Intensity of transmitted photon spectra as a function of transverse and longitudinal dimensions of soil medium using 137Cs.

Transmitted photon spectra (pulse-height distribution) of (137)Cs gamma rays are measured through longitudinal and transverse dimensions of the soil medium. The intensity (photon fluence rate) of the multiple-scattered peak, appearing in the lower energy part of the spectrum, has been studied as a function of longitudinal as well as transverse thickness of the soil medium. The intensity of multiple-scatter peak decreases exponentially with the increase in longitudinal thickness of the soil medium, whereas a reverse trend of multiple-scatter peak intensity with transverse thickness occurs as the longitudinal dimension increases. The soil medium as a whole takes part in multiple scattering affecting the intensities of transmitted photons.

Differential effects of a steroidal antiestrogen, EM-139, on prostaglandin and cyclic adenosine 3',5'-monophosphate production in the circular and longitudinal layers of bovine myometrium.

The regulation of prostaglandin (PG) production and cAMP generation was studied in vitro in cultured smooth muscle cells isolated specifically from the circular or longitudinal layers of the bovine myometrium. We found that prostacyclin (PGI2) was the principal PG produced by the myometrium, especially in the longitudinal layer, followed closely by PGE2 and marginally by PGF2 alpha. The PG production (fg/ml, mean +/- SD) in the circular and longitudinal layers was, respectively, PGE2 (424.4 +/- 162.0) > PGI2 (189.5 +/- 19.0) > PGF2 alpha (9.5 +/- 3.0) versus PGI2 (751 +/- 36) > PGE2 (515.7 +/- 94.0) > PGF2 alpha (16.3 +/- 3.0); production was stimulated up to 15-fold 24 h after addition of phorbol 12-myristate (PMA; 100 nM). Hormonal control of PG production was assessed by use of a steroidal antiestrogen, EM-139. PG production was inhibited in a concentration-dependent manner by EM-139 in both circular and longitudinal layers, with maximal inhibition at 1 microM. In parallel studies, chronic treatment with EM-139 resulted in significant increases in isoproterenol-induced cAMP production in both muscle layers, but more especially in the circular layer. This antiestrogenic effect was reversed by addition of 17 beta-estradiol. These results indicate that the two smooth muscle layers of the bovine myometrium have distinct patterns of PG production and that the adenylate cyclase/cAMP response of the circular layer is more sensitive to estrogen modulation. Our findings with a cell culture model of separated myometrial layers provide strategic information for a better understanding of the regulation of uterine contractility during pregnancy.

Igf1 promotes longitudinal bone growth by insulin-like actions augmenting chondrocyte hypertrophy.

Longitudinal bone growth, and hence stature, are functions of growth plate chondrocyte proliferation and hypertrophy. Insulin-like growth factor 1 (Igf1) is reputed to augment longitudinal bone growth by stimulating growth plate chondrocyte proliferation. In this study, however, we demonstrate that chondrocyte numbers and proliferation are normal in Igf1 null mice despite a 35% reduction in the rate of long bone growth. Igf1 null hypertrophic chondrocytes differentiate normally in terms of expressing specialized proteins such as collagen X and alkaline phosphatase, but are smaller than wild-type at all levels of the hypertrophic zone. The terminal hypertrophic chondrocytes, which form the scaffold on which long bone growth extends, are reduced in linear dimension by 30% in Igf1 null mice, accounting for most of their decreased longitudinal growth. The expression of the insulin-sensitive glucose transporter, GLUT4, is significantly decreased and the insulin-regulated enzyme glycogen synthase kinase 3beta (GSK3) is hypo-phosphorylated in Igf1 null chondrocytes. Glycogen levels were significantly decreased and ribosomal RNA levels were reduced by almost 75% in Igf1 null chondrocytes. These data suggest that Igf1 promotes longitudinal bone growth by 'insulin-like' anabolic actions which augment chondrocyte hypertrophy.

The longitudinal primary care clerkship at Harvard Medical School.

