Retention of the p53 codon 72 arginine allele is associated with a reduction of disease-free and overall survival in arginine/proline heterozygous breast cancer patients.

PURPOSE: The arginine to proline substitution at codon 72 represents a common aminoacidic polymorphism of the p53 protein. Recent data suggest that p53 codon 72 may modulate the response to cancer therapy. The aim of this study was to test the hypothesis that the p53 codon 72 genotype, evaluated in the tumor tissue and in the disease-free lymph node, is related to differences in disease-free and overall survival among breast cancer-affected patients. EXPERIMENTAL DESIGN: We assessed the p53 codon 72 genotype in DNA from disease-free lymph nodes and neoplastic tissues obtained from 67 women affected by breast cancer who underwent surgical resection at the Bologna Breast Cancer Surgical Unit from 1993 to 1995. RESULTS: We found that the retention of the p53 codon 72 arginine allele in the tumor tissue of proline/arginine heterozygous breast cancer patients is associated with statistically significant reduced disease-free and overall survivals. CONCLUSIONS: Our findings suggest that the genotyping for p53 codon 72 locus in both the tumor tissue and in the lymph node of breast cancer patients could contribute to identify a subset of arginine/proline heterozygous patients who have a reduced survival that is associated with the specific retention of the arginine allele in the tumor tissue.

Age and ethnicity as prognostic factors influencing overall survival in breast cancer patients in the Auckland region. Auckland Breast Cancer Study Group.

AIMS: to assess the effect of age at diagnosis and ethnicity on overall survival from breast cancer. METHODS: information was collected from 2706 breast cancer patients in the Auckland region, diagnosed between 1976 and 1985. Age at diagnosis was categorised into four groups: < 35 years, 35-49 years, 50-74 years and 75 years or older. Ethnicity was self reported in three categories: European, Maori and Pacific Island Polynesian. Independent effects were assessed by controlling for extent of disease, specifically metastases at presentation, nodal status and size of tumour. RESULTS: age and ethnic group were both significantly related to overall survival in univariate analyses. In multivariate analysis, age had an independent effect on survival mainly due to a significant survival difference between women aged less than 35 years and those aged 35 to 49 years (p < 0.0001; RR = 2.02). Survival was not significantly different between other age groups when adjustments were made for extent of disease. There was no significant effect of ethnicity on survival in the reduced dataset used for the multivariate analysis. Separate analyses suggested that ethnic differences in the extent of disease at diagnosis may be the cause of the apparent initial effect of ethnicity on survival. CONCLUSIONS: women < 35 years at diagnosis have a significantly poorer prognosis than women aged 35-49 years. Other differences between age groups were not significant when stage of disease was taken into account. Ethnicity was not an independent factor influencing survival after controlling for extent of disease but numbers in the Maori and Pacific Island groups were too small to conclusively evaluate any effect of ethnic group on prognosis.

[Overall long-term survival in lung cancer analyzed in 610 unselected patients].

INTRODUCTION AND OBJECTIVES: Many studies of lung cancer survival are carried out in patients selected for certain features that usually influence prognosis favorably. The objective of this study was to assess the overall survival of unselected patients with a diagnosis of lung cancer in our practice. PATIENTS AND METHODS: We studied 610 patients for whom survival information was available, a population comprising 88% of the 694 with lung cancer diagnosed in our hospital from 1991 through 1998. The variables analyzed for their correlation with survival were age, sex, histology, tumor-node-metastasis (TNM) stage, treatment, and time of diagnosis (with patients grouped by 2-year periods). RESULTS: The cases of 596 men and 14 women with a mean age of approximately 67 years were studied. Small cell tumors were found in 141, non-small cell tumors in 447, and other tissue types in 22. Surgical excision was carried out on 118 (19.3%), and treatment was confined to control of symptoms for 6.4% of the patients with small cell tumors and 40.5% of those with non-small cell cancer. Symptomatic treatment alone was more common for patients older than 70 years (52.5%) and less common during the last 2 years of the study period (1997-1998: 19%). Overall 5-year survival was 7.9% (2.8% in small cell cancer and 9.4% in non-small cell cancer). Survival rates were lower in patients over 70 years of age. Significant differences in survival were seen for successive TNM stages, with the exception of IIIA and IIIB. The 1997-1998 period saw better survival rates, at 40.8% after 1 year and 11.2% after 5 years. CONCLUSIONS: The survival rates in lung cancer patients in our hospital practice are low because the rate of surgical resections is low owing to the high percentage of cases found in advanced stages. Our observations are similar to those reported from other European countries.

