[Comparative studies on oral glucose tolerance and serum lipids under the influence of a new oral contraceptive (SH B 209 AB: 2 mg cyproterone acetate + 0.05 mg ethinyl estradiol), the combination D-norgestrel/ethinyl estradiol and in a control group (author's transl)].

24 healthy women given the combination of cyproterone acetate (2 mg) + ethynyl estradiol (0.05 mg) [SH B 209 AB] and D-norgestrel (0.25 mg) + ethinyl estradiol (0.05 mg) [Neogynon] were compared with a control group in oral glucose tolerance tests. The volunteers were divided into three groups. Two received alternately both preparations with each treatment cycle preceded by one cycle without medication. The third group served as an independent control. On the 25th day of each cycle blood was withdrawn before and 15, 30, 45, 60, 90 and 120 min after the carbohydrate load. Blood glucose and insulin and free fatty acids in serum were determined. Fasting values of triglycerides and cholesterol in serum were determined. The following results were obtained: 1. The mean fasting levels of blood glucose, serum insulin and free fatty acids were unchanged. 2. The total area under the blood glucose curve over the initial value was not altered 2 h after treatment with SH B 209 AB or Neogynon. The total area under the insulin and free fatty acids curves were also unchanged after the oral glucose load. 3. There was no change in serum cholesterol. 4. Serum triglycerides were significantly increased after SH B 209 AB. 5. The implications of the structural differences between both gestagens are discussed.

[Treatment of polycystic ovary syndrome and related infertility with clomiphene resistance by compound cyproterone acetate].

OBJECTIVE: To assess the efficacy and safety of compound cyproterone acetate (CPA Co, or Diane-35) in the treatment of polycystic ovary syndrome (PCOS) and to explore its potential role in improving the outcome of ovulation induction in clomiphene (CC) resistant cases. METHODS: Twenty-nine proven PCOS patients were enrolled. Seventeen out of them, including 16 who were resistant to CC, had anovulatory infertility. CPA Co was given for 4-6 cycles. Serum luteinizing hormone(LH), follicle stimulating hormone(FSH), estradiol(E2), testosterone(T), androstenedione(A), dehydroepiandrosterone (Ds), sex hormone binding globvlin (SHBG), insulin concentrations, total cholesterol, triglycerides(TG), high density lipoprotein-cholesterol(HDL-C), low density lipoprotein-cholesterol (LDL-C), apolipoprotein A, apolipoprotein B and transvaginal pelvic ultrasonography were determined before, 2-3 cycles and 4-6 cycles after treatment. After stopping the drug, CC or human menopausal gonadotropin were re-administered in 12 patients (22 cycles) and 5 cases (6 cycles) respectively for induction of ovulation. RESULTS: All patients had regular uterine bleeding during CPA Co therapy. Acne improved in 8 cases. Hirsute score didn't decrease significantly. Serum LH, FSH, E2, A, T, Ds concentrations decreased significantly (P < 0.05-0.01), while SHBG levels increased by 5.1 times after treatment. Meanwhile, bilateral ovarian volumes shrunk and follicle numbers decreased significantly (P < 0.05-0.01). Serum insulin levels did not change significantly. As to the lipid profile, the most striking change was 55.1% increase of HDL-C and 23.0% decrease of LDL-C, although TG also increased by 36.2%. 58.6% of patients had mild and transient adverse effects. Serum alanine transaminase increased in 3 cases. After stopping treatment, CC induced 5.3% (by cases) or 45.4% (by cycles) ovulation rate and 2 pregnancies achieved in 12 CC resistant patients. CONCLUSION: CPA Co has antiandrogenic effects on PCOS patients clinically, biochemically and in ovarian morphology. Pretreatment of CPA Co may play a role in improving the outcome of ovulation induction in CC resistant cases.

Differences in the mechanism of action of medrogestone and cyproterone acetate.

In the intact adult rat, medrogestone was found to be effective in causing the involution of the prostate and seminal vesicle. Medrogestone, however, does not function through an antiandrogenic action. In the castrate animal medrogestone was unable to block the stimulating action of exogenous testosterone on the sex accessory organs as shown by effects on tissue weight and DNA synthesis. In addition, medrogestone was unable to block the uptake of labeled androgen into prostatic nuclei. The effectiveness of medrogestone on sex accessory tissue may be accounted for by its ability to lower plasma testosterone levels. This is achieved by inhibiting testosterone biosynthesis. The action of medrogestone was compared to that of cyproterone acetate and it was concluded that these drugs inhibit prostatic growth through two different mechanisms.

