[Artificial intranasal bacterial invasion in the newborn calf. 2. Artificial bacterial invasion using gram-positive and gram-negative strains].

A micrococcal and a diplocaccal strain isolated from the nasal space of a clinically intact nursed calf were used for artificial bacterial invasion in the first phase of the experiment. Application of bacterial suspension prepared from those strains had no effect upon the rise of coli counts in the nasal secretion of nursed calves during their first days of age nor upon the morbidity or mortality of all 677 test animals in comparison to 665 controls. Therefore, an avirulent E.-coli strain was used in subsequent bacterial invasion experiments. The strain was retrievable up to the seventh day of age, the count having been about 10(5) bacteria per gram nasal secretion. Application of a bacterial suspension prepared from that E.-coli strain did not reduce morbidity and mortality among 820 test animals that were compared to 809 controls. Results are discussed in this paper with reference to literature.

An improved system for quantifying AgNOR and PCNA in canine tumors.

BACKGROUND: Quantifying silver stained nucleolar organizer regions (AgNORs) and proliferation cell nuclear antigens (PCNA) are useful techniques to measure proliferative activity of tumor cells; however, the nonspecific deposition of stains and overlappings of AgNOR and PCNA counts between grades of tumors hamper their applications. MATERIALS AND METHODS: Fifty-two surgical specimens from dogs, including mast cell tumors, perianal gland tumors and hyperplasias, fibromas, fibrosarcomas, and normal tissues were studied. The 3 microns dewaxed sections of formalin-fixed tissues were stained to detect AgNORs by a modified inverted incubation technique in a newly developed silver staining device. Data were collected and analyzed using a high-resolution digital microscope camera and image analysis software. Sequential sections were also stained for PCNA using an immunohistochemical method. RESULTS: The improved system for quantifying AgNOR provided more accurate and non-overlapping mean AgNOR counts, which enable us to distinguish benign states from malignant changes. The mean AgNOR cut-off points that discriminated grade II or III mast cell tumors from grade I, perianal gland carcinomas from adenomas (or hyperplasia), fibrosarcomas from non-fibrosarcoma tissues, were 6.0, 14.1, 9.4, and 8.8 respectively. The mean AgNOR areas, relative AgNOR areas, and PCNA positive rates of some malignant and non-malignant tissues (benign tumor and normal tissues) were significantly different (P < 0.05). CONCLUSIONS: This improved system is a sensitive and rather precise method for quantifying the AgNOR and PCNA. It provides a valuable objective measurement for differentiating benign and malignant tumors.

Evaluation of coliforms as indicators of water quality in India.

The total and thermotolerant coliform counts in rural drinking water derived from ground water, piped supplies and surface water are reported for a number of areas in India. To evaluate these counts as indicators of recent faecal contamination the total coliforms and thermotolerant coliforms isolated have been identified. Thermotolerant Escherichia coli formed 11.7% of the total coliforms and 75.1% of the thermotolerant coliforms. Citrobacter sp. (20.4%) and Klebsiella sp. (50.9%) were the other common total coliforms isolated and, among the thermotolerant coliforms, Klebsiella sp. (16.4%) was the only other organism frequently encountered. The total coliform counts were significantly correlated with water temperature. The applicability in tropical areas of standards developed for temperate climates is discussed.

Capillary density in rat myocardium during timed plasma staining.

In urethan-anesthetized thoracotomized rats the density of myocardial capillaries that had been stained during timed infusions of a plasma label was determined. gamma-Globulin-coupled fluorochromes [lissamine-rhodamine B 200 (RB 200) and fluoresceinisothiocyanate (FITC)] were applied to the vascular system for different intervals of time. The circulation in the heart was freeze stopped, and the dye-containing capillaries were counted in histological cross sections of the organ. When the dyes had been infused into the left atrium for 1, 2, 5, or 10 s the number of stained vessels continuously increased (1,960 +/- 170, 2,780 +- 160, 3,310 +/- 130, and 3,340 +/- 140 cap/mm2 in the subepicardium and 1,990 +/- 210, 2,490 +/- 190, 3,090 +/- 160, and 3,210 +/- 160 cap/mm2 in the subendocardium, respectively). However, further prolongation of dye exposure (2, 10, or 30 min) did not increase significantly the number of marked capillaries. The induction of a general hypoxia (30-s stop of ventilation) after dye application also did not induce higher capillary counts than were found in the normoxic animals during long-time plasma labeling. Changes in red blood cell-containing capillaries were not covered by these observations. The distribution of stained capillaries in the evaluated areas was more inhomogeneous after incomplete filling with RB 200 for 1 s than after complete staining with FITC for 4 min. The results favor the view that in the rat heart at rest the blood plasma passes the microcirculation in an inhomogeneous manner; in all capillaries the slowest transit times do not exceed 5 s, however.

