[The characteristics of the effect of point and classical massage on the hemodynamics of patients with a history of transient ischemic attacks in the vertebrobasilar system].

Clinical and instrumental studies have revealed differences in effects of nerve ending and classic massage on hemodynamics in 41 patients early after transitory ischemic attacks in the vertebrobasilar area. Point massage produced more potent vasotropic effect, contraindications to it are minimal. It can be considered as a pathogenetic therapy aimed at correction of cerebral circulation in patients with vertebrobasilar area applicable early after acute cerebrovascular episodes.

Perception of tactile massage as a complement to other forms of pain relief in rheumatic disease.

OBJECTIVE: To investigate how patients with rheumatic disease perceive tactile massage as a complement to other pain alleviation methods. METHODS: A phenomenographic approach with semi-structured interviews was employed on a strategic sample of 14 patients with various rheumatic diseases, both inflammatory and non-inflammatory, who had been admitted to Spenshult Rheumatic Hospital. RESULTS: Three descriptive categories with a total of nine conceptions emerged. The descriptive categories were: experiencing alleviation, experiencing trust, and gaining insight. Experiencing alleviation described how patients experience relaxation, pleasure and respite. Experiencing trust described how patients experience a sense of security, confirmation and inner peace. Gaining insight described how patients get to know themselves, see possibilities, and experience wholeness. CONCLUSIONS: When caring for patients who have a chronic illness involving considerable changes to their lives, it is important for health professionals not only to be aware of their physical needs but also to take account of the whole person. This study demonstrates the importance of offering patients a complement to conventional treatment. Tactile massage is a method that promotes a holistic view of patients with chronic pain as well as allowing them to find a new focus in terms of the disease and how they cope with it in their daily lives.

Hyperkalemic cardiac arrest, prolonged heart massage and simultaneous hemodialysis.

A 53 year old diabetic patient underwent CABG and aortic valve replacement in another institution and developed postoperative oliguric and hyperkalemic acute renal failure. Shortly after transferring to our unit a cardiac arrest occurred. Immediate resuscitative measures were ineffective. The serum potassium level was 10.2 mmol/l. Conventional arteriovenous hemodialysis was initiated while the patient was still undergoing cardiac massage. When the serum potassium level was lowered to 6.5 mmol/l, 90 min later, the heart began to beat. After hemodialysis was discontinued the patient was reactive and fully conscious. The use of simultaneous hemodialysis with prolonged mechanical heart massage as a reliable method for recovery in hyperkalemic cardiac standstill is stressed.

Patrons of massage parlors: some facts and figures.

A survey of 183 patrons of a massage parlor in Illinois indicated that the typical customer is a 35-year-old married white male from out of town with 13 years of education who is employed in a lower- or middle-class job and goes to church on Sunday. He reports having had a variety of heterosexual experiences in the past, has come to the parlor because of lack of sexual partner at this particular time or because of curiosity, will come to orgasm during the genital massage, and will find it sexually satisfying. He is likely to have high self-esteem, consider himself personally and sexually adjusted, consider his value system liberal, and to be somewhat sympathetic to the goals of the women's rights movement.

Detection of telomerase activity in prostate massage samples improves differentiating prostate cancer from benign prostatic hyperplasia.

PURPOSE: We performed a case-control study in which we tested the ability of a non-invasive assay to detect telomerase activity and to distinguish between prostatic cancer (Pca) and benign prostatic hyperplasia (BPH) on samples of epithelial cells obtained after prostatic massage. METHODS: Telomerase activity was determined by a telomeric repeat amplification protocol (TRAP) assay. We selected 60 patients with histologically proven Pca (30 cases) or BPH (30 cases). Specimens included in this study were from patients who had no suspicious findings on digital rectal examination for cancer, had clinical evidence of lower urinary tract symptoms, had no sonographic signs of Pca at the transrectal ultrasound evaluation, had total PSA values moderately elevated (2.6-15 ng/ml), and had no evidence of other urological cancers. The whole procedure was conducted in double blind between pathologists and molecular biology operators. RESULTS: Telomerase activity was detected in 90% of Pca cases and in 13% of BPH cases. The sensitivity (90%) and specificity (76%) of this method were calculated. The positive predictive value, negative predictive value, and diagnostic efficiency were 87%, 90%, and 88% respectively. CONCLUSION: Our data indicate that telomerase activity detected by TRAP assay on prostate epithelial cells collected by prostate massage can substantially improve the distinction between Pca and BPH conditions. One of the clinical benefits resulting from the use of this new assay would be to refine the biopsy indication and to avoid for several patients without Pca the unnecessary cost and the complications of prostate biopsy.