[Clinical effects of hormone replacement therapy with estradiol valerate and cyproterone acetate in perimenopausal women].

BACKGROUND: In perimenopause many women complain of psychogenic and organic disorders often connected with initial and increasing levels of hypoestrogenism. In this study we evaluated the effects of hormone replacement therapy (HRT) in symptomatic women in perimenopause. METHODS: We enrolled 100 healthy and symptomatic women who were randomly distributed to two groups for a 12-month study. Group A: 50 patients received HRT with estradiol valerate and cyproterone acetate (Pausene(R)); Group B: 50 patients were used as a control group (treated with Cacit Vitamin D30(R)). A number of clinical and instrumental tests were performed at 0, 6 and 12 months to evaluate any changes in vasomotor and urogenital symptoms, bone and serum homeostasis and possible sexual disorders. The statistical analysis was performed using the c2 test. RESULTS: The group of women receiving HRT showed a significant reduction in vasomotor clinical symptoms (p<0.001) and sexual disorders (p<0.002); this was not reflected in the control group where libido decreased steadily with apparent slight improvements only at the end of the study. The urogenital disorders disappeared almost completely in Group A, whereas they diminished in Group B. HRT patients also showed a slight increase (p<0.001) in the initial value of BMD (bone mineral density), a reduction in the marker for bone turnover and an improved lipid profile (p<0.05). These changes were unfortunately not noted in Group B. CONCLUSIONS: On the basis of these results we conclude that HRT appeared to have a beneficial effect on perimenopausal clinical symptoms over the 12-month period, leading to marked improvements in the psychophysical wellbeing of symptomatic women in perimenopause.

The effect of 2 combined oral Contraceptives containing either drospirenone or cyproterone acetate on acne and seborrhea.

A new oral contraceptive has been developed that contains a unique progestogen, drospirenone (DRSP), and that has both antiandrogenic and antimineralocorticoid activity. Our objective was to compare the effect of 30 microg ethinyl estradiol (EE)/3 mg DRSP (EE/DRSP; Yasmin, Schering AG, Berlin, Germany) with that of 35 microg EE/2 mg cyproterone acetate (EE/CPA; Diane-35, Schering AG, Berlin, Germany) on mild-to-moderate cases of acne. Diane-35 is used worldwide (it is not on the market in the United States and Japan) as a hormone treatment for acne, with additional contraceptive benefits. This multicenter, double-blind, randomized study was completed over 9 treatment cycles. A total of 128 women with mild-to-moderate facial acne, with or without seborrhea and/or hirsutism, were randomized. Treatment with either EE/DRSP or EE/CPA was assigned in a 2:1 ratio. Acne lesions, sebum production, and hair growth on the upper lip, chin, and chest were assessed, as well as levels of total and free testosterone, androstenedione, dehydroepiandrosterone sulfate (DHEAS), sex hormone-binding globulin (SHBG), and luteinizing hormone (LH). At study completion, dermatologists, gynecologists, and subjects gave their overall assessment of the effect of treatment on acne. After 9 treatment cycles, the median total acne lesion count was reduced markedly by 62.5% in the EE/DRSP group and 58.8% in the EE/CPA group. A comparison of the 2 groups revealed that EE/DRSP was at least as effective as EE/CPA. Both preparations also reduced sebum production and hair growth on the upper lip and chin. A 3-fold increase in the levels of SHBG was observed in both treatment groups, and levels of androgens and LH decreased. Treatment differences were not seen. Subjective evaluation of the effect of treatment on facial acne by dermatologists, gynecologists, and the subjects themselves indicated an excellent or good improvement for most subjects in both groups. EE/DRSP has been shown to be as effective for treating mild-to-moderate acne as a preparation containing EE/CPA. This new preparation may provide useful hormone therapy for women with androgen-dependent disorders who also require contraception.

Effects of tamoxifen, mifepristone and cyproterone on the electroconvulsive threshold and pentetrazole-induced convulsions in mice.