A score test for overdispersion in zero-inflated poisson mixed regression model.

Count data with extra zeros are common in many medical applications. The zero-inflated Poisson (ZIP) regression model is useful to analyse such data. For hierarchical or correlated count data where the observations are either clustered or represent repeated outcomes from individual subjects, a class of ZIP mixed regression models may be appropriate. However, the ZIP parameter estimates can be severely biased if the non-zero counts are overdispersed in relation to the Poisson distribution. In this paper, a score test is proposed for testing the ZIP mixed regression model against the zero-inflated negative binomial alternative. Sampling distribution and power of the test statistic are evaluated by simulation studies. The results show that the test statistic performs satisfactorily under a wide range of conditions. The test procedure is applied to pancreas disorder length of stay that comprised mainly same-day separations and simultaneous prolonged hospitalizations.

Non-Gaussian smoothing of low-count transmission scans for PET whole-body studies.

A non-Gaussian smoothing (NGS) technique is developed for filtering low count transmission (TR) data to be used for attenuation correction (AC) of positron emission tomography (PET) studies. The method is based on a statistical technique known as the generalized linear mixed model that allows an inverse link function that avoids the inversion of the observed transmission data. The NGS technique has been implemented in the sinogram domain in one-dimensional mode as angle-by-angle computation. To make it adaptive as a function of the TR count statistics we also develop and validate an objective procedure to choose an optimal smoothing parameter. The technique is assessed using experimental phantoms, simulating PET whole-body studies, and applied to real patient data. Different experimental conditions, in terms of TR scan time (from 1 h to 1 min), covering a wide range of TR counting statistic are considered. The method is evaluated, in terms of mean squared error (MSE), by comparing pixel by pixel the distribution for high counts statistics TR scan (1 h) with the corresponding counts distribution for low count statistics TR scans (e.g., 1 min). The smoothing parameter selection is shown to have high efficiency, meaning that it tends to choose values close to the unknown best value. Furthermore, the counts distribution of emission (EM) images, reconstructed with AC generated using low count TR data (1 min), are within 5% of the corresponding EM images reconstructed with AC generated using the high count statistics TR data (1 h). An application to a real patient whole-body PET study shows the promise of the technique for routine use.

Suppression of caries-related microorganisms in dentine lesions after short-term chlorhexidine or antibiotic treatment.

This study investigated the efficiency of a chlorhexidine varnish and an antibiotic paste in suppressing the cultivable microflora of deep dentine cavities in a stepwise excavation procedure. Subsequent to enamel preparation and removal of the central biomass, infected dentine was sampled from the cavity floor. Ten cavities each were either covered with the 1% chlorhexidine- and 1% thymol-containing varnish Cervitec (CE), the demeclocycline hydrocortisone-containing ointment Ledermix (LE) or received no treatment as control (CO). A compomer composite was used as intermediate restorative. Cavities were reassessed after 6 weeks and again dentine samples were microbiologically investigated for total viable counts, mutans streptococci and lactobacilli. After 6 weeks a significant reduction of the total viable counts was observed in the LE group (p = 0.011) compared to the control, whereas no differences were found in the CE group (p > 0.05). Mutans streptococci were rarely recovered at baseline and after 6 weeks. Compared to the CO group counts of lactobacilli were significantly reduced in the CE and LE groups (p < 0.05). Lactobacillus species were frequently recovered at baseline and after 6 weeks of observation. Lactobacillus rhamnosus was the predominant species in all samples investigated. Application of CE or LE resulted in reduced counts of lactobacilli after a period of 6 weeks. Although none of the materials completely eliminated the viable microorganisms, the use of LE was more effective than CE in reducing the total anaerobic microorganisms associated with carious dentine.

Effect of chlorhexidine varnish on Actinomyces naeslundii genospecies in plaque from dental fissures.

