[Clinical efficacy of Viagra with behavior therapy against premature ejaculation].

OBJECTIVE: To study the efficacy of Viagra combined with behavior therapy against premature ejaculation (PE). METHODS: Sixty PE patients were divided into two groups randomly: control group (behavior therapy alone) and the group of Viagra combined with behavior therapy. Intra-vaginal ejaculation latency time (IELT) and the coitus satisfaction of the patient and the partner were recorded before and after treatment. RESULTS: The IELTs of the two groups were 0.80 +/- 0.20 and 0.73 +/- 0.24 minutes respectively before treatment, and 1.82 +/- 0.54 and 3.63 +/- 0.55 minutes respectively after treatment. As for IELT and satisfaction degree, Viagra produced better result than behavior therapy. CONCLUSION: During this clinical trial, Viagra combined with behavior therapy prolonged IELT, which suggests that Viagra may be helpful for the treatment of premature ejaculation.

[Effect of successive ejaculation on semen analysis parameters in normal men].

OBJECTIVE: To study the effect of successive ejaculation on semen analysis parameters in normal men. METHODS: Eight ejaculates were collected at daily intervals from 8 normal men. The semen parameters were analyzed according to WHO laboratory manual for the examination of human semen and sperm-cervical mucus interaction. RESULTS: The semen volumes and total sperm counts decreased gradually day by day, significantly lower from the 5th to the 8th day than the 1st (P < 0.01). The sperm densities also declined day by day, but not significantly. The sperm viability and motility (a + b grade) presented an increasing trend, but with no significance except on the 7th day. CONCLUSION: With the increase of the ejaculatory frequency, human semen volumes and the total counts significantly decrease, while sperm viability and motility gradually increase. Successive ejaculation does not affect the quality of human semen.

[Comparison between sildenafil plus sertraline and sertraline alone in the treatment of premature ejaculation].

OBJECTIVE: To compare the efficacy and safety of sildenafil plus sertraline with those of sertraline alone in the treatment of premature ejaculation (PE). METHODS: Seventy-two patients with PE but without any obvious organic cause were enrolled in this study. They were randomly divided into Groups A and B of equal number. Group A received 50 mg sertraline daily 4 to 6 hours before planned sexual activity for 12 weeks, and Group B were given 50 mg sertraline daily plus 50 mg sildenafil as needed, 1 hour before planned sexual activity, for 12 weeks. Before and after the treatment, the mean intravaginal ejaculation latency time, the intercourse satisfaction, the mean number of coituses per week and the drug-related side effects were evaluated. RESULTS: The mean intravaginal ejaculatory latency time was (0.59 +/- 0.12), (3.9 +/- 0.15) minutes (P < 0.001) at baseline and post-treatment in Group A, and (0.56 +/- 0.11), (5.6 +/- 0.12) minutes (P < 0.001) in Group B, improved in both of the 2 groups, but more significantly in Group B (P < 0.05). Before and after the treatment, the mean intercourse satisfaction domain values of the IIEF were (8.9 +/- 1.2), (10.8 +/- 1.1) (P < 0.05) and (8.8 +/- 1.1), (13.8 +/- 1.3) (P < 0.001) in Groups A and B, respectively, significantly greater in Group B than in Group A (P < 0.05) after the treatment; the mean numbers of coituses per week in Groups A and B were (0.9 +/- 0.2), (1.9 +/- 0.3) (P < 0.05) and (1.0 +/- 0.2), (2.7 +/- 0.2) (P <0.001) respectively, significantly larger in Group B (P<0.05) after the treatment. As for the side effects, there was a higher rate of headaches (P < 0.01) and flushing episodes (P < 0.001) in Group B than in Group A. CONCLUSION: Sertraline combined with sildenafil can produce significantly better results than sertraline alone in patients with premature ejaculation. However, the combined treatment is associated with a slight increase in the drug-related side effects.

[Central and peripheral neuromodulation of ejaculation. An updating contribution].

Some recent results of both experimental and clinical researches have brought to the attention the role of new neurotransmitters and neuroreceptors involved in the process of seminal ejaculation. In the central neurochemical system (neurotransmitters and neurohormones), besides the known monoamines, several aminoacids, peptides, steroids, kinins, PGs--some of which produced by the glia--have been recently evidenced. Among the peripheral receptors, besides the muscarinic and adrenergic ones, some neuropeptidic (VIP, Neuropeptide Y, endogenous opioids, etc.) take part in the effectorial process of ejaculation. Many pharmacological agents may interfere with the ejaculation, at both central and peripheral levels.

Ejaculation↗

Neuroanatomy and physiology of ejaculation.

