Alpha E beta 7 (CD103) expression identifies a highly active, tonsil-resident effector-memory CTL population.

The characterization of antiviral CTL responses has largely been limited to assessing Ag-specific immune responses in the peripheral blood. Consequently, there is an incomplete understanding of the cellular immune responses at mucosal sites where many viruses enter and initially replicate and how the Ag specificity and activation status of CTL derived from these mucosal sites may differ from that of blood-derived CTL. In this study, we show that EBV-specific CTL responses in the tonsils are of comparable specificity and breadth but of a significantly higher magnitude compared with responses in the peripheral blood. EBV-specific, tonsil-resident, but not PBMC-derived, T cells expressed the integrin/activation marker CD103 (alphaEbeta7), consistent with the detection of its ligand, E-cadherin, on tonsillar squamous cells. These CD8-positive, CD103-positive, tonsil-derived CTL were largely CCR7- and CD45RA- negative effector-memory cells and responded to lower Ag concentrations in in vitro assays than their CD103-negative PBMC-derived counterparts. Thus, EBV-specific CTL in the tonsil, a crucial site for EBV entry and replication, are of greater magnitude and phenotypically distinct from CTL in the peripheral blood and may be important for effective control of this orally transmitted virus.

[Immunodiagnosis of chronic tonsillitis].

Current methods of allergo- and immunodiagnosis of chronic tonsillitis are reviewed. Among them, attempts to use serological tests and reactions of cell type for defining course and form of chronic tonsillitis. Single tests, especially those basing on the suggested etiological factors, are thought uneffective. The available knowledge on functional characteristics of the lymphopharyngeal ring allows to offer some immunological tests for development of the laboratory-diagnostic algorithm aimed primarily at assessing feasibility of tonsil tissue repair for decisions on therapeutic policy rather than determination of the tonsillitis form.

Differential expression of cellular prion protein on human blood and tonsil lymphocytes.

BACKGROUND AND OBJECTIVE: The expression of cellular prion protein (PrPc) on the surface of peripheral lymphocytes has been previously reported, but little is known about its expression on lymphoid cells from secondary lymph organs. In this report, we compare the surface expression of PrPc on human blood lymphocytes and tonsil lymphocytes. DESIGN AND METHODS: This analysis was performed by cytometry on live lymphocytes isolated from healthy donors or from the tonsils of adults or children. RESULTS: Human peripheral lymphocytes and tonsillar lymphoid cells, but not erythrocytes or granulocytes, express PrPc at their surfaces. Interestingly, we found significantly less PrPc on freshly isolated tonsil lymphocytes, both B and T, than on blood cells. Although tonsil cells bear less PrPc than circulating blood lymphocytes, they are able to express high quantities of PrPc on their surface when placed in culture. However, contrary to previous results, mitogen stimulation does not affect this expression on B- or T-cells. INTERPRETATION AND CONCLUSIONS: We suggest that the PrPc expression by lymphocytes may be modified by interactions occurring during intratissular migration or during cell-to-cell contacts. Whether PrPc plays a role in intracellular communication at this location, as it does in the nervous system, remains an open question.

[Palatine tonsils and immunity. IV. Lymphocyte blast transformation in various lymphoid organs of animals immunized with microbial antigens].

For the purpose of ascertaining the peculiarities of cellular differentiation of lymphoid cells of the palatine tonsils experiments were conducted on rabbits immunized intravenously and subcutaneously with streptococcus and paratyphoid B antigens; a study was made (in the blast-transformation reaction) of a comparative response of the lymphocytes of the palatine tonsils, the thymus, the spleen, the appendix and the regional lymph node. In case of stimulation with the specific microbial antigens and PHA the proliferative response was greater in the culture of the splenic lymphocytes and the regional lymph node than in the culture of the lymphocytes of the palatine tonsils and the appendix. The thymus lymphocytes responded to PHA only. A conclusion was drawn that the paratyphoid B and streptococcus antigens penetrating into the lymphoid tissue by hematogenic route stimulated a relatively low percentage of lymphocytes of the palatine tonsils and the appendix in comparison with the regional lymph node and the spleen.

[The clinical meaning and determination of the superoxid dismutase activity and malondialehyde content in the serum of the patients suffering from chronic tonsillitis].

The activity of SOD and MDA content in the serum of patients suffering from chronic tonsillitis were determined one day before operation and one week after of operation. It was found that the activity of T-SOD Mn-SOD and CuZn-SOD were decreased in the serum of patients with chronic tonsillitis than that in the normal control group with statistical significance (P < 0.01). T-SOD activity was increased and MDA content was decreased significantly after operation than the time before operation (P < 0.001, P < 0.01). This study indicates that tonsil excision could decrease MDA content and increased SOD activity for the chronic tonsillitis patient.

