Clinical manifestations of erythema chronicum migrans Afzelius in 161 patients. A comparison with Lyme disease.

Clinical symptoms were studied in 161 consecutive patients with erythema chronicum migrans Afzelius and in a follow-up study signs of late manifestations were investigated. General symptoms such as headache, fever, myalgia and/or arthralgia were found in about half of the patients with a disease duration of less than or equal to 3 weeks. Three patients had coexisting lymphadenosis benigna cutis. Two untreated patients developed meningitis/meningoradiculitis and one untreated patient arthritis. The importance of a sufficient antibiotic therapy to prevent late manifestations is stressed. Although there are many similarities between erythema chronicum migrans Afzelius and Lyme disease, the results of the present study also point to differences. Multiple skin lesions, pronounced general symptoms, laboratory abnormalities and major symptoms from the joints were less common in patients with erythema chronicum migrans Afzelius than reported in patients with Lyme disease, but a prolonged course of the skin eruption was more common.

[The effect of dilutions of Apis mellifica and Apium virus on ultraviolet light-induced erythema in the guinea pig].

Dilutions of Apis mellifica (obtained from the whole bee) and Apium virus (obtained from bee venom) are used classically in homeopathy for inflammatory symptoms with edema, erythema and pruritus (Lewis triad). Using a method examining the evolution of UV induced erythema in the guinea pig, the authors show the following dilutions of Apis mellifica 7 CH(10(-14)), 9 CH(10(-18)) and of Apium virus 5 CH(10(-10)), 7 CH(10(-14)), 9 CH(10(-18)) exert an action on experimental erythema. The results are statistically significant for the dilutions at the 48th hour after irradiation.

[Erythema action spectrum in xeroderma pigmentosum group A patients].

Twenty patients with xeroderma pigmentosum group A (XP-A) and 30 healthy controls were studied to determine the energy requirements for UV-erythema with monochromatic light between 255 and 340 nm. As test sites, unexposed skin on the upper back was selected for irradiation using a prism monochromator and a 1 kW xenon arc source. The minimal erythema doses (MEDs) were determined at each erythemal response peak, at 24 hours post irradiation in normal controls and at 72 hours in XP-A patients, respectively. Compared with normal subjects, XP-A patients had extremely low MEDs at all wavelengths tested. The erythema action spectrum showed 2 peaks at 265 and 290 nm in XP-A patients and the greatest effectiveness at 290 nm. These results indicate that XP-A patients should be protected strongly against not only UVB but also UVA, because the photo-hypersensitivity in XP-A patients has been extended from the UVB to UVA region.

Rapidly progressive (crescentric) glomerulonephritis in erythema nodosum leprosum: case report.

A middle aged man (48 years) with short duration of illness (7 days) was admitted in the state of acute renal failure with erythema nodosum leprosum. He had repeated episodes of erythema nodosum leprosum in the past. His blood pressure was normal (150/80 mmHg). During his hospital stay he was in the state of progressive anaemia (Hb = 8.8 g/dl to 7.2 g/dl), oliguria (urine out-put = 250-350 ml/day), azotaemia (blood urea = 198 mg/dl to 218 mg/dl) and impaired renal function tests with fatal outcome. Kidneys were smooth, congested and weighing 150 g each with histological features of rapidly progressive (crescentric) glomerulonephritis, a result of immune complex deposition from recurrent erythema nodosum leprosum episodes.

Quantitative assessment of UV-induced pigmentation and erythema.

In this paper we present methods that we have developed to measure pigmentation in human skin. This involves the measurement of diffuse reflection spectra from human skin in vivo and referencing them to either totally depigmented skin, or the skin of the same individual if we are measuring variations in pigmentation. Changes in the pigmentary system brought about by UV radiation can be measured for each individual. Absolute measurements lead to estimates of the melanin concentration in the skin, while differential measurements lead to estimates of the quantity of additional or reduced pigment content of some skin lesions. The same instrumentation has been successfully used to assess UV-induced erythema as well as other vascular changes. The determination of the minimum detectable erythema dose can be performed even in the darkest-skinned subjects without loss of sensitivity as in the case of laser Doppler instruments. It has been shown that what is perceived by the eye as erythema is a very complex phenomenon, encompassing a large number of vascular reactions that can be studied in detail through diffuse reflection spectroscopy. Some of the possible responses are presented, as well as the contributing chromophores that have been identified so far.

