Intracellular mislocalization of mutant podocin and correction by chemical chaperones.

The NPHS2 gene encoding the podocin protein was causally linked to the autosomal recessive type of steroid-resistant nephrotic syndrome. In this study, we investigated the consequence of the R138Q mutation of podocin, one of the most common missense mutations in the NPHS2 gene, by examining the expression of the wild-type and R138Q mutant podocins in mammalian cells. Either myc- or FLAG-tagged wild-type podocin was strongly stained in plasma membrane, particularly in the fine processes wherein the protein was colocalized with actin stress fibers. On the other hand, the R138Q mutant podocin was completely retained intracellularly and colocalized with the endoplasmic reticulum (ER) marker, calnexin. These results suggest that the R138Q mutation affected podocin protein folding, thereby interfering with the mutant protein's departure from the ER. To determine if the ER retention of R138Q mutant is correctable, cells were incubated with the chemical chaperones glycerol, trimethylamine-N-oxide, and DMSO. Using these two methods, namely, cell surface labeling with sulfo-NHS-S-S-biotin and Alexa 488-streptavidin, and immunostaining to detect the podocin protein close to the plasma membrane, we confirmed that these chemical chaperone treatments elicit a cellular redistribution of R138Q podocin. Our results reveal defective cellular processing of the mutant podocin, and provide evidence for pharmacological correction of the processing defect.

Rapid detection and identification of Clostridium chauvoei by PCR based on flagellin gene sequence.

We developed a one-step polymerase chain reaction (PCR) system that specifically detects Clostridium chauvoei. Oligonucleotide primers were designed to amplify a 516-bp fragment of the structural flagellin gene. The specificity of the PCR was investigated by analyzing 59 strains of clostridia, and seven strain of other genera. A 516-bp fragment could be amplified from all the C. chauvoei strains tested, and no amplification was observed by using DNAs from the other strains tested, including Clostridium septicum. Similarly, this PCR-based method specifically detected C. chauvoei DNA sequences in samples of muscle and exudate of obtained from mice within 12h of inoculation. In tests using samples of muscle or liver, the limit of detection was about 200 organisms per reaction. These results suggest that the one-step PCR system may be useful for direct detection and identification of C. chauvoei in clinical specimens.

[The characteristics of the antibacterial activity of garlic (author's transl)].

It has been found by Cavallito (1944) that garlic (Allium sativum L.) has a strong antibacterial activity and the active principle of garlic is named "Allicin" and it has been confirmed by Stoll et al. (1948) that the allicin is derived from the alliin-alliinase system. Nevertheless, one worker in Japan (1951) stands on another view point which the effective component of garlic is some sugar-proteim because he could not find alliin in the native garlic. We intend to make sure that the alliin is contained in the native garlic, by using Hitachi Amino acid Analyser with Fraction Collecter and compared with garlic of other countrys' production. As the result of examinations, we found that the good amounts of alliin is contained in the native garlic also. The contents of alliin in garlic is varied by the species, the cultivation mode, and the district of production. The form of alliin in the tissue is a free style, and thereupon it is easy to change its chemical conformation by the method of extraction. We compared the relative capacity between 1 allicin unit by Klein (1954) and 1 cup unit by Schmidt et al. (1947). Furthermore we tested the antibiotic spectrum, using the pure crystallized alliin with the sufficient amount of raw alliinase gained by the isoelectric point precipitation and checked upon wide spectrum for Mycobacterium tuberculosis, Gram-positive and negative bacteria, and some fungi.