The aim of this study was to evaluate the efficacy of three antihormones, tamoxifen (TXF, an antiestrogen), mifepristone (MIF, an antiprogesterone) and cyproterone (CYP, an antiandrogen) in two major models of experimental epilepsy, electrically and pentetrazole (PTZ)-evoked seizures in mice. TXF (20-50 mg/kg) significantly raised the threshold for electroconvulsions in female mice, whereas CYP was active in male mice. Similar effects were observed in castrated mice. Different data were obtained in sexually immature animals since both TXF and CYP exerted anticonvulsive effects in animals of both genders. MIF (5-50 mg/kg) remained without effect on electrically evoked seizures in mice. The anticonvulsive action of TXF was reversed by aminophylline, bicuculline, kainic acid and N-methyl-D-aspartic acid, but not by estradiol or strychnine. The protective action of CYP was reversed by aminophylline and bicuculline, but not by testosterone, kainic acid, N-methyl-D-aspartic acid or strychnine. All three antihormones were ineffective against PTZ-induced convulsions in mice. Our results suggest that the action of TXF and CYP might be indirectly associated with the respective hormonal receptor-mediated events, but the nature of this dependence is unclear and further investigations are needed to elucidate this phenomenon.

Urinary androgen response of 8 girls with precocious puberty given cyproterone acetate.

The effects of cyproterone acetate (CA), daily oral administration of 100 mg/m2, on the levels of urinary androgen excretion and velocity of skeletal maturation were studied in 8 girl patients with precocious puberty. Under therapy, testosterone excretion fell within a short time, and velocity of skeletal maturation normalized when the treatment was prolonged. Other beneficial effects such as stoppage of vaginal bleeding and no further growth of breast size and public hair, were also observed. Of 3 subjects, who were given CA for a period of 2--3 yrs, 2 showed further increments in testosterone excretion levels.

[Cyproterone acetate - ethinylestradiol treatment of hirsutism, acne, seborrhea and alopecia].

Twenty-five female patients, aged 14 to 49, and suffering from syptoms of virilization (hirsutism, acne, seborrhea and alopecia) were treated with an estrogen-progestogen combinantion, i.e. cyproterone acetate and ethinylestradiol, having antiandrogenic and antiovulatory action. Patients were submitted to monitoring by means of a series of laboratory tests in order to detect any toxic or hormonal effects that might occur. In most cases symptoms of virilization were significantly reduced, while untoward side effects were not noted.

[Evaluation of efficacy of cyproterone acetate monotherapy in prostatic cancer].

The trial conducted in the Primorsk Regional Cancer Hospital of Vladivostok and P. A. Herzen Moscow Research Cancer Institute studied effectiveness and tolerance of cyproteron acetate (CA) monotherapy and maximal androgenic block (MAB) (CA + surgical or CA + drug castration). A total of 70 patients with localized, locally advanced and generalized prostatic cancer entered the trial. They were divided into three groups: group 1 (54.3%) patients received CA monotherapy--300 mg/day, group 2 (27.1% patients) was exposed to MAB by surgical castration and received CA in a dose 200 mg/day, group 3 (18.6% patients) were exposed to MAB by drug castration and received 200 mg/day CA. The results obtained show that CA monotherapy and MAB have similar effectiveness in favourable prognostic signs. CA monotherapy was less toxic than CA combination with castratio.

The effect of cyproterone acetate on the antler cycle in red deer (Cervus elaphus L.).

The aim of the study was to test the effect of antiandrogen, cyproterone acetate (CA) on the antler cycle in the red deer (Cervus elaphus). CA was administered to three adult red deer stags (Edward, Fuks and Gacek) in weekly intervals. Edward and Fuks were given 600 mg + 600 mg of CA, whereas Gacek was given 600 mg + 300 mg. CA was injected during the hard antler phase: in mid-October (Edward), at the end of November (Fuks) and at the end of January (Gacek). CA caused the antler casting 17 to 22 days after the first injection. In all stags, the casting of antlers was followed by a period of intensive growth of new antlers. Edward was given CA at the end of October. This treatment was responsible for occurrence of the two antler cycles in the year of the experiment. When CA was administered during the middle of the hard antler phase an additional short antler cycle occurs followed by new antler growth. CA treatment in the later part of hard antler phase may cause a prolonged antler cycle.