The primary care clerkship (PCC) at Harvard Medical School was established in 1997. The goals are to provide students with longitudinal experiences with patients and to include modern themes in the curriculum: managing illness and clinical relationships over time; finding the best available answers to clinical questions; preventing illness and promoting health; dealing with clinical uncertainty; getting the best outcomes with available resources; working in a health care team; and sharing decision making with patients. The PCC, a required course in the clinical years, meets one afternoon a week for nine months. Students spend three afternoons per month in primary care practices, where they see three to five patients per session and follow at least one patient ("longitudinal patient") over time. Classroom sessions, in both large- and small-group formats, promote a common educational philosophy and experience, and reinforce habits of problem-based learning established in the preclinical years. The students rated 74% of their preceptors excellent, especially praising their ability to facilitate and support good interpersonal relationships with patients, their ability to encourage students' independent evaluation of patients (as opposed to shadowing), and their enthusiasm for teaching. Students saw their longitudinal patients a mean of 4.8 times; 83% saw their patients at least three times. The PCC complements the curriculum of block clerkships in hospitals, and because the two are offered concurrently, students are required to come to terms with two substantially different cultures within medicine. Other medical schools are beginning to develop longitudinal clerkships to ensure that students have essential educational experiences that are difficult to achieve in block, hospital-based clerkships.

Longitudinal tibial stress fractures: a report of eight cases and review of the literature.

PURPOSE: We present the imaging features of longitudinal stress fractures in eight patients and evaluate the literature to determine which tests are most useful for making this diagnosis. METHOD: Three musculoskeletal radiologists retrospectively reviewed eight cases of longitudinal tibial stress fractures presenting between 1988 and 1994. Reports on 36 cases, cited in the literature between 1960 and 1996, also were reviewed. Imaging modalities used and characteristic imaging features were noted. RESULTS: Plain radiographs had a characteristic appearance in approximately 28% of patients. Initially, plain films were negative, and in most cases, they eventually showed characteristic radiographic features. However, 25% of cases failed to demonstrate a plain film abnormality, with the diagnosis made by other modalities. CT or MRI is often diagnostic, and longitudinal stress fractures have characteristic imaging features with each modality. While sensitive, bone scan has lower specificity than either CT or MRI. CONCLUSION: Longitudinal stress fractures have characteristic imaging features, and familiarity with these features is necessary for timely and efficient diagnosis, avoiding morbidity due to delay or use of unnecessary tests.

Combined effects of blood pressure and body mass index on left ventricular structure in middle-aged males: cross-sectional and 2-year longitudinal results.

OBJECTIVE: To compare the 2-year longitudinal with the cross-sectional relationships of blood pressure and body mass index with echocardiographic left ventricular measurements in middle-aged males with no history of cardiovascular disease or hypertension. METHODS: M-mode echocardiograms of adequate quality were obtained at initial and 2-year follow-up examinations in 177 subjects. Measurements of left ventricular wall thickness and internal dimensions were made, and estimates of left ventricular mass/height were calculated. Longitudinal changes in left ventricular measurements and risk factors were computed as the differences between the follow-up and initial values. RESULTS: Systolic blood pressure (SBP) was significantly associated with left ventricular mass/height and wall thickness in cross-sectional and in longitudinal analyses. Similar results were observed after adjusting for age, body mass index, sport activity and heart rate. Although body mass index was strongly related to left ventricular mass/height, wall thickness and internal dimension in the cross-sectional study, no significant associations were observed between changes in body mass index and in left ventricular measurements. CONCLUSIONS: The present study emphasizes the differential effects of spontaneous changes in blood pressure and body mass index on the evolution of the left ventricular mass in middle-aged males. Spontaneous changes in SBP during the 2-year follow-up period were associated with rapid changes in left ventricular structure. The 2-year period might not have been sufficient for body mass index to induce changes in left ventricular structure. The duration and amplitude of body weight changes which entail changes in left ventricular mass remain to be determined by further longitudinal investigations.

Relation of left ventricular longitudinal and circumferential shortening to ejection fraction in the presence or in the absence of mild hypertension.