Separation of Tc-99 in soil and plant samples collected around the chernobyl reactor using a Tc-selective chromatographic resin and determination of the nuclide by ICP-MS

Technetium (Tc) is known to have high mobility in a soil-water system and also high bioavailability for plants, because the most stable form of Tc in natural surface environment is thought to be TcO4- which is highly soluble. The chemical form of Tc, however, changes with environmental conditions. Thus, it is necessary, for realistic assessment, to obtain transfer parameters, such as transfer factors, under natural conditions. However, it is difficult to obtain these parameters using global fallout 99Tc in actual fields due to its low concentration. In this study, 99Tc concentrations in surface soil and plant leaf samples collected from forest sites within the 30-km zone around the Chernobyl reactor were measured for the first time. In the case of soil samples, a simple and rapid analytical method for determination of 99Tc is used which consists of volatilizing and trapping Tc in a combustion apparatus, purifying the Tc with an extraction chromatographic resin, and measuring it by inductively coupled plasma mass spectrometry (ICP-MS). For plant samples, a wet digestion method in combination with the resin is applied and the 99Tc is measured by ICP-MS. Concentrations of 99Tc in organic soil samples and leaves of strawberry (Fragaria vesca) range from 1.1-14.8 Bq kg(-1) dry weight and 0.2-6.0 Bq kg(-1) dry weight, respectively. Our results indicate that soil-to-plant transfer factors for Tc are similar to those for Cs.

[Pancreatic anastomosis after pancreatoduodenectomy].

Leakage from a pancreatic anastomosis after a pancreatoduodenectomy is one of the serious problems that can bring on life-threatening complications and lead to operative mortality. To minimize these complications, various reconstruction methods have been proposed. A series of 183 patients underwent pancreatoduodenectomy between August 1981 and December 1996 at 1st Department of Surgery, Teikyo University School of Medicine. Our pancreatic anastomoses were classified into four groups based on "Classification of Pancreatic Carcinoma (Japan Pancreas Society, FirstEnglish Edition, Kanehara, Tokyo 1996)",: (a) mucosa-to-mucosa pancreatico jejunostomy in 58 cases, (b) tube assisted (telescoped typed) pancreaticojejunostomyin 104, (c) pancreaticojejunostomy through dunking method in 2, and (d) panc reaticogastrostomy in 19. The leakage occured in 5 out of 104 of tube-assisted pancreaticojejunostomy, in 1 out of 19 of pancreaticogastrostomy. Due to the smooth healing process of anastomosis, mucosa-to-mucosa pancreaticojejunostomy have been applied, leading to results were no leakage occurred in 58 consecutive cases.

Rapid assay of beta-galactosidase and sialyltransferase by lectin affinity high performance liquid chromatography with fluorescence detection.

Fluorescein isothiocyanate (FITC)-labelled asialotransferrin and pyridyl aminated oligosaccharides were prepared from asialotransferrin and human milk using affinity chromatography and high performance liquid chromatography (HPLC), respectively. These substances were incubated with galactosidase or sialyltransferase and then examined by lectin affinity HPLC. The elution patterns changed according to the period of incubation and amount of enzyme. This analytical method using lectin affinity HPLC with fluorescence labelled glycoprotein or oligosaccharides as the substrates has great value for detecting these enzyme under the same chromatographic conditions. In addition, differences were noted in the activity of beta-galactosidase toward oligosaccharides having the Gal beta(1----3)GlcNAc or Gal beta(1----4)GlcNAc structure at reducing termini.

[Occurrence of granulomatous appendicitis in rabbits].

Granulomatous appendicitis was observed in all of the 45 Japanese white rabbits examined. Histologically, multiple microgranulomas were accompanied with foreign body giant cells and focal calcifications in lymph nodules of appendix. Foreign body giant cells contained hair coat and larvae of Passalurus ambiguus. In addition, the sacculus rotundus and mesenteric lymph nodes were affected with the same lesions. PAS- and Gram- positive bacilli were phagocytized in the microgranulomas and macrophages. They were also stained positively with the immunoperoxidase method for the auto-sera of rabbits. Isolation of these bacilli in pure culture has not yet been accomplished. The occurrence of granulomatous lesions due to tuberculosis, pseudotuberculosis, tularemia and actinomycosis seems unlikely in the present cases because none of these organisms nor characteristic lesions were detected. It was suggested that the Gram-positive bacilli appeared to play a role in granulomatous appendicitis.

Conversion of percutaneous transhepatic cholangiodrainage to internal drainage in obstructive jaundice.

