repository of the Clinical Outcomes Research Initiative (CORI), all of which had been described by the reporting endoscopist. These images were presented to 52 experienced endoscopists recruited from the clinical affiliates of CORI who were asked to assign each a term from the Minimum Standard Terminology for Digestive Endoscopy. Proportion of agreement with the endoscopist varied by finding from 84.3% to 51.0% (overall 67.6% with 95% CI 63.4-71.8%). Proportion of agreement among the subjects varied by finding from 76.3% to 38.5%.(overall 55.6% with 95% CI 52.4-58.8%). Possible reasons for this lack of agreement are discussed.

A comparison of semantic categories of the ISO reference terminology models for nursing and the MedLEE natural language processing system.

Natural language processing (NLP) systems have demonstrated utility in parsing narrative texts for purposes such as surveillance and decision support. However, there has been little work related to NLP of nursing narratives. The purpose of this study was to compare the semantic categories of a NLP system (Medical Language Extraction and Encoding [MedLEE] system) with the semantic domains, categories, and attributes of the International Standards Organization(ISO) reference terminology models for nursing diagnoses and nursing actions. All but two MedLEE diagnosis and procedure-related semantic categories mapped to ISO models. In some instances, we found exact correspondence between the semantic structures of MedLEE and the ISO models. In other situations (e.g. aspects of site or location), the ISO model was not as granular as MedLEE. For clinical procedure and non-invasive examination, two ISO nursing action model components (action and target) were required to represent the MedLEE semantic category. The ISO model requires additional specification of selected semantic categories for the abstract semantic domains in order to achieve the objective of using NLP to parse and encode data from nursing narratives. Our analysis also suggests areas for extension of MedLEE.

MeSH and specialized terminologies: coverage in the field of molecular biology.

There is real need among researchers for data in molecular biology, due to the explosive development in this field. Information extraction techniques have been developed in this specific domain, to extract structured information from the literature. The National Library of Medicine provides access to 12 million MEDLINE citations, accessible via PubMed. Information retrieval techniques which are used to select documents that are relevant according to user request. As other domains in medicine, biomolecular scientific literature is indexed by MeSH terms. Automatic identification of protein and gene names in scientific articles is an important step in the indexing process, since proteins and genes are often associated with multiple names, and authors alternate between these synonyms. In this article we assess the contribution of specialized terminologies in the field of molecular biology compared to MeSH.

[New and old definitions and COPD terminology].

Terminology of the disease currently known as chronic obstructive pulmonary disease has gone through a long and non-uniform development. Various definitions have been gradually developed and chronic bronchitis was separated from pulmonary emphysema. Various umbrella names have been suggested to unite this group of diseases. At present a classification based on World strategy for diagnosing, treatment and prevention of the chronic obstructive pulmonary disease from 2000 which has been published in our country too is used.

[The review of anatomical and clinical terminology concerning vocal folds].

The review of the anatomical and clinical literature of the vocal folds was presented. The attention was paid on the lack of homogenous terminology in this subject. Different authors use different terms concerning the vocal folds: vocal cords, vocal lips and vocal folds. The necessity of developing common, current nomenclature of the vocal folds, also due to clinical reasons was emphasized.