This study describes the effects of varnish containing 40% chlorhexidine diacetate on Actinomyces naeslundii populations in plaque from human molar fissures. In each of 15 subjects two dental fissures with high levels of mutants streptococci were selected. The experimental treatment consisted of the single application of a small amount of chlorhexidine varnish onto the selected fissures. The varnish was removed 15 min after application. One month after varnish application a significant increase was observed in A. naeslundii counts while the number of mutans streptococci had decreased significantly compared with preexperimental levels. From 85 randomly selected Actinomyces isolates taken from blood agar plates before varnish application, 44% belonged to A. naeslundii genospecies 1 and 56% to A. naeslundii genospecies 2. From 106 isolates taken 1 month after chlorhexidine varnish application, 42% belonged to A. naeslundii genospecies 1 and 58% to A. naeslundii genospecies 2. At baseline 28% of A. naeslundii genospecies 1 strains were catalase-positive, but 1 month after varnish application 4% of the strains were catalase-positive (p < 0.05). It is concluded that chlorhexidine varnish application caused an increase of A. naeslundii in dental plaque, but induced no significant changes in the distribution of the two A. naeslundii genospecies.

Rapid G+ count and subpopulation assessment of the intestinal bacteria in Apodemus sylvaticus and Mus musculus by flow cytometry.

We report a novel application of calcein-acetomethyl ester in flow cytometry for rapid estimation of the number of G+-bacteria in rodent feces (Apodemus sylvaticus and Mus sp.f. muridae). We also use the combined application of flow cytometry and Syto-13 or Sypro Orange staining to count rapidly the total bacterial population and to describe bacterial subpopulations in the intestine.

Detection of photoreactivity demonstrated in a modified local lymph node assay in mice.

Photoallergic and phototoxic reactions are an increasing problem in dermatologic practice. Currently available test models are heterogeneous and do not allow differentiation between photoallergic and phototoxic reactions. We have modified the local lymph node assay to screen for photoreactive compounds. We found an ultraviolet A (UVA) irradiation-dependent increase in the weights and cell counts of the auricular lymph nodes after application of 8-methoxypsoralen, tribromosalicylanilide, trichlorosalicylanilide, chlorpromazine and promethazine. In contrast to these photoreactive compounds, contact sensitizers did not produce any reaction to UVA irradiation in the local lymph node assay. This assay system has the advantage of being less expensive and less time-consuming and might even be able to differentiate between phototoxic, photoallergic and contact allergic reactions.

The mechanisms of immune suppression by high-pressure stress in mice.

The effects of high-pressure stress on the induction of anti-sheep red blood cells (SRBC) and of plaque-forming cells (PFC), and on thymus weight, were studied in BALB/c mice in-vivo and in-vitro. The efficacy of high-pressure stress in suppressing PFC and thymic involution was maximum when the stress was applied 1 h day(-1) for 2 days before immunization with SRBC. Both effects were blocked by administration of indomethacin, atropine, naloxone or phentolamine before the first application of stress, whereas hexamethonium and propranolol had no such effect. Hexamethonium, naloxone and propranolol administered before the second application of high-pressure stress blocked both effects. Prostaglandin and acetylcholine given 24 h before application of high-pressure stress caused a marked reduction in PFC count, but not in thymus weight. The reduced PFC count caused by acetylcholine was blocked by pretreatment with indomethacin. When adrenaline was injected 24 h after application of high-pressure stress a marked reduction in PFC was observed, but without thymic involution. When adrenaline was injected 24 h after prostaglandin injection the PFC count was also markedly reduced, but not thymus weight. The decrease in PFC caused by two exposures to stress or one exposure to stress plus injection of adrenaline was blocked by diethylcarbamazine before the second exposure to stress or the injection of adrenaline. In addition, normal spleen cells, were induced as suppressor cells when incubated with the serum of stressed mice, but not when supplemented with anti-leukotriene C4, D4 antibody. These data suggest that mice fall into a pre-stress condition via the release of prostaglandin after the first stress, and then immunosuppression is induced in these prestressed mice via the release of leukotriene C4, D4, caused by the activation of the autonomic nervous system by the second exposure to stress.

Role of data aggregation in biosurveillance detection strategies with applications from ESSENCE.