[Chronic tonsillitis: therapeutic and diagnostic approaches].

The article is a brief review of clinical and topographic anatomy of the palatine tonsils. Their role in formation of general and local immunity is shown. Etiological and pathogenetic aspects of chronic tonsillitis are considered: most frequent causing agents, underlying pathogenetic and immunological mechanisms. B.S. Preobrazhensky's and V.T. Palchun's clinical classification of chronic tonsillitis is presented. New approaches to etiopathogenetic treatment of chronic tonsillitis are described.

Lymphangiectatic fibrolipomatous polyp of the palatine tonsil.

A rare benign polypoid tumor of the right palatine tonsil is described in a 23 year old male. It contained dilated lymphatic channels surrounded by fibrous tissue and foci of mature fat. The features of this lesion add support to the hypothesis that benign tumors of tonsil may be hamartomas of tonsil rather than true neoplasms. The differential diagnosis of polypoid lesions of the tonsil is discussed.

[Detection of Arcanobacterium haemolyticum phospholipase D neutralizing antibodies in patients with acute tonsillitis].

Neutralization test carried on a non-nutrient blood agar prepared from sheep erythrocytes sensibilized with equi factor of Rhodococcus equi was used for the detection of antibodies against phospholipase D (PLD) of Arcanobacterium haemolyticum. Altogether, 433 sera from 404 patients aged 1 to 25 years with the signs of acute tonsillitis were examined. Antibodies against PLD were found in 28 patients (6.9%). In 116 sera from persons without the signs of tonsillitis the antibodies against PLD were detected in one case (0.9%). The titres reached the values from 4 to 64. Out of 84 paired sera, the significant change of the titre, i.e. at least the four-fold one, was observed five times: three times as the seroconversions, twice as the declines of the titre. The antibodies were found most often in children aged 11 to 15 years, 10 times out of 97 (10.3%). In 253 patients with the tonsillitis, the result of the parallel bacteriologic examinations of throat swabs was at the disposal. A. haemolyticum was successfully isolated in 3 cases, but only owing to the inhibition of staphylococcal hemolysis around its colonies. All three cases were also serologically positive. Consequently, our results show that neutralizing antibodies against PLD of A. haemolyticum can be detected in patients with tonsillitis.

An unusual case of recurrent tonsillitis due to Pseudomonas aeruginosa.

An unusual case of recurrent tonsillitis due to pseudomonas aeruginosa. Pseudomonas (P.) aeruginosa in the head and neck region of an immunocompetent patient is mainly seen in ear infections, and sometimes in sinusitis. P. aeruginosa is an occasional finding in tonsil smears as part of normal microbial flora, but it rarely produces suppurative tonsil infection. We report a case of a previously healthy young female with recurrent episodes of tonsillitis due to P. aeruginosa infection. Although the patient received complete regimens of antibiotics (orally and intravenously) repeatedly, definitive eradication was only achieved after tonsillectomy.

Role of surface swab, core swab and fine needle aspiration in isolating the core bacteria in inflammed tonsils.

The microbial flora of 75 patients with chronic tonsillitis were sudied to identify the commonest organism causing chronic tonsillitis and to compare surface swab, fine needle aspiration (FNA) and core swab in isolating the pathogens. Staphylococcus aureus was the predominant isolate from surface core swab FNA and surface swab. In our study the correlation between surface swab with core swab was 47.9% and between surface swab with FNA 57.3%. The correlation between FNA with core swab of 69.3% showed that FNA for culture was superior to surface swab in predicting the core culture. Hence FNA of the tonsil has more advantage over the routinely performed surface swab in representing the core pathogen. Thus the rationale of treating chronic tonsillitis medically should be based on the knowledge of the common core pathogen.

[Lipid complex in subpalatine tonsils in normal state and pathology].

A fat-acid compound of lipids in the subpalatine tonsils was studied by gas chromatography in normal state, chronic tonsillitis and hypertrophy of tonsils. We have determined that in saturation of the lipid complex raised in the subpalatine tonsils due to an increase in the level of arachidonic acid. It gives evidence for an activation of the lipid peroxidation leading to a pathological process.

Spontaneous and PHA-induced rosetting of human blood, tonsil lymphocytes and MLC blasts with sheep, human and horse erythrocytes.

