[Staghorn calculi in the child. Apropos of 17 cases representing 20 kidneys].

The authors report their experience involving 72 cases of lithiasis in the child, 17 involving staghorn calcuti. Of a total of 79 staghorn calculi, 17 affected children. Such staghorn calculi in the child were almost always infected (proteus), often extensive but not associated with any impairment of renal function for a long period. No metabolic disturbance which could explain the formation of these calculi was found, in particular no cases of hyperparathyroidism. Treatment is surgical. The operation is long, difficult and delicate, always requiring peroperative contact films and often clamping of the renal pedicle in the case of nephrotomy. Such clamping is much better tolerated with the use of contact refrigeration of the kidney. On the basis of the results of quantitative renal scintigraphy, nephrotomy seemed better tolerated by the kidneys of children than by the kidneys of adults. True recurrences were rare (3/20) and small "forgotten" calculi (2/20) were often well tolerated.

Spontaneous proteus nephritis among male C3H/HeJ mice.

The spontaneous occurrence of nephritis and resultant mortality was investigated among 2,836 control or treated (dermal carcinogenesis bioassays) C3H/HeJ male mice. Overall incidence of nephritis putatively caused by Proteus mirabilis was 1.2%. Incidences were similar in control and treated mice and ranged from 2.5--32.5% among 73 unrelated treatment or control groups. Diagnosis was based upon characteristic gross or histologic renal lesions. Grossly affected kidneys contained either multiple, discrete tan foci or elevated pale tan patches. Histologic renal lesions consisted of a multifocal necropurulent nephritis with numerous gram negative bacteria present in affected tubules. Necrotizing pyelitis and papillitis usually was present, an necropurulent cystitis was present in half of the cases examined histologically. Proteus mirabilis, isolated in pure culture from an affected kidney, produced similar renal lesions after intraperitoneal or intravenous inoculation of clinically normal C3H/HeJ males. The organism was reisolated from the kidneys of the experimentally produced cases. It was concluded that nephritis putatively caused by Proteus mirabilis was an important cause of mortality in male C3H/HeJ mice.

[The protective activity of a multicomponent vaccine made from the antigens of opportunistic microorganisms when administered orally].

Multicomponent vaccine VP-4, prepared from Streptococcus, Klebsiella, Proteus and Escherichia coli antigens, was tested for protective activity by oral administration to mice with experimental Klebsiella, Proteus and S. pneumoniae infections. Vaccine VP-4, when introduced orally (in 2-3 administrations of 2 mg), was found to produce a protective effect with respect to K. pneumoniae (the effectiveness index was equal to 5.7 and 4.3). With respect to P. vulgaris, the effectiveness index was 2.7 (immunization was made in two oral administrations of 2 mg); in pneumococcal infection preliminary oral immunization with vaccine VP-4 in two administrations in a dose of 2-4 mg produced high protective effect. Thus, immunization with vaccine VP-4, introduced orally in comparatively small doses, proved to be effective in animal models used in the experiment.

[HLA-system antigens in patients with food poisonings caused by opportunistic bacteria].

A total of 286 convalescents after toxico-infections caused by opportunistic microorganisms were examined. The panel of anti-HLA sera permitted the typing of 34 HLA antigens, class 1. As a result, the development of the disease was found to be directly associated with antigen HLA-B17 and inversely with antigen HLA-A2. The relationship of antigens of the HLA system with alimentary toxico-infections caused by opportunistic microorganisms manifested not by the absolute resistance or predisposition to the disease, but by an increased severity of the pathological process, paralleled by a decreased level of protective antigens and an increased one of provoking antigens of the HLA system.

[The sanitary parasitological characteristics of different sources of drinking water supply].

In some CIS areas (Khabarovsk Territory, Moscow, Grodno, Odessa, Astrakhan, and Kzyl-Orda provinces), sewage, animal sewage, and sewage from the populated areas (82.3% of the samples tested), water in the surface reservoirs, sources of potable water supply (54.0%), and potable water (6.5%) were shown to contain viable agents of intestinal parasitic diseases, such as cysts of Lamblia and Balandia, eggs of ascarides, Trichuris trichiura, Diphyllobothrium, oncospheres of Taenia, etc.). This should be borne in mind in planning and implementing measures for sanitary protection of surface and underground potable water supply sources, recreational [correction of recreative] areas, for prevention of helminthiases and intestinal protozoan [correction of Proteus] infections.

Oral vaccination against Porteus mirabilis.

Mice given a single dose of proteus vaccine orally were protected against 1 MLD (minimum lethal dose) of autologous Proteus mirabilis by the fourth day after vaccination. Three doses of oral vaccine induced protection against 1 MLD autologous challenge for 7 days after vaccination and partial protection for a further 8 weeks. Cross-protection against different strains of Pr. mirabilis and against some strains of Pseudomonas aeruginosa, Serratia marcescens and Providencia species was found in mice immunized orally on three consecutive days with a bivalent proteus vaccine.

The value and limitation of pleural fluid pH measurement.

Analysis of an aliquot of fluid from a pleural effusion will often provide the clinician with the information needed to establish a presumptive or definitive diagnosis of the patient's illness. A properly performed pleural fluid pH can maximize the diagnostic yield and provide guidelines for further clinical interventions.
