[Clinical investigation on anacusis associated with chronic suppurative otitis media].

OBJECTIVE: To investigate the risk factors and clinical features of anacusis or severe deafness associated with chronic suppurative otitis media (CSOM ). METHOD: A retrospective review of 62 ears consecutive cases of severe deafness (15 ears) and anacusis (47 ears) associated with CSOM were investigated . There were 5 ears severe deafness deteriorated to anacusis after procedures. This represented 6.42% of our total series of CSOM with or without cholesteatoma in adults and children (965 ears) in 1993-2000. The series with cholesteatoma or granuloma or tympanosclerosis were 46 (74.19%) ears 13 (20.97%) ears and 3 (4.84%) ears respectively. The symptoms and operative findings were analyzed. All patients underwent preoperative CT scans and preoperative audiometry. With respect to surgical technique, the canal-wall-down procedure was performed in(50/62) 80.6%, and classical modified mastoidectomy was performed in(12/62) 19.4%. RESULT: The results demonstrated that chronic otorrhea with bloody, ossicular chain destructive and erosion of the supraplate structure or disappear of footplate were helpful predicting indicators for anacusis or severe deafness. There was a high significant association (Chi-square Fisher exact test P < 0. 001) with these factors compared with CSOM without anacusis or severe deafness. Labyrinthine fistula was also correlated to anacusis or severe deafness (P < 0.05). There were no significant with the courses and facial paralysis and revision mastoidectomy. CONCLUSION: Anacusis or severe deafness intimately associated with chronic otorrhea with bloody, ossicular chain destructive, erosion of the supraplate structure or disappear of footplate and labyrinthine fistula. The surgical procedure should be performed earlier to eradicate the chronic or acute infection and to avoid anacusis or severe deafness.

[Use of multi-dimensional statistic analysis for integral evaluation of treatment quality in patients with suppurative-inflammatory diseases of maxillofacial region].

Possibilities of modern methods of maths statistics for comparative evaluation of efficacy of different methods of treatment of patients with suppurative-inflammatory diseases of maxillo-facial region were analyzed and an integral index of treatment efficacy was suggested. Application of the integral index of treatment efficacy showed that intravenous injection of sodium hypochlorite and autohemotherapy with oxidized blood led to the closest apposition of cluster center of these patients to the cluster center of healthy persons. It testifies to greater efficacy of such treatment of the disease in comparison to the other methods.

[Microorganisms isolated from chronic suppurative otitis media and their antimicrobial sensitivities].

In this study mycologic and bacteriologic cultures were made in 100 patients who had chronic suppurative otitis media. We observed 58.8% pure, 35.3% mixed growth (more than one bacteria or bacteria and fungus) in the cultures. In pure cultures, we found mostly Proteus sp., S. aureus and Pseudomonas sp. respectively. In 134 bacteriologic cultures most frequently isolated bacteria was S. aureus (36.6%). In Sabouraud agar, we found mostly Aspergillus sp. and Candida sp. (28.6%). In antimicrobial sensitivity test, gram negatives were found to be sensitive mostly to ofloxacin (73.5%), pseudomonas was found to be sensitive mostly to tobramycin (70%). In the staphylococci the antimicrobic which had the highest sensitivity rate was gentamicin (75.5%), but at the same time we observed 24.5% resistance to this antimicrobic agent. Following this respectively, ofloxacin 71.4% sensitivity 4.1% resistance, cefuroxime 69.4% sensitivity 10.2% resistance rate were detected.

[Clinical-microbiological evaluation of the efficacy of combined use of chitosan, low intensity laser radiation and photosensitizer in treatment of patients with acute suppurative maxillofacial periostitis].

Results are presented on treatment of 68 patients with acute suppurative periostitis of maxillofacial region. After surgical interventions in patients of the study group (48 patients) the wounds were cleansed by 1% chitosan on 0.2% HC1 in combination with methylene blue and irradiated by IR laser beam. The wounds healed in 2-3 days. In the control group (20 patients) for wound dressing chlorhexidine as a standard procedure was used, length of the healing process was 5-6 days. After combined treatment the number of microflora in the wound was reduced and microflora did not show the signs of pathogenicity.

Chronic suppurative otitis media and related complications at the University Clinic of Kinshasa.

During the five-year period from 1998 to 2002, the ENT department of the University Clinic of Kinshasa treated 343 patients suffering from chronic suppurative otitis media (CSOM) and complications. Two hundred and seven patients had simple, 28 had cholesteatomatous form of CSOM, and 108 patients presented with intratemporal or intracranial complications. Roughly half of the patient group consisted of children younger than 10 years. The simple form of CSOM affects men and women equally, but the cholesteatomatous form was more prevalent among men. Otorrhoea and hearing loss were the major presenting symptoms. Hearing loss was moderate to severe in 2/3 and slight in 1/3 of the audiometrically tested ears. A mastoidectomy was performed on 55% of the patients. Roughly 1/3 of the patients presented with complications, the majority of which (69) were subperiostal abscesses. Complications other than subperiostal abscesses were associated with a high morbitidy and a few patients even died. Our data suggest that the prevalence of CSOM in the Kinshasa area is at least 4% and, in fact, has not declined over the last 20 years.