OBJECTIVES: To study left ventricular longitudinal shortening in arterial hypertension and the relative contribution of longitudinal and circumferential fiber shortening to ventricular ejection. METHODS: Two-dimensional and M-mode echocardiograms were obtained for 50 normotensive subjects (aged 49 +/- 12 years) and 50 never-treated mild hypertensive patients (aged 49 +/- 11 years), to measure the minor-axis endocardial and midwall shortening, long-axis shortening and ejection fraction. RESULTS: The midwall shortening was lower in hypertensive than it was in normotensive subjects (P < 0.001) and was related inversely to the circumferential wall stress for both groups (P < 0.04 and 0.0001, respectively). The long-axis shortening in hypertensive patients (22.2 +/- 4.2%) and in normotensives (23.6 +/- 5.4%) was not statistically different, and was not related either to the meridional or to the circumferential wall stress. The ejection fraction was also similar for the two groups (68.2 +/- 6.3 versus 68.6 +/- 5.6%). Both for normotensive and for hypertensive subjects, the ejection fraction was influenced mainly by the midwall shortening (61 and 40% of the variance for normal and hypertensive individuals, respectively), with a minor contribution from the long-axis shortening, which was 7% for normotensive subjects and 18% for hypertensive patients, a statistically significant difference (P < 0.001). The combined effect of midwall and longitudinal shortenings on the ejection fraction was regulated by the relative wall thickness, and was maximal for hypertensive patients with an ejection fraction greater than that predicted by the midwall shortening. CONCLUSIONS: Left ventricular ejection is produced principally by circumferential shortening and is related independently to the relative wall thickness. In the presence of arterial hypertension and an altered cardiac load, longitudinal shortening becomes an important mechanism by which to augment ejection, thereby offsetting the reduction in midwall shortening.

The role of calcium metabolism abnormalities in the development of ossification of the posterior longitudinal ligament of the cervical spine.

To study the role of calcium metabolism in the development of ossification of the posterior longitudinal ligament of the cervical spine, 39 patients with affected cervical spines were investigated using an oral calcium tolerance test and followed by cervical spinal radiography to evaluate disease progression. The average follow-up was 6 years. The patients were divided into two groups according to their responsiveness to an oral calcium load: a group of 14 patients with decreased calciuric responses and another group of 25 with normal responses. The incidence of the development of cervical ossification of the posterior longitudinal ligament in the decreased-response group was significantly higher than that in the normal group. Because these patients were managed operatively by laminoplasty, however, the incidence was low irrespective of their calciuric responses. These results suggest that the development of cervical ossification of the posterior longitudinal ligament is associated with decreased intestinal calcium absorption but that treatment with laminoplasty alters the mechanical environment of the cervical spine (eg, range of motion of the cervical spine is limited). The development of cervical ossification of the posterior longitudinal ligament may be controlled by this procedure.

The generation and detection of longitudinal guided waves in thin fibers using a conical transformer.

This paper describes a technique to couple ultrasonic energy from a piezoceramic disc transducer into a fiber waveguide to induce longitudinal propagation. A polymer cone is utilized to bond the fiber waveguide onto the surface of the disc and to behave as a mechanical transformer, converting lateral displacements at its base into longitudinal displacements at its apex. Wideband finite element analysis (FEA) results are provided to show that the bond efficiently couples the radial modes of a disc transducer into fiber waveguides for longitudinal mode excitation. Furthermore, narrowband FEA is utilized to investigate how the geometry and material properties of the bond and waveguide influence the coupling efficiency. The technique is then quantified in terms of signal-to-coherent noise ratio (SCNR), reflecting its ability to generate the desired longitudinal waveguide mode and reject erroneous modes. Finally, design parameters are outlined for the successful implementation of this technique.

Differential expression of P2X-purinoceptor subtypes in circular and longitudinal muscle of canine colon.