Conversion of percutaneous transhepatic cholangiodrainage to an internal drainage is safe and effective in rerouting bile back to the intestinal tract in obstructive jaundice without major operative procedure. The method consists of insertion of a drainage tube with several side holes beyond the bile duct stricture. Usually, a single lumen tube is used. However, double lumen tube is preferred when the amount of bile discharge is excessive. This simple technique is found to be useful in the management of obstructive jaundice not only for preoperative decompression of biliary tree but also for the inoperable lesions.

A technique of cervical aortic graft transplantation in mice.

A new method of mouse aortic graft transplantation into carotid artery was developed with cuff technique. By harvesting the descending aorta of the donor using a small Teflon cuff (external diameter 0.6 mm, internal diameter 0.4 mm) and super fine-tip forceps, and modifying the method of mouse heterotopic heart transplantation with cuff technique, donor descending aortic allografts could be interposed in the common carotid artery of recipient mice. Histological analysis demonstrated neither evidence of tissue damage nor intimal thickening in isograft implanted over 100 days. We strongly recommend that this new model of aortic transplantation in mice is a simple and useful technique for vascular transplantation research.

Electron transferase activity of diaphorase (NADH: acceptor oxidoreductase) from Bacillus stearothermophilus.

Electrochemical kinetic measurements were carried out for electron-transfer between NADH and the oxidized forms of mediators (ferrocenylmethanol (FMA), ferrocenyl-1-ethanol (FEA), N,N,N',N'-tetramethylphenylenediamine (TMPD), Co(Phen)2+(3) and Fe(CN)4-(6)) catalyzed by diaphorase (NADH: acceptor oxidoreductase, EC 1.6.99.-) purified from Bacillus stearothermophilus. Cyclic voltammograms for the mediators with excess NADH in the presence of diaphorase gave steady-state currents. The quantitative analysis of the dependence of the current on the mediator concentration yielded a Michaelis constant (Km) and molecular activity (ko), which are difficult to determine by the conventional spectrophotometric method. Small Km and large ko values were observed for the oxidized forms of FMA, FEA and TMPD compared to those for Co(Phen)3+(3) and Fe(CN)3-(6). It is suggested that the reaction pocket of the present diaphorase is hydrophobic. The present electrochemical procedure for the determination of the kinetic parameters is applicable widely to similar enzyme reactions.

Role of energy metabolism in drug-induced acute gastric mucosal injuries in humans.

BACKGROUND: In various drug-induced gastric mucosal injuries, it has been speculated that changes in gastric mucosal energy metabolism differ according to the cause of injury. This study was conducted to investigate the role of energy metabolism in two drug-induced gastric mucosal injuries in humans. METHODS: The subjects were patients with acute gastric mucosal injury due to non-steroidal anti-inflammatory drugs (NSAID) or steroids, and normal controls. Two sets of tissue specimens from the antrum and corpus of the stomach were obtained. One specimen from each area was used for histological analysis and the other was stored in liquid nitrogen. The stored tissues were homogenized. Adenosine triphosphate (ATP), adenosine diphosphate (ADP) and adenosine monophosphate (AMP) were measured by the luciferin-luciferase method. RESULTS AND CONCLUSIONS: In the NSAID group, the ATP levels and the total adenine nucleotide (TAN) levels in both the antrum and corpus were significantly lower than in the control group. In the steroid group, no significant differences were observed in either the ATP or TAN levels. The NSAID decreased energy metabolism in the entire stomach while the steroid had a negligible effect on energy metabolism.

A novel model of cutaneous candidiasis produced in prednisolone-treated guinea-pigs.

In an attempt to develop an animal model of cutaneous candidiasis useful for the pre-clinical evaluation of antifungal drugs, experimental cutaneous Candida albicans infections were produced in mature and immature guinea-pigs treated with prednisolone. The morbidity of this model in terms of the extent and the duration of the superficial infection was compared with that of two other reported models: those produced by alloxan treatment, or the use of occlusive dressings. Infected animals were also included which received neither of these treatments. The Candida infection continued steadily for 14 days or more in both mature and immature animal groups treated subcutaneously with prednisolone and appeared to be more suitable than that produced with alloxan treatment or under occlusive dressings. In prednisolone-induced infection, most of the micro-organisms were confined to the epidermis and there was no evidence of their dermal penetration during the 14 day experimental period. In view of these facts, we attempted a more quantitative method by counting viable Candida in order to evaluate antimycotics in a shorter therapy period during which animals would receive less exposure to the vehicle. These findings appear to indicate that the experimental model of cutaneous C. albicans infection produced in prednisolone-treated mature and immature guinea-pigs would be useful for studies on the therapeutic efficacy of antifungal agents against cutaneous candidiasis.