Compared to peripheral blood lymphocytes the ability of human tonsil T cells and MLC blasts to bind sheep, human and horse erythrocytes was found to be increased. Tonsil and MLC T cells were able to bind sheep red blood cells without any cold incubation, i.e. they were 'early' rosettes, and higher percentage of human and horse erythrocyte rosettes were formed by these cells. Low doses of phytohaemagglutinin increased the proportion of rosettes between peripheral blood, tonsil, MLC cells and human and horse erythrocytes. PHA acted only on T cells, and not on B cells, lymphoblastoid B and other cell lines. On the ground of the stronger rosetting property of MLC blasts and tonsil cells, it is likely that the T cells responsible for binding of horse and human erythrocytes after PHA treatment are 'early' or 'active' rosetting cells.

Morphological study of development and functional activity of palatine tonsils in embryonic age.

Palatine tonsils play an important role in the development of the immune system, being the first organ in the lymph system which analyses and reacts to antigenic stimulation. In this study, the peritonsillar area of Waldeyer's ring was investigated in 88 normal human embryos which were examined histologically and immunohistochemically. The progressive development of palatine tonsils during embryonic life is discussed. The first appearance of tonsils is in about the 14th-15th week followed by a parallel development of B- and T-cell regions which accounts for the high incidence of non mucosa-associated lymphoid tissue lymphomas among all tonsillar lymphomas and the higher incidence of T-cell-lymphomas, in comparison to the mucosa-associated lymphoid tissue of the digestive system. The way in which the human body develops the palatine tonsils quickly and prepares them to react to the first antigenic stimulation, are discussed.

[The role of Toxoplasma gondii in the etiology of chronic tonsillitis in children].

Toxoplasma gondii is known to have an affinity to lymphatic tissue. A series of 506 children with chronic tonsillitis were examined serologically for toxoplasmosis by means of CBR, IIR and ELISA. In 7.7% of the children examined low toxoplasma antibody titers were recorded. Isolation experiments performed on tonsillar tissue of the children with the highest toxoplasma antibody titers were negative. Direct microscopic evidence of tissue toxoplasma cysts failed to be established and the cytopathologic picture characteristic of nodal toxoplasmosis was not present in smears of cells aspirated from lymphatic nodes regional to palatine tonsils. The authors assumed that chronic toxoplasma tonsillitis did not occur in the series studied. In our ecological conditions, T. gondii may not be involved in the etiology of chronic tonsillitis, or it may participate to an extent which is epidemiologically negligible. (Tab. 4, Ref. 14).

[The role of nitric oxide in antimicrobial defense in chronic tonsillitis].

Nitric oxide (NO) degradation products were assayed in lacunar discharge in chronic compensated and decompensated tonsillitis before and after treatment on the ultrasound device Tonsillor. It was found that pretreatment concentration of NO degradation products (nitrites and nitrates) differed in decompensated and compensated chronic tonsillitis. The treatment reduced content of NO degradation products in both forms of tonsillitis. Use of indirect values of NO concentration are proposed for differential diagnosis between chronic compensated and decompensated tonsillitis as well as for monitoring of treatment efficacy.

[Clinical analysis of infectious mononucleosis misdiagnosised as tonsillitis].

OBJECTIVE: Discussing the clinical features of infectious mononucleosis misdiagnosised as tonsillitis. METHOD: We used the methods of variant lymphocyte count, heterophil agglutination test and EBVCA-IgA detection to diagnose 6 cases of infectious mononucleosis misdiagnosised as tonsillitis. RESULT: Six cases of infectious mononucleosis misdiagnosised as tonsillitis are cured without upper airway obstruction. CONCLUSION: We must pay attention to the variant lymphocyte count when tonsillitis is diagnosed. Some cases of infectious mononucleosis in convalescence or without hepatic functional lesion and renal functional lesion are easily misdiagnosised and missed.

[Activity of acid aspartic proteinase Candida strains isolated from palatine tonsils].

We have evalueated the activity of acidous asparagine protease in 48 strains of Candida isolated from homogenates of palatine tonsils from 116 patients after tonsilectomy. Strains most frequently isolated were C. albicans (66.7 +/- 6.8%) and C. tropicalis (14.6 +/- 5.09%). The range of proteolysis zone for Candida strains was 4.6 to 12.5 mm. Over 60% of the studied strains showed high and very high proteolytic activity (>6.6 mm) and the remaining strains showed moderate activity (4.6-6.5). There were no strains with low medium activity (to 4.5 mm zone). The range of the number of rosettes E formed by lymphocytes obtained from the studied tonsils was 0-268.3. Lymphocytes isolated from 70 % of tonsils infected with Candida formed only 0-130 rosettes E; in the remaining cases the number ranged between 131 and 220. The number of lymphocytes T forming rosettes E isolated from palatine tonsils from patients not infected with fungi was 2.5 times higher. There was no correlation between the size of proteolysis zone and the number of rosettes E.