Adenosine triphosphate (ATP) mediates excitatory junction potentials through P2X receptors in many smooth muscles. However, relatively little is known about postjunctional intestinal P2X receptors. We examined the effect of exogenous ATP on circular and longitudinal myocytes of canine colon using the patch clamp technique at 32 degrees C. In both cell types, ATP induced inward currents (I(ATP)) at -70 mV in a concentration-dependent manner. The potency profile of ATP analogues in circular myocytes was: ATP approximately 2-methylthio-ATP > alpha,beta-methylene ATP, and that in longitudinal myocytes was: alpha,beta-methylene ATP approximately ATP > 2-methylthio-ATP. Pretreatment of circular myocytes with alpha,beta-methylene ATP inhibited the response to subsequent ATP, suggesting receptor desensitization. I-V relationships of I(ATP) were linear with inward rectification and E(rev) of -13 mV. I(ATP) at -70 mV was carried predominantly by Na+ as determined by shifts in E(rev) when extracellular Na+ was lowered. In RT-PCR, circular myocytes expressed mRNAs encoding P2X2, 3 and 4, while longitudinal myocytes expressed mRNAs for P2X3 and 5. P2X7 was absent in both cells. Fragments of each subtype were cloned and sequenced. We failed to clone P2X1 and P2X6 genes. Overall, different P2X receptor subtypes are expressed in circular and longitudinal canine colonic myocytes. Their activation produces non-selective cation currents that can depolarize and excite muscles of both layers.

Acute desensitization of muscarinic receptors in the isolated guinea-pig ileal longitudinal muscle.

1. The effects of acute desensitization of muscarinic receptors mediating contractile responses of the guinea-pig ileal longitudinal muscle were studied in vitro, using similar conditions for both functional and radioligand binding studies. 2. The pA2 values for a number of muscarinic antagonists (pirenzepine, methoctramine, (+/-)para-fluoro-hexahydro-siladifenidol and 4-diphenylacetoxy-N-methyl piperidine-methiodide) indicated that the contractile response to carbachol was mediated through an M3 muscarinic receptor. In binding experiments the muscarinic receptor subtype population in ileal longitudinal muscle was found to be heterogeneous, consisting of approximately 77% M2 and 23% M3 receptors. 3. Pre-exposure of ileal longitudinal muscle to 10 microM carbachol for 30 min produced a reduction (28 +/- % of control maximum) in the maximum contractile response and a dextral shift in the concentration-effect curve to carbachol. Prior equilibration (60 min) with (+/-)p-F-HHSiD (1 microM), but not with methoctramine (1 microM) or pirenzepine (0.3 microM), prevented the desensitization. Desensitization under these conditions did not alter either the apparent affinity, the total number of binding sites or the relative, proportions of M2 and M3 muscarinic receptors, as determined in radioligand binding studies. Desensitization did not cause any meaningful change in either the apparent affinity of carbachol or the proportion of the high and low affinity binding sites. 4. It is concluded that desensitization of the contractile responses of the guinea-pig ileal longitudinal muscle is a result of M3 but not M2 muscarinic receptor desensitization. Acute desensitization, therefore, is not accompanied by meaningful changes in the total number of both M2 and M3 receptors or by alterations in the affinity of the receptor to ligands.

Dynamics in longitudinal eye movements and corneal shape.

The magnitude and character of longitudinal movements of the eye were studied in the context of high speed videokeratoscopy. It was of interest to determine whether these dynamic changes in the eye movements can affect the corneal shape and its estimation. A high speed videokeratoscope with a sampling frequency of 50 Hz was used for measuring the corneal apex movements as well as for measuring variations in the best-fit sphere radius and central radius of curvature. The magnitude of the measured longitudinal apex movements could reach over 200 microm showing a slow trend. The estimated local changes of the apex movements that could be associated with the cardiopulmonary system were about 40-50 microm. The temporal variations in the equivalent estimated central radius of curvature ranged between 10 and 15 microm. Spectral analysis of the longitudinal eye movements revealed strong signatures of the pulse and respiration signals as well as the assumed blink control signal. No clear association between the longitudinal apex movements and the corneal curvature was found. However, very slow significant changes in the corneal shape were observed. The central radius of curvature of the cornea revealed slow changes of up to 120 microm. Understanding the nature of such changes will be of benefit in ophthalmic applications requiring highly accurate measurements of corneal shape, such as contact lens design and refractive surgery.

Morphine-tolerant longitudinal muscle strip from guinea-pig ileum.

