An integrated system for demographic estimation from two age distributions.

This paper presents a simple method for estimating a birth rate and a level of mortality for an intercensal period. The birth rate is estimated from the intercept of a line fitted to data and the level of mortality from the slope of that line. The formula that is developed is based upon a recent generalization of stable population relations. An estimate of childhood mortality level is an optional but significant piece of additional input. An important by-product of the procedure is an estimate of the true age distribution.

[Systemic integration of dominating motivation].

The paper analyzes the investigations into the central architectonics of biological motivations, which have been performed by the P.K. Anokhin scientific school. Each biological motivation is postulated to be a result of specific integration of heterochemical mechanisms of biological motivations. The chemical plasticity of biological motivations is evidenced by the findings of the duplication of different neurotransmitters in cortical-subcortical interrelations, as well as the potentialities of the chemical rearrangement ("chemical compensation") of neurotransmitter mechanisms of biological motivations in case of bilateral failure of hypothalamic motivational centers of the hypothalamus. Some cerebral oligopeptides are found to be able to temporarily and selectively modify the pattern of motivational behavioral responses evoked by electric stimulation of various hypothalamic formations. It is concluded that the biological motivations are a result of the integrative performance of the whole brain wherein hypothalamic pacemakers play the leading but not final role.

An integrated system of community services for the rehabilitation of chronic psychiatric patients in Shenyang, China.

The evolution of the Zhengyang Community Mental Health Rehabilitation Centre described in this paper proves that community-based mental health services initiated and developed by enthusiastic and committed community members can be successful in the Chinese setting. The most important step is to utilise community resources to secure a stable source of income, preferably by establishing a profitable welfare enterprise that can both finance the provision of other services and provide work for mentally ill clients who cannot obtain regular employment. The ultimate goal is to reintroduce as many clients back into the community as possible, so a range of services is needed: supervision of medication, social skills training, occupational rehabilitation, and, most importantly, work placement. The vitality of the organisation depends on its flexibility and responsiveness to the changing needs of patients; it must use its experience to guide policy decisions about the development of new services and the alteration or termination of existing services.

Managing conflict in an integrated system.

The current turmoil and uncertainty in the health care industry are causing increasing tension and conflict among the groups responsible for rendering and managing care (physicians, nurses, hospital executives, and board members), groups already having adversarial histories. This chapter examines ways to recognize and manage conflict, or situations of potential conflict.

Continuous physiological monitoring: an integrated system for use in neonatal intensive care.

A general purpose, multi-channel digital data acquisition and analysis system (Amlab, Associative Measurements, Sydney) configured to collect and process physiological signals including electroencephalograph and blood pressure is described. Its advantages over analog and single-purpose instrumentation are discussed as is its potential, by the addition of transducers, to replace single-purpose instruments.

The heart as a self-regulating system: integration of homeodynamic mechanisms.

In the past the study of mechanical and electrical properties of the heart has been disjointed with minimal overlap and unification. The fact remains that these features are tightly coupled and central to the functioning heart. The maintenance of adequate cardiac output relies upon the highly integrated autoregulatory mechanisms and modulation of cardiac myocyte function. Regional ventricular mechanics and energetics are dependent upon muscle fiber stress-strain rate, the passive properties of myocardial collagen matrix, adequate vascular perfusion, transcapillary transport and electrical activation pattern. Intramural hydraulic "loading" is regulated by coronary arterial and venous dynamics. All of these components are under the constant influence of intrinsic cardiac and extracardiac autonomic neurons, as well as circulating hormones. A brief overview of the putative regulation of these various components is presented in this paper.

Initiatives to build a competitive healthcare system: integrating service redesign and clinical pathways.

In this article, the authors describe how two separate initiatives (the redesign of hospital services and the development and implementation of clinical pathways) were integrated at an academic medical center. The lessons learned in integrating projects that were not conceived at the same time and in developing multidisciplinary teams to produce new case management tools may be used by clinicians and managers elsewhere. The service redesign project developed a role for senior, experienced nurses with clinical expertise and patient management skills to manage the care of groups of similar patients, focusing on patient outcomes. The clinical pathway project developed diagnostic related groups-based clinical pathways. The steps involved in clinical pathway development-stakeholder cooperation building, initiation of data collection tools, data analysis, implementation of the role of the expert nurse in "championing" the pathway, and evaluation of the effects of pathways-are discussed.