INTRODUCTION: Syndromic surveillance systems are used to monitor daily electronic data streams for anomalous counts of features of varying specificity. The monitored quantities might be counts of clinical diagnoses, sales of over-the-counter influenza remedies, school absenteeism among a given age group, and so forth. Basic data-aggregation decisions for these systems include determining which records to count and how to group them in space and time. OBJECTIVES: This paper discusses the application of spatial and temporal data-aggregation strategies for multiple data streams to alerting algorithms appropriate to the surveillance region and public health threat of interest. Such a strategy was applied and evaluated for a complex, authentic, multisource, multiregion environment, including >2 years of data records from a system-evaluation exercise for the Defense Advanced Research Project Agency (DARPA). METHODS: Multivariate and multiple univariate statistical process control methods were adapted and applied to the DARPA data collection. Comparative parametric analyses based on temporal aggregation were used to optimize the performance of these algorithms for timely detection of a set of outbreaks identified in the data by a team of epidemiologists. RESULTS: The sensitivity and timeliness of the most promising detection methods were tested at empirically calculated thresholds corresponding to multiple practical false-alert rates. Even at the strictest false-alert rate, all but one of the outbreaks were detected by the best method, and the best methods achieved a 1-day median time before alert over the set of test outbreaks. CONCLUSIONS: These results indicate that a biosurveillance system can provide a substantial alerting-timeliness advantage over traditional public health monitoring for certain outbreaks. Comparative analyses of individual algorithm results indicate further achievable improvement in sensitivity and specificity.

Epidemiologic considerations in reporting herd somatic cell counts.

The reporting of herd summaries of individual cow somatic cell counts differs between the nine Dairy Records Processing Centers. The objectives of this paper are to review the present reporting; to compare the methods used to calculate herd somatic cell counts; and to discuss the epidemiologic implications of these methods. Estimates of central tendency and frequency distributions are the most commonly used herd summary statistics for somatic cell counts. Analyses of the frequency distributions are less widely available. Potential uses of herd somatic cell counts are to measure one aspect of herd milk quality; to estimate the prevalence of mastitis; to analyze mastitis patterns within the herd; and to investigate new areas for mastitis control research. Presently, the best available methods for these applications appear to be bulk tank somatic cell counts or weighted arithmetic means for gauging milk quality; frequency distributions for estimating mastitis prevalence; analyses of frequency distributions of mean linear score by lactation and stage of lactation for identifying mastitis patterns within a herd; and frequency distributions or estimated incidence rates, adjusted for differences in age and stage of lactation, for investigating new areas for mastitis control research.

Sporicidal efficacy of hydrogen peroxide aerosol.

Because of its high sporicidal activity but low toxicity, hydrogen peroxide (H2O2) is a useful agent for gentle antimicrobial treatment of irritable goods. Above all to optimise practical aspects of H2O2 application, sporicidal efficacy of aerosols based on 1% H2O2 solution was tested. Using commercially available bioindicators with Bacillus subtilis spores immobilized on filter paper strips, a 7 h lasting aerosol treatment at room temperature resulted in a reduction of viable count of 2.5 log10 (median, n = 5). By combined application of 0.3% sorbitan monolaurate (Span 20) and 1% H2O2 aerosol at room temperature, a reduction of viable count of >6.3 log10 (median, n = 5) could be reached within 2 h, already. Following aerosol treatment at 60 degrees C, all bioindicators tested (n = 15) have been found without growth after 10 min, corresponding to a reduction of viable count >6.3 log10. However, this efficacy was possible only if the spores were immobilized on filter paper strips. Changing the carrier materials lower sporicidal activity of H2O2 aerosol was found resulting in a reduction of viable count between >5.4 log10 (median, n = 15) on polyethylene foil and 0.4 log10 (median, n = 15) on glass slides. These differences could not be correlated with surface qualities as porosity or wettability.

Dopaminergic modulation of transcallosal activity of cat motor cortical neurons.

The effects of dopamine (DA) and its antagonists on the transcallosal activity of pyramidal tract neurons (PTNs) and non-PTNs in the anesthetized cat motor cortex were studied with iontophoretic applications; dopamine, SCH 23390 (D1 antagonist), sulpiride (D2 antagonist) and haloperidol. Neuronal activity was recorded with a multi-barreled glass microelectrode. Transcallosal neuronal activity was evoked by stimulation of the contralateral motor cortex. The number of spikes thus activated was counted for the control and test conditions after application of each drug: (1) dopamine application decreased the number of spikes evoked by transcallosal stimulation; (2) application of SCH 23390, sulpiride and haloperidol restored these decreased spike numbers to the control level; (3) latency of neuronal response to transcallosal stimulation was not affected by the application of either DA, SCH 23390, sulpiride or haloperidol; and (4) there was no significant difference between PTNs and non-PTNs in the manner of response to DA and its antagonist applications. Our conclusion is that dopamine modulated the transcallosal neuronal response in the cat motor cortex in a suppressive manner. This fact suggested that interhemispheric neuronal communications could be subjected to suppressive modification by the dopaminergic system.