Broadly reactive and highly sensitive assay for Norwalk-like viruses based on real-time quantitative reverse transcription-PCR.

We have developed an assay for the detection of Norwalk-like viruses (NLVs) based on reverse transcription-PCR (RT-PCR) that is highly sensitive to a broad range of NLVs. We isolated virus from 71 NLV-positive stool specimens from 37 outbreaks of nonbacterial acute gastroenteritis and sequenced the open reading frame 1 (ORF1)-ORF2 junction region, the most conserved region of the NLV genome. The data were subjected to multiple-sequence alignment analysis and similarity plot analysis. We used the most conserved sequences that react with diverse NLVs to design primers and TaqMan probes for the respective genogroups of NLV, GI and GII, for use in a real-time quantitative RT-PCR assay. Our method detected NLV in 99% (80 of 81) of the stool specimens that were positive by electron microscopy, a better detection rate than with the two available RT-PCR methods. Furthermore, our new method also detected NLV in 20 of 28 stool specimens from the same NLV-related outbreaks that were negative for virus by electron microscopy. Our new assay is free from carryover DNA contamination and detects low copy numbers of NLV RNA. It can be used as a routine assay for diagnosis as well as for elucidation of the epidemiology of NLV infections.

Ultraviolet Rayleigh-Mie lidar with Mie-scattering correction by Fabry-Perot etalons for temperature profiling of the troposphere.

A Rayleigh-Mie-scattering lidar system at an eye-safe 355-nm ultraviolet wavelength that is based on a high-spectral-resolution lidar technique is demonstrated for measuring the vertical temperature profile of the troposphere. Two Rayleigh signals, which determine the atmospheric temperature, are filtered with two Fabry-Perot etalon filters. The filters are located on the same side of the wings of the Rayleigh-scattering spectrum and are optically constructed with a dual-pass optical layout. This configuration achieves a high rejection rate for Mie scattering and reasonable transmission for Rayleigh scattering. The Mie signal is detected with a third Fabry-Perot etalon filter, which is centered at the laser frequency. The filter parameters were optimized by numerical calculation; the results showed a Mie rejection of approximately -45 dB, and Rayleigh transmittance greater than 1% could be achieved for the two Rayleigh channels. A Mie correction method is demonstrated that uses an independent measure of the aerosol scattering to correct the temperature measurements that have been influenced by the aerosols and clouds. Simulations and preliminary experiments have demonstrated that the performance of the dual-pass etalon and Mie correction method is highly effective in practical applications. Simulation results have shown that the temperature errors that are due to noise are less than 1 K up to a height of 4 km for daytime measurement for 300 W m(-2) sr(-1) microm(-1) sky brightness with a lidar system that uses 200 mJ of laser energy, a 3.5-min integration time, and a 25-cm telescope.

[A clinical study on primary renal pelvic tumors].

Sixteen cases of primary renal pelvic tumor treated at our Department between July, 1971 and June, 1984, were reviewed. The sixth decade predominated over other age groups and occupied 47.3% of all cases (average: 63.9 years). The sex ratio was 4.3:1 with male patients predominating over female patients. The incidence of the affected side was equal; 8 cases in the right and 8 cases in the left renal pelvis. The most common initial symptom was macroscopic hematuria in 13 cases (81.2%), followed by flank pain in 2 cases (12.5%). The major findings in IVP were filling defect in 11 cases (68.7%) and non-visualizing kidney in 4 cases (25.0%). Positive urinary cytology was obtained in 6 cases (49.1%) by voided urine specimen and 4 cases (50.0%) by catheterized urine specimen. Histologically, all cases were transitional cell carcinoma; 11 of them were low stage and 5 were high stage at the initial diagnosis. Nine patients(56.1%) were treated by total nephroureterectomy associated with partial cystectomy. The over all survival rate at 1,2,3,4 and 5 years was 86%, 78%, 78%, 68% and 68%, respectively, by the Kaplan-Meier method. The five year survival rate was 80% for the low stage group and 0% for the high stage group. (p less than 0.05, generalized Wilcoxon test). The five year survival rate was 83% for the low grade group and 40% for the high grade group. (p less than 0.05, Generalized Wilcoxon test) Among several factors, stage and grade of the tumor were the most influencing factors for prognosis.

