[Structure of treatment expenditures for hemodialysis patients (data of Kaunas University of Medicine Hospital in the year 2001)].

Prevalence of the end-stage renal disease patients is rising every year. In Lithuania the cost of hemodialysis procedure is under regulation of the Minister of Health. The objective of our study was to evaluate expenditures for hospitalizations and medical ambulatory treatment using data of Kaunas University of Medicine Hospital dialysis center in the year 2001. We analyzed ambulatory case records of all 111 end-stage renal disease patients dialyzed in our clinic in November 2001. Retrospective data about hospitalizations, hospital beds, expenditures for hospitalizations and medicamental ambulatory treatment were collected. There were 123 hospitalizations in 75 patients, 1967 total hospital days. Admission rate was 1.34+/-1.5 per patient year, 19.82+/-25.96 hospital beds per patient year, the mean duration of each hospitalization - 16.26+/-11.57 days. Expenditures for hospitalizations were 1415 litas ($ 354) per patient year. In average, hemodialysis procedures were carried out 2.5 times a week per patient (or 130 times a year) and expenditures for hemodialysis procedure were 36400 litas ($ 9100) per patient year. The mean number of medications monthly prescribed to hemodialyzed patients was 6.83+/-2.21. Expenditures for these medications were 16240 Lt ($ 4060) per patient year including expenditures for erythropoietin 14976 Lt per patient year. All health insurance expenditures for hemodialyzed patients were 54055 litas ($ 13514) and this was 2-4 fold lower than reported in developed countries.

[Control of viral hepatitis in Lithuanian hemodialysis centers in 1997-2001].

Hemodialysis patients are a high-risk group for hepatitis B and C virus infections. The aim of the study was to evaluate the prevalence of B and C viral hepatitis, level of its control among patients on hemodialysis. In December of 1997, 1998, 1999, 2000, 2001 we visited all hemodialysis centers of Lithuania and gathered information about these infections in patients on hemodialysis. Eleven percent (11.4%) of all hemodialysis patients were not examined for HB(s)Ag and 15.2% for anti-HCV, 67.3%--for anti-HB(s) and 57.7% for anti-HBc in 2001. The number of examined patients for the markers of hepatitis had increased in 2001 in comparison with 1997. The same number of hemodialysis patients with HB(s)Ag was found in each year of study (14% in 1997, 14.4% in 2001). We observed the decrease in percentage of anti-HCV positive patients from 23% in 1998 till 15.4% (p<0.01) in 2001. Only 10.6% hepatitis B virus vaccinated patients was registered in 2001 and this percentage increased if compared to 6.3% in 1999. About (1/4) of anti-HB(c) positive patients were HB(s)Ag positive in 2000-2001. Chronic hepatitis B could be diagnosed for them. The duration of hemodialysis of HB(s)Ag and anti-HCV positive patients was longer compared to HB(s)Ag and anti-HCV negative patients (p<0.001).

Evaluation of exposure to cosmic radiation of flight crews of Lithuanian airlines.

BACKGROUND: In Lithuania the average annual effective dose due to cosmic radiation at the sea level is 0.38 mSv. The dose rate caused by cosmic radiation increases with altitude due to the decrease in attenuation of cosmic radiation by atmosphere. Dose rates at altitudes of commercial flights are tens times higher than those at the sea level. For this reason people who frequently fly receive higher doses which might even be subject to legal regulations. The European Council Directive (96/29/Euratom) on basic radiation safety standards requires that doses of aircrews members be assessed and the appropriate measures taken, depending on the assessment results. OBJECTIVES: The aim of this study was to evaluate potential doses, which can be received by members of aircrews of Lithuanian Airlines. The assessment was done by performing measurements and calculations. METHODS: Measurements were performed in flying aircrafts by thermoluminescent detectors, Geiger Muller counters and neutron rem counter. Such an approach lead to evaluation of doses due to directly ionizing particles and neutrons. Calculations were done with the help of the code CARI-6M. Such parameters as flight route, solar activity, duration and altitudes of flight were taken into account. Doses received during different flights and in different aircrafts were assessed. The results of measurements and calculations were compared and differences discussed. The results were also compared with the data obtained in other similar studies. RESULTS: It was found that the highest doses are received in flights to Paris, London, Amsterdam, and Frankfurt by aircraft B737. A number of flights causing annual doses higher than 1 mSv was estimated. CONCLUSIONS: Despite the fact that only European flights are operated by Lithuanian Airlines the dose of 1 mSv may be exceeded under some circumstances. If it happens some radiation protection measures shall be taken. These measures are also discussed.

