Development of mini-gel technology in two-dimensional electrophoresis for mass-screening of samples: application to tears.

Despite the extensive literature available on tear proteins and lipids, very little has been reported on the tear fluid as a whole and it's changes in contact lens wear or ocular diseased patients. Initially a human reflex tear two-dimensional map was created by Molloy et al. (Electrophoresis 1997, 18, 2811-2815), using this information a process for mass-screening was established. The large format two-dimensional technique was evaluated, using a basal tear reference map, and modified to describe a fast, efficient and cost effective method of protein separation. The use of one pH 3-10 18 cm nonlinear immobilised pH gradient (IPG) strip and two mini-gels for the second-dimensional sodium dodecyl sulphate-polyacrylamide gel electrophoresis (SDS-PAGE) results in an effective separation of tear proteins which will be applied in diagnostic studies of tear samples.

Evaluation of mass screening program for stomach cancer.

Particular method and program of mass survey examination of gastric cancer were introduced into Miyagi Prefecture and the results accumulated during the past 14 years are presented. During this period, 1,427 cases of gastric cancer (0.18%), as well as many cases of other diseases of the stomach and duodenum, were found by the mass survey. 450 cases were those of early cancer in which invasion of carcinoma was limited to the mucosa and submucosa. The ratio of surgically confirmed early cancer cases to all the stomach cancer cases was 36.4%. Almost in all cases of early stomach cancer there were neither complaints nor clinical symptoms; that is, they were the cases of so-called preclinical cancer of the stomach. The prognosis after surgery of early stomach cancer was remarkably favorable with a five-year survival rate over 90% and the death rate due to stomach cancer was actually decreased in the surveyed population.

[Automation of mass screening for glaucoma].

An experimental model (mock-up) of automated complex "Glaucoma" has been produced for mass population screening. It comprises five ophthalmologic instruments: for history data collection; for visual acuity examining; for examining the visual fields limits (perimeter); for determination of paracentral sites within visual field (campimeter), and finally a device for intraocular tension measurement. A special computer serves to control the operation of these instruments, collection, processing and display of information. A set including magnetic recorder, projector and synchronizer provides instructions for patients. The latter were examined following special lecture explaining the purpose of investigation, operation rules for these instruments and sequence of procedures.

[Organization of prophylactic mass screening for tuberculosis of the population in the south of the Aral sea cost region].

The results of mass prophylactic screening of the population for tuberculosis in the south of the Aral sea costal region have been generalized with consideration of its geographic and sanitary--epidemiologic conditions. A mixed model of comprehensive prophylactic screenings was used for the first time with integration, for the first time, of the traditional (team) and field-work methods. The obtained results proved the efficiency of this approach: during one year a total of 1,217,400 subjects or 80% of the relevant population were involved in the prophylactic screening for the first time in one region. As a result, the epidemiologic situation related to tuberculosis was revealed and a complex of the therapeutic and health-improvement measures accomplished. The proposed measures have been tested and made the basis of a section in a complex program "Health of the population of the Aral sea costal region up to the 2000th year" which is recently being introduced in other regions of the republic.

Detection of succinylacetone and the use of its measurement in mass screening for hereditary tyrosinemia.

A technique designed to measure quantitatively succinylacetone (4,6-dioxoheptanoic acid) is presented. It essentially involves the inhibition of delta-aminolevulinate dehydratase (EC 4.2.1.24) by succinylacetone. Prior to their use in the assay, the samples are heated at 100 degrees C for 30 min in order to transform all succinylacetoacetate (3,5-dioxooctanedioic acid) to succinylacetone. By this transformation of the first abnormal metabolite specific to hereditary tyrosinemia to the second and last one, which is a powerful inhibitor of delta-aminolevulinate dehydratase, we determine in one sensitive assay the total amount of both. Succinylacetone was measured in sera and urines from 19 patients with hereditary tyrosinemia. All sera and urines contained succinylacetone at concentrations ranging, respectively, from 2 to 100 mumol/l and from 190 to 6000 mumol/g creatinine. The technique was also adapted to dried blood spots on paper and was used as a test complementary to blood tyrosine determination in mass screening for hereditary tyrosinemia. A total of 2412 samples having concentrations of 60 mg/l or more of tyrosine were assayed, and ten showed the presence of succinylacetone. These were all from newborns with hereditary tyrosinemia. The test has proven to virtually eliminate false positives, and, thereby, much clerical work and parental anxiety.

A mass screening device of genome by polymerase chain reaction-restriction fragment-single strand conformation polymorphism analysis.

We have developed a new genome screening method and named it as polymerase chain reaction-restriction fragment-single strand conformation polymorphism (PCR-RF-SSCP) analysis. This method consists of three steps: (1) amplification of long DNA by PCR; (2) digestion of the amplified PCR products by restriction enzyme(s) and labelling the restriction sites; (3) polyacrylamide gel electrophoresis under non-denaturating conditions (SSCP analysis) and under denaturating conditions containing 8M urea. Theoretically, this method enables us to detect even a base substitution, deletion or insertion in up to 22,000 base pairs amplified from genomic DNA by PCR in one analysis. The procedures are very simple, reproducible and require no special apparatus. This method is applicable to any genes already known and we believe that this method is very useful for mass screening of the genome.
