Personalized cardiovascular risk management linking SCORE and behaviour change to Web-based education.

The PULSE (Personalization Using Linkages of SCORE and behaviour change readiness to web-based Education) project objectives are to generate and evaluate a web-based personalized educational intervention for the management of cardiovascular risk. The program is based on a patient profile generated by combining: (a) an electronic patient data capture template (DCT); (b) the Systematic COronary Risk Evaluation (SCORE) algorithm; and (c) a Stage of Change determination model. The DCT inherently contains a set of evidence-based parameters for patient description and disease evaluation. The patient's stage of behaviour change determines messages consistent with the individual's change processes, decisional balance, and self-efficacy. The interventions are designed to address both medical and psychosocial aspects of risk management and, as such, we combine staged lifestyle modification materials and non-staged messages based on Canadian clinical guidelines to motivate personal risk management. The personalization decision logic is represented in Medical Logic Modules implemented in Java. An intelligent interactive system generates the personally relevant materials and delivers the education to the patient via the Web. An evaluation study will be conducted to determine whether web-based personalized educational strategies exert favourable influence on patient's interest, knowledge, and perceived compliance to the suggested lifestyle modifications.

[Information guided technology and procedure in neurosurgical field].

Translational Research is the research to apply and mediate a fundamental research result to the clinical field effectively. The integration of the diagnosis and the treatment is an important item in the life science field from the viewpoint of quality of therapy and minimally invasive therapy. The standardization of mechanical interface of the medicine related machine, system and a medical information system are important from the viewpoint such as prevention of medical malpractice. A high function operating robot (manipulator) as an endoscopic robot and image-guided minimally invasive device are important in the point of view of the development of a medical device shown with a medical device industry vision. We describe an outline of our ongoing development of endoscope system, intraoperative MRI (intelligent operating theater) and robot surgery system in neurosurgical field.

[Aspects of the development and sex specific prevalence of psychogenic diseases in children and adults in relation to genetic and environmental factors].

We investigated a sample of one hundred twins as to their neurotic disturbances in childhood and adulthood. The hereditary factor was of greater effect with the continuing neuroses of childhood than with the weakening neuroses in childhood. But a certain effect of the hereditary factor could not be denied with neuroses manifesting themselves only in adulthood. According to our results the hereditary factor in childhood is not stronger than in adulthood, with adults it is as effective. Particular neurotic symptoms show influences of the hereditary factor in respect to depressive, maladaptive oral and maladaptive aggressive disturbances of behavior and difficulties in forming interpersonal relationships. In respect to stuttering, disturbances of the urogenital system and intelligence and occupational development a certain hereditary factor is very probable. Sex influences also have a certain effect: Behavioral disorders in childhood are found in the male sex, the female sex rather shows psychic symptoms.

Evaluation of a new infrared tympanic thermometer: a comparison of three bands.

As technology advances, it is important for nurses to compare various types of equipment used on pediatric patients to determine the cost effectiveness, risk, and ease of use. Presently, there are several types of electronic thermometers in use to measure body temperature. The following article discusses the comparison of the Thermoscan Instant Thermometer (Thermoscan Inc., San Diego, CA) with the First Temp infrared (Intelligent Medical Systems, Carlsbad, CA) and IVAC electronic thermometer (IVAC Corporation, San Diego, CA).

The cytomorphologic and cytometric manifestations of cervical reparative processes.

The process of tissue repair or regeneration is the attempt of the injured tissue to be restructured and to reestablish its function as it existed prior to injury. The cytologic alterations depend on the original tissue site, the elapsed time after the incurred injury as well as on the type of the injury itself. From a practical standpoint, the importance of the recognition of tissue repair alterations as such, is the avoidance of false positive cytologic diagnoses and/or the under evaluation of existing malignant lesions with potentially severe clinical consequences. The computerized on-line file of patients examined cytologically and histologically since the year 1959 which contains over two million patient records was searched for those cases identified as "tissue repair". The major criteria of cytomorphology of tissue repair and differential diagnoses are described. For the purpose of enhancing the differential diagnostic accuracy two procedures useful as diagnostic aids are presented: (1) local administration of estrogens, and (2) computerized cell image analyses evaluated and guided by an artificial intelligence expert system.

BioSYNTHESIS: bridging the information gap.