A comparative study of subcutaneous mastectomy with radical mastectomy.

The purpose of this study was to compare the results of 133 cases (131 patients) of subcutaneous mastectomy with axillary dissection between 1983 and 1999 and 910 cases of radical mastectomy during the same period. The median follow-up period of the subcutaneous mastectomy group and the radical mastectomy group were 66 months and 81 months, respectively. The age at operation was significantly (p<0.01) younger in the subcutaneous mastectomy group than in the radical mastectomy group and the clinical stage was significantly (p<0.01) earlier. Lymph node metastasis was significantly (p<0.01) higher in the radical mastectomy than in the subcutaneous mastectomy group. There was no difference in ER status between the two groups. There was local recurrence in 5 (3.8%) members of the subcutaneous mastectomy group and in 12 (1.3%) members of the radical mastectomy group. There was no difference in disease-free survival and overall survival between the two groups. Divided into two subgroups by lymph node status, there was no difference in disease-free survival and overall survival between the two groups. Local recurrence occurred more frequently (p<0.05) in the subcutaneous mastectomy group, however, than in the radical mastectomy group when no lymph node metastasis was found. Multivariate analysis using the Cox hazard model showed that operation method and lymph node status were independent prognostic factors for local recurrence, whereas, lymph node status and ER status were independent prognostic factors of disease-free survival. In conclusion, subcutaneous mastectomy presents a risk factor for local recurrence, but the survival rate of the subcutaneous mastectomy group is as favourable as the radical mastectomy group.

Results of radiation therapy for superficial esophageal cancer using the standard radiotherapy method recommended by the Japanese Society of Therapeutic Radiology and Oncology (JASTRO) Study Group.

BACKGROUND: Superficial esophageal cancer (SEC) is defined as esophageal cancer limited to the submucosal layers, including mucosal cancer and submucosal cancer, and is squamous cell carcinoma in most patients. In 2000, the Japanese Society of Therapeutic Radiology and Oncology (JASTRO) Study Group for SEC published a consensus guideline of standard radiotherapy methods. In this study, the interim treatment outcomes of SEC patients, who had received radiation therapy following the standard radiotherapy methods, were investigated. PATIENTS AND METHODS: From 2000 to 2003, a total of 141 SEC patients were treated in 24 institutions in Japan. RESULTS: The 1-, 2- and 3-year survival rates were 95%, 90% and 90%, respectively, for patients with mucosal cancer and 90%, 81% and 70%, respectively, for patients with submucosal cancer. The overall survival was better in patients who had undergone chemotherapy than in patients who had received radiation therapy alone, though the difference was not statistically significant. The clinical target volume (CTV) did not influence overall survival and intracavitary irradiation did not influence the local control rate in either patients with mucosal or submucosal cancer. Radiation-induced esophageal ulcer was not observed in this series. CONCLUSION: The standard radiotherapy methods are safe and effective for treating SEC. However, the usefulness of chemotherapy and intracavitary irradiation and the optimal setting of the CTV should be clarified by future randomized trials.

Plasma histamine levels in normal and atopic children.

Plasma histamine was measured in 258 children by using a sensitive high performance liquid chromatographic method. A normal level of histamine, which was high at a lower age and low at a higher age, was determined. Atopic children had higher levels than normal children. The extent of atopic disease had a relationship with higher levels. The correlations between plasma histamine and basophil count and between plasma histamine and IgE were significant in atopic children but not in normal children.