Effects of intrajugular hypertonic saline, vaginal distension and vulvar massage on activity of supraoptic neuroendocrine cells.

Activity of 40 single antidromically identified supraoptic neurons was recorded and evaluated in response to a combination of tactile, vulvar massage, vaginal distension, and slow intrajugular 1.2 M sodium chloride infusion in unanesthetized, randomly hydrated ewes. Estradiol-implanted Southdown ewes were prepared according to techniques described by Jennings et al. Only 4 spontaneous firing patterns were observed in the supraoptic nuclei. Analysis of evoked activity indicated that each stimulus evoked alterations in mean firing rates or increased numbers of short interspike intervals in some cells. The resultant activity of units to sequential vulvar massage and 1.2 M sodium chloride infusion suggests a possibility of separate mechanisms of release of oxytocin and vasopressin.

Elevation of serum prostatic acid phosphatase levels after prostatic massage.

The effect of prostatic massage on the serum prostatic acid phosphatase (PAP) levels determined by radioimmunoassay (RIA) was studied in 29 patients with benign prostatic hyperplasia (BPH) and 7 patients with prostatic carcinoma (CA). Among the BPH patients, 77 per cent (P less than 0.001) showed an increase in post-massage PAP levels but only 3 (10%) showed an increase to more normal levels.

Mechanism of liver gene transfer by mechanical massage.

Many metabolic diseases are caused by defects in the metabolic pathways in the liver. Others result from the absence of specific proteins normally produced and secreted by the liver. Because these metabolic disorders are usually caused by single gene defect, they are ideal candidates for gene therapy. We have previously shown that mouse liver can be transfected by mechanically massaging the liver (MML) after intravenous injection of naked plasmid DNA. We now show a significant linear relationship between the level of liver gene expression and the venous blood pressure, supporting the idea that gene transfer by MML is due, at least in part, to pressure-mediated effect. Liver transfection could not be blocked by co-injection of excess irrelevant DNA or poly I, suggesting that there is no involvement of receptors, including the scavenger receptor, in MML. Moreover, the level of gene expression could be further enhanced by a combination of MML and an increase in DNA retention-time in the liver. Persistence of gene expression could be significantly improved using an EBV-based plasmid vector. Our data suggest the mechanical massage produces transient membrane defects through which naked DNA can enter into the liver cells by simple diffusion.

Effects of closed-chest cardiac massage on intracranial pressure.

Closed-chest cardiac massage results in a marked increase in intrathoracic pressure and unusual patterns of blood flow. Among the physiological consequences of these changes appears to be a marked increase in intracranial pressure associated with chest compression as documented by the following patient case studies. While temporary, the marked nature of this rise in intracranial pressure suggests that the technique of closed-chest massage may, in itself, be responsible for clinically significant alterations in cerebral hemodynamics.

The effect of acupressure with massage on fatigue and depression in patients with end-stage renal disease.