1. Implantation of morphine pellets in guinea-pigs produced a high degree of tolerance and dependence within 3 days.2. The contractions of the longitudinal muscle induced by electrical stimulation of the myenteric plexus-longitudinal muscle preparations obtained from tolerant animals were less depressed by morphine than the contractions evoked in preparations from non-tolerant animals.3. Naloxone did not alter the size of the evoked twitch but antagonized the depressant action of morphine in tolerant and in non-tolerant animals. When given to tolerant guinea-pigs, naloxone caused an increase in intestinal activity in vivo.4. The contractile response of the longitudinal muscle to acetylcholine was the same in preparations obtained from tolerant and non-tolerant animals. Electrically evoked contractions of the myenteric plexus-longitudinal muscle preparations from tolerant animals showed reduced sensitivity to the depressant effects of adrenaline, isoprenaline, and particularly, dopamine.

Influence of N-ethylmaleimide on cholinoceptors and responses in longitudinal muscles from guinea-pig ileum.

1 The binding of carbamylcholine to membranes prepared from the longitudinal muscle of guinea-pig ileum was determined from its inhibition of the binding of [3H]-3-quinuclidinyl benzilate. Carbamylcholine binding was resolved into high and low affinity components with apparent dissociation constants of 0.11 +/- 0.02 and 11 +/- 1 microM; 42% of the receptors displayed high affinity carbamylcholine binding. 2 Alkylation of longitudinal muscle membranes with N-ethylmaleimide increased muscarinic receptor affinity for carbamylcholine in a manner consistent with a conversion of low affinity to high affinity receptors. After exposure the muscle membrane fragments to 1 mM N-ethylmaleimide for 20 min at 35 degrees C, carbamylcholine binding was resolved into two components with apparent dissociation constants of 0.11 +/- 0.01 and 9 +/- 2 microM, with 74% of the receptors displaying the higher affinity. 3 Exposure of longitudinal membranes mounted in an organ chamber to 1 mM N-ethylmaleimide for 30s depressed isometric contractions in response to acetylcholine by 80%, while contractions induced by K+ and Ba2+ were reduced by less than 20% and 10%, respectively. Acetylcholine dose-response curves were shifted to the right while Ba2+ curves were unaffected. 4 It is suggested that N-ethylmaleimide has a selective effect on muscarinic responses in the longitudinal muscle by disrupting processes occurring after receptor occupancy but before the induction of phospholipid turnover or calcium influx in the postsynaptic membrane.

Changes in the mechanical properties of the longitudinal and circular muscle tissues of the rat myometrium during gestation.

Changes in the mechanical properties of the longitudinal and circular muscle tissues of the rat myometrium during gestation were investigated. In isolated longitudinal and circular muscles of the rat myometrium, spontaneous contractions and contractions per unit cross-sectional area induced by 128 mM K+ and 1 X 10(-5) M acetylcholine (ACh) increased with the progress of gestation. These increases appeared in longitudinal muscles to a greater extent than in circular muscles. ACh induced the largest contraction for both intact muscle tissues, at all stages of gestation. In both muscle layers, the ACh-induced contraction reached the same amplitude as the 1 X 10(-5) M Ca-induced contraction of skinned muscles, except for the longitudinal muscle at the 22nd day of gestation. In Ca-free solution containing 2 mM EGTA, ACh produced contraction in both intact muscle tissues at all stages and the amplitude was increased during the progress of gestation, whereas the K-induced and spontaneous contractions ceased. In saponin-treated skinned muscles of both layers, the free Ca concentration required to produce contraction was lowered, the maximum amplitudes of the contraction were increased and the pCa-tension relationships shifted to the left during the progress of gestation. The results indicate that during the progress of gestation, Ca sensitivity of of the contractile proteins and mechanical responses to agonists increased and that the properties of the intracellular Ca store site were also altered in both muscle layers.

Simultaneous measurement of contractile effects in the circular and longitudinal smooth muscle of the rat vas deferens by drugs perfused externally or via the lumen.