BioSYNTHESIS is a prototype intelligent retrieval system under development as part of the IAIMS project at Georgetown University. The aim is to create an integrated system that can retrieve information located on disparate computer systems. The project work has been divided in two phases: BioSYNTHESIS I, development of a single menu to access various databases which reside on different computers; and BioSYNTHESIS II, development of a search component that facilitates complex searching for the user. BioSYNTHESIS II will accept a user's query and conduct a search for appropriate information in the IAIMS databases at Georgetown. For information not available at Georgetown, such as full text, it will access selected remote systems and translate the search query as appropriate for the target system. The search through various computer systems and different databases with unique storage and retrieval structures will be transparent to the user. BioSYNTHESIS I is complete and available to users. The design work for BioSYNTHESIS II is under development and will continue as a multiyear technical research effort of the proposed Georgetown IAIMS implementation project.

[Chaotic attractor of the left ventricular wall motion in normal heart healthy subject].

To analyze the entire left ventricular wall motion, not the decomposed motion, nonlinear mathematical techniques were used to describe graphically time series data for the left ventricular contraction. Time series data of the left ventricular wall motion were recorded by M-mode echocardiography, and embedded into three-dimensional phase space. The largest Lyapunov exponents, a measure of the rate of divergence in phase space, were calculated to detect the sensitive dependence on initial conditions. The reconstructed attractor of the left ventricular contraction showed a lower dimensional strange attractor, and calculation of the Lyapunov exponents indicated sensitivity to initial conditions. These results suggest that time series data of the left ventricular contraction show the characteristics of deterministic chaos. The chaotic system can provide an intelligent and flexible control system. Our results suggest that the circulatory control system, which mediates the cardiac contractility, may be a flexible dynamic compatible with deterministic chaos.

ALERT: a clinical case simulator program for serious, communicable, and rare diseases.

ALERT is an intelligent Tutoring System (ITS) based on clinical case simulation. Its purpose is to assist in the training of general practitioners regarding the diagnosis and the control of serious, communicable, and rare diseases, such as anthrax, plague, and smallpox. ALERT provides both monitoring techniques and treatment protocols and is structured into two independent sections: one devoted to the simulation of the clinical case and the other to detailed description of disease.

Drug delivery and targeting.

When a pharmaceutical agent is encapsulated within, or attached to, a polymer or lipid, drug safety and efficacy can be greatly improved and new therapies are possible. This has provided the impetus for active study of the design of degradable materials, intelligent delivery systems and approaches for delivery through different portals in the body.

Data compression for medical report archiving.

In a hospital environment, where large numbers of reports are compiled, data compression offers a method for substantially reducing the required disk space whilst protecting the confidentiality of patient data. An analysis is given of the most important parameters that influence compression of free-text data, to realize an efficient implementation of an archiving system for medical reports. An overall reduction of the required disk space by more than 50% can be attained, as well as supplementary level of protection. A slight increase in access time is thereby inevitable, but almost insignificant. Such an archiving system constitutes a necessary component for an intelligent information retrieval system. The access to information contained in medical documents (by means of natural language processing and artificial intelligence techniques) is considered to be one of the key issues in the field of medical informatics for the coming decades.

Integrated radiology information system, picture archiving and communications system, and teleradiology--workflow-driven and future-proof.

The proliferation of integrated radiology information system/picture archiving and communication system (RIS/PACS) and teleradiology has been slow because of two concerns: usability and economic return. A major dissatisfaction on the usability issue is that contemporary systems are not intelligent enough to support the logical workflow of radiologists. We propose to better understand the algorithms underlying the radiologists' reading process, and then embed this intelligence into the software program so that radiologists can interact with the system with less conscious effort. Regarding economic return issues, people are looking for insurance against obsolescence in order to protect their investments. We propose to future-proof a system by sticking to the following principles: compliance to industry standards, commercial off-the-shelf (COTS) components, and modularity. An integrated RIS/PACS and teleradiology system designed to be workflow-driven and future-proof is being developed at Texas Tech University Health Sciences Center.

Expert Systems Research.

Artificial intelligence, long a topic of basic computer science research, is now being applied to problems of scientific, technical, and commercial interest. Some consultation programs, although limited in versatility, have achieved levels of performance rivaling those of human experts. A collateral benefit of this work is the systematization of previously unformalized knowledge in areas such as medical diagnosis and geology.

RGSS-ID: an approach to new radiologic reporting system.

RGSS-ID is a developmental computer system that applies artificial intelligence (AI) methods to a reporting system. The representation scheme called Generalized Finding Representation (GFR) is proposed to bridge the gap between natural language expressions in the radiology report and AI methods. The entry process of RGSS-ID is made mainly by selecting items; our system allows a radiologist to compose a sentence which can be completely parsed by the computer. Further RGSS-ID encodes findings into the expression corresponding to GFR, and stores this expression into the knowledge data base. The final printed report is made in the natural language.