[Clinical results and hormonal aspects of the treatment of virilizing symptoms in women with cyproterone acetate (author's transl)].

28 cases of moderate to severe hirsutism, two cases of acne and one case of axilary hidrosadenitis were tested continuously on average over 6 months by the daily administration of 100 mg cyproterone acetate. This drug was then given together with 1-2 mg oestradiol valerate over a further 6 month period by reversed sequential therapy. The effect of treatment was judged as being moderate only in 7 cases of idiopathic and one case of adrenal hirsutism and good in the remaining cases. However, this regimen caused a marked lowering of the urinary dehydroepiandrosterone sulphate and low polar oestrogen levels and also the plasma testosterone level. At the same time, according to preliminary results, the fraction of sex hormone binding globulin was raised. The plasma hydrocortisone level was depressed by about 20%. The fractions of 17-KS and 17-KGS decreased slightly at first, but then returned to normal. The side effects of continuous treatment were mainly gestagenic and of reversed sequential therapy mainly oestrogenic in nature. The continuation rate was 81% during a treatment period of two years.

Cyproterone acetate in the therapy of prostate carcinoma.

Prostatic carcinoma is one of the most commonly neoplasm in men, with the strongest incidence around the age of 70 years. In consideration of the high hormonodependence of prostatic neoplasm the reduction of testosterone levels represents the choice of treatment for the patients with metastatic disease, and has an application also in the treatment of patients with a more limited disease, but not elegible to local treatment with a curative aim. The circulating testosterone can be maintained to the lowest levels by the use of drugs that can obtain, with different mechanism of action, a "medical castration". Cyproterone acetate (CPA) is a steroideal antiandrogen which has affinity with progesteron and with glucocorticoidal receptors. It centrally inhibits the release of lutehinizing hormon blocking in this way the secretion of testosterone of testicular origin. Besides, it inhibits the action of the androgens of surrenalic and testicular origin to the cellular level through a competitive direct interaction with the cellular receptors. This review illustrates the main evidences of efficacy and safety of CPA in the treatment of prostatic carcinoma.

Longitudinal study of the behaviour of certain hormonal parameters in women undergoing treatment with cyproterone acetate.

A study has been made of the behaviour of certain hormonal parameters (LH, FSH, PRL, HGH, ACTH, cortisol and testosterone) in five women aged from 17 to 28 years, during two successive menstrual cycles, the first under basal conditions and the second while under treatment with cyproterone acetate (CPA) 100 mg/day), given from the fifth to the twenty-fifth day of the cycle. On the first day of the initial cycle and the twenty-ninth day of the second cycle, the secretion of gonadotropins, prolactin, and somatotropin were measured before and after stimulation with GnRH (50 microgram, i.v.), sulpiride (200 mg, i.m.) or insulin (0.1 U/kg). During treatment with CPA the ovulatory peak of LH was abolished, with a reduction in the levels of gonadotropin and testosterone. No changes were observed for PRL, HGH, ACTH or cortisol. The study of the pituitary reserves showed a reduced response of LH but not of FSH to GnRH. There were no changes in the response of PRL and HGH to stimulation. The data obtained confirm the inhibitory effect of CPA on gonadotropin and testosterone secretion, while the other parameters studied did not vary significantly.

Cyproterone acetate-promoted prevention of renal cortical necrosis following testosterone and vasopressin administration.

Bilateral focal renal cortical necrosis was observed after vasopressin administration in rats pretreated for 10 days with testosterone phenyl propionate. When the androgen-receptor blocking-agent cyproterone acetate was administered together with the testosterone, the subsequent vasopressin treatment did not cause renal cortical necrosis. The results suggest the role of the androgen receptors in the kidney in the induction of the phenomenon.

[Prevention of testosterone-vasopressin induced necrosis of the kidney cortex by cyproterone acetate].

After a ten days pretreatment with testosteron-propyonate administration of vasopressin induces bilateral cortical necrosis of kidneys in rats. When testosteron and cyproteron-acetate an androgen-blocking agent were given simultaneously vasopressin did not induce necrosis of the renal cortices. These findings suggest that androgen receptors of the kidney may take a part in eliciting this phenomenon.