Ejaculation is the final stage of coitus in the mammalian male and results in the expulsion of sperm out of the urethral meatus. Two successive phases, emission and expulsion, can be distinguished during ejaculatory response. Normal anterograde ejaculation requires close coordination of sympathetic, parasympathetic, and somatic components commanding the different peripheral anatomical structures (accessory sexual glands, ducti, and striated muscles) involved in ejaculation. The efferent pathways innervating these anatomical structures drive motor outputs originating from spinal thoracolumbar and lumbosacral nuclei. These spinal ejaculatory centers, the synchronized activation of which is likely carried out by a group of spinal cells, are under the control of both peripheral sensory afferents coming from genital areas and supraspinal information arising from specific brain regions.

[Proteolytic activity of human ejaculate].

Studies of the ejaculate proteolytic activity, viscosity, and protein concentration in patients with various spermatogenesis levels have revealed that viscosity and proteolysis directly depend on the count of spermatozoa in the ejaculate. Lower values were associated with azospermia. A clear-cut correlation was detected between these parameters and the inflammatory process in the prostate. Exacerbation of chronic prostatitis results in decrease of the ejaculate viscosity and activation of the proteolytic enzymes, this being confirmed by sperm protein electrophoresis, whereas the protein concentration is essentially reduced. These findings necessitate further investigations of the aforesaid parameters over the course of treatment for chronic prostatitis.

Retrograde ejaculation: successful treatment with artificial insemination.

Retrograde ejaculation is characterized by aspermia or oligospermia and results from an incompetent bladder neck, often due to a dysfunction of the internal sphincter. In almost 3 years, eight couples who suffered from infertility due to retrograde ejaculation were treated with inseminations with spermatozoa gained from the urine. Ovulation was predicted on the basis of blood LH levels. The urine-semen sample was collected in 100 mL of Hepes medium and 5 mL 1% human albumin (pH 7.4). After centrifuging, the remaining sperm pellet was dispersed on a Percoll gradient. After centrifuging and resuspending, followed by two washing procedures with Ham's F-10 and human albumin 1%, the remaining sample was used for intrauterine insemination. Twelve pregnancies were thus achieved; two women became pregnant twice and one three times. The pregnancy rate per cycle was 44.4%. In seven couples, pregnancy was achieved within three cycles. Four pregnancies ended in spontaneous abortion and five ended in the birth of a healthy child; three pregnancies were continuing at the time of writing. Retrograde ejaculation can be treated successfully with inseminations using spermatozoa obtained from urine. It seems important to collect the urine-semen sample in a buffered medium and to time the insemination on the basis of the LH surge.

Painful ejaculation associated with antidepressants in four patients.

BACKGROUND: Painful ejaculation associated with tricyclic antidepressants is rarely reported in the medical literature. METHOD: Painful ejaculation following the administration of imipramine and clomipramine is described in four patients. RESULTS: The phenomenon occurred in all patients during the first 3 weeks of treatment and disappeared within several days when the tricyclic dosage was reduced or the medication was withdrawn. CONCLUSION: Painful ejaculation was apparently evoked by tricyclic antidepressant administration. Clinicians should be aware of this underreported side effect.

[Vibration-induced reflex ejaculation in patients with spinal cord injuries].

Thirty-six men (aged 18-42 years, average age 27 years) with known spinal cord lesions for 0.3-39 years and levels of the lesions from C3 incomplete to L1 complete were submitted to penile vibrations with the objective to obtain reflex ejaculation. Sperm cells were demonstrated in a total of 27 of the participants, antegrade ejaculation in 16 cases and 11 retrograde ejaculations. In general, the semen quality was very poor with reduced total number of spermatozoa and increased percentage of immobile and/or morphologically abnormal spermatozoa compared with normal values. In connection with the vibration procedures, three cases of autonomous hyperreflexia followed by transient slight headache which did not require treatment occurred while an isolated case was treated with 0.5 mg glycerine nitrate (Nitromex) administered sublingually with good effect. No other side effects were observed.

A new sperm collection method for treatment of retrograde ejaculation.

Retrograde ejaculation is an uncommon but treatable form of male infertility. Successful recovery of motile spermatozoa from a post-ejaculatory urine for artificial insemination is dependent on careful regulation of osmolarity and acidity of the bladder contents into which the ejaculate emanates, and the separation of the motile spermatozoa from the debris and cells which are found in these specimens. A pregnancy established by artificial insemination of spermatozoa recovered from bladder contents of a patient suffering from retrograde ejaculation is presented. The techniques for preparing the urinary bladder for spermatozoal survival and the removal of debris and cells by delicate centrifugation are discussed.

[Retrograde ejaculation as a cause of aspermia following retroperitoneal lymph node excision and the effective use of alpha sympathomimetic drugs].