Prevalence of chronic suppurative otitis media among the children living in two selected slums of Dhaka City.

Chronic Suppurative Otitis Media (CSOM) is one of the important health problems in our country. Serious complications may arise from it. As such the study was aimed to find out the prevalence of CSOM among the children of two-selected slum dwellers in Dhaka City. This was a descriptive type of cross-sectional study. A total of 203 samples were interviewed and examined. Altogether 7.39% of children were observed to have CSOM. Among CSOM cases, 60% had hearing impairment. Ear discharge and perforation were observed in 26.7% in right ear, 46.7% in left ear and 26.7% in both ears. Central perforation was present in 93% cases. Cholesteatoma was present in 6.7% cases. Among CSOM cases, 53.3% children were in 2-5 years age group, 80% were dependent, 53.3% cleaned ear by cotton buds, 93.3% lived in Kutcha house, 73.3% had a monthly income of TK. 1001-2000 and 46.7% belonged to families having 6 members. Out of 15 CSOM, 11 cases were seen among children of 141 illiterate mothers. Four cases of CSOM were found among children of 62 literate mothers. Among the 203 mothers, 65.52% were found aware, and 34.48% were not aware about the disease and sequel of CSOM. Here 39.9% mothers had knowledge and 60.01% had no knowledge about the treatment of CSOM. Among the CSOM cases 46.67% cases did not receive any treatment. In treatment receiving group, only 3(20%) children received it from MBBS doctor or Hospital.

[First experience in Hungary with the use of a garamycin sponge in the treatment of suppurative bone and joint wounds].

Authors have implanted Garamycin sponge in 51 patients, suffering from suppuration of bones and joints. This is a sponge-like collagene material prepared from cattle tendon and containing Gentamycin. Their experiences have shown that this product is especially suitable for the treatment of infected cavities of minor tubular bones, whereas it is less suitable for the treatment of extensive cavities of long tubular bones. Attention is called to the fact, that the product had to be tried on "contraselected" material and they had only 5 exceptional cases in which it could be used as a "first therapeutic measure". Under such conditions the early, 50 per cent good results can be considered as promising. Authors close their paper with the description of 4 cases.

Bacteriological study of paediatric and adult chronic suppurative otitis media.

Chronic suppurative otitis media (CSOM) is one of the commonest illnesses in ENT practice which requires medical attention all the more in children of poor socio-economic status having in past inadequate treatment and negligent medical care. The present study was conducted to find out the various aerobic and anaerobic microorganisms associated with CSOM in paediatric and adult cases and their current antimicrobial susceptibility pattern as a guide to therapy. Samples were collected from 109 clinically diagnosed cases of CSOM and processed according to standard protocols. Out of 74 paediatric CSOM cases, 72 (97.2%) were bacterial culture positive while out of 35 adult CSOM cases, 28 (80%) were culture positive. Bilateral CSOM was slightly more common in adults (25%) than paediatric (21.4%) age group. Polymicrobial nature of CSOM was noted in both paediatric (70.8%) and adult (71.4%) cases while number of organisms isolated per lesion was slightly higher in adults (2.5) as compared to paediatric (1.95) cases. In paediatric CSOM, Staphylococcus aureus was the commonest aerobic isolate while in adult CSOM, Pseudomonas aeruginosa was the commonest one. Among anaerobes Peptostreptococcus spp. was commonest in CSOM where as Prevotella melaninogenica in adult CSOM. Sensitivity of S. aureus to cefuroxime was 72.2% while that of gram negative bacilli was higher to cefotaxime (90 to 100%). Among anaerobes higher sensitivity was seen to metronidazole (98.6%), clindamycin (95.7%) and chloramphenicol (98.6%). In view of the polymicrobial etiology of CSOM, prompt appropriate antimicrobial therapy can effectively reverse the disease process thereby preventing longterm sequelae.

[Hyperchronic bronchopulmonary suppurations (clinical study of 100 cases)].

Following an analysis of the clinical observations files the authors present a series of aspects and clinical data, as well as functional results in connection with the evolution in out-bursts of 100 cases with bronchopulmonary suppurations dating for more than 10 years. These anatomo-clinical entities, developed either in healthy or on pre-existing lesions, of polymorphous aspect and frequently invalidating, with evident tendencies to relapses, determining considerable difficulties in the application of therapeutical measures, make necessary the dispensarization of patients and this must be organized in the frame of the network for pneumophthysiology, in view of providing the high quality assistance that is necessary in such cases.