UV-induced erythema model: a tool in dermatopharmacology for testing the topical activity of non-steroidal anti-inflammatory agents in man.

UV-induced erythema is a well known inflammatory model applied both in animal and human skin to test the activity of topical non-steroidal anti-inflammatory compounds in a great variety of pharmaceutical formulations. The aim of this study was to evaluate the inhibitory efficacy of piroxicam in two different topical formulations (cream 0.5, 1 and 1.5% and gel 1%) as compared to three non-steroidal compounds, benzydamine, etofenamate and indomethacin (cream 5%), on erythema induced after UV-injury on the back of 5 healthy subjects. The results showed that piroxicam in cream formulation, indomethacin cream and etofenamate gel have a similar effect, decreasing the erythema size 7 h after irradiation. However, benzydamine cream and piroxicam gel showed no effect with this method. We may conclude that this model is adequate and precise for selecting the most appropriate galenic dosage form for an active compound in terms of its clinical efficacy when topically administered.

Chemotherapy-induced acral erythema in patients receiving bone marrow transplantation.

Chemotherapy-induced acral erythema is an uncommon and distinctive syndrome of intense macular erythema of the palms and fingers seen in patients treated with high-dose chemotherapy. It is painful, may form bullae, and heals uneventfully with desquamation. The incidence (35%) of this complication in patients receiving bone marrow transplantation at our institution is quite high and probably reflects the exceptional doses of chemotherapy and concomitant total body irradiation these patients receive. Biopsy specimens showed vacuolar change, spongiosis, necrotic keratinocytes, and epidermal atypia. These findings probably result from direct toxic effect and mimic those of acute graft-vs-host disease. Awareness of chemotherapy-induced acral erythema is important to avoid its misdiagnosis as a cutaneous sign of acute graft-vs-host disease. This distinction can usually be made on clinical grounds. If necessary, serial skin biopsy specimens are helpful.

[Light-induced postherpetic erythema exsudativum multiforme].

A 37-year-old man had erythema multiforme over a period of 20 years following recurrent herpes simplex infections. On the basis of the results of provocative UV-A tests as well as histological findings a diagnosis of photosensitive erythema multiforme was made. The erythema multiforme exsudativum like variant of polymorphous light eruption is discussed in the differential diagnosis.

Quantification of UV-induced erythema with a portable computer controlled chromameter.

A sensitive computer controlled tristimulus color analyzer (Minolta Chromameter II Reflectance) was used to measure UV-induced erythema quantitatively. Of the 5 available chromaticity measuring modes in the instrument, one of the values (a*) was enough to specify the erythema quickly. A comparison has been made between results using the instrument and a graded score based on visual perception of erythema. This easy to operate instrument is suitable for both clinical and field conditions.

An annular erythema of infancy.

An unusual annular and arciform erythema in an infant is reported. Skin lesions appeared when the child was 4 days old, and multiple lesions have been present during the subsequent two years. Individual lesions gradually enlarge over a period of a few weeks, and they resolve spontaneously without any residual cutaneous abnormality. This annular erythema is clinically and histologically distinct from previously described annular erythemas of infancy, with which it is compared.

[Paraneoplastic palmar erythema].

The authors one human case of symmetric palmar erythema with chronic evolution going before subclinical metastasis of gastric adenocarcinoma. Short review of palmar erythema cases associated with others diseases is presented. They point out the similarity of clinical aspect and iconography with another case associated to Hodgkin disease. Considering the few bibliographical references on this topic, they suggest the systematic investigation of neoplastic disease when a palmar erythema is detected without any other etiology.

[Erythema migrans disease. 3 case examples with neurologic complications].