Overall HIV-1 prevalence in pregnant women over-estimates HIV-1 in the predominantly rural population of Afar Region.

The appropriateness of sentinel sero-surveillance based upon ante-natal clinic (ANC) attendees to estimate HIV-1 prevalence in the general population has been questioned. In Ethiopia, where the population is heterogeneous and where economic and practical barriers to ANC attendance exist, problems of extrapolation may be exacerbated. We planned an unlinked anonymous sero-survey which included data on basic population characteristics to investigate whether sero-surveillance data from ANCs in Afar Region might be taken to represent the situation among the general population of the Region. 371 pregnant women attending Dubti Hospital and Assayta Health Centre were tested for HIV-1 (using a single ELISA test) and active syphilis (RPR test). Socio-demographic characteristics were collected for each woman. Of the women tested, 278 (75%) were 28 years of age or younger. Two hundred eighty (76%) were urban residents and 237 (64%) described themselves as being of Amhara ethnicity. Overall, 73 (19.7%) were HIV-1 positive, but prevalence was three times higher among the 237 women of Amhara ethnicity compared to the 112 of Afar ethnicity (24.9% vs 8.0%, p < 0.001), and almost three times higher for urban compared to rural residents (23.2% vs 8.8%, p < 0.001). Positive RPR results were strongly associated with HIV-1 infection (OR 3.37, 95% CI 1.47-7.71). According to the Demographic and Health Survey (DHS) 2000, only 4.5% of the population of Afar Region is of Amhara ethnicity, and 7.8% urban residents. We have demonstrated that basing ANC sero-surveillance in urban areas of Afar Region over-samples urban residents of Amhara ethnicity and yields a major over-estimation of overall HIV-1 prevalence for the Region. Reliable estimation of HIV-1 prevalence in Afar Region will require more flexible strategies that permit sampling of rural Afar residents.

[Choice of therapy and overall survival in patients with chronic myeloproliferative diseases].

A retrospective analysis of 106 case histories of primary chronic myeloproliferative diseases (CMPD) was undertaken: idiopathic myelofibrosis--71 (67%), polycythemia vera--29 (27.3%), and essential thrombocythemia--6 (5.7%), median age--65 years (26-84 yrs). Hydroxyurea and myelosan were mostly used as cytostatic drugs while erythrocyte mass transfusions and hemoexfusions (phlebotomy)--for life-support. Median overall survival in patients untreated with cytostatics was 95.2 years as compared with 156 months in recipients of such drugs. Survival rates in all CMPD patients with hypocellular bone marrow who had received cytostatics were lower than in those with normal or hypercellular marrow (p=0.005). Cytostatic therapy had either no impact on survival in patients with hypocellular bone marrow or was followed by decrease. Among CMPD patients who had received erythrocyte mass transfusions survival rates were significantly lower than in intact ones (p=0.0009). Median overall survival in patients receiving hemoexfusions was 193.6 months, as compared with 110.3 months in intact ones (p=0.008). Our data may be useful in selecting therapy for CMPD.

Association of disease progression and poor overall survival with detection of circulating tumor cells in peripheral blood of patients with metastatic breast cancer.

The aim of this study was to define the frequency and clinical relevance of cytokeratin positive metastatic tumor cells in the peripheral circulation of patients with stage IV breast cancer. Peripheral blood was collected from 32 consecutive patients with metastatic breast cancer and 23 healthy donors. Tumor cells were enriched using positive selection with anti-HEA125-microbeads and cytospins were prepared of the positive selection eluate. Slides were incubated with a Fab2 fragment of the pancytokeratin antibody A45-B/B3 conjugated with alkaline phosphatase (AKP) and a CAM5.2-AKP monoclonal antibody and developed with an alkaline phosphatase anti-alkaline phosphate reaction (APAAP). All samples were evaluated using light microscopy and an automated image analysis system. In 8/32 (25%) patients cytokeratin positive (CK+) cells could be detected after anti-HEA125 enrichment in the peripheral blood whereas in none out of 23 healthy donors. One to 1000 (median 5) positive cells per patient sample were observed and cluster of tumor cells in one patient. Automated image analysis was as powerful in detecting micrometastases as conventional light microscopy. All patients with CK+ cells in the peripheral circulation (8/8, 100%) showed progressive disease at the time-point of blood draw whilst only 9/24 (37.5%) showed disease progression without detection of positive cells. The median overall survival of CK+ patients was 4+/-2 months compared to 13+/-7 months of CK- patients (p<0.001). CK+ cells are detectable in the peripheral circulation of 25% of patients with metastatic breast cancer after positive selection with anti-HEA125. Detection of tumor cells in the peripheral circulation might be correlated with progression of disease and shorter overall survival.

