Radiographic evaluation of apical periodontitis and endodontic treatment results: a computer approach.

Chronic apical periodontitis is diagnosed primarily by radiography. Whereas subjective, chairside evaluations of radiographs have serious limitations as regards accuracy of endodontic diagnosis, digitalization of radiological data allows quantification of the information obtained. In clinical practice, digitalization may improve the diagnostic sensitivity and reduce the radiation dose; in clinical endodontic research, digitalization of radiographs provides a means of unbiased and quantitative assessment of apical periodontitis. Automated measurements of the radiographic density of the periapical area have been applied in a study of the effect of different endodontic sealers on the healing of apical periodontitis. Changes in radiographic density could be detected as early as 3 weeks after completion of endodontic treatment, and the computer-based analyses provided numeric values for the progression of the healing process in the periapical region.

Attainment of hermetic seal with decalcified allogenic bone matrix grafts--surgical vs. conservative approach.

Twenty four non vital upper anterior teeth from twelve healthy patients, between the ages of 18-30 years, having definite radiolucent areas at the periapex, were taken for this study. A comparison was made between the surgical and conservative approach, using decalcified allogenic bone matrix (DABM) grafts to achieve a physiologic seal at the root apex. The period of observation were 60 days, 120 days and 180 days. The clinical and radiological finding of this study strongly support the observation that DABM accelerates osteogenesis and cementogenesis which seal the root apex through a physiological process. The process of healing was quicker when the surgical approach was used. However a longer period of observation and histological study is required before any conclusion can be drawn, as regard the choice of a surgical or conservative approach.

[Prognosis of canal treatment. Success and failure].

This clinical review confirms the very positive prognosis for endodontic therapy, conditional to strict compliance with imperatives. Endodontics failures are transient and can be remedied. The authors define the attitude to be adopted and the measures to be taken in accordance with the type of difficulty encountered.

Quantity of apical extruded material and efficiency of five different methods of removing gutta-percha and sealer from root canals--"in vitro" evaluation.

The objective of this study was the comparison, in three clinical situations, of five different methods of removing gutta-percha and sealer from root canals necessitating endodontic retreatment "in vitro". In the first method only hand instruments (files) were used; in the second and third Sonic and Ultrasonic devices were used respectively, and in the fourth and fifth methods Sonic and Ultrasonic devices were used with hand instruments. In order to evaluate these methods, the time required to remove the filling material, the amount of debris remaining in the canals after the deobstruction and the quantity of substances extruded beyond the apex were investigated. The results showed that while the hand method and the combined methods had no statistically significant differences in respect of time spent, the amount of remaining debris was greater in the combined method. On the other hand, the sonic and ultrasonic techniques demonstrated that more time was required for deobstruction when compared to the hand instruments technique, besides a greater quantity of remaining debris in the canal walls. The extruded material beyond the apex was considered non significant in all methods.

[Indications for surgical endodontics].

In the last decade the number of absolute indications for periapical surgery (no alternative, except for extraction) is increasing. In an increasing number of clinical situations periapical surgery has to be considered as a kind of ultimum refugium-therapy to save the tooth. Because of this shift the percentage of success of the surgical procedure has decreased. Moreover, the degree of difficulty of the treatment is increasing by a dorsal shift with regard to the teeth to be treated.