Fatigue and depressive mood are the most significant symptoms experienced by patients with end-stage renal disease. The purpose of this study was to examine the effectiveness of acupressure with massage in fatigue and depression in patients with end-stage renal disease (ESRD) receiving hemodialysis treatment. The study applied an experimental pretest and posttest design. Sixty-two hemodialysis patients participated in the study. Data were collected from two hemodialysis clinics in major hospitals in southern Taiwan. Following consent to the study, subjects were randomly assigned to an acupressure group or a control group. Patients in the acupressure group received acupoint massage for 12 minutes per day, three days per week, for four weeks. Subjects in the control group only received routine unit care. The measures included the Revised Piper Fatigue Scale, and Beck ' s Depression Inventory. Descriptive statistics, chi 2 tests, t-test and analyses of covariance were used for data analysis. The results indicate that subjects experienced a moderate level of fatigue. Nearly 65 % of hemodialysis patients had a depressed mood. ANCOVA results indicated that fatigue (F((1.54)) = 9.05, p =.004) and depression (F((1.54)) = 4.20, p =.045) among patients in the acupressure group showed significantly greater improvement than patients in the control group. The findings of this study provide an interventional model for nurses taking care of ESRD patients.

Comparison between the sperm morphology in semen samples obtained from yearling beef bulls by transrectal massage of the ampullae and cauda epididymal dissection.

As electroejaculation (EEJ) is prohibited for use on unanaesthetized animals in Sweden, there is a need for an alternative method of semen collection from bulls in the field. The aim of the present study was to evaluate the use of transrectal massage (TM) of the ampullae to collect semen from yearling beef bulls under field conditions in Sweden. Transrectal massage was performed on 52 yearling beef bulls. Volume of semen collected, duration of procedure, percentage progressively motile sperm, and sperm concentration were measured. Smears were prepared for sperm morphology examination. Semen samples were obtained from 47 of 52 bulls. Mean volume was 3.2 ml (SD +/- 3.7), mean duration of collection was 7.4 min (SD +/- 2.8), mean percentage progressively motile sperm was 43.5% (SD +/- 29.2) and mean concentration was 201.9 x 10(6) spermatozoa/ml (SD +/- 278 x 10(6)). Twenty-three of the 52 bulls were slaughtered 3-4 days after semen collection and aliquots of the cauda epididymal contents were collected for sperm morphology examination. The percentages of proximal droplets, abnormal tails and abnormal midpieces were significantly higher (p < 0.05) and the percentage of detached heads was significantly lower (p < 0.05) in the post-mortem samples compared with those in the TM samples. However, importantly there was no significant difference between the two sample types in the percentages of abnormal heads. This study demonstrates that semen can be collected from yearling beef bulls by TM. We think that TM constitutes a useful tool, when semen collection with EEJ or artificial vagina (AV) is not possible under field conditions, when included in the bull breeding soundness evaluation (BBSE) protocol. However, further studies are needed, and presently being carried out, to evaluate if semen samples collected by TM are comparable with semen samples collected by AV.

[The effects of a foot-reflexo-massage education program on foot care in diabetic patients].

PURPOSE: This study was conducted to evaluate the effects of foot care education program using foot-reflexo-massage in diabetic patients. METHOD: A convenience sample of non-equivalent control group time series design was used. It provided foot care education to diabetic patients through small book for both group. For the experimental group, foot-reflexo-massage was taught by a researcher and research assistants. Analysis was done by Repeated Measured ANOVA. RESULT: There was significant increase in foot care knowledge, self care behavior, between the experimental group and the control group over three different times. There was significant in skin temperature and pulse of foot over three different times and interaction by groups or over time, but there was no significant difference between groups. There was no significant difference in blood flow volume and capillary filling time over three different times, between groups, but there was interaction by groups or over time. But there was significant difference in discrimination in change of dosalis pedis artery blood flow. CONCLUSION: Findings indicate that this study may contribute to develop nursing intervention for foot care of diabetic patients.

Cytologic examination of post prostatic massage specimens as an aid in diagnosis of carcinoma of the prostate.

The results of cytologic examination fo post-massage prostatic secretion from symptomatic and asymptomatic subjects were evaluated by comparison with available tissue diagnosis. Specimens studied consisted of post-prostatic massage secretion and urine. The latter specimen gave much superior results. Although possibility of the cytologic diagnosis in prostatic carcinoma cases was limited, it proved to be reliable. Presently available short follow-up data indicate that this technique may play a role only as a diagnostic aid, however, its real value in prostatic cancer detection, could be only evaluated by long term follow-up of the "high risk" group.