[Mortality from external causes: influence of age and gender].

UNLABELLED: The article AIMS to evaluate mortality from external causes in Lithuania in 1990 and in 1997 and influence of age and gender on it. MATERIAL AND METHODS: The primary data of official demographic statistics were used for analysis, after adjustment by age - European standard. Four external causes of death (suicides, homicides, traffic accidents and all external causes) were examined. Years of potential life lost (YPLL) due to premature death from external causes were calculated. THE RESULTS obtained showed that age adjusted male suicides mortality increased by 7.7%, from homicides - by 3.0% and from all external causes - by 3.2% per year. Female suicide mortality increased by 5.9%, from homicides - by 1.7% and from all external causes increased by 2.0% per year. Male traffic accidents decreased approximately by 4.8% and those of females approximately 5.0% per year. CONCLUSIONS: Males had higher mortality in almost all age groups. Male suicides and all external causes mortality increased from 10-14 years to maximum at the age of 50-54 years and then decreased. Female mortality from these two reasons from 10-14 years increased with age. Maximal female homicide mortality was found at the age of 35-49 years. Male traffic accident mortality at the age of 20-24 years and that of females at the age of 10-14 and 20-29 years increased from 1990 to 1997 despite decline in other age groups. The YPLL per 100000 due to all external causes increased from 4598 in 1990 to 5297 in 1997 for males and from 940 to 1106 for females.

Golden hour - early postinjury period.

Quality and adequacy of specialized first aid for patients affected by high energy trauma is extremely important factor in order to reduce postraumatic disability and mortality of polytrauma patients. Treatment strategies for high energy trauma management are in early stages of development. Adequate aid can be rendered only in a few centers of Lithuania. Pre-hospital and very early hospital stages of patients with high energy trauma, which significance is proven, are unsatisfactory and inadequate. A retrospective study was performed in order to analyze efficacy and adequacy of pre-hospital (Kaunas Emergency Station, KES) and very early hospital (Kaunas University of Medicine Hospital Emergency Room, ER) management stages for 53 patients affected by high energy trauma and admitted to Kaunas University of Medicine Hospital during the period of 2001-2002. Averaged injury severity score, according to ISS, was 21.3, mortality rate was 34%. It was established long duration of pre-hospital and early hospital stage of management (accordingly 34+/-6.5 and 50+/-17.2 minutes), extremely rare monitoring of vital signs in pre-hospital stage (breathing was evaluated for 1.9% of patients, heart rate for 26.4% of patients). Fluid therapy as a part of complex treatment was applied for 7.5% of patients in pre-hospital stage and 3.8% in very early hospital stage.

[Motivation to participate in mammography screening program for breast cancer].

The rates of incidence and mortality of breast cancer in Lithuania are increasing. The causal prophylaxis is unknown. The early mammography diagnosis improves results of treatment. The aim of the study is to investigate the motivation to participate in mammography screening program for breast cancer and to evaluate the relationship between these motives and women age, education, family history, personal income. Two hundred three women were interviewed about the motivation to participate in mammography screening program for breast cancer. The strength of motivation was estimated in parametric variable. It was attempted at evaluating the influence of age, education, family history and personal income on the level of motivation. Logistic analysis was used. Asymptotic women participated in mammography screening program for breast cancer. Complaints about their breasts as a motive to participate indicated only 31.8% of all women. After the survey it was observed that women understand the danger of breast cancer (86.1%) and believe that early diagnosis can protect from the poor outcome of the disease. Out of interviewed women, 85.3% (independently from their income, age, education and family history) think that it is essential for every woman to participate in mammography screening program for breast cancer. The majority of all surveyed women were informed about the program by their doctors. This motive became the main one to consult about breast cancer for 63% of women. The mass media made no significant impact to participate in mammography screening program for breast cancer.

[Documentation of good distribution practice of medicines and its implementation in Lithuanian drug distribution companies].