The erythema-migrans-disease is closely related to Lyme-disease as described in the USA, and represents a tickborne spirochetosis with highest incidence during summer and autumn. Besides nonspecific general symptoms in its earlier stages, it is characterized by a distinctive erythema migrans; later, additional neurological, cardiac and/or arthritic symptoms may develope. Its typical neurologic complication, the lymphocytic meningoradiculitis Garin-Bujadoux-Bannwarth, was observed in three children whose clinical features illustrate the great variability of symptoms of Erythema-migrans-disease. Serological tests for specific spirochete antibodies are valuable aids in differential diagnosis, especially for cases with late complications if a tick bite is not remembered, or as is not uncommon, the typical skin lesion has not occurred, or has not been noted. Antibiotic therapy with penicillin or tetracycline is a successful treatment of the earlier stages of the disease and can prevent, or at least ameliorate, subsequent complications.

[Histopathology of erythema nodosum].

The histopathology of erythema nodosum is discussed with reference to the literature and of 19 cases of our clinic. There is a significant vascular involvement especially of the subcutaneous tissue. The veins and arteries are equally affected. The histological picture consists not only of an acute inflammatory process in the subcutaneous tissue, but also of inflammatory cells which area see to a lesser extent in the dermis. In many cases the typical small granulomatous foci of MIESCHER which are considered to be pathognomonic for erythema nodosum do not develop. In the cases which we examined we were not always able to tell from the histological picture how long the erythema nodosum had been present. Multinucleated giant cells which are often found in the septal connective tissue are seen even in relatively acute cases.

Infantile epidermodysplastic erythema gyratum responsive to imidazoles. A new entity?

A 3 1/2-year-old girl had a three-year history of chronic annular erythema that more closely mimicked erythema gyratum repens of adults than other annular erythemas of infancy. Histopathologic study revealed bowenoid characteristics in the epidermis. No fungi were ever demonstrated in this patient's skin lesions, but they consistently responded to treatment with ketoconazole and flared immediately after cessation of treatment with that drug.

[Comparison of the antiphlogistic effect of mucopolysaccharide-polysulfate ointments with heparin-containing ointments in the UV erythema test].

It could be demonstrated by means of the UV erythema model that four different ointment preparations containing mucopolysaccharide polysulfate (MPS) have a marked antiinflammatory effect and inhibit the formation of erythemas to a large extent. In this trial arrangement the different preparations containing MPS were significantly superior to altogether eight commercial heparin ointments, which were tested, too. With an inhibition of 69% of the erythema induced by UV rays, Hirudoid 40000 ointment is the most effective of all preparations tested.

[Erythema gyratum repens of Gammel and Hodgkin's disease].

A 65 year old male with Hodgkins disease, and generalised figurate Erythema, which during his period of hospitalisation migrated and became much more evident, disappearing after initial therapy. Diagnosed as "Erythema gyratum repens" reported by Gammel, an uncommon form of paraneoplasic migrant figurate Erythema, we review the 33 previous cases of this process, and find that, although 30 were related to other processes.

'Persistent' annular erythema of infancy.

An unusual annular erythema occurred in a 6-month-old girl. Recurrent annular lesions appeared on the face and back, but annular lesions persisted for ten months on the distal extremities. Eleven months after the eruption appeared, it resolved completely without recurrence. This seemingly unique condition is compared with other cases of chronic annular erythemas, especially those previously described as erythema gyratum perstans.

Recall of UVB-induced erythema in breast cancer patient receiving multiple drug chemotherapy.

One day after sunbathing, a breast cancer patient received intravenous methotrexate, cyclophosphamide and 5-fluorouracil and had a recall of her UV erythema over the following week. Phototesting with UVA and UVB prior to and after a subsequent chemotherapy treatment showed a UVB-induced recall of erythema, as well as a phototoxicity-like response. Skin biopsies from test sites were compatible with phototoxic dermatitis. The occurrence of both erythema recall and the phototoxicity-like reaction suggests that more than one mechanism is involved in this side effect.