Patients' perceptions of nurse staffing, nursing care, adverse events, and overall satisfaction with the hospital experience.

Noticeably absent in the discussions surrounding nurse staffing and adverse events is the patients' perspective, except for their satisfaction with various aspects of their hospitalization experience. The results of a study undertaken to test a theoretical model of patients' perceptions of nurse staffing, the nursing care they receive, self-reported adverse events, and overall satisfaction with the hospital experience are presented. Perception of nurse staffing only weakly predicted the number of adverse events reported by patients, but was a strong predictor of the perception of nursing care received, and the perception of nursing care received was the only significant predictor of overall satisfaction with the hospital experience.

Intercellular adhesion molecule-1(ICAM-1), CD44s expression and serum level of sICAM-1 in disseminated non-Hodgkin's lymphoma: correlation with overall survival.

BACKGROUND AND PURPOSE: Uncontrolled growth, invasion and distant spread are the characteristics of neoplastic cells. Changes in adhesion molecule expression, such as loss of expression, de novo expression or functional alterations, can be observed in each of these steps. The aim of the present work was to study the expression of two adhesion molecules [standard CD44 (CD44s) and intercellular adhesion molecule-1 (ICAM-1)] and the soluble form of ICAM-1(sICAM-1) in adult disseminated non-Hodgkin's lymphoma (D-NHL) and to correlate the findings with overall survival. PATIENTS AND METHODS: The expression of CD44s and ICAM-1 adhesion molecules and the level of sICAM-1 were studied in 34 cases of (D-NHL). They included: 13 cases of lymphoblastic lymphoma (LBL), 5 cases of Burkitt's lymphoma (BL), one case of diffuse large cell lymphoma (DLCL), 14 cases of small lymphocytic lymphoma (SLL), one case of mantle cell lymphoma (MCL). Ten apparently healthy individuals were taken as a control group. CD44s expression was evaluated by direct immunofluoresence, ICAM-1 by immunoperoxidase on cyto-preps and serum level of sICAM-1 by ELISA. RESULTS: ICAM-1 was positive in 6/34 cases (17.6%) of D-NHL. ICAM-1 was expressed in 2/15 (13.3%) of low-grade NHL and 4/19 (21%) of high-grade lymphoma with no significant difference between the two groups (p=0.6). CD44s was expressed in 13/34 cases (38.2%) of D-NHL; in 4/15 (26.6%) of low grade NHL and 9/19 (47.3%) of high grade lymphoma with no significant difference between the two groups (p=0.2). The serum levels of sICAM-1 were elevated in all patients with D-NHL compared to healthy controls with a statistically significant difference (p <0.001), (median 1160 ng/ml, range from 150 to 2500 ng/ml, and 375 ng/ml, range from 270 to 620 ng/ml, respectively). The median of sICAM-1 in patients with high-grade D-NHL was significantly (p=0.006) lower than that of those with low-grade D-NHL (median 890 ng/ml, range from 150 to 1540 ng/ml and 1440 ng/ml, range from 380 to 2500 ng/ml respectively). The median follow-up duration for all patients was 18 months. No statistical significant difference was achieved (p=0.8) on comparing overall survival pattern between D-NHL lymphoma patients with positive or negative expression of ICAM-1 or sCD44 expression. Also, no statistically significant difference (p=0.9) was found between patients with sICAM (above and below 600 ng/ml) at 18 months from diagnosis. CONCLUSION: There is a marked heterogeneity in cell adhesion molecule (CAM) expression in NHL and this may correlate with degree of differentiation of malignant lymphocytes. However the exact significance of these findings will require functional studies to determine the role of these CAMs in each subtype of NHL.

Effects of ivermectin and diethylcarbamazine on microfilariae and overall microfilaria production in bancroftian filariasis.

Ivermectin and diethylcarbamazine (DEC) are used in mass treatment programs for the elimination of lymphatic filariasis because of their strong effects on microfilaremia. However, the effects of treatment on adult worms and the degree of individual variation in efficacy are unclear. We analyzed series of microfilaria (Mf) counts from individuals treated with a single dose of 400 microg/kg ivermectin or 6 mg/kg DEC (N = 23 in each group; 1 year follow-up). For each individual, we estimated the microfilaricidal effect and the reduction in overall Mf production (e.g., caused by death or sterilization of worms, or inhibited Mf release from the female worm uterus). Ivermectin on average killed 96% of Mf and reduced Mf production by 82%. DEC killed 57% of Mf and reduced Mf production by 67%, with some individuals responding very poorly. The strong reduction in overall Mf production is good news for control of lymphatic filariasis, but the prospects of elimination will be diminished if part of the population systematically responds poorly to treatment.