Good Distribution Practice of medicinal products for human use is a quality warranty system, which includes requirements for purchase, receiving, storage and export of drugs, intended human consumption. A drug is a specific product and its mishandling is dangerous to human health and life. Therefore it is necessary to strictly control the movement of the drug from the producer to the consumer so that poor quality drugs do not have access to the market. Good Distribution Practice rules set the general requirements for good wholesale distribution practice of drugs, intended for human consumption. In order for company to meet the specified requirements, the drug distribution company must have all suitable and necessary premises, machinery, equipment, the required number of employees and specified documentation. The preparation of the Good Distribution Practice documentation is one of the most important and complex aspects when implementing the Good Distribution Practice in the companies. The article deals with the analysis of results obtained during the research of drug distribution companies in Lithuania. The research revealed that drug distribution companies put emphasis on the equipment of storage premises. Less attention is being paid to the preparation of the documents of Good Distribution Practice. The article thus presents the analysis of Good Distribution Practice documents prepared by the drug distribution companies.

[Influence of pharmaceutical law on the formation of the monopoly of pharmacists in Kaunas province in the 19th century].

The aim of this work is to estimate the influence of pharmaceutical law on the development of pharmacy in Kaunas province in the 19th century. In czarist Russia of the 19th century (which covered the majority of Lithuania) the trade of medicines was regulated severely. A statute of pharmacy issued in 1836 claimed that whenever giving a permission to establish a new pharmacy, the agreement of the owners of the nearest pharmacies should have been got beforehand. In such a way the law gave an indirect right for pharmacists to influence the establishment of new pharmacies. It also became an obstacle for a natural formation of the network of pharmacies and, thus, stimulated a creation of the monopoly of pharmacists. After the proclamation of the statute, i. e. from 1836 till 1840, only two new pharmacies were opened in Kaunas province, and later, during a thirteen-year period, (1841-1853) not a single pharmacy was founded there. Pharmacists, due to the avoidance of competition and fear to lose the monopoly of the trade of medicines in a region, tried to prevent other people from the establishment of pharmacies. They used to establish pharmacies in neighbouring towns themselves, to create the branches of their pharmacies or the networks of pharmacies of their family. In fact, the unfounded decrees of the government institutions of czarist Russia ampered the formation of a sufficient network of pharmacies and the development of the care of public health in the provinces of czarist Russia.

Concentration and species diversity of airborne fungi near busy streets in Lithuanian urban areas.

The investigations on air pollution in industrial cities of Lithuania: Vilnius, Alytus, Kaunas, Marijampole and Elektrenai were aimed at detecting the presence of fungi and aerosol particles during different seasons of the year. Sampling of fungal spores was carried out at 20 sampling sites. Active air sampling was performed simultaneously with the use of passive sedimentation plates. Data on the spread of various micromycete species in the air of cities contaminated with various pollutants are presented. Micromycetes of 430 species belonging to 165 genera, 19 families, 13 orders, 4 classes, and 3 phylla were isolated and identified. We found 21 species, 11 genera, 7 families from Ascomycota, 6 species, 1 genus, 1 family from Oomycota, 45 species, 15 genera, 8 families from Zygomycota. Mitosporic fungi comprised 138 genera, 358 species, and comprised the vast majority of identified species: 358 out of 430 (83.25 %). Conditionally pathogenic species were also isolated. It was concluded that the abundance of such fungi as Aspergillus niger, A. fumigatus, Cladosporium herbarum, Alternaria alternata, Aureobasidium pullulans in the air can be a significant criterion for the evaluation of air pollution.

Overweight and increased blood pressure in preschool-aged children.

This paper presents the data on height, weight, body mass index, systolic and diastolic blood pressure of 3-7 year old children. A sample of preschoolers (n=1441) was drawn from the kindergartens in Kaunas city (Lithuania). The 5 ( th ), 10 ( th ), 50 ( th ), 90 ( th ) and 95 ( th ) percentiles were estimated for studied variables and the criteria for the overweight and increased blood pressure were calculated. It was shown that body mass index could be used as an indicator of the child's physical development. Its value 14-18 kg/m ( 2 ) indicates an optimal child's growing, value < or = 14 kg/m ( 2 ) - the underweight, value > or =18 kg/m ( 2 ) - the overweight. According to these criteria it was found that 7.4% of 3-7 year old children were underweight, 5.8% overweight. Increased blood pressure (over 90 ( th ) percentile of systolic or/and diastolic blood pressure) had 21.4% of examined children. The prevalence of increased blood pressure was significantly higher in obese than in normal weight children. We conclude that the significant prevalence of childhood overweight and increased blood pressure emerge in preschool aged children. Thus, we recommend investigations of prevention and intervention programs to be used in the preschool setting.