Retroperitoneal lymphadenectomy may produce in the majority of patients an irreversible loss of normal ejaculation due to lesion of presacral sympathetic nerve fibers. Libido and orgasm did remain unchanged. In 14.3% of the patients a retrograde ejaculations could evaluated. By application of alpha-sympathicomimetic drugs this could reversed to an antegrade ejaculation. However, fertility was altered strongly because of motility loss.

Glycerylphosphorylcholine levels in the coagulational groups of human ejaculates.

The amount of coagulum present in fresh ejaculates of men (among infertile couple) varied directly with the levels of seminal glycerylphosphorylcholine (GPC; P less than 0.001), which is secreted predominantly by the epididymis. GPC concentrations (mg/ml) of the normally and poorly coagulating ejaculates revealed close similarities with those of the presumably fertile (1.72 +/- 0.10) and infertile semen (1.13 +/- 0.08) respectively. The study suggests that the degree of coagulation of human ejaculates may be correlated with epididymal function.

The characterisation of non-sperm cells in the ejaculates of fertile men using transmission electron microscopy.

The non-spermatozoal cells (NSC) in the semen samples of 20 fertile human males have been studied by transmission electron microscopy (TEM), in order to accurately distinguish between the different types of cell present and to give a quantitative profile of their relative proportions. The main seminal constituents of the average fertile man were found to be the germinal elements, accounting for 84.0% of the total NSC population. Of this percentage the anucleate bodies, designated "CM" in this study, were the greatest component (43.0%, SEM +/- 4.7). Spermatids were the next commonly occurring cell (22.2%, SEM +/- 2.9), the anucleate cellular masses with organelles, CM(O)'s and the spermatocytes making up the remaining 18.8%. Leucocytes accounted for 13% of the total NSC, respectively: neutrophils - 12% (SEM +/- 4.5); macrophages - 0.9% (SEM +/- 0.3) lymphocytes - 0.1% (SEM +/- 0.1). The remaining 3% consisted of 2.3% (SEM +/- 0.7) epithelial cells and 0.7% (SEM +/- 0.4) Sertoli cells. The principal conclusions reached were: that some normal ejaculates host active phagocytosis and possibly macrophage activation; that anucleate bodies which form a major component of the ejaculate merit further quantitative study and that the shedding of spermatids is an interesting and important aspect of ejaculation.

Abnormalities of ejaculation.

The normal physiologic processes of emission and ejaculation require coordination of neurophysiologic, anatomic, and, in certain cases, psychological phenomena. Disruption of any component, from the embryologic development of the müllerian duct through the medications used for nonrelated systemic disease, can alter the efficient function of ejaculation. Evaluation of the urologic patient who has any of a number of abnormal ejaculatory states requires an understanding of the many possible mechanisms of failure. The majority of these men need evaluation because of a possible male-factor infertile marriage. The potential for improvement is significant, given the development of improved techniques to stimulate ejaculation and the promise shown by extracorporeal fertilization techniques such as in vitro fertilization and gamete intrafallopian tube transfer. These patients deserve complete assessment and optimization of any factors that will enable them to achieve their goal of procreation.

[The collection of sperm in men unable to ejaculate].

Electro-ejaculation and surgical collection of sperm by vasotomy enable to obtain sperm in men unable to ejaculate because of a neurological spinal disorder. A multicenter study of electro-ejaculation is currently in process in the USA, but as these are limited experiments, the preliminary results do not permit to draw any conclusion.

Relationship between the coagulation-liquefaction property of human ejaculates and their volume, sperm count and motility.

In ejaculates from selected infertile couples, the degree of coagulation revealed significant positive correlations with liquefaction time, sperm count and motility, but negative correlation with semen volume. Comparison of presumptively fertile and infertile ejaculates also showed significant variations in their amount of coagulum and liquefaction time. The study suggested a possible relationship between the coagulation-liquefaction property of human ejaculates and their semen quality.

[Liquefaction disorders of the ejaculate--diagnosis and therapy].

We are introducing a simple and reliable method for evaluation of liquefaction disturbances of human ejaculate. This technique requires to try to draw threads out of the ejaculate by using plastic hootes. If this can still be achieved after a period of 20 minutes after ejaculation, liquefaction is positively disturbed. The therapeutical possibilities have been investigated in a clinical study. 37 patients of the andrological department of the university hospital for gynecology and obstetrics in Graz have been treated with Ichthyol-medicaments for six weeks. 18 patients had a sit-bath therapy with Ichthobad three times a week, 19 patients were using Ichtopur suppositoria two times daily. In both variants of treatment a significant decrease of the liquefaction time occurred, 3 pregnancies resulted. This new diagnostic method and the encouraging results after treatment with Ichthyol improve the therapeutical possibilities for the andrologist in the complex field of seminal liquefaction disturbances.