Breast cancer incidence and its possible spatial association with pesticide application in two counties of England.

BACKGROUND: This study examines the spatial distribution of breast cancer incidence in Lincolnshire and Leicestershire, and its association with the application of selected pesticides. METHODS: The Black-White (BW) join-count statistic and Moran I coefficient were used to investigate the localized distribution of breast cancer. Linear regression techniques were applied to examine the association between the breast cancer incidence rate and pesticide application. RESULTS: The results from the BW join-count test and the Moran I coefficient test showed no overall evidence of localized distribution, 'clusters', of breast cancer. The regression analyses showed no sign of spatial association between breast cancer and application of pesticides in the urban areas as expected. The findings, however, did reveal a spatial association between the breast cancer incidence rates and the application of three selected pesticides [Aldicarb, Atrazine and Lindane (thought to potentially mimic oestrogen)] in rural Leicestershire. No such association was seen in Lincolnshire. CONCLUSIONS: Pesticides vary significantly in their spatial application. Overall, consistent associations between breast cancer incidence rates and the selected pesticides were not found, although the ecological study design has limitations and these are discussed. This approach is able to rule out strong associations; assessment of smaller risk would require a large and expensive study in individuals. The results of this study, although derived in the UK, have significance to the debate concerning the possibility of environmental compounds that mimic oestrogen and their consequences for human health.

Routine application of the proton-pump inhibitor pantoprazole in patients with gastric lymphoma undergoing chemotherapy.

OBJECTIVE: The stomach is the most common site of origin for extranodal lymphomas. While resection has played a major part in the management of such patients in the past, in recent years there has been a change towards organ-conserving therapies. However, the risk of perforation and bleeding in patients undergoing organ-conserving therapy has been used as an argument against primary application of chemotherapy. In this article, we present our experience with the prophylactic use of oral pantoprazole. MATERIAL AND METHODS: All patients undergoing chemotherapy for gastric lymphoma at our institution were prophylactically given oral pantoprazole (2x40 mg) for the duration of chemotherapy. Compliance with intake of the proton-pump inhibitor (PPI) was assessed at every visit for application of chemotherapy and at routine blood counts taken 10-14 days after each cycle of treatment by direct questioning of the patient. RESULTS: A total of 82 patients (median age 69 years, range 33-93) received chemotherapy for gastric lymphoma: 51 had diffuse large B-cell lymphoma (DLBCL), 24 had mucosa-associated lymphoid tissue (MALT) lymphoma and 7 had DLBCL+MALT lymphoma. Compliance with intake of the PPI was excellent, as only two patients reported irregular intake and only one patient refused regular medication with pantoprazople. All patients responded to chemotherapy, with 74 (90%) achieving complete remission and 8 (10%) partial remission. After a median follow-up time of 50 months (range: 9-84 months) only one of these 82 patients (1.22%), the patient who discontinued intake of pantoprazole, died from gastric perforation, while none of the other patients had gastrointestinal bleeding or perforation. CONCLUSIONS: Judging from these data, continuous PPI intake is feasible and has a high rate of compliance. In the absence of randomized trials, routine application of oral pantoprazole in patients given chemotherapy for gastric lymphoma, especially DLBCL, appears to be a reasonable approach.

[Changes of laboratory hematologic indices in patients with chronic glomerulopathies following treatment with superhigh doses of cyclophosphamide].

41 patients with active glomerulopathies treated with super high doses of Cyclophosphamide were studied. The following indices were followed up dynamically: hemoglobin concentration, number and morphology of erythrocytes, number and morphology of leucocytes and differential count. Hemoglobin concentration and erythrocyte count remain in the reference ranges in most of the patients. They fall more considerably compared to the initial values between the 90-120 day after the treatment. This fall improves spontaneously and does not require discontinuance of the treatment. The single application of super high dose of Cyclophosphamide does not influence significantly the leucopoiesis. Because of the parallel application of glucocorticosteroids no fall of the leucocyte count is observed and they remain in their reference ranges.