Overall prognosis in younger patients with colorectal carcinoma.

AIM: Prognosis of colorectal cancer in younger patients is a controversial issue. While survival after curative resection seems not worse than in older patients, few data are available about the overall survival of younger patient with colorectal cancer irrespective of receiving a curative treatment or not. In this study we have reviewed our experience with patients under 45 years of age affected by colorectal cancer. METHODS: A total of 358 patients with colorectal cancer were operated on between 2001 and 2005. Nineteen patients were younger than 45. Schedules were reviewed as to palliative vs curative resection, Duke's classification, tumor grading, and compared with data from patients older than 45. RESULTS: Though no significant difference in survival can be detected after curative resection, fewer patients less than 45 years are eligible to be treated curatively. Significantly worse tumor grading and rates of curative resection make for a poorer prognosis in younger patients when compared with older ones. CONCLUSIONS: Prognosis for colorectal cancer in the overall group of patients less than 45 years is worse than in older patients. Younger patients more frequently present with distant metastases or peritoneal carcinosis which preclude the possibility of a curative treatment.

[Evaluation of the protective properties of nonwoven materials for throw-away medical overalls].

The permeability of varying density felts (Spandbond, SMS, Tyvek, Sontara) for throw-away medical overalls was experimentally assessed under statistic and dynamic conditions, by using model test aerosols, including bacterial one. Their permeability was shown to decrease with the higher density of the materials under study. Laminated Span-bond contributes to an abrupt reduce in the bacterial penetration of this kind of tissues and enhances their protective properties. The felts CMC, Tyvek, Sontara, and laminated Spanbond were found to have the highest protective properties. In terms of their barrier and protective properties, the felts surpass the tissues used to make re-usable protective medical overalls.

[Overall migartion testing with alternative fatty food simulants].

Overall migration from commercial samples of food plastics packaging into 95% ethanol and isooctane--the alternative fatty food simulants--was determined. Testing conditions--time and temperature of the contact of food packaging material with food simulants reflected real or predicted use of given packaging. The analytical methods according to European Standard EN 1186 were used. Test methods by filling the artiecles, total immersion, forming a pouch or using the cell were used for migration as appropriate to allow contact with liquid food simulant only this surface which will come into the contact with food during normal use of the product. The overall migration from the samples of the tested products into both food simulants (95% ethanol and iso-octane) was well below the limit (10 mg/dm2), except of the migration from plasticized PVC films which markedly exceeded above limit, showing that these films must not be used for packaging of food containing fat. Application of food simulants as a alternative to olive oil markedly shortens analytical procedure and reduces workload, both important in routine testing.

A computational model for the overall pattern of ocular dominance.

In layer IV of the primary visual cortex, in both the macaque monkey and the cat, geniculocortical terminals representing the two eyes are segregated into alternating zones known as ocular dominance bands. Viewed tangentially, in the monkey these bands take the form of a series of branching parallel stripes that run roughly perpendicular to the border of striate cortex. In the cat, the overall ocular dominance pattern consists of irregularly branching, beaded bands that exhibit no predominant orientation. If the striking differences in the appearance of these two patterns reflect important differences in the basic rules governing cortical ocular dominance, then this poses a problem for attempts to formulate general principles of visual cortical organization. However, it has been suggested that the differences in the appearance of the ocular dominance patterns in these two species could result simply from known differences in the boundary conditions of their geniculocortical pathways. This article describes the formulation and testing of a single computational model that accurately predicts the quite dissimilar ocular dominance patterns in cats and monkeys. This model also generalizes to predict the different ocular dominance patterns observed in young and old three-eyed frogs, supporting the notion that the overall pattern of ocular dominance is governed by a common set of rules. The significance of these results is discussed in terms of previous models, which have focused largely on local processes underlying the development of ocular dominance segregation. Although the present model is not a developmental one, it does shed some light on potential mechanisms for establishing retinotopy in striate cortex and on possible developmental relationships between the geniculostriate pathway and intrinsic modularity of the striate cortex.

The effect of enzyme-enzyme complexes on the overall glycolytic rate in vivo.