Slow stroke back massage: its effect on patients in a rehabilitation setting.

A quasi-experimental design was used to determine the effects of three consecutive days of slow stroke back massage (SSBM) on adult patients in a rehabilitation setting. This study used the Huckstadt Touch Instrument to assess physiological and psychological responses to touch, as well as the recipients' perceptions of touch. The convenience sample comprised 24 adult patients in a rehabilitation hospital in southeastern North Carolina. Subjects' ages ranged between 52 and 88 years with a mean of 71.8 years. There was a significant decrease in systolic and diastolic blood pressure after SSBM on all 3 days. There was a statistically significant decrease in mean heart rate and mean respiratory rate on Days 1 and 3. There was no psychological change in any of the patients. Perception scores, however, indicate a positive response to SSBM. Patients perceived it as being comfortable, good, pleasant, and warm. On all occasions, their responses indicated that the intervention made them feel cared for, happy, physically relaxed, less anxious, calm, restful, and gave them a feeling of closeness with the nurse.

An apparatus for applying a mechanical massage to rat hearts inside a wide-bore NMR spectrometer.

A device for applying mechanical massage to the isolated perfused rat heart inside a wide-bore NMR spectrometer was developed. This device exerts a squeezing pressure on a fibrillating heart placed inside a NMR spectrometer without interfering with the NMR measurements. It appears that the apparatus is important for obtaining reliable results in the postischemic reperfusion period.

Zinc in post prostatic massage (VB3) urine samples: a marker of prostatic secretory function and indicator of bacterial infection.

The value of biochemical examination of post-prostatic massage (VB3) urine samples has been investigated. Measurement of zinc levels provides a good marker of prostatic secretory function. In the absence of prostatic carcinoma findings of greater or equal to 100 micrograms zinc in VB3 urines is strong evidence (2% false negatives) of an absence of infectious or inflammatory prostatic disease. Less than 40 micrograms of zinc is suggestive (14% false positives) of prostatitis. There is little need to consider the endogenous urinary zinc levels. This simple test should be of particular value in cases where an expressed prostatic secretion is not obtained (about 40% of this series of patients), when it would be a valuable adjunct to cytological and bacteriological examination of split urine samples.

[2-stage rupture of the liver as a complication of external heart massage].

A delayed rupture of the liver after resuscitation is reported. In the case of a 27-year-old woman, pulmonary embolism was followed by asystolia, which was remedied by external cardiac massage. This procedure caused ruptures of the liver, which first led to the formation of a subcapsular hematoma and 5 1/2 hours later, due to a secondary rupture of the capsule, led to internal bleeding and death. Degenerative changes of the parenchyma cells in the vicinity of the rupture were recognized, as well as fibrin separations on the detached Glisson's capsule, as signs of a survival period of several hours. Histological investigation of the lungs revealed a hepatocellular embolism and--in the absence of rib or sternum fractures--fat and bone-marrow embolisms. The morphological findings are discussed with reference to pertinent reports in the literature.

Atrial fibrillation precipitated by carotid sinus massage in a patient with unexplained syncope.

BACKGROUND: Carotid sinus massage (CSM) is a useful tool in the investigation of unexplained syncope, but is associated with risks. AIM: To describe the first reported case of atrial fibrillation precipitated by CSM during the investigation of a patient with recurrent unexplained syncope. RESULTS: A 58-year-old female underwent CSM as part of a tilt test protocol for the investigation of recurrent unexplained syncope. Left CSM revealed the diagnosis of mixed cardioinhibitory and vasodepressor carotid sinus syndrome but also precipitated atrial fibrillation, an arrhythmia previously unreported as a complication of the procedure. CONCLUSIONS: CSM is a useful tool in the investigation of patients with recurrent unexplained syncope, but may cause atrial fibrillation in addition to other previously described complications and should, therefore, be used with caution.