The prevalence of overweight and obesity in relation to social and behavioral factors (Lithuanian health behavior monitoring).

The aim of the study is to estimate the trends in the prevalence of overweight and obesity among Lithuanian adult population and to examine the association of social and health behavior factors with overweight and obesity. Since 1994 five cross-sectional surveys have been conducted among adult population aged 20-64 within the international FINBALT HEALTH MONITOR project. An independent national random sample of 3000 inhabitants of Lithuania was drawn from National Population Register for each survey. The data were collected through mailed questionnaires (covering sociodemographic characteristics, health behaviors, and self-reported height and weight). Overweight was defined as BMI>25 kg/m ( 2 ) and obesity - as BMI>30 kg/m ( 2 ). In 2002 the overall prevalence of obesity was 16% and the prevalence of overweight - 49.1%. Since 1994 both indicators have increased in men while in women the prevalence of overweight has decreased. The prevalence of obesity and overweight increased with age. Obesity and overweight were least prevalent among the highly educated women, but most prevalent among the highly educated men. Women living in cities were less obese and overweight than those living in towns and villages. Obesity and overweight were less prevalent among daily smokers and physically active persons. Women drinking beer at least once a week were less obese and overweight than those who consumed beer more rarely. There was no consistent association between nutrition habits and the prevalence of obesity and overweight. The National obesity control program aimed at promotion of healthy nutrition and physical activity should be elaborated in order to decrease the prevalence of obesity and overweight in Lithuanian population.

[Prevalence and risk factors of age-related maculopathy among middle aged people].

OBJECTIVE: To investigate the prevalence of age-related maculopathy and the relationships of age-related maculopathy to age, sex, ocular factors, education, work exposures. A population-based cohort study included 1357 subjects (35-64 years of age) living in Kaunas (Lithuania), who were examined during the interval from 2001 to 2002. The diagnosis of age-related maculopathy was based on ophthalmoscopic findings or on fundus photographs. The prevalence of early age-related maculopathy was 7.3% in persons 40-64 years of age. No late age-related maculopathy (AMD) was found. Prevalence of early age-related maculopathy was strongly related to age (P<0.001). No significant prevalence differences were between the sexes. The frequency of age-related maculopathy was higher in eyes with light iris color but association was not significant. No associations were found between cataract or cataract type and ARM. There was no association between education and age-related maculopathy. Exposure to harmful work was found to be related to the presence of early age-related maculopathy in all. The prevalence of age-related maculopathy was significantly higher between men working in high temperature.

Airborne fungi in industrial environments--potential agents of respiratory diseases.

Investigations on airborne fungi in a poultry house, a swinery, a feed preparing and storing house, a grain mill, a wooden panel producing factory, and organic waste recycling facilities have been carried out in Lithuania. Low concentrations of fungal spores were detected in the wooden panel producing factory, the swinery, the feed preparing and storing house, and the poultry house; moderate concentrations were found in the organic waste recycling facilities; high concentrations were revealed at the grain mill. Species of Aspergillus oryzae, A. nidulans, P. expansum, Penicillium olivinoviride, P. claviforme and Botryotrichum longibrachiatum prevailed in the poultry farm; Geotrichum candidum, Cladosporium cladosporioides, C. herbarum, Penicillium viridicatum and P. fellutanum dominated in the swinery. Fungi of Penicillium viridicatum, P. expansum, Staphylotrichum coccosporum and Aspergillus oryzae prevailed in the feed preparing and storing house at the swinery. Cladosporium cladosporioides, C. herbarum, Penicillium viridicatum and Geotrichum candidum prevailed in the grain mill. Fungi ascribed as Paecilomyces puntonii, Rhizopus nodosus and R. stolonifer dominated in the wooden panel producing factory. Species of Aspergillus raperi, P. paxilli, P. oxalicum, and Cladosporium herbarum prevailed at the organic waste recycling facilities. According to published data, the majority of the identified fungal species are characterized as allergenic and an exposure to their spores may provoke adverse health effects (such as allergic rhinitis, bronchial asthma or extrinsic allergic alveolitis) in susceptible individuals.