The effects of noradrenaline and barium chloride were studied in the rat isolated vas deferens by perfusion of drugs either externally or through the lumen of the organ. Two effects were recorded simultaneously in the same preparation: (a) isometric contractions, due to the tension elicited by drugs on the external (longitudinal) smooth muscle layer and (b) pressure of internal perfusion, due to contractions of the internal (circular) smooth muscle layer. It was found with the longitudinal muscle that: (a) the potency, expressed as pD2 values, and the maximum response to noradrenaline were lower if the drug was perfused internally rather than externally; (b) the differences in maximum effects were pronounced on the prostatic half but were not observed on the epididymal half; (c) the maximum response obtained by internal perfusion could be increased by simultaneously adding the same dose of drug externally; (d) when barium chloride was used instead of noradrenaline no significant differences were observed on pD2 values, but differences on maximal responses were similar to that observed for noradrenaline; (e) it was possible to block completely the effect of internal or external noradrenaline on the longitudinal muscle, by perfusing external phenoxybenzamine. In these conditions the responses of the circular muscle to the agonist were only partly blocked. With the circular muscle, the differences related to internal and external perfusion were less marked than in the longitudinal muscle. However, unlike the latter, the circular layer was slightly more sensitive to drugs applied internally, in relation to pD2 values. It is suggested that the difference in pD2 values may be due to the removal of noradrenaline by the neuronal uptake process, whereas the difference in maximal effect is due to the inaccessibility of part of the receptor population when drugs are added through the lumen.

Bradykinin stimulation of phosphoinositide hydrolysis in guinea-pig ileum longitudinal muscle.

1. Bradykinin (BK)-induced contraction of ileal smooth muscle is assumed to be due to phosphoinositide hydrolysis but this has never been reported. We have investigated whether BK receptors are linked to this transduction mechanism in guinea-pig ileum longitudinal muscle and determined whether these receptors are equivalent to those labelled in [3H]-BK binding assays. 2. In membranes prepared from longitudinal muscle, [3H]-BK bound to a single class of sites with high affinity. Characterization of the binding with BK analogues indicated that the radioligand selectivity labelled a B2 type receptor. 3. BK significantly elevated tissue levels of [3H]-inositol phosphates in longitudinal muscle slices preincubated with [3H]-myo-inositol. The agonists potencies of BK, Lys-BK, Met-Lys-BK, Tyr5-BK and Tyr8-BK were in agreement with their relative potencies in the binding assay. The B1 receptor agonist des-Arg9-BK, did not stimulate inositol phosphate production. The response to BK was blocked by known B2 receptor antagonists but not by the B1 antagonist des-Arg9, Leu8-BK. 4. BK-induced phosphoinositide hydrolysis was unaffected by exposure of muscle slices to either atropine or indomethacin. 5. The results indicate that the B2 receptors linked to phosphoinositide turnover in ileal longitudinal muscle exhibit properties similar to those involved in contractile responses. Also, the receptor mediating the phosphoinositide response is likely to be that labelled in the [3H]-BK binding studies.

Longitudinal metric properties of disability rating scales for Parkinson's disease.

OBJECTIVES: This study analyzes the longitudinal metric attributes of three Parkinson's disease (PD) disability scales, taking Hoehn and Yahr (HY) staging as the reference measure of PD progression. METHODS: A sample of 87 PD patients was assessed during regular medical visits, using the HY, the Unified Parkinson's Disease Rating Scale--Activities of Daily Living Section (UPDRS-ADL), the Schwab and England Scale (SES), and the Intermediate Scale for Assessment of PD (ISAPD), across a follow-up period of 2.6 +/- 1.0 years. RESULTS: The following cross-sectional attributes were analyzed, at baseline and again on conclusion of the study: floor and ceiling effects, convergent validity, reliability, and standard error of measurement, all of which were found to be adequate. Longitudinal reproducibility values (intraclass correlation coefficient) were 0.81 (ISAPD) to 0.84 (UPDRS-ADL). Insofar as longitudinal validity was concerned, the change scores of the three disability scales correlated significantly with the HY change score, absolute value r = 0.33 to 0.45, P < 0.003. Slightly lower values were found when taking the annual rate of change, absolute value r = 0.20 to 0.36. The three scales were acceptable, even though there were small differences among them. The "minimal clinically important difference" proposed for these scales is: SES, -6; UPDRS-ADL, +2; ISAPD, +1.5 points. CONCLUSIONS: The three scales proved adequate for longitudinal assessment of PD disability. UPDRS-ADL was more precise and ISAPD more consistent. Magnitude of change and correlation with change in HY were slightly higher with the ISAPD. Effect size and standardized response mean for the minimal change in HY were higher for the UPDRS-ADL.