Recent studies have demonstrated that most glycolytic enzymes can reversibly associate to form heterogeneous enzyme-enzyme (binary) complexes in vitro. However, kinetic analysis of these complexes has shown that the individual enzymes have a varied response to complex formation: some enzymes are inhibited, some are activated and some are unaffected. In order to determine the potential role of binary complexes in regulating glycolytic flux, we have mathematically calculated enzyme distributions and activities using data from in vitro binding and kinetic studies. These calculations suggest that, overall, formation of binary complexes would lower flux through phosphofructokinase and aldolase, would increase flux through glyceraldehyde-3-phosphate dehydrogenase and lactate dehydrogenase, and would not affect flux through triosephosphate isomerase, phosphoglycerate kinase and pyruvate kinase. The implications of these results are discussed with respect to the effect of complex formation on overall glycolytic flux and on the flux through individual enzyme loci.

Cephalometrics and facial shape: new thresholds by an overall approach to classic standards.

Pretreatment lateral skull radiographs were studied to analyze the characteristics of facial morphology. Standard cephalometric measurements were compared with those provided by a Fourier description of the overall morphology. Skull patterns were divided into hyperdivergent, neutral, and hypodivergent groups (1) according to the standard thresholds of facial height ratio and (2) according to the mathematical distance between their Fourier coefficients and the Fourier coefficients of three reference plots. Linear regression analysis showed that all the Fourier variables were significantly correlated. The standard thresholds of normality used to classify patients as normal, hyperdivergent, or hypodivergent did not correlate with each other. Fourier analysis resulted in new thresholds of normality that are more accurate than previous norms developed by standard cephalometric measurements. These new thresholds take into account the overall morphologic characteristics of the patient and each separate measurement is strictly correlated with the others.

[Hygienic assessment of protective effects of overalls and individual protective devices used in the production of ampicillin trihydrate and its dosage form, ampicillin sodium salt].

The working clothes and individual protective devices recommended by the industrial Rules for the production of ampicillin trihydrate and its medicinal derivatives such as natrium salt ampicillin, do not provide adequate protection of the skin and respiratory mucous from antibiotic contamination. The use of the overalls in combination with the pressurized helmet (designed and produced at the 'Sintez' enterprise) which provided air supply for breathing and, partially, under the overalls, significantly decreased contamination of the body. At the stages of manual handling of the finished products, adequate protection was attained by using the protective paste produced at the 'Altaivitamine' enterprise.

Relationship of sex hormone binding globulin to overall adiposity and body fat distribution in a biethnic population.

Previous data have indicated that decreased sex hormone binding globulin (SHBG) is associated with increased overall and upper body adiposity and higher levels of glucose, insulin and triglyceride (TG) and decreased levels of high-density lipoprotein (HDL) cholesterol. Since Mexican Americans have greater overall and upper body adiposity, higher rates of non-insulin-dependent diabetes mellitus, higher TG and lower HDL levels than non-Hispanic whites, we postulated that they would also have lower levels of SHBG. We measured total testosterone and total estradiol using a commercial radioimmunoassy and SHBG using a dextran-coated charcoal technique in premenopausal women (61 Mexican American and 39 non-Hispanic white) as part of the San Antonio Heart Study, a population-based study of diabetes and cardiovascular risk factors. There were no significant ethnic differences in total testosterone or total estradiol. SHBG, however, was lower in Mexican American (0.285 micrograms/dl) than in non-Hispanic white women (0.429 micrograms/dl) (P = 0.009). After adjustment for body mass index (BMI), ratio of waist-to-hip circumference (WHR) and ratio of subscapular-to-triceps skinfolds (centrality index), SHBG remained lower in Mexican Americans (0.307 micrograms/dl) than in non-Hispanic whites (0.396 micrograms/dl), although this difference was no longer statistically significant (P = 0.083). BMI, WHR and centrality index were all negatively associated with SHBG (P less than 0.01). The lower levels of SHBG in premenopausal Mexican American women compared to non-Hispanic white women may reflect greater in-vivo androgenicity and may be related to a variety of metabolic abnormalities seen in this ethnic group.

The quality of pain in arthritis: the words patients use to describe overall pain and pain in individual joints at rest and on movement.

One hundred and five patients with polyarthritis chose from 14 sensory and 5 affective pain descriptors derived from the McGill Pain Questionnaire (MPQ), and completed visual analogue scales (VAS) for overall and individual joint pain at rest and on movement. The relative frequency of sensory pain descriptor choice varied for the 4 different circumstances of pain. Over one third of patients volunteered affective words not included in the MPQ. This study suggests that conventional overall measures of pain, such as the MPQ, when used for patients with arthritis may neglect differences in pain experience perceived in individual joints and on movement.
